1.A single-center analysis of the short-term efficacy and safety of RAPN in 45 patients with non-metastatic pT 3a renal cell carcinoma

Objective:To analyze the short-term efficacy and safety of robot-assisted laparoscopic partial nephrectomy(RAPN)for non-metastatic pathological stage T 3a renal cell carcinoma. Methods:The clinical and pathological data of 45 patients with pathologically confirmed non-metastatic T 3a renal cell carcinoma who underwent RAPN at Sun Yat-sen University Cancer Center between January 2016 and December 2023 were retrospectively reviewed. There were 30 males and 15 females. The average age of the cohort was(54.3±10.7)years,and the average clinical tumor diameter was(4.9±1.8)cm. Of all the patients,35(77.8%)were asymptomatic,7(15.6%)presented with hematuria,and 3(6.7%)presented with lumbar pain. Preoperative imaging assessed 34 patients(75.6%)as having clinical stage T 3a,all suspected of involving the collecting system or perirenal fat invasion;the remaining 11 patients(24.4%)were assessed as having stage T 1-2 disease. The median R.E.N.A.L. nephrectomy score was 8.0(7.0,10.0). A history of hypertension,diabetes,or chronic kidney disease was present in 18 patients(40.0%). The primary endpoint was progression-free survival,and the secondary endpoints included postoperative complications and short-term renal function outcomes. Survival curve was estimated using the Kaplan-Meier method,and renal function comparisons were made using the paired t-test. Results:The RAPN was performed through a transabdominal approach in 32 patients(71.1%),with a median estimated blood loss of 150.0(50.0,300.0)ml. Seven(15.6%)patients required intraoperative blood transfusion. The median length of postoperative hospital stay was 4.0(4.0,6.0)days. Postoperative complications occurred in 6 patients(13.3%),including 5(11.1%)with mild complications and 1(2.2%)with a severe complication. Renal function returned to baseline in 24 of 39 evaluable patients(61.5%),while 3 patients(7.7%)developed surgery-related chronic kidney disease 3 to 12 months postoperatively,but none required dialysis. The median follow-up time was 31.8(22.7,50.9)months,12(26.7%)patients received programmed cell death protein 1 inhibitor adjuvant therapy postoperatively. During follow-up,3 patients experienced tumor recurrence,the 3-year progression-free survival rate of the entire cohort was 95.4%.Conclusions:For some carefully selected patients with T 3a renal cell carcinoma,RAPN performed by experienced surgeons is a feasible and safe option,providing excellent short-term oncological outcomes,complication control,and renal function recovery. The long-term efficacy remains to be seen.
2.Multivue reconstruction technique during mitral valve transcatheter edge-to-edge repair: A case report

Echocardiography is an important imaging technique in mithal valve transcatheter edge-to-edge repair (TEER). During the operation, mitral leaflets capture and clamping have the highest requirements for ultrasound image quality and should be performed under the guidance of high-quality commissural view. However, standard commissural view cannot be obtained in some patients due to cardiac enlargement, transposition or limited esophageal acoustic window. In this condition, the optimal view can be obtained by Multivue technology. This paper reports a male patient aged 67 years with successful mitral valve TEER under the real-time guidance of Multivue technology, and summarizes the key points of this technology.
3.Renal cell carcinoma with segmental renal vein invasion:diagnostic challenges and short-term outcomes of robot-assisted laparoscopic partial nephrectomy

Objective To evaluate the short-term outcomes of robot-assisted partial nephrectomy(RAPN)in the treatment of renal cell carcinoma(RCC)involving segmental renal vein invasion,and to summarize relevant diagnostic experience and surgical techniques.Methods A retrospective analysis was conducted on the clinicopathological data of 23 RCC patients who were found to have segmental renal vein invasion during RAPN at Sun Yat-sen University Cancer Center during 2022 and 2024.All procedures were performed by the same experienced urologic surgeon(>1000 robotic cases).Preoperative computed tomography(CT)scans of all patients failed to reveal segmental renal vein invasion.Two experienced radiologists re-evaluated the imaging postoperatively.The clinical and pathological data of patients were analyzed to preliminarily explore the short-term efficacy,imaging,intraoperative findings and pathological characteristics.Results All surgeries were completed successfully via the transperitoneal approach with renal preservation.The median operation time was 151.0(125.5,182.0)min,and median blood loss was 180.0(100.0,300.0)mL;2 patients(8.7%)required intraoperative transfusion;3(13.0%)experienced postoperative complications,all of which were minor(Clavien-Dindo grade 1-2).Postoperative pathology confirmed venous invasion in 2 patients.The diagnostic rates of segmental renal vein invasion by the two radiologists were 47.8%(11/23)and 21.7%(5/23),respectively,with concordance in only 3 cases.During a mean follow-up of 9.1 months,1 patient developed iliac lymph node metastasis without local recurrence 12.4 months after surgery.After reoperation to remove the metastatic focus,the patient was treated with Axitinib combined with Toripalimab and has survived to date.The remaining patients were followed up for an average of 8.9 months,with no recurrence or metastasis observed.Conclusion Preoperative detection of segmental renal vein invasion in RCC is challenging,and pathological confirmation is often inconsistent with intraoperative findings.For RCC patients with intraoperatively identified segmental renal vein invasion,RAPN performed by experienced surgeons is feasible and safe on the premise of ensuring complete tumor resection,with favorable short-term oncologic outcomes.Long-term results require further follow-up.
4.Animal study results of a novel designed transcatheter mitral valve replacement system

Objective:To preliminarily assess the biocompatibility and durability of the TruDelta TM transcatheter mitral valve replacement (TMVR) system. Method:Six adult sheep were divided into 3 groups based on the duration of follow-up: 30 days ( n=1), 90 days ( n=3) and 180 days ( n=2). The TruDelta TM TMVR system was implanted through a transapical approach under transesophageal echocardiographic guidance. The operability of the TMVR system was evaluated using an instrument performance evaluation scale (consisting of 39 items), with scores ranging from 1 (worst) to 10 (best) assigned by the operator. Echocardiography was conducted preoperatively, immediately after surgery, and at 30, 90, and 180 days post-implantation. At the last follow-up time point, the intervention mitral valve membrane and major organs were dissected for observation. The artificial valves were taken for hematoxylin eosin (HE) staining and observed under a scanning electron microscope. Result:All six procedures were successfully completed using 29S size TruDelta TM TMVR device. At the final follow-up, echocardiogram demonstrated good valve function without obvious paravalvular leakage, with a transvalvular gradient of (7.8±3.2) mmHg (1 mmHg=0.133 kPa) and a mitral valve orifice area of (1.8±0.2) cm 2. Autopsy findings revealed no structural valve failure and almost complete endothelialization (>75%) with 90 to 180 days. Both HE staining and scanning electron microscopy confirmed optimal endothelialization of the valve stent. Conclusion:The preclinical animal study indicates that the TruDelta TM device exhibits favorable biocompatibility and durability.
5.Transthoracic Echocardiography-guided Double Cavity Permanent Pacemaker Implantation in Pregnant Women:Two Cases Report

X-ray is usually used to determine anatomy and localization during conventional permanent pacemaker implantation.But X-ray exposure might induce radiation injury to fetus.In this paper,we reported 2 cases successful double cavity permanent pacemaker implantation in pregnant women woman at 13 weeks of gestation under the guidance of transthoracic echocardiography.The postoperative pacing parameters were good,the whole process was finished with zero radiation,and there were complications during pregnancy.Both cases resulted in full-term pregnancies and the natural delivery of healthy newborns.Pacemaker electrodes were in normal position as confirmed by post-delivery X-ray examinations.

10.Analysis of pediatric pre-prescription review orders based on PCNE classification system

OBJECTIVE To provide reference for improving the pre-prescription review system and reducing the occurrence of medication error by analyzing the drug-related problems(DRPs)in the pre-prescription review orders of pediatric outpatient clinics using the Pharmaceutical Care Network Europe(PCNE)classification system.METHODS The data of pre-prescription review orders were retrospectively collected from outpatient department of Shanghai Children's Medical Center,Shanghai Jiao Tong University School of Medicine from July 2022 to June 2023;DRPs in the pre-prescription review orders were classified and summarized by using the PCNE classification system(version 9.1),and then analyzed in terms of types and causes of issues,and the acceptance of interventions.RESULTS A total of 66 017 DRPs orders were included,involving 41 165 patients.The proportion of DRPs orders in children aged ≤5 years old was the highest(58.25%),followed by children aged 6-12 years old(33.52%);the department with the highest proportion of DRPs was internal medicine of pediatrics department(71.41%);the department with the highest incidence of DRPs was thoracic surgery department(9.73%);top three drug categories of DRPs orders were systemic anti-infective drugs(25.26%),Chinese patent medicines(24.74%)and respiratory drugs(22.38%).Referring to PCNE classification system,the types of DRPs mainly focused on treatment safety(64.86%);the reasons of DRPs orders mainly focused on dose selection(82.09%),of which 41.26%were due to excessive drug dosage;92.13%of interventions could be accepted and fully executed by doctors.CONCLUSIONS DRPs orders identified by the pre-prescription review system can be effectively analyzed by using PCNE classification system.Pharmacists should focus on medication use in children aged<5 years old,update and develop personalized prescription review rules timely,and meet the rational needs of clinical medication for children.
