ObjectiveTo explore the effect of Polygonati Odorati Rhizoma polysaccharides on hyperlipidemia in mice by modulating the gut microbiota. MethodsNinety male C57BL/6J mice were randomized into the following groups (n=15): control, model, simvastatin, low- (100 mg·kg-1), medium- (200 mg·kg-1), and high-dose (400 mg·kg-1) Polygonati Odorati Rhizoma polysaccharides groups. Other groups except the control group were fed with a high-fat diet for the modeling of hyperlipidemia, and drug interventions lasted for 12 weeks. Serum levels of total cholesterol (TC), triglycerides (TG), low-density lipoprotein cholesterol (LDL-C), and high-density lipoprotein cholesterol (HDL-C) were measured by an automatic biochemical analyzer. The pathological changes in the liver and epididymal fat were observed by hematoxylin-eosin staining, and lipid accumulation in the liver was assessed by oil red O staining. The gut microbiota was analyzed by 16S rRNA gene sequencing. ResultsCompared with the control group, the model group exhibited an increase in body weight (P<0.01), along with marked elevations in serum levels of TC, TG, and LDL-C (P<0.05,P<0.01). Furthermore, the model group showcased increase in the liver index and epididymal fat coefficient (P<0.05), increased liver fat accumulation, enlargement of adipocytes in the epididymal fat, decreases in both alpha and beta diversity of the gut microbiota, and an increase in the relative abundance of Allobaculum (P<0.01). Compared with the model group, Polygonati Odorati Rhizoma polysaccharides suppressed the increase in body weight (P<0.01), lowered the serum levels of TC, TG, and LDL-C (P<0.05,P<0.01), reduced the liver index and epididymal fat coefficient (P<0.05), alleviated liver fat accumulation, and decreased the size of adipocytes in the epididymal fat. Furthermore, it enhanced the alpha and beta diversity of the gut microbiota in mice, reduced the relative abundance of Allobaculum, Erysipelotrichaceae, and Clostridium (P<0.01), and increased the relative abundance of Akkermansia and Blautia (P<0.01). ConclusionPolygonati Odorati Rhizoma polysaccharides can ameliorate hyperlipidemia induced by a high-fat diet in mice by regulating the diversity and composition of the gut microbiota.

BACKGROUND:
The available evidence regarding the benefits of percutaneous coronary intervention (PCI) on patients receiving dialysis with coronary artery disease (CAD) is limited and inconsistent. This study aimed to evaluate the association between PCI and clinical outcomes as compared with medical therapy alone in patients undergoing dialysis with CAD in China.
METHODS:
This multicenter, retrospective study was conducted in 30 tertiary medical centers across 12 provinces in China from January 2015 to June 2021 to include patients on dialysis with CAD. The primary outcome was major adverse cardiovascular events (MACE), defined as a composite of cardiovascular death, non-fatal myocardial infarction, and non-fatal stroke. Secondary outcomes included all-cause death, the individual components of MACE, and Bleeding Academic Research Consortium criteria types 2, 3, or 5 bleeding. Multivariable Cox proportional hazard models were used to assess the association between PCI and outcomes. Inverse probability of treatment weighting (IPTW) and propensity score matching (PSM) were performed to account for potential between-group differences.
RESULTS:
Of the 1146 patients on dialysis with significant CAD, 821 (71.6%) underwent PCI. After a median follow-up of 23.0 months, PCI was associated with a 43.0% significantly lower risk for MACE (33.9% [ n = 278] vs . 43.7% [ n = 142]; adjusted hazards ratio 0.57, 95% confidence interval 0.45-0.71), along with a slightly increased risk for bleeding outcomes that did not reach statistical significance (11.1% vs . 8.3%; adjusted hazards ratio 1.31, 95% confidence interval, 0.82-2.11). Furthermore, PCI was associated with a significant reduction in all-cause and cardiovascular mortalities. Subgroup analysis did not modify the association of PCI with patient outcomes. These primary findings were consistent across IPTW, PSM, and competing risk analyses.
CONCLUSION
This study indicated that PCI in patients on dialysis with CAD was significantly associated with lower MACE and mortality when comparing with those with medical therapy alone, albeit with a slightly increased risk for bleeding events that did not reach statistical significance.

Triple-negative breast cancer is therapeutically challenging due to the low expression of tumor markers and 'cold' tumor immunosuppressive microenvironment. Here, we present a dual-targeting peptide-drug conjugate (PDC) for tumor inhibition. Our PDC efficiently and selectively delivers cytotoxic Monomethyl Auristatin E (MMAE) into tumor cells via C-X-C chemokine receptor type 4 (CXCR4) and folate receptor 1 (FOLR1) for synergistic inhibition of growth and metastasis. Our results show that the dual-targeting PDC has potent antitumor activity in cultured human cells and several murine transplanted tumor models without apparent toxicity. The combination of dual-targeting PDC and radiotherapy modulates the tumor immunosuppressive microenvironment by increasing CD8+ T cell infiltration and attenuating the proportion of myeloid-derived suppressor and regulatory T cells. Therefore, our dual-targeting PDC represents a promising new strategy for cancer therapy that rebalances the immune system and promotes tumor regression.

Objective:To investigate the effect of the number of positive preoperative serological tumor markers on the surgical approach and prognosis of patients with intrahepatic cholangiocarcinoma. Methods:This is a retrospective case-series study. Data from 548 patients with intrahepatic cholangiocarcinoma after radical resection from October 2010 to April 2019 were retrospectively collected in 10 hospitals of China. There were 277 males and 271 females with an age of (57.8±10.2)years(range:23 to 84 years). Four hundred and twenty-six patients(77.7%) had at least one positive preoperative serum tumor marker. The data collection included the results of 4 preoperative serological tumor markers, other preoperative indicators(5 prodromal symptoms, 6 medical history,8 preoperative serological indicators,5 preoperative imaging indicators, and 14 preoperative pathological examination indicators),baseline data (gender and age),surgical methods, and prognostic follow-up data. Four preoperative results of serologic tumor marker and surgical procedure were converted into categorical variables. The number of positive preoperative serum tumor markers was used as the treatment variable, the surgical method was used as the mediating variable, and the survival time was used as the outcome variable. Univariate and multivariate analysis were used to screen for other preoperative indicators which were independent factors that influenced the surgical procedure and the prognosis of patients as covariates to analyze the mediating effect. Results:Of the 548 patients included in the study, 176 patients (32.1%) underwent partial hepatectomy,151 patients(27.5%) underwent hemihepatectomy, and 221 patients(40.3%) underwent partial hepatectomy or hemihepatectomy combined with other treatments. The results of the univariate and multivariate analysis showed that the number of positive serum tumor markers, intrahepatic bile duct dilatation, portal vein invasion, pathological differentiation, pathological type, vascular invasion,T stage,N stage and maximum tumor diameter were independent factors influencing the surgical procedure(all P<0.05). Intrahepatic bile duct dilatation, pathological differentiation and T stage were independent prognostic factors for patients with intrahepatic cholangiocarcinoma(all P<0.05). Intrahepatic bile duct dilatation, differentiation and T stage were included as covariates in the mediation effect model. The results showed that the number of positive serum tumor markers before surgery had a negative predictive effect on the survival time of patients with intrahepatic cholangiocarcinoma ( β=-0.092, P=0.039),and had a positive predictive effect on the surgical method ( β=0.244, P<0.01). The number of positive serum tumor markers had a negative predictive effect on the survival time of patients with intrahepatic cholangiocarcinoma ( β=-0.151, P=0.002). Direct and indirect effects accounted for 71.3% and 28.7% of total effects, respectively. Conclusions:The higher the positive number of preoperative tumor markers, the worse the prognosis of patients with intrahepatic cholangiocarcinoma. The number of positive cells not only directly affects the prognosis of patients, but also indirectly affects the prognosis of patients by affecting the surgical method.

Objective:To investigate the influencing factors of falls in patients with chemotherapy-induced peripheral neuropathy for gynecological cancer, and establish a Nomogram model based on independent influencing factors. Methods:A total of 216 patients with peripheral neuropathy who received chemotherapy for gynecological cancer in The First Affiliated Hospital of Army Military Medical University of the People′s Liberation Army from February 2021 to April 2023 were retrospectively selected, and were divided into the modeling set and the validation set according to the ratio of 7∶3 using random number table method. According to the occurrence of falls in the modeling set, the patients were divided into the fall group and the non-fall group. Independent influencing factors were obtained through univariate analysis and multivariate Logistic regression analysis, and a prediction model was constructed based on the regression analysis. A Nomogram map was drawn using R language software. Receiver operating characteristic curve (ROC) and calibration curve were used to test the prediction efficiency, and the predictive benefits were evaluated by clinical decision curve. Results:Among the 216 patients, 151 were included in the modeling set and 65 in the validation set. Among the patients in the modeling set, 53 cases were in the fall group, aged (62.89 ± 8.58) years old, and 98 cases were in the non-fall group, aged (59.48 ± 8.12) years old. Chemotherapy cycle, Neuropathy Impairment Score-Lower Limbs, Barthel index, Home Falls and Accidents Screening Tool, regular exercise habits, diabetes and depression were all independent influencing factors for falls in patients with chemotherapy-induced peripheral neuropathy of gynecologic cancer ( OR values were 0.312-3.727, all P<0.05). The area under the curve (AUC) of the established Nomogram model was 0.873, with good discriminating ability. The mean absolute error (MAE) of calibration curve was 0.048, which fitted the ideal curve, indicating that the model has good calibration performance. The AUC of the validation set was 0.873 and the MAE of calibration curve was 0.073, indicating that the model has good external prediction efficiency. Conclusions:This Nomogram model can predict falls in patients with chemotherapy-induced peripheral neuropathy of gynecological cancer and provide evidence for clinical prevention of falls.

BACKGROUND:Minipigs are often used in research fields such as skin injury, vascular trauma and cosmetic medicine because they are highly similar to human beings in terms of skin tissue structure and cardiovascular system.Hydrogel as a wound repair drug possesses a variety of excellent physicochemical properties such as strong water retention and adhesion, which can provide isolation moisturization and drug release for wounds. OBJECTIVE:To summarize and conclude the progress of the application of trauma models for different experimental purposes of hydrogel therapy for minipigs,to reveal the development status of various types of minipig trauma models,to analyze the deficiencies of minipig trauma models at the present stage. METHODS:The relevant articles published in Web of Science database and CNKI database from the establishment of each database to 2023 were checked, using"piglet, miniature pig, minipig, miniature pig;gel, hydrogel;trauma, injury, wound, lesion, incision"as Chinese search terms and"Miniature Swine,Miniature pig, minipig;gel, hydrogel;injury, wound, lesion, incision"as English search terms.A total of 438 Chinese and English documents were retrieved, and 59 documents were included in the study through the inclusion and exclusion criteria. RESULTS AND CONCLUSION:(1)At present, the main models used clinically for trauma repair are large animal species(dogs and pigs),rabbits, and rodents(rats and mice).Because the skin structure of the minipig is more like that of humans, the minipig is the most ideal animal model for trauma repair.(2)In the in-vitro skin injury model, skin defect model is the basic wound model, which can be divided into full skin defect model and medium-thickness skin defect model according to the depth of the wound defect.Burn wound model and infected wound model are multidimensional models with hot metal scald and bacterial culture imposed on the basis of the skin defect model, which have the advantages of high safety coefficient and low operation difficulty.(3)In the in-vivo trauma repair model,mini-pigs are used as esophageal cricothyrotomy model which is more in line with the pathological state of clinical diseases.Mini-pigs are used in the gastric perforation and vascular hemostasis model, which can visually demonstrate the stronger organ adhesion, hemostatic properties and tissue regeneration-promoting effects of the hydrogel.(4)The specific parts of the pig also has the corresponding mode of use:pig ear is usually used to evaluate the hydrogel drug delayed-release effect.Porcine cellular proteins and pig skin collagen are mostly used to prepare composite hydrogels of tissue origin.
