1.Comprehensive Analysis of Flavonoid Content in Chrysanthemum Indicum:Germplasm Variation,Tissue Distribution,and Developmental Stage Changes

Objective To provide a scientific basis for the quality control of Chrysanthemi Indici Flos medicinal materials and the sustainable development of this resource,the germplasm differences,tissue distribution,and developmental stage changes of flavonoid content in Chrysanthemum indicum was explored.Methods High-performance liquid chromatography(HPLC)was used to quantitatively analyze the five main flavonoid compounds,including linarin,tilianin,cynaroside,apigenin,and acacetin.Real-time fluorescence quantitative PCR(qPCR)technology was used to evaluate the relative expression levels of the rhamnosyltransferase(RhaT)gene at different developmental stages.Results The study found significant differences in flavonoid accumulation among different Chrysanthemum indicum germplasms,with the highest content of linarin observed in the Hubei diploid.Flavonoid content was significantly higher in leaves,flower buds,and flowers compared to stems and roots.Linarin and tilianin peaked at the flower bud stage and then gradually decreased,while the contents of apigenin and acacetin increased with flower development.The qPCR analysis further showed that the expression level of the RhaT gene was highly consistent with the accumulation pattern of linarin.Conclusion The results reveal that flavonoid content in Chrysanthemum indicum is significantly affected by germplasm,tissue type,and developmental stage,providing important data support for the quality evaluation of Chrysanthemum indicum medicinal materials and the rational development of this resource.
2.Establishment of an evaluation system for conversion to laparotomy in laparoscopic cholecystectomy and exploration of surgical grading management

Objective To develop and validate a scoring system to predict the possibility of laparoscopic cholecystectomy (LC) conversion to laparotomy based on preoperative clinical data, and to establish a grading management model of surgery. Methods A retrospective analysis was conducted on the clinical data of 9 414 patients who underwent LC at Renhe Hospital and Huashan Hospital from June 2013 to June 2018. The patients were divided into two groups: the LC group (9 246 patients who successfully underwent LC) and the conversion to laparotomy group (168 patients who required conversion to open surgery). The data of two groups were compared, and the risk factors affecting conversion to laparotomy were screened out by single factor analysis of Chi-square test. Then, the risk factors were analyzed by multiple Logistic regression, and the pre-coefficient of each variable of the risk factors was assigned according to the established conversion to laparotomy possibility function. After calculating the score of each case, the difference in the actual conversion rate of each group was compared. The area under receiver operating characteristic (ROC) curve was calculated to evaluate the performance of the scoring system. According to the scoring system, LC surgical grading management model was created and verified. Results The following factors were identified as significant risk factors for conversion to laparotomy (P < 0.001): body temperature ≥ 38.5℃, frequency of acute cholecystitis ≥3 times, maximum thickness of gallbladder wall ≥ 5 mm, gallbladder neck stone incarceration, diameter of common bile duct ≥8 mm, and surgical experience ≤50 cases were the risk factors for conversion to laparotomy (P < 0.001). A score >3 points was associated with a high risk of conversion to laparotomy. Conclusions The LC scoring system and surgical grading management are reliable and effective tools for predicting and reducing the conversion rate of LC to laparotomy.
3.Analysis of 20 cases of primary coarctation of the aorta in children treated with transcatheter covered stent implantation

Objective:To evaluate the safety and early-to-mid-term efficacy of Cheatham-Platinum (CP) covered stent in treating primary coarctation of the aorta (COA) in children.Methods:?A retrospective self-controlled study was conducted on 20 pediatric patients with primary COA who underwent CP covered stent implantation at the Department of Pediatric Cardiology, Guangdong Provincial People′s Hospital, between January 2006 and December 2024. Clinical baseline characteristics, procedural details, and postoperative follow-up records were collected. Changes in aortic pressure gradients before and after the procedure, as well as the complication rates, were assessed. Comparisons between pre-and post-operative parameters were performed using paired Student′s t-tests and rank sum test. Results:Among the 20 patients, 14 were males and 6 were females, with the age of 12 (11, 13) years and the weight of 43 (36, 49) kg. All of the patients underwent successful implantation of CP-covered stents, with a technical success rate of 100%. The immediate peak gradient across the coarctation segment decreased significantly from 49 (33, 58) mmHg (1 mmHg=0.133 kPa) preoperatively to 3 (0, 5) mmHg postoperatively ( Z=3.92, P<0.001). The narrowest vessel diameter increased from (5.6±3.2) mm preoperatively to (16.9±5.4) mm postoperatively ( t=14.73, P<0.001). Following stent implantation, all patients exhibited a significant reduction in blood pressure, with left upper arm systolic blood pressure decreasing from(141±19) mmHg preoperatively to (122±11) mmHg postoperatively ( t=4.47, P<0.001). Immediate complications occurred in 3 cases: one pseudoaneurysm, one left subclavian artery occlusion, and one access site hematoma. During a follow-up period of 2 (1, 3) years, 16 patients maintained blood pressure within the normal range, while 4 had residual hypertension. Restenosis occurred in 3 patients: 1 patient underwent re-dilation at 24 months postoperatively; and the other 2 patients, who exhibited an upper-to-lower limb systolic blood pressure gradient <20 mmHg, did not require intervention at the time of reporting. All stents remained well-positioned. Follow-up CT angiography at 1 year in 8 patients demonstrated stent patency without evidence of fracture or aneurysm. Conclusion:CP-covered stent demonstrates high safety and favorable short-to-mid-term efficacy in treating primary COA in children.
4.Genomic analysis and multidrug resistance of monophasic Salmonella enterica serovar Typhimurium isolates from Henan

Objective:To analyze the multidrug resistance and genomic characteristics of Monophasic variant of Salmonella enterica serovar Typhimurium (monophasic Salmonella enterica serovar Typhimurium) isolates from clinical patients and food sources in Henan province. Method:A total of 91 monophasic S.Typhimurium strains isolated from clinical and food sources in Henan from 2021 to 2023 were analyzed for antimicrobial sensitivity and underwent whole genome sequencing. Multilocus sequence typing(MLST), multidrug resistance genes and plasmid types were identified using the sequencing data. Phylogenetic tree based on core genome multilocus sequence typing (cgMLST) and single nucleotide polymorphism (SNP) sites was constructed to analyze the genetic evolutionary relationship by comparing with international popular strains in public databases. The Chi-square test was used to compare drug resistance rates. Results:No significant difference was observed in the drug resistance rates between the clinical strains and food strains in Henan [82.19%(60/73) and 11/18, χ2=2.614, P=0.106]. The overall multidrug resistance (MDR) rate was 78.02%(71/91). Most strains were resistant to ampicillin, tetracycline chloramphenicol, β-lactam and sulfonamides. Resistance genes carried by the isolates varied, as well as the drug-resistant phenotypes. MLST showed that 91 strains of S.Typhimurium belonged to ST34 and carried aminoglycoside acetyltransferase gene aac(6′)-Iaa and mobile genetic elements such as plasmids IncQ1 and IncHI2/IncHI2A. The results of cgMLST typing phylogenetic trees showed that food and clinical isolates from the same region in Henan were identified as the same cgST type and clustered in the same branch, which indicated the risk for cross-infection between animal and human. The phylogenetic tree of monophasic S.Typhimurium constructed based on SNP sites showed that the majority of monophasic S.Typhimurium strains in Henan were close to the strains from other provinces in China, other strains were close to the strains from Europe and Southeast Asia, suggesting the possibility of cross regional transmission of the strains. Pork was identified as the main food source. Conclusion:The prevalent ST type of monophasic S.Typhimurium isolated from Henan was ST34, which carried multiple antibiotic resistance genes and widespread drug resistance phenotypes. Most of the monophasic S.Typhimurium isolates in Henan showed a specific phylogenetic lineage with the foreign epidemic strains, indicating the possibility of dissemination of strains between humans and food.
5.Exploration of the integration path of health science popularization and communication practice in on-cology teaching

Objective To explore the necessity,significance,and implementation forms of integrating tumor health sci-ence popularization education and communication practice into clinical undergraduate oncology teaching.Methods Science pop-ularization education was conducted among clinical undergraduate oncology students at Guangzhou Medical University who partici-pated in science communication activities.Before and after the activities,online questionnaires were distributed via Wenjuanxing.Student questionnaires were collected within the deadline,while two different versions of questionnaires were randomly distributed to 30 parents and 20 faculty members involved in oncology teaching to assess their views and attitudes toward the necessity of pub-lic cancer prevention science communication,as well as the integration of science popularization education and communication practice into oncology teaching,and to evaluate its educational significance.Results The analysis of the questionnaires showed that nearly all or the vast majority of students,parents,and teachers highly recognized the necessity of public cancer prevention science education and awareness-raising,as well as the importance of medical students disseminating tumor health knowledge.They also strongly supported the inclusion of science popularization education in oncology teaching.Most students felt that their knowledge level improved significantly after the education sessions.Conclusion The integration of tumor science popularization education and communication practice into oncology teaching is highly necessary and meaningful.It significantly enhances students' health science literacy,sense of mission,and responsibility.It is recommended to incorporate such content into oncolo-gy teaching.
6.Cloning and Prokaryotic Expression of Three Flavonoid Glycoside Synthesizing UGT Genes of Chrysanthemum indicum.L

Objective The three UGT genes involved in the synthesis of flavonoid glycosides in the wild chrysanthemum were cloned,fused and expressed in prokaryotic system,which provided a basis for further study of the functions of these genes in the synthesis of flavonoid compounds.Methods three candidate UDP-glycosyltransferase(UGT)family genes were successfully cloned from flower tissue cDNA of diploid Chrysanthemum indicum.L in Hubei,namely:CiUGT1(1425 bp),CiUGT2(1311 bp)and CiUGT3(1314 bp)encoded opal fraction of 51.72,48.08 and 48.28 kDa,respectively.After the physical and chemical properties of the protein were analyzed,the recombinant plasmid was constructed by seamless Klon technology,and the recombinant proteins of CiUGT2 and CiUGT3 were successfully expressed in Escherichia coli.Results SDS-PAGE analysis showed that the purified CiUGT2 and CiUGT3 protein samples were about 0.2 mg/mL and 1.4 mg/mL,respectively.Conclusion Two candidate UGTs were obtained to catalyze the synthesis of flavonoid 7-O-glucosides in Chrysanthemum indicum.L,which provide the theoretical basis for the molecular mechanism of flavonoid glycosides biosynthesis in the capitulum of Chrysanthemum indicum.L.
7.Advances in diagnostic difficulties and targeted therapy of IgG4-related sclerosing cholangitis

IgG4-related sclerosing cholangitis (IgG4-SC) is an IgG4-related disease charac-terized by bile duct wall thickening and luminal stricture. Its pathogenesis involves complex multi-dimensional regulation, including immune imbalance, genetic epigenetic regulation, microbial ecolo-gical abnormalities, and activation of fibrotic pathways. Due to the lack of specificity in clinical mani-festations, IgG4-SC is easily misdiagnosed as biliary tract diseases such as cholangiocarcinoma and primary sclerosing cholangitis in the early stage, leading to diagnostic difficulties. At present, gluco-corticoids are the preferred treatment option. Although most patients can quickly recover in the early stages, high recurrence rates and hormone dependence limit the long-term effectiveness of treatment. The authors combine the latest research progress at home and abroad to deeply analyze the pathogenesis, clinical manifestations, and diagnosis of IgG4-SC, focusing on treatment strategies and cutting-edge advances, in order to provide reference and inspiration for clinical practice.
8.Analysis of quality of life of rectal cancer patients after transanal total mesorectal excision and laparoscopic total mesorectal excision:a retrospective cohort study

Background and purpose:There are few studies on the quality of life after transanal total mesorectal excision(TaTME)for rectal cancer,and there is a lack of evidence-based research.This study aimed to evaluate the impact of TaTME in comparison to laparoscopic total mesorectal excision(LaTME)on postoperative quality of life in patients.Methods:A retrospective analysis was conducted on clinical and pathological data from rectal cancer patients who underwent LaTME and TaTME between September 2019 and September 2022 at Renhe Hospital,Baoshan District,Shanghai,and Fudan University Shanghai Cancer Center.All the patients completed a set of validated questionnaires one year postoperatively,including the EORCT QLQ-C30,Low Anterior Resection Syndrome(LARS)score,International Prostate Symptom Score(IPSS),Generalized Anxiety Disorder-7(GAD-7),and Patient Health Questionnaire-9(PHQ-9).Comparative analysis of these survey results was performed.The study was approved by the Ethics Committee of Renhe Hospital,Baoshan District,Shanghai.The Strengthening the Reporting of Observational Studies in Epidemiology(STROBE)checklist was followed for this study.Results:A total of 39 patients in the TaTME group and 38 patients in the LaTME group were included.There were no statistically significant differences between two groups regarding demographic variables such as gender and age,as well as clinical factors including tumor distance from the anal verge,tumor stage,and preoperative neoadjuvant treatment protocols.Analysis of EORCT QLQ-C30 results indicated equivalent overall quality of life between the two groups;however,the TaTME group reported significantly more severe insomnia(P=0.020).No significant differences were observed in LARS severity or total scores between the groups,though the TaTME group demonstrated superior functional outcomes in terms of incontinence for liquid stools and clustering of stools(P=0.007,P=0.004).Additionally,both groups exhibited comparable results in IPSS,and levels of depression and anxiety.Conclusion:The findings suggest that TaTME and LaTME yield similar outcomes in terms of anal and urinary function,overall quality of life,and psychological health,indicating that it is a viable and safe alternative to LaTME in terms of postoperative quality of life.
