1.Research Progress of Anti-lung Cancer Drug-related Interstitial Lung Disease.
Chinese Journal of Lung Cancer 2025;28(4):309-318
Lung cancer is the cancer with the highest incidence and mortality rate worldwide. In addition to the diversified treatment and prolonged lifespan in view of the development of medical technology, the side effect of medicine should not be ignored. Drug-induced interstitial lung disease (DI-ILD) is also commonly encountered during this process, and ILD triggered by the treatment of lung cancer characterized by the inflammation and scarring of lung tissue after the antitumor treatment in lung cancer leads to a poor prognosis and high mortality. The diagnosis and treatment of ILD caused by anti-lung cancer agents remains challenging in clinical settings and requires joint efforts from multidisciplinary team (MDT). This review systematically updates the epidemiology, molecular pathogenesis, genomics/genetics study, diagnosis and treatment of ILD related to anti-lung cancer agents. By the integration of the latest evidences, the paper offers clinical work references for early diagnosis of ILD related to anti-lung cancer agents to enhance the survival and quality of life of the lung cancer patients.

3.Research advances on molecular mechanisms of lung cancer regulated by environmental factors via DNA methylation and clinical translation
Journal of Environmental and Occupational Medicine 2025;42(12):1536-1545
Environmental factors play a pivotal role in the pathogenesis and progression of lung cancer, with epigenetic regulation, particularly DNA methylation, serving as a central molecular mechanism. This review systematically elaborated on the mechanisms by which environmental exposures dynamically regulate lung cancer development through influencing DNA methylation and summarized recent advances in the clinical translation of related epigenetic biomarkers for early diagnosis and targeted therapy. Relevant studies published between December 2018 and March 2025 were retrieved from Chinese and English databases, and 64 articles were included in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Environmental exposures can specifically induce abnormal methylation of tumor suppressor genes such as the aryl hydrocarbon receptor repressor (AHRR) and Ras‑associated domain family 1 isoform A (RASSF1A), thereby promoting genomic instability and activation of oncogenes. In terms of clinical translation, methylation detection of genes including short stature homeobox 2 (SHOX2) and RASSF1A in plasma has demonstrated favorable diagnostic performance, with a sensitivity of 87.0% (95%CI: 80.2%, 91.5%) and a specificity of 98.0% (95%CI: 94.9%, 99.4%), indicating its applicability for early screening of lung cancer. However, although current DNA methyltransferase (DNMT) inhibitors can achieve epigenetic reprogramming, their applications still face challenges related to insufficient selectivity among different tumor subtypes. Given that environmental factors drive lung carcinogenesis via epigenetic mechanisms, there is an urgent need to establish a three‑tier prevention and control framework encompassing “environmental monitoring – methylation biomarker screening – targeted intervention.” Future efforts should focus on deepening integrated multi‑omics analyses, developing subtype‑selective DNMT inhibitors, and optimizing multimodal detection strategies for methylation biomarkers to advance precision prevention and treatment of lung cancer.

Objective:To observe the clinical effect of comprehensive reconstruction of grades Ⅰ-Ⅱ thumb and finger defects with hallux osteo-onychocutaneous flaps of great toe. Methods:This is a retrospective study. From June 2020 to December 2023, comprehensive reconstruction surgery for Grade Ⅰ-Ⅱ digital defect were performed with hallux osteo-onychocutaneous flaps of great toe for 3 thumbs and 7 fingers in 9 patients in the Department of Hand and Microsurgery of Baoji Third Hospital. Causes of digital injury were: 4 of machine crush, 3 of electric saw cut, 1 of door crush, and 1 of electrical burn. There were 6 grade I digital defects (beyond the nail root) and 4 grade Ⅱ defects (last segment of digit). The defects of the digits were reconstructed by taking references of the shape and structure of the contralateral normal thumbs and fingers. Then the hallux osteo-onychocutaneous flaps of great toe were designed and harvested accordingly from the left and right great toes. Donor sites were covered by skin grafting or local dressing change. One patient was treated in emergency surgery, 6 in sub-emergency surgery and 2 in elective surgery. Integrated perioperative patient management was provided to all of the patients. Postoperative follow-ups were conducted through the visit of outpatient clinic, telephone calls or WeChat interviews. Flap survival, appearance and sensation recovery were evaluated according to the Evaluation Standard of Upper Limb Functional of Hand Surgery of Chinese Medical Association. Results:Vascular insufficiency of 1 digit occurred in surgery, and relieved by local treatment with lidocaine and warm saline. All 10 digits successfully survived, and all donor sites healed spontaneously. The postoperative follow-up period was 10 to 30 months, with an average of 18 months. One transferred nail was found in poor appearance (not flat), the rest of the reconstructed digits were good in appearance and function. The nail, finger print and fine sensation (TPD 5~8 mm), as well as digital flexion, extension, grasping and opposition of the reconstructed digits were all good. According to the Evaluation Standard of Upper Limb Functional of Hand Surgery of Chinese Medical Association, at the last follow-up visit, 5 digits were in excellent, 4 in good and 1 in fair. Conclusion:The comprehensive reconstruction of grades Ⅰ-Ⅱ digital defects with hallux osteo-onychocutaneous flap of great toe is an ideal surgical technique that can reconstruct the nail, finger print and sensation of a digit, with good postoperative function as well as an aesthetic and realistic appearance.
