1.A qualitative study on economic toxicity perceptions and experiences of caregivers of colorectal cancer patients from the perspective of social ecological systems theory

Objective To explore the economic toxicity perceptions and experiences of caregivers of colorectal cancer patients from the perspective of social ecological systems theory.Methods Using purposive sampling,18 caregivers of colorectal cancer patients hospitalized in the gastrointestinal sur-gery and oncology departments of a tertiary grade A hospital in Yangzhou were selected for semi-struc-tured interviews.The Colaizzi 7-step analysis method was employed to organize and analyze the data.Results Three main themes and nine sub-themes were extracted regarding the economic toxicity expe-riences and needs of caregivers of colorectal cancer patients.Microsystem included multiple negative experiences,impacted individual health,and difficulties in surrogate decision-making;mesosystem in-cluded heavy family financial burden,altered family lifestyle,and tense family atmosphere;macrosys-tem included needs for the scope and intensity of medical insurance reimbursement,needs for medical resources and services,and a desire for social support.Conclusion The ecological system of caregiv-ers of colorectal cancer patients is not optimistic,and is generally affected by economic toxicity.Inter-vention strategies can be sought from multiple aspects,including alleviating the negative experiences of caregivers,strengthening social support,and paying attention to the needs of caregivers,aiming to re-duce the level of economic toxicity among caregivers of colorectal cancer patients.
2.Analysis of literature case reports on hemolytic anemia induced by piperacillin-tazobactam

Objective To discuss the occurrence and characteristics of hemolytic anemia induced by piperacillin-tazobactam to provide a reference for clinically safe drug use.Methods PubMed,Web of Science,CNKI,WanFang Data,and VIP were electronically searched to collect case reports of hemolytic anemia induced by piperacillin-tazobactam from inception to December 2023,and statistical analysis was conducted on patients'basic information,drug use,hemolytic anemia,complications and outcomes.Results A total of 27 patients were included in 25 literature,including 14 males(51.85%)and 13 females(48.15%),aged from 4 days to 77 years old,and the time of first occurrence of hemolytic anemia ranged from 24 h to 18 days.The minimum hemoglobin level of hemolytic anemia was determined in 25 cases,ranging from 27 to 113 g·L-1,of which 14 cases were severe or extremely severe anemia(<60 g·L-1).After drug withdrawal,antibiotic change,and symptomatic supportive treatment,the hemolytic anemia of 25 patients(92.59%)was improved or recovered.Among them,the anemia of 1 patient was improved after stopping piperacillin-tazobactam,but the patient died of multiple organ failure 15 days after drug withdrawal.The other two patients of anemia did not improve,and eventually discharged themselves or died clinically.Conclusion Hemolytic anemia is a rare adverse reaction of piperacillin-tazobactam,and the clinical symptoms have no obvious specificity.Hemoglobin levels should be monitored in clinical use,and once the abnormality is found,if it is clear that piperacillin-tazobactam is the suspicious drug,it should be promptly discontinued,and measures such as red blood cell transfusion should be taken if necessary.

Objective:
To investigate the risk factors of postoperative acute kidney injury (AKI) in patients aged between 40 and 50 years old undergoing cardiac valvular surgery and the impact on outcome.
Methods:
The clinical data of 286 patients aged between 40 and 50 years old undergoing cardiac valve surgery in Guangdong Provincial People′s Hospital from January 2012 to December 2016 were analyzed retrospectively. Preoperative coronary angiography was performed in all patients. All patients enrolled were divided into AKI group and non-AKI group according to the existence or not of postoperative AKI. Patients with AKI were further divided into AKI stage 1, stage 2, and stage 3 groups according to KDIGO guideline. Demographic characteristics, preoperative clinical data including serum creatinine, estimated glomerular filtration rate, hemoglobin, uric acid, urinary protein, presence or absence of chronic kidney disease, left ventricular ejection fraction, pulmonary artery pressure, New York Heart Association (NYHA) functional classification, preoperative co-morbidity (hypertension, diabetes, anemia, cerebrovascular disease, peripheral artery disease), preoperative medication(vasoactive drugs, diuretic, renin-angiotensin system inhibitor (RASI), surgical data (contrast dosage in coronary angiography, type of cardiac valve surgery) were recorded and analyzed in this retrospective study. Objective To analyze the epidemud growth factor receptor(EGFR)mutations in NSCLC patients in Fujian province.Methods Fresh specimens of lung cancer and corresponding normal lung tissue were collected from 50 cases of NSCLC patients.After DNA extraction,nested polymerase chain reaction (nested PCR)and direct deoxyribonucleic acid(DNA)sequencing were used to analyze EGFR gene mutations in NSCLC patients.Results EGFR mutations in tumors were identified from 13 of 50(26%)patients,including 10 cages of in-frame deletion in exon 19 and 3 cases of amino acid substitution in exon 21.Conclusion The mjor type of EGFR mutation in NSCLC patients in Fujian is in-frame deletion in exon 19.
5.Hepatotoxicity of Cyclosponne in Renal Allograft Recipients

Hepatotoxicity of cyclosporine A was investigated in 366 patients who received renal allografts, of whom 94 (25.68%) presented hepatotoxicity and 13 (3.55%) died. Hepatotoxicity appeared between 7 and 84 d, and lasted for 5 to 96 d. Our study suggested that cyclosporine A is not suitable for older patients with positive HBsAg and liver disfunction.
