1.Physical circumference development in Chinese children aged 3-6 years

Objective:
To understand the growth and development levels of four physical measurements in children aged 3-6 years in China, so as to provide a reference for child nutrition improvement and health promotion.
Methods:
A stratified random sampling method was used to collect physical measurement data from 120 kindergartens 25 842 children aged 3-6 years across 24 provinces and cities in seven natural geographical regions of North China, Northeast China, East China, Central China, South China, Southwest China and Northwest China from 2020 to 2023. The development levels of head circumference, chest circumference, waist circumference, and hip circumference were evaluated using a grading method. The analysis of gender and age differences was conducted using Mann-Whitney U- test and Kruskal-Wallis test, and the comparison of abnormal detection rates for different genders was conducted using Chi square test.
Results:
The distribution range of children aged 3-6 was 51.74(50.08, 53.33) cm in terms of head circumference, 55.73(52.09, 59.04) cm in terms of chest circumference, 53.04(48.92, 56.40) cm in terms of waist circumference, and 59.36(56.30, 62.32) cm in terms of hip circumference. The detection rate of abnormal head circumference in boys and girls aged 3-6 years old was relatively high (19.71%-42.02%), and the detection rate of abnormal physical circumference development levels in boys was higher than that in girls of all ages ( χ 2=5.63-83.35, P <0.05). The detection rate of abnormal hip circumference (4.89%-6.53%) and chest circumference (4.51%-6.38%) in boys and girls aged 3-6 was relatively low, and there was no statistically significant difference in the abnormal rate between different ages and genders ( χ 2=0.00-1.61, 0.00-3.71, P >0.05). The detection rate of abnormal waist circumference in boys and girls aged 3-6 was relatively high (13.70%-42.45%), and the detection rate of abnormal waist circumference in girls was higher than that in boys aged 4-6 groups ( χ 2=10.49-58.18, P < 0.05).
Conclusions
The overall physical development of children aged 3-6 years in China is improving, but the abnormal detection rates for head circumference and waist circumference are relatively high. Child healthcare should focus on preventing and treating abdominal obesity, with differentiated health intervention strategies based on different age groups and genders.

Yttrium-90 selective internal radiation therapy (90Y-SIRT) is a treatment technique that delivers radioactive microspheres precisely to the arterial vascular bed of neoplasms, utilizing beta radiation to administer a high local dose of radiation to the neoplasm tissues. This technology has demonstrated significant efficacy in patients with unresectable pirmary liver cancers and liver metastases. This article systematically reviews the development history and clinical application status of 90Y-SIRT in the treatment of liver cancer, and looks forward to future development directions.
3.Correlation of Serum miR-21 and miR-23a Levels with Cognitive Function and Inflammatory Response in Patients with Parkinson's Disease

Objective To investigate the expression levels of serum microRNA-21(miR-21)and microRNA-23a(miR-23a)in patients with Parkinson's disease(PD)and their correlations with cognitive function and inflammatory responses.Methods A total of 120 PD patients admitted to the Second Affiliated Hospital of Hebei North University between December 2019 and January 2022 were enrolled,along with 115 healthy controls from the same period.Serum miR-21 and miR-23a levels were measured by quantitative real-time PCR,while serum levels of IL-6,CRP,and TNF-α were determined by ELISA.According to Mini-Mental State Examination(MMSE)scores,PD patients were classified into a cognitive impairment group(MMSE<26,n=72)and a normal cognition group(MMSE≥26,n=48).General characteristics in clinical and biochemical indicators levels were compared between the two groups.Spearman correlation analysis was used to assess the relationships of miRNAs and MMSE scores.Multivariate logistic regression analysis was employed to identify risk factors for cognitive impairment.The predictive value of miR-21 and miR-23a was evaluated using Receiver Operating Characteristic(ROC)curve analysis.Results Serum miR-21,miR-23a,IL-6,CRP,and TNF-α levels were significantly higher in the PD group than in the control group(P<0.01).The cognitive impairment group showed higher levels of miR-21,miR-23a,and inflammatory factor than the cognitively normal group(P<0.01).Correlation analysis revealed that miR-21 and miR-23a levels were negatively correlated with MMSE scores(r=-0.472,-0.514;P<0.001)and positively correlated with IL-6,CRP,and TNF-α(P<0.001).Multivariate Logistic regression analysis revealed that high expression of miR-21,miR-23a,and a higher UPDRS score,were independent risk factors for cognitive impairment in PD patients(P<0.05).Combined detection of miR-21 and miR-23a showed higher predictive accuracy for cognitive impairment than either marker alone(P<0.05).Conclusion Serum expression levels of miR-21 and miR-23a was upregulated in PD patients,which were associated with cognitive function and inflammatory response.Combined detection shows good predictive value for cognitive impairment..
4.The relationship between the expression of TLR2 and TLR4 in peripheral blood mononuclear cells and Th1/Th2 immune response in children with recurrent respiratory tract infection

Objective To investigate the expression of Toll-like receptor 2(TLR2)and Toll-like receptor 4(TLR4)in peripheral blood mononuclear cells(PBMC)in children with recurrent respiratory tract infection(RRTI)and its relationship with T helper cell 1(Th1)/T helper cell 2(Th2)immune response.Methods A total of 65 children diagnosed with RRTI who admitted to the hospital from December 2020 to December 2022 were enrolled in the study as the RRTI group,and 45 healthy children who underwent physical examination in the hospital during the same period were enrolled as the control group.The relative expression levels of TLR2 and TLR4 mRNA in PBMCs were detected by real-time fluorescence quantitative PCR(qPCR).The expres-sion rates of TLR2 and TLR4 protein in PBMCs were detected by flow cytometry.The levels of Th1 cytokine interferon-γ(IFN-γ),Th2 cytokine interleukin-4(IL-4)and their ratio(IFN-γ/IL-4)in plasma were detected by enzyme-linked immunosorbent assay(ELISA).Pearson correlation analysis was used to analyze the corre-lation between TLR2,TLR4 protein expression rates and plasma IFN-γ,IL-4 levels.Results The RRTI group had significantly higher plasma level of Th2 cytokine IL-4 than the control group,significantly lower plasma level of Th1 cytokine IFN-y than the control group,and significantly lower ratio of IFN-γ/IL-4 than the con-trol group,the differences were all statistically significant(P<0.05).The relative expression levels of TLR2 and TLR4 mRNA and protein expression rates in PBMC of children in the RRTI group were higher than those in the control group,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that the protein expression rates of TLR2 and TLR4 in PBMC of children with RRTI were both nega-tively correlated with both plasma IFN-γ levels and IFN-γ/IL-4(P<0.05)and positively correlated with plasma IL-4 levels(P<0.05).Conclusion The expression of TLR2 and TLR4 in PBMC and plasma Th1/Th2 cytokines in children with RRTI may be involved in the occurrence and development of the disease.Ex-cessive activation of TLR2 and TLR4 may weaken Th1 function and enhance Th2 function.
5.Research advances in visualized microspheres

Microspheres are commonly used as embolic materials in vascular interventional operations.However,due to the limitation of materials,almost all microspheres cannot be detected by imaging equipment in vivo.The visualization of microspheres refers to the addition of various materials to the microspheres that enable the microspheres to be displayed on the images of imaging equipment.In order to optimize the embolization process and facilitate postoperative follow-up,a lot of visualized microspheres have been reported so far,such as X-ray visualized microspheres,MRI visualized microspheres,ultrasonic visualized microspheres,etc.Clinical experience has shown that these microspheres can provide true spatial distribution and real-time intraoperative feedback,which contributes to the optimization,personalization,and improvement of vascular embolization technology.This paper aims to make a comprehensive review about the recent advances in researches about the above mentioned visualized microspheres.
6.A case of anti-glomerular basement membrane nephritis complicated with anti-neutrophil cytoplasmic antibody associated vasculitis and IgA nephropathy

Anti-glomerular basement membrane (GBM) nephritis and anti-neutrophil cytoplasmic antibody (ANCA) -associated vasculitis are both autoimmune diseases that can involve multiple systems throughout the body, and both can cause crescent glomerulonephritis. At present, cases of such "double-positive" antibodies have been reported at home and abroad , but the cases of acute progressive glomerulonephritis (RPGN) with IgA nephropathy that are both positive for GBM and ANCA are relatively rare. This case was a middle-aged female patient, whose clinical manifestations were consistent with rapidly progressive glomerulonephritis, positive for anti-GBM antibodies and anti-myeloperoxidase. After renal puncture biopsy, she was diagnosed as anti-GBM nephritis complicated with ANCA associated vasculitis and IgA nephropathy.After treatment with plasma exchange, hemodialysis, glucocorticoids, cyclophosphamide, etc., the condition improved and the patient was discharged. But the patient did not follow the doctor's instructions to take medication and continue hemodialysis treatment after discharge, and the condition worsened, ultimately leading to death.
7.Interventional precision shunting for portal hypertension

Portal hypertension is a pathological elevation of portal pressure gradient(PPG),often leading to severe complications.Interventional shunting such as transjugular intrahepatic portosystemic shunt(TIPS)has gradually become an important means of treatment,but the postoperative hemodynamic changes are uncertain,therefore,individualized treatment scheme with precision shunting is needed.The purpose of precision shunting is to balance the amount of shunting blood with the amount of blood perfusion in the liver tissue,which can minimize the incidence of complications while relieving symptoms.The determination of hemodynamic goals after interventional shunting is influenced by various factors,and the procedural differences and individual variances should be taken into consideration when making reasonable postoperative hemodynamic targets(optimal PPG).The preliminary results of clinical practice of precision shunting indicates that accurate measurement of preoperative portal pressure and effective monitoring of postoperative hemodynamic status should be emphasized.Future studies should further improve the interventional shunting technique to achieve a more individualized precision treatment.
8.Models based on contrast enhanced CT radiomics and imaging genomics for predicting prognosis of ovarian serous cystadenocarcinoma

Objective To explore the value of model established with radiomics features based on contrast enhanced arterial phase CT and model with radiogenomics for predicting prognosis of ovarian serous cystadenocarcinoma(OSC).Methods Enhanced arterial phase CT images of 110 OSC patients were retrospectively collected from 2 centers and The Cancer Imaging Archive(TCIA)database.The radiomics features were extracted,among those related to prognosis were selected to establish a radiomics Cox regression model.Genes data of 399 OSC patients were obtained from The Cancer Genome Atlas(TCGA)database,and genes related to the radiomics features included in the above radiomics model were identified with high Pearson correlation coefficient,and then enrichment gene analyses were performed.For 57 OSC cases with complete enhanced CT and gene data,the hub genes which had the highest connectivity with radiomics prognosis predicting model were detected using Cox regression and protein-protein interaction(PPI).Furthermore,a radiogenomics prognosis predicting model was established with the hub genes.The efficiencies of these 2 models for predicting prognosis of OSC patients were analyzed.Results Finally,the radiomics model included 5 OSC prognosis-related radiomics features,with C-index of 0.782 and 0.735 in corresponding training and test set,respectively.Meanwhile,the radiogenomics model included 30 prognostic hub genes,with C-index of 0.673 and 0.659 in corresponding training and test set,respectively.The survival rates of patients with better predicted prognosis according to radiomics model and radiogenomics model were both higher compared with the others(both P<0.05).Totally 1 135 mRNA genes were found being associated with radiomics model,including biological behaviors such as cell adhesion,and signaling pathways such as PI3K-Akt,extracellular matrix receptor interaction pathway and type 1 diabetes pathway.Conclusion The radiomics model was effective for predicting prognosis of OSC patients.Analysis of mRNA bioinformatics in OSC patients might provide biological interpretations for the radiomics model.
9.Risk factors for lung metastasis of differentiated thyroid cancer

Objective:To investigate the risk factors for lung metastasis from differentiated thyroid cancer (DTC), predict the occurrence of lung metastasis, and diagnose lung metastasis early.Methods:From April 2013 to May 2023, 442 DTC patients (145 males, 297 females; age (41.6±13.1) years) who received 131I treatment in the First Affiliated Hospital of the Air Force Medical University were retrospectively analyzed. All patients were divided into lung metastasis group ( n=124) and non-lung metastasis group ( n=318) according to pathology or clinical, imaging and laboratory test results. Patients were randomly divided into training set ( n=309) and validation set ( n=133) at the ratio of 7∶3. The differences of clinical data between the two groups were compared by χ2 test and Mann-Whitney U test. Factors affecting lung metastasis were analyzed by multivariate logistic regression analysis. ROC curve analysis was used to evaluate the predictive ability of the model. Results:The differences of sex, primary tumor type, multifocal, extra thyroid tissue invasion, number of operations, thyroglobulin (Tg) level before 131I treatment, maximum diameter of primary lesion, lymphocyte absolute value, neutrophil absolute value and B-Raf proto-oncogene, serine/threonine kinase (BRAF) V600E mutation between the two groups were statistically significant ( χ2 values: 7.72-107.77, z values: from -6.50 to -2.44, all P<0.05). Multivariate logistic regression analysis showed that multifocal (odds ratio ( OR)=5.646, 95% CI: 1.763-18.089, P=0.004), BRAF V600E mutation ( OR=0.184, 95% CI: 0.062-0.543, P=0.002), Tg level before 131I treatment ( OR=1.015, 95% CI: 1.004-1.025, P=0.005), lymphocyte absolute value ( OR=0.395, 95% CI: 0.166-0.940, P=0.036) and maximum diameter of primary lesion ( OR=1.932, 95% CI: 1.207-3.093, P=0.006) were independent factors affecting lung metastasis. The AUCs of the training set and validation set obtained by the nomogram prognostic model were 0.899 and 0.889, respectively. Conclusion:Large primary tumor, multiple focus, non-mutated BRAF V600E gene, high Tg level before 131I treatment and low lymphocyte absolute value may be considered as risk factors for lung metastasis of DTC.
10.Differential metabolites of bronchoalveolar lavage fluid from coal worker's pneumoconiosis patients

Background It is a research hotspot to study the changes of metabolites and metabolic pathways in the process of coal worker's pneumoconiosis (CWP) by metabonomics and to explore its pathogenesis. Objective To study the change of metabolites in bronchoalveolar lavage fluid (BALF) of patients with CWP and explore the metabolic regulation mechanism of the disease. Methods Patients with CWP who met the national diagnostic criteria according to Diagnosis of occupational pneumoconiosis (GBZ 70-2015) and underwent massive whole lung lavage were selected as the case group, and patients with tracheostenosis who underwent bronchoscopy were selected as the control group. BALF samples were collected from the cases and the controls. After filtering out large particles and mucus, the supernatant was stored in a −80 ℃ refrigerator. The samples were detected and analyzed by liquid chromatography-mass spectrometry after adding extraction solution, cold bath ultrasonication, and high-speed centrifugation, and the metabolic profiles and related data of CWP patients were obtained. The differential metabolites related to the occurrence and development of CWP were screened by multiple statistical analysis; furthermore, we searched the Kyoto Encyclopedia of Genes and Genomes (KEGG) database for potential metabolic pathways involved in the progression. Results There was no significant difference in the general conditions of the subjects, such as weight, height, age, and length of service among the stage I group, the stage II group, the stage III group, and the control group (P˃0.05). When comparing the CWP stage I group with the control group, 48 differential metabolites were screened out, among which 14 were up-regulated and 34 were down-regulated. A total of 66 differential metabolites were screened out between the patients with CWP stage II and the controls, 14 up-regulated and 52 down-regulated differential metabolites. Compared with the control group, 63 differential metabolites were screened out in the patients with CWP stage III, including 11 up-regulated and 52 down-regulated differential metabolites. There were 36 differential metabolites that may be related to the occurrence of CWP, among which 11 differential metabolites were up-regulated, and 25 were down-regulated. Four significant differential metabolic pathways were identified through KEGG database query: linoleic acid metabolic pathway, alanine metabolic pathway, sphingolipid metabolic pathway, and glycerophospholipid metabolic pathway. Conclusion The metabolomic study of BALF show that there are 36 different metabolites in the occurrence and development of CWP, mainly associating with linoleic acid metabolism, alanine metabolism, sphingolipid metabolism, and glycerophospholipid metabolism pathways.
