1.PCDHGB7 Methylation Characteristics and Their Clinical Significance in Lung Adenocarcinoma Based on the TCGA Database

Objective This study used The Cancer Genome Atlas (TCGA) database to investigate the methylation levels of PCDHGB7 in lung adenocarcinoma tissues and analyze its association with clinical pathological features of lung adenocarcinoma, and its prognostic predictive value. Methods Methylation data of PCDHGB7 from 473 lung adenocarcinoma tissues and 32 normal tissues in the TCGA database were derived and analyzed to assess between-group variance and determine the correlation of methylation levels with clinical pathological features and overall survival. Results PCDHGB7 exhibited significantly higher methylation levels in lung adenocarcinoma tissues than in normal tissues (P<0.001), demonstrating a strong discriminative ability for lung adenocarcinoma (AUC=0.926, P<0.001). The methylation level was strongly associated with gender (P=0.047) and T stage (P=0.013). The critical prognostic sites of PCDHGB7 were identified through univariate Cox regression analysis. Rheumatoid arthritis(RA)is a chronic,systemic,autoimmune inflammatory disease.Bone destruction caused by RA is a key factor in disability,involving abnormal activation of immune cells,overproduction of inflammatory factors,and imbalances in bone remodeling process.Ferroptosis,a form of cell death associated with disturbed iron metabolism,inactivation of the antioxidant system and lipid peroxidation,plays a complex role in RA bone destruction.Traditional Chinese medicine(TCM),with its multi-targeted intervention properties,has shown significant potential and advantages in treatment of RA bone destruction.A variety of TCM can regulate the ferroptosis pathway and influence osteoblast function,which has an important role in slowing down RA bone destruction.This review focuses on the latest research progress in the regulation of ferroptosis pathway by TCM to alleviate RA bone destruction,aiming to provide new perspectives and theoretical basis for the treatment of RA bone destruction.

Rheumatoid arthritis(RA)is an autoimmune disease characterized by synovitis,synovial cell proliferation,neo-vascularization,and bone and cartilage destruction.Its pathogenesis is complex and has not yet been fully elucidated.A variety of cells,cytokines and signaling pathways are involved in the pathogenesis of RA.Hypoxia-inducing factor(HIF)and oxygen-sensing signaling pathway(PHD-HIF-VHL)are closely related to the occurrence and development of RA,and play an important role in syno-vial cell proliferation,inflammatory response and cartilage destruction.In this study,the research progress of oxygen sensing signaling pathway in RA was described from the aspects of the mechanism of oxygen sensing signaling pathway and its involvement in the patho-genesis of RA,in order to provide ideas and theoretical basis for the research of anti-RA drugs by targeting important molecules of oxy-gen sensing signaling pathway.

Depression is a frequently-seen mental disorder that profoundly affects the survival and quality of life of individuals.Present clinical medicine therapies for depression are not fully efficacious and novel therapeutic agents and targets remain necessary.Bupleuri Radix-Paeoniae Radix(BR-PRA),an essential and crucial compo-nent of traditional antidepressant compound,possesses the beneficial effect of lowering toxicity and amplifying the antidepressant effect when utilized in combination.The underlying mechanisms of these synergistic effects may involve the suppression of inflammation and oxidative stress,the regulation of monoamine neurotransmitters,brain-derived neurotrophic factors,the modulation of the hypothalamus-pituitary-adrenal axis,and the metabolism of various amino acids and energy.This article summarizes the synergistic effects and antidepressant pharmacological effects of BR-PRA herb-pair,thereby providing valuable insights into the potential advantages of this combination and its potential mechanisms of antidepressant action.

Rheumatoid arthritis(RA)is a chronic and progressive autoimmune disease.Current clinical treatments for RA often exhibit inadequate drug responses and severe adverse effects,underscoring the necessity for alternative therapies.Baicalin,a flavonoid derived from Scutellaria baicalensis roots,possesses anti-inflammatory,antioxidant,and immunomodulatory properties.Research suggests that baicalin can inhibit the release of inflamma-tory mediators,thereby reducing inflammation.Moreover,it demonstrates antioxidant effects by decreasing reactive oxygen species production and mitigating synovial damage induced by oxidative stress.Baicalin may impede the activation of inflammatory cytokines through pathways such as NF-κB,MAPKs,and JAK-STAT signaling cascades while inducing apoptosis,regulating immune cell activity,and balancing immune mediators to impact synovitis progression.Clinically,both monotherapy with baicalin or its combination with other formulations have exhibited efficacy and safety profiles.The advancement in targeted drug delivery systems has improved its bioavailability further suggesting promising clinical applications.This review provides an overview of research progress on baicalin's mechanisms in inhibiting RA synovitis along with its clinical applications.

Total hip arthroplasty(THA)is an effective treatment for elderly femoral neck fractures,mid-to late-stage femoral head necrosis,and end-stage hip osteoarthritis.However,serious complications such as aseptic loosening of the prosthesis,peripheral fractures,and dislocation of the prosthesis still exist following THA,which makes the selection of the appropriate hip prosthesis type and placement position before THA an important challenge for surgeons.Currently,the commonly used preoperative planning methods for THA mainly rely on static images from two-dimensional(2D)X-ray or three-dimensional(3D)computed tomography(CT),which fail to adequately consider the hip joint in weight-bearing as well as motion,lumbar-hip joint changes,and prosthetic impingement during motion.Recently,the dual fluoroscopic imaging system,as a new in-vivo,dynamic radiological imaging technology,provides comprehensive and accurate dynamic 3D data for THA preoperative planning.However,the technical process and expert consensus on preoperative 3D planning of THA using a dual fluoroscopic imaging system have not yet been established,which affects the promotion and application of this technology.In light of the above,national orthopaedic experts and related professional representatives discussed and proposed seven consensus issues,and the'expert recommendation rate'and'strong recommendation rate'were obtained through a questionnaire survey on the recommendations of the participating experts.This consensus aims to provide guidance and reference for the standardised application of preoperative 3D planning of THA using the dual fluoroscopic imaging system.

10.Clinical study on the therapy for a herniated lumbar intervertebral disc with Wechsler's bone setting manipulation

Objective:To evaluate the efficacy of Wechsler's bone setting manipulation in the treatment of lumbar disc herniation.Methods:A randomized controlled trial was carried out. 50 LDH patients from the Spinal Surgery Department of Ruikang Hospital Affiliated to Guangxi University of Chinese Medicine and the Ruikang Hospital Traditional Chinese Medicine Master Hall Affiliated to Guangxi University of Chinese Medicine were selected as observation subjects from January to November 2021. They were divided into two groups using a random number table method, with 25 patients in each group. The control group was treated with traditional bone setting techniques, while the observation group was treated with Wechsler's bone setting correction techniques. Both groups were treated for 3 weeks. VAS scale was used to evaluate the degree of lower back and leg pain, and the Japanese Orthopedic Association was used to evaluate treatment (JOA score) to evaluate lumbar function; the sacral tilt angle (SS), lumbar lordosis angle (LL), and coronal pelvic tilt angle values were measured on the lateral X-ray film of the entire spine; adverse reactions were recorded and clinical efficacy was evaluated.Results:The total effective rate of the observation group was 92.0% (23/25), and the control group was 68.0% (17/25), with statistical significance between the two groups ( χ2=4.50, P=0.034). The VAS score of the observation group after treatment was lower than that of the control group ( t=16.25, P<0.01); the JOA score was higher than that of the control group ( t=-29.39, P<0.01). The SS [(36.00 ± 1.87) ° vs. (34.16 ± 1.57) °, t=-10.64] and LL [(38.08 ± 2.60) ° vs. (34.96 ± 1.90) °, t=-10.04] in the observation group were higher than those in the control group ( P<0.01); the coronal pelvic tilt angle value [(0.47 ± 0.14) ° vs. (0.69 ± 0.14) °, t=42.57] was lower in the control group ( P<0.01). No adverse reactions occurred during the treatment period in both groups. Conclusion:Wechsler's bone setting manipulation can improve the pain symptoms of patients with LDH, and restore the spine pelvis biomechanical balance to a certain extent.
