Objective To investigate the impact of catheter ablation(CA)combined with left atrial appendage closure(one-stop procedure)on cardiac function in atrial fibrillation(AF)patients, and analyze the risk factors for late AF recurrence after one-stop pro-cedure.Methods A total of 225 patients who visited the Inner Mongolia Autonomous Region People's Hospital for AF from January 2020 to December 2023 were included, and they were divided into the one-stop procedure group(n=93)and the CA group(n=132)ac-cording to the procedure, and 55 patients with matching clinical baseline characteristics were respectively screened out at a ratio of 1:1 by the method of propensity score matching, namely the CA group(n=55)and the one-stop procedure group(n=55),and the changes in cardiac function between the two groups were compared between preoperative and 1-year postoperative periods.Risk factors for late post-operative recurrence of AF and their predictive value in the one-stop procedure group were assessed using multivariate Logistic regression analysis and receiver operating characteristic(ROC)curve. Results At 1 year postoperatively,N-terminal B-type natriuretic peptide(NT-proBNP),left atrial internal diameter(LAD),and left ventricular end-diastolic diameter(LVEDD)were significantly lower and left ventricular ejection fraction(LVEF)was significantly higher in both groups compared with the preoperative period(P<0.05),but there was no statistically significant difference in the improvement degree of cardiac function between the two groups at 1 year postopera-tively(P>0.05).The results of the univariate analysis showed that there were statistically significant differences in NT-proBNP,LAD,and left ventricular end-systolic diameter(LVESD)in the one-stop procedure late postoperative recurrence group compared with late postoperative non-recurrence group(P<0.05),and the results of multivariate Logistic regression analysis showed that the preoperative NT-proBNP(OR=1.075,95%CI:1.029-1.124,P=0.001)and preoperative LAD(OR=1.222,95%CI:1.094-1.365,P<0.001)were the risk factors for late recurrence of AF after one-stop procedure. The results of the ROC curve analysis showed that the area under the curve(AUC)of NT-proBNP,LAD,and the combination of the two in predicting late recurrence of AF after one-stop procedure were 0.788,0.772 and 0.846,respectively,with a sensitivity of 84.4%,68.8%and 87.5%,and a specificity of 68.9%,78.7%and 77.0%,respectively.Conclusion Both one-stop procedure and CA can significantly improved cardiac function in patients with AF with comparable results.Preoperative LAD enlargement and NT-proBNP elevation are independent risk factors for late recur-rence of AF after one-stop procedure, and the combination of the two can effectively improve the efficacy of AF recurrence risk assess-ment.

Objective:To explore the value of dual-layer detector spectral CT quantitative parameters in evaluating the treatment response of neoadjuvant chemoradiotherapy (nCRT) in patients with locally advanced rectal cancer (LARC).Methods:The study was a cross-sectional study. From May 2021 to March 2023, a total of 52 patients with LARC who received complete nCRT and were pathologically confirmed rectal adenocarcinoma at the Guangdong Province Hospital of Traditional Chinese Medicine were retrospectively enrolled. Each patient underwent spectral CT examination before and after nCRT, including plain scan, arterial phase (AP), and venous phase (VP) scans. According to the tumor regression grade, the patients were divided into the good response ( n=20) and the poor response group ( n=32). Measurements of the primary tumor′s spectral CT parameters, including effective atomic number (Z eff) at plain scan, iodine concentration (IC), CT values of 40 keV and 100 keV virtual monochromatic image (VMI) at dual-enhanced phases, were taken before and after nCRT. Additionally, the normalized iodine concentration (NIC), spectral curve slope (λHU), and the change rate of the above parameters before and after nCRT were calculated. The independent sample t-test or Mann-Whitney U test was used to compare the differences between the two groups. The receiver operating characteristic (ROC) curve was used to assess the efficacy of various metrics in evaluating the tumor treatment response of nCRT. A binary logistic regression analysis of combined parameter results was performed for the parameters with the areas under curve (AUC)>0.75, and the AUC of the combined parameter was evaluated. Results:There were significant differences in NIC AP and λHU VP before nCRT, NIC VP and λHU VP after nCRT, and the change rates of Z eff, NIC AP, NIC VP and λHU AP between the good response group and the poor response group ( P<0.05). The remaining parameters showed no statistically significant difference ( P>0.05). The ROC curve results showed that the AUCs of the above 8 parameters for evaluating tumor treatment response of nCRT were 0.702, 0.655, 0.695, 0.769, 0.738, 0.807, 0.791, and 0.677, respectively. The AUC of the combined model of the three parameters with AUC>0.75 (λHU VP after nCRT, the change rate of NIC AP and NIC VP) was 0.869, with 80.0% sensitivity and 84.4% specificity. Conclusion:The quantitative parameters derived from spectral CT may provide new markers for evaluating the response to nCRT treatment in patients with LARC. The multi-parameter combined model can improve diagnostic efficacy.

Journal of Army Medical University 2024;46(7):715-724
Objective To explore the effects of HPV16 E6 on genes and signaling pathways in cervical epithelial cells and to screen genes associated with oncogenic transformation. Methods HUCEC models infected with HPV16 E6 were constructed, and transcriptome sequencing was performed to screen for differentially expressed genes(DEGs),which were subjected to Gene Ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment to analyze the differential signaling pathways.RT-qPCR was used to validate major differentially down-regulated expressed genes. After predicting the major differentially expressed proteins by molecular docking analysis, the expression of major differential genes in HUCEC cell model was verified by RT-qPCR and Western blotting. In addition,RT-qPCR and Western blotting were used to further verify the expression of major differential genes in cervical cancer cell lines,SiHa and CaSki.Results A total of 55 genes with more than two-fold differential expression were screened. The results centering on down-regulated genes showed that the negatively regulated differential gene was mainly enriched in redox processes;KEGG enrichment analysis revealed that it was mainly associated with carbohydrate metabolism and cancer.RT-qPCR results showed that the down-regulated differential expression trends of the selected 10 genes were basically consistent with the sequencing results.Molecular docking analysis predicted an interaction between DHRS2 and HPV16 E6,and RT-qPCR and Western blotting confirmed that HPV16 E6 down-regulated DHRS2 mRNA(P<0.01)and protein(P<0.05)and ETV5 protein expression(P<0.01).In SiHa and CaSki cells, compared with the control group, the mRNA and protein expression of DHRS2 was downregulated and positively correlated with the trend of P53 protein expression(P<0.05).Conclusion HPV16 E6 can mediate oncogenic transformation of cervical cells and promote cervical carcinogenesis through downregulating DHRS2 expression.
