Transplantation-associated thrombotic microangiopathy(TA-TMA)is one of the severe complications after hematopoietic stem cell transplantation, and its specific pathogenesis has not yet been fully elucidated. In recent years, with in-depth research on TA-TMA,its pathogenesis has been gradually elucidated. It primarily involves multiple factors, including endothelial injury, aberrant activation of the complement system, and oxidative stress.Regarding treatment,in addition to conventional supportive therapy and etiology-directed management, various targeted therapies have emerged, including drugs like Eculizumab, recombinant thrombomodulin, defibrotide, and N-acetylcysteine.This article aims to systematically review the current research achievements and latest progress on the pathogenesis and treatment methods of TA-TMA,to provide references for the early prevention and treatment of TA-TMA.

International Eye Science 2024;24(7):1168-1172
AIM: To preliminarily assess the efficacy and safety of PAUL glaucoma implant in glaucoma management.METHODS: Clinical data of 10 glaucoma patients(10 eyes)who treated with Paul's glaucoma implant from March 2022 to January 2023 were retrospectively analyzed. A follow-up of 12 mo was performed to observe changes in visual acuity, intraocular pressure, and number of anti-glaucoma medications before and after surgery.RESULTS: Included patients had no significant change in visual acuity from preoperative to last follow-up; preoperative intraocular pressure was 19-60(median 28)mmHg, and it was 10-18(median 14)mmHg at last follow-up. All patients required two to four antiglaucoma medications preoperatively, and only 1 case needed medications at final follow-up. Four patients had a significant decrease in corneal endothelial cell density compared with the preoperative period, and no corneal-related complications occurred. At final follow-up, all 10 patients met surgical success criteria.CONCLUSION: PAUL glaucoma implant has significant IOP lowering efficacy and that corneal endothelial cell damage may be a pitfall of its presence.

Objective To compare and analyze clinical effects of three correction methods in type Ⅲnipple retraction. Methods A total of 93 patients with type Ⅲ nipple retraction were retrospectively enrolled at Clifford Hospital between May 2018 and December 2023.Based on the different surgical methods employed, they were categorized into three groups:group A(silk traction method,n=30),group B(areola double-flap method,n=31),and group C(areola single-flap method,n=32).The study compared the operation time, therapeutic efficacy, hemodynamic disorders, improvement in nipple appearance and function, complications, patient satisfac-tion,and recurrence rates among these three groups. Results The operation duration was significantly longer in group B compared to groups A and C(P<0.05).Group C exhibited a significantly higher total response rate than groups A and B(P<0.05),while no significant difference was observed between groups A and B(P>0.05).There were no significant differences in the incidence of hemodynamic disorders among the three groups(P>0.05).The improvement scores for nipple appearance and function were significantly higher in group C compared to groups A and B(P<0.05),with no significant difference between groups A and B(P>0.05).The incidence of complications was lower, satisfaction was higher, both being statistically significant,in group C compared to groups A and B(P<0.05),but there were no significant differences in the incidence of complications or satisfaction between groups A and B(P>0.05).The recurrence rate was significantly lower in group B and group C than in group A(P<0.05).Conclusion The correction effect of the areola single-flap method is superior to that of the silk trac-tion method and areola double-flap method in patients with type Ⅲ nipple retraction, thereby enhancing clinical efficacy, patient satisfaction, nipple aesthetics, and functionality while reducing complications and recurrence rates.

Objective To investigate the protective effect of total flavonoids of Salvia divinorum extract against acetaminophen(APAP)-induced acute liver injury(ALI)and its molecular mechanism. Methods The main chemical constituents of total flavonoids of Salvia divinorum were obtained through literature search, and their pharmacological mechanisms were predicted using bioinformatics analysis. In a mouse model of APAP-induced ALI,the protective effects of 100,200 and 400 mg/kg total flavonoids of Salvia miltiorrhiza and 150 mg/kg bifidus were evaluated by observing changes in blood biochemistry and liver histopathology and detecting expressions of the key proteins in the Nrf2/HO-1 signaling pathway. Results Network pharmacology analysis suggested that the main active components in total flavonoids of Salvia divinorum for regulating APAP-induced liver injury included quercetin, lignocerol, caruric acid, and kaempferol, for which GO function enrichment analysis yielded 632 GO entries, including 472 involving biological processes,42 involving cellular composition, and 118 involving molecular function.KEGG enrichment analysis showed that the total flavonoids of Salvia divinorum regulated APAP-induced liver injury mainly through ferroptosis-related signaling pathway. In mice with APAP-induced ALI,treatment with the total flavonoids significantly lowered ALT and AST levels, improved liver histopathology and inflammatory cell infiltration, reduced iron deposition in liver tissues, improved lipid peroxidation-related indexes, promoted the expressions of Nrf2,HO-1,SLC7A11,and GPX-4 proteins, and inhibited the expression of keap1 protein. Conclusion The total flavonoids of Salvia divinorum alleviate APAP-induced ALI in mice possibly by suppressing hepatocyte ferroptosis via activating the Nrf2/SLC7A11/GPX-4 signaling pathway.

Objective:To introduce a reliable and easy method of philtrum crest and philtrum repairing in secondary repair of cleft lip. Methods:Lift the upper lip flap after removing the scar on upper lip. Then the superficial orbicularis oris muscle was carefully separated, which formed the abnormal muscular eminence in the affected side after the first cleft lip repair. The pedicle of the orbicularis oris muscle the reconstructing philtrum crest. The lingual orbicularis oris muscle flap was lifted and curled from outside. The superficialorbicularis oris muscle was cut in philtrum longitudinally and turned outward. The two evertedorbicularisoris muscle flaps were sutured on both sides and fixed them on the reconstructing philtrum crest. The exposed deep orbicularis oris muscle in philtrum was sutured with the upper dermis, which formed the prominentphiltrum crest and deeper philtrum. Results:We reviewed the hospital records of patients from July 2008 to November 2021. In total, 201 patients were included in this study. All of these patients underwent the reconstruction of philtrum crest and philtrum with orbicularis oris muscle intertangling and philtrum plasty in secondary cleft lip repairing. During the treatment, the patients had no complications such as infection, hematoma and flap necrosis.198 patients were observed from 1 months to 36 months (7.8 months on average). All of them improved in different degrees in early postoperative period. Most reconstructed philtrum crest was raised and basically symmetrical with the contralateral side in shape. The philtrum was clear and the scar was not obvious. The reconstructed philtrum crest would decrease with time. 91.7% (90/98) patients were satisfied with the postoperative effects after three months. Conclusions:The method of reconstruction of philtrum crest and philtrum with orbicularis oris muscle intertangling and philtrum plasty in secondary cleft lip repairing is safe and effective. The long term postoperative effect is stable with high satisfaction as well. As a result, this method can be used as the philtrum crest and philtrum correction scheme for secondary cleft lip repairing.
