1.Long-term efficacy of three intra-articular injections in the treatment of knee osteoarthritis:a network meta-analysis

OBJECTIVE To evaluate the long-term efficacy of intra-articular injection of hyaluronic acid (HA), platelet-rich plasma (PRP), and micro-fragmented adipose tissue (MFAT) in treatment of knee osteoarthritis (KOA). METHODS Randomized controlled trials (RCTs) of KOA patients receiving intra-articular injection of the three agents were collected from PubMed, Web of Science, Embase, Cochrane Library, CNKI, Wanfang Data, VIP, Chinese Biomedical Literature Service System,and clinical trial registration platforms. The retrieval time limit was from January 2016 to December 2025. After screening the literature, extracting the data,and evaluating the quality of the literature, network meta-analysis was performed using RevMan 5.3 and Stata MP 16.0 software. RESULTS A total of 31 RCTs involving 2 964 patients were included. For pain relief, after 6 months of treatment, the Visual Analogue Scale (VAS) scores in the PRP+HA and PRP groups were significantly lower than those in the saline and HA groups ( P <0.05), PRP+HA and PRP were ranked as the top two by surface under the cumulative ranking curve (SUCRA). After 12 months of treatment, the VAS scores in the PRP group were significantly lower than those in the HA and saline groups ( P <0.05), PRP and PRP+HA were ranked as the top two by SUCRA. For functional recovery, after 6 months of treatment, the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) scores in the PRP group were significantly lower than those in the HA or saline groups ( P <0.05). After 12 months of treatment, the WOMAC scores in the MFAT, PRP and PRP+HA groups were significantly lower than those in the HA or saline groups ( P <0.05). After 6 and 12 months of treatment, International Knee Documentation Committee (IKDC) scores in the MFAT and PRP groups were significantly higher than the saline group ( P <0.05), and after 12 months of treatment, the PRP group was significantly higher than the HA group ( P <0.05). MFAT and PRP were ranked as the top two by SUCRA for both the WOMAC and IKDC scores. CONCLUSIONS For pain relief, PRP+HA showed a significant advantage at 6 months after treatment, whereas PRP alone demonstrated a notable superiority at 12 months after treatment. For functional recovery, MFAT exhibited a marked advantage at both 6 and 12 months after treatment.
2.Rapid health technology assessment of insulin icodec for the treatment of type 2 diabetes mellitus

OBJECTIVE To comprehensively evaluate the efficacy, safety and cost-effectiveness of insulin icodec in treating type 2 diabetes mellitus (T2DM), providing evidence-based guidance for new drug selection in hospital and clinical medication decision-making. METHODS PubMed, Cochrane Library, Embase, CNKI, Wanfang, VIP and foreign health technology assessment (HTA) websites were searched by using rapid health technology assessment from inception to 15 July 2025 for systematic reviews/meta-analyses, pharmacoeconomic studies, and HTA reports on insulin icodec in the treatment of T2DM. After data extraction and quality assessment, the findings of the included studies were analyzed descriptively. RESULTS Ten systematic reviews/meta-analyses and three pharmacoeconomic studies were included. Among them, 4 systematic reviews/meta-analyses were of high quality; the overall quality of the 3 pharmacoeconomic studies was relatively good. Regarding efficacy, insulin icodec was superior to once-daily basal insulin in reducing glycated hemoglobin (HbA1c) and in achieving the target of HbA1c<7% (P<0.05). No significant differences were observed between icodec insulin and comparators in lowering fasting plasma glucose (P>0.05). For safety, insulin icodec did not increase the incidence of any adverse events (AEs), serious AEs, clinically significant hypoglycemia (random glucose<3 mmol/L), injection-site reactions, or allergic reactions, compared with once-daily basal insulin overall (P> 0.05); however, insulin icodec was associated with a significant increase in body weight (P<0.05). Domestic economic evaluations indicated that insulin icodec was more cost-effective than insulin glargine and insulin degludec when its annual costs were in the range of 784.90-1 145.96 and 597.66-736.34 US dollars, respectively. CONCLUSIONS Insulin icodec demonstrates favorable efficacy and safety profiles in the treatment of T2DM; however, attention should be paid to the risk of weight gain. Under China’s healthcare system, insulin icodec demonstrates greater economic value only when the patient’s weekly required basal insulin dose falls within a specific range,and clinical practice requires individualization.
3.Genetic insights into thymic carcinomas and thymic neuroendocrine neoplasms denote prognosis signatures and pathways.

BACKGROUND:
Thymic carcinomas (TCs) and thymic neuroendocrine neoplasms (TNENs) are two aggressive subtypes of thymic malignancy. Traditional therapy for advanced TCs and TNENs has limited outcome. New genomic profiling of TCs and TNENs might provide insights that contribute to the development of new treatment approaches.
METHODS:
We used gene panel sequencing technologies to investigate the genetic aberrations of 32 TC patients and 15 TNEN patients who underwent surgery at Shanghai Chest Hospital between 2015 and 2017. Patient samples were sequenced using a 324-gene platform with licensed technologies. In this study, we focused on clinically relevant genomic alterations (CRGAs), which are previously proven to be pathogenic alterations, to identify the pathology-specific mutational patterns, prognostic signatures of TCs and TNENs.
RESULTS:
The mutational profiles between TCs and TNENs were diverse. The genetic alterations that ranked highest in TCs were in CDKN2A, TP53, ASXL1, CDKN2B, PIK3C2G, PTCH1, and ROS1 , while those in TNENs were in MEN1, MLL2, APC, RB1 , and TSC2 . Prognostic analysis showed that mutations of ROS1, CDKN2A, CDKN2B, BRAF, and BAP1 were significantly associated with worse outcomes in TC patients, and that mutation of ERBB2 indicated shortened disease-free survival (DFS) and overall survival (OS) in TNEN patients. Further investigation found that the prognosis-related genes were focused on signal pathways of cell cycle control, chromatin remodeling/DNA methylation, phosphoinositide 3-kinases (PI3K)/protein kinase B (AKT)/mammalian target of rapamycin (mTOR), and receptor tyrosine kinase (RTK)/RAS/mitogen-activated protein kinase (MAPK) signaling.
CONCLUSION
We profiled the mutational features of 47 Chinese patients with thymic malignancy of diverse pathologic phenotypes to uncover the integrated genomic landscape of these rare tumors, and identified the pathology-specific mutational patterns, prognostic signatures, and potential therapeutic targets for TCs and TNENs.

4.Effects of rapid drug sensitivity testing for multidrug-resistant bacteria on the prognosis of patients with severe intra-abdominal infection

Objective:To examine the clinical value of rapid detection of drug-resistant bacteria by immunochromatography and the effects of rapid detection on the prognosis of patients with severe intra-abdominal infection complicated by carbapenem-resistant Enterobacteriaceae (CRE) bloodstream infection.Methods:This was a retrospective cohort study. We analyzed clinical data of 73 patients with severe abdominal infections with sepsis or septic shock complicated by CRE bloodstream infection admitted to the general surgery department of Jinling Hospital between February 2022 and February 2023. Patients were divided into a colloidal gold immunochromatographic assay (GICA) group (17 patients) and conventional testing group (56 patients) based on whether a GICA for CRE had been performed on the patients' first blood culture sample during the diagnosis and treatment process. There were no statistically significant differences between the GICA and conventional testing groups in age ([55.9±17.3] vs. [47.6±16.4] years), sex ([16 men vs. one woman ] vs. [41 men vs. 15 women]), median Charlson comorbidity index (3.0[2.0,4.0] vs. 3.0[2.0, 4.8]), septic shock (10 vs. 39), or acute kidney injury (8 vs. 40) (all P>0.05). Both groups routinely underwent traditional bacterial identification and drug susceptibility testing. Additionally, patients in the GICA group were tested directly for positive blood cultures using a GICA carbapenemase test kit. The main outcomes were mortality rates on Days 28 and 90 after the first identification of CRE bloodstream infection in both groups. We also compared the microbial clearance rate, duration of hospitalization and intensive care unit stay, and time from onset of CRE bloodstream infection to initiation of targeted and appropriate antibiotics between the two groups. Results:The rate of microbial clearance of bloodstream infection was significantly greater in the GICA group than in the conventional testing group (15/17 vs. 34/56 [60.7%], χ 2=4.476, P=0.034), whereas the 28-day mortality tended to be lower in the GICA than conventional testing group [5/17 vs. 44.6% [25/56], χ 2=1.250, P=0.264). The 90-day mortality (8/17 vs. 53.6% [30/56], χ 2=0.222, P=0.638), median duration of hospitalization (37.0 [18.0, 46.5] days vs. 45.5 [32.2, 64.8] days, Z=-1.867, P=0.062), and median duration of intensive care unit stay (18.0 [6.5, 35.0] days vs. 32.0 [5.0, 51.8] days, Z=-1.251, P=0.209). The median time between the onset of bloodstream infection and administration of antibiotics was 49.0 (38.0, 69.0) hours in the GICA group, which is significantly shorter than the 163.0 (111.8, 190.0) hours in the conventional testing group ( Z=-5.731, P<0.001). The median time between the onset of bloodstream infection and administration of appropriate antibiotics was 40.0 (34.0, 80.0) hours in the GICA group, which is shorter than in the conventional testing group (68.0 [38.2, 118.8]) hours; however, this difference is not statistically significant ( Z=-1.686, P=0.093). Conclusions:GICA can provide information on carbapenemase- producing pathogens faster than traditional drug sensitivity testing, enabling early administration of the optimal antibiotics. The strategy of 'carbapenemase detection first' for managing bacterial infection has the potential to improve prognosis of patients and reduce mortality rate.

6.Film analysis algorithm of isocenter error based on Hough transform for the CyberKnife system

Objective:A new algorithm based on Hough transform (HT) was proposed to improve the accuracy and stability of the film image analysis of Automatic Quality Assurance (AQA) test, and to explore the influence of the resolution of film image on the test results.Methods:Nine pairs of films were obtained for AQA modules in this study. Firstly, the median filter was used to preprocess the grayed-out film image to remove noise interference. Then, a global threshold was utilized to binarize the image. The images were edge-detected and the film edge line was extracted by Hough transform. The film image was transformed to the correct position. Finally, the edge of the field shadow circle and the shadow circle of the tungsten ball were extracted by the edge detection method and Hough transform. The radial error was finally obtained by analyzing the concentricity.Results:There was no significant difference in the accuracy between the test results yielded by the HT method and the AQA software ( P>0.05). The difference in the standard deviation of the test results was statistically significant ( P=0.027), indicating that the algorithm increased the stability while ensuring the accuracy of film analysis. Increasing the resolution of film scanning failed to significantly improve the accuracy and stability of film analysis in both two methods. Conclusions:The algorithm used in this study can eliminate the human error caused by film scanning placement while ensuring the accuracy of film analysis, providing a more stable way for the AQA test of the CyberKnife system.
7.Simultaneous determination of four kinds of components in Gegen-Huangqin-Huanglian Decoction by UPLC-MS/MS

Objective:To develop an UPLC-MS/MS method for the simultaneous determination of baicalin, berberine, glycyrrhizic acid and puerarin in Gegen-Huangqin-Huanglian Decoction. Methods:Isocratic elution was carried out with mobile phase consisting of acetonitrile - 4 mmol/L ammonium formate. The separation was performed on ACE Excel 3 C18 (2.1 mm × 100 mm, 3 μm), and the mass spectrometer was operated in the positive and negative ionization electrospray (ESI) mode using multiple monitoring (MRM) for analysis of four components. External standard method was used for fix quantity. The precursor to product ion transitions monitored for baicalin, berberine, glycyrrhizic acid and puerarin were m/z 447.0→271.0, 336.2→320.2, 821.4→350.9 and 415.2→294.9, respectively. Results:Baicalin, berberine, glycyrrhizic acid and puerarin were all analyzed exactly, the linear ranges were 0.002-0.080, 0.002-0.080, 0.001-0.040, 0.002-0.080 ng, respectively. The r were 0.998 3, 0.999 4, 0.997 9 and 0.999 5, respectively. The recoveries of four analytes ranged from 98.75% to 100.86% and the relative standard deviations were all below 0.74%. Conclusions:UPLC-MS/MS method is sensitive, accuate with rapid speed, which could be used for the determination of baicalin, berberine, glycyrrhizic acid and puerarin in Gegen-Huangqin-Huanglian Decoction.
8.Effect of siRNA silencing apoptosis signal-regulating kinase 1 on inflammatory response of human alveolar epithelial A549 cells induced by lipopolysaccharide

Objective To study the effect of small interfering ribonucleic acid (siRNA) silencing apoptosis signal-regulating kinase 1 (ASK1) on inflammatory response of lipopolysaccharide-induced alveolar epithelial A549 cells and its mechanism.Method Cell inflammation model of A549 cells was induced by lipopolysaccharide.The expression of ASK 1 in A549 cells was silenced by liposome transfection of siRNA.The mRNA and expression levels of ASK1,interleukin 6 (IL-6),interleukin 8 (IL-8) and tumor necrosis factor alpha (TNF-α) in A549 cells were detected by immunoblotting,real-time fluorescence quantitative polymerase chain reaction and enzyme-linked immunosorbent assay.Result The expression of IL-6,IL-8 and TNF-α in the experimental group was significantly higher than that in the control group (P<0.001),which indicated that the inflammatory model of A549 cells was successfully constructed.The mRNA level and expression of ASK1 in the interference group was significantly lower than that in the negative control group and the blank control group (P<0.01),indicating that silencing ASK1 was also successful.The expressions of IL-6,IL-8 and TNF-α in the interference group (0.37±0.04,0.32±0.04,0.48 ±0.13) were significantly lower than those in the negative control group (1.04±0.11,1.22±0.19,0.93±0.14) and the blank control group (1.01±0.14,1.01 ±0.23,1.02±0.25).The expression of IL-6,IL-8 and TNF-α protein in the interference group (pg/ml) (122.6± 11.0,537.2±42.4,159.2± 19.6) were also significantly lower than those in the negative control group (267.4±20.4,1 289.8±55.3,327.0±26.3) and blank control group (246.6±18.7,1 300.3±35.6,325.2± 18.3),with significant difference (P<0.05).There was no significant difference in each value between negative control group and blank control group (P>0.05).Conclusion Silencing ASK1 by siRNA can down-regulate the expression of IL-6,IL-8 and TNF-α in A549 cells,suggesting that ASK 1 may be involved in the regulation of lipopolysaccharide-induced inflammation in A549 cells.
9.Effect of lisinopril on cardio-pulmonary function and 6 minutes walking distance in patients with hypertension

Objective To compare the difference of antihypertensive efficacy,the result of heart and lung function test(CPET)and 6 minutes walking distance(6MWD)in hypertension patients with different angiotensin converting enzyme(ACE)gene polymorphisms.Methods 120 patients with hypertension were randomly divided into four groups:the control group treated with nifedipine zyban,perimental group of group A(II),group B(ID),group C(DD)treated with lisinopril tablets.The blood pressure control situation,the changes of the CPET and 6MWD results were compared in the four groups 3 and 6 months after treatment.Results After 3 months,there were no significant changes in blood pressure drop of the four groups,and the differences were no statistically significant among the four groups in the range of blood pressure drop(all P>0.05),the body mass,maximum oxygen uptake(VO2/kg)and 6MWD of the four groups were not significantly changed..After 6 months,the blood pressure of the four groups decreased,and there were no statistically significant differences between the four groups in the drop of blood pressure(all P>0.05),VO2/kg(mL):the control group(17.94±1.51)mL,group A(18.04±1.85)mL,group B(19.70±1.25)mL,group C(21.25±2.20)mL and 6MWD:the control group(448.66±50.26)m,group A(445.07±41.21)m,group B(488.56±55.66)m,group C(500.54±53.25)m.The improvement range of VO2/kg and 6MWD was group C>B>A(group B compared with group A:ct=12.01,P=0.03;group C compared with group B:dt=17.26,P=0.02),there were no statistically significant differences between the control group and group A(all P>0.05).Conclusion This study found no ACE genotype associated with the antihypertensive effects of lisinopril,but found improvement of CPET and 6MWD result in different groups,DD type is superior to the ID,the ID type is better than type II.
10.Application and advantage of near infrared spectroscopy technology in authentication of Chinese materia medica.

This review introduced the application of near infrared spectroscopy (NIRS) in the identification of species, production area and processing method as well as quality evaluation of Chinese materia medica (CMM). By comparing with other authentication methods of CMM, the NIRS technology showed great advantages in exclusive identification of CMM, both identification and quality evaluation of CMM as well as identification of famous-region medicinal material. Finally, the future direction of the authentication of CMM by NIRS technology was suggested.
