1.History and prospects of the military hospital preparation rooms

Military hospital preparation rooms are an important part of military medical institutions and have played an important role in military pharmacy support in history. However, with the development of science and technology, the improvement of domestic pharmaceutical production and innovation capabilities, and the adjustment of the military establishment and system, the establishment structure, functional tasks, and business forms of military medical institutions have undergone significant changes. The historical evolution of military preparation rooms were reviewed, the current situation were analyzed and the development challenges faced were identified. It was also explored how military hospital preparation rooms, as an important link in military pharmaceutical support, can face new situations and adapt to new forms of warfare. By enhancing the military efficiency of preparation rooms, it could play a greater role in improving medical support capabilities and enhancing the combat effectiveness of troops.
2.Cyclocarya paliurus Polysaccharide Inhibits Benign Prostatic Hyperplasia by Reducing 5α-Reductase 2

ObjectiveTo investigate the effect and mechanism of polysaccharide in water extract of Cyclocarya paliurus (CPWP) in inhibiting benign prostatic hyperplasia (BPH). MethodsCPWP was obtained by heating reflux, aqueous extraction, alcohol precipitation, and freeze drying. The chemical composition and structural properties of CPWP were analyzed by high performance liquid chromatography with 1-pheny-3-methyl-5-pyrazolone pre-column derivatization and infrared spectroscopy. Male SD rats were randomly assigned into control, model, finasteride (ig 5 mg·kg-1), and low-, medium-, and high-dose (ig 50, 75, 100 mg·kg-1) CPWP groups, with 8 rats in each group. The BPH model was established by subcutaneously injecting propionate testosterone in castrated rats. The rats in the drug intervention groups were administrated with corresponding drugs, and those in the control group were administrated with an equal volume of normal saline each day. After 30 consecutive days, the rats were sacrificed, and the prostate tissue was separated and weighed. The effects of drug interventions on the body weight, prostate wet weight, and prostate index of rats were examined. The prostate tissue was stained with hematoxylin-eosin (HE) for observation of pathological changes. Enzyme-linked immunosorbent assay was employed to measure the level of dihydrotestosterone (DHT), and immunohistochemical staining was used to detect the expression of steroid 5 alpha-reductase 2 (SRD5A2) and Ki67 in the prostate tissue. ResultsCPWP was identified as a saccharide, with characteristic absorption peaks of saccharides. CPWP showed the total sugar content of 44.15% and molecular weight within the range of 5.5-78.8 kDa, being composed of mannose, rhamnose, galacturonic acid, glucose, galactose, xylose, and arabinose. Compared with the control group, the model group had significantly increased prostate wet weight and prostate index (P<0.01), thick and tall prostate epithelial cells, increased internal wrinkles, papillary expansion into the cavity, an elevation in DHT level in the serum, and up-regulated expression of SRD5A2 and Ki67 in the prostate tissue (P<0.05, P<0.01). Compared with the model group, both the finasteride and CPWP groups showed decreases in prostate wet weight and prostate index (P<0.05, P<0.01), thinned prostate epithelial cells, with only a small portion of internal wrinkles and papillary expansion into the cavity, shortened papillary protrusions, lowered DHT level in the serum, and down-regulated expression of SRD5A2 and Ki67 in the prostate tissue (P<0.01). Moreover, CPWP exerted effects in a dose-dependent manner. ConclusionCPWP inhibits BPH by regulating the expression of SRD5A2.
3.A qualitative study on the experience of publishing case reports by resident physicians: challenges, strategies, and expectations

Objective:To investigate the experiences of residents in completing case reports, the support needed in writing case reports, and the necessary conditions for future case report education.Methods:Using purposive sampling, one-on-one semi-structured in-depth interviews were conducted with residents who had published case reports. A thematic analysis was employed to analyze the data.Results:The study included 15 residents [3 males, 12 females; mean age: (27.20±2.57) years], with 8 undergoing standardized residency training and 7 receiving specialized physician training. The study identified three themes, including the challenges, strategies, and desired support during data collection prior to writing the case report, the writing process, and the submission and publication. The following specific needs were identified. Before writing, residents sought access to free and official research resources, guidance on the value of case publication, high-quality case resources, comprehensive supervision of data collection, access to interdepartmental collaboration, and funding support. During the writing period, they sought case report templates, experiences in writing case reports, guidance on article composition, and motivational support. After writing, they expected operational guidance of submission websites, recommendations for target journals, and templates for revision response letters.Conclusions:This study explored the challenges, strategies, and expected support in the continuous stages of before, during, and after writing case reports, thereby initially identifying the essential support conditions for case report instruction from the perspective of resident physicians.
4.Analysis of the metabolic profile in 4-minute low-volume high-intensity intermittent training based on the W'balance model

Objective To evaluate the effectiveness of low-volume high-intensity interval training(LV-HIIT)protocols defined by the W'Balance(W'BAL)model in achieving maximal activation of both anaerobic and aerobic energy systems.Methods Twenty-eight national-level athletes(age:20±1 years old;height:174±9 cm;weight:65.1±9.4 kg)completed an incremental exercise test,followed by six supra-critical power(supra-CP)and five sub-CP constant-load tests to determine VO2max,maximal accumulated oxygen deficit(MAOD),critical power(CP),and W prime(W').They then performed three randomly ordered LV-HIIT protocols(each of 4-minute total duration),with a 10-second inter-val for passive recovery between bouts.The protocols consisted of 10s(HIIT10/10),20s(HIIT20/10),and 30 s(HIIT30/10).Exercise intensities were individually prescribed using the W'BAL model.Accu-mulated oxygen deficit(AOD)and net AOD(NAOD)were calculated for each protocol.Results Final 10-s oxygen uptake(VO2)reached 77%,88%,and 89%of VO2max in HIIT10/10,HIIT20/10,and HIIT30/10,respectively(P<0.05).VO2 in HIIT10/10 was significantly lower than in HIIT20/10 and HIIT30/10(P<0.05),with no significant difference between the latter two(P>0.05).AOD did not differ significantly from MAOD among the three protocols(P>0.05),whereas NAOD was significantly lower than MAOD in all cases(P<0.05).Additionally,NAOD in HIIT10/10 was significantly lower than in HIIT20/10 and HI-IT30/10(P<0.05),with no significant difference between the latter two(P>0.05).Conclusion The HIIT20/10 and HIIT30/10 intensity established by the W'BAL model can substantially elicit maximal activation of both anaerobic and aerobic energy systems.While the W'BAL model demonstrates potential for set-ting exercise intensities in LV-HIIT,future studies are necessary to develop specific W'recovery rate models for targeted populations and to refine CP models that are better suited for intermittent exercises.
5.Genetic analysis of a family with Dentinogenesis imperfecta type Ⅰ caused by a novel mutation in the COL1A2 gene.

OBJECTIVE:
To investigate the clinical phenotype and genetic characteristics of a family with Dentinogenesis imperfecta type Ⅰ(DGI-Ⅰ).
METHODS:
Clinical data were collected from a patient with DGI-Ⅰ admitted to the Reproductive Medicine Department of the Affiliated Hospital of Jining Medical University in March 2024. Clinical and familial data were retrospectively collected. Peripheral blood samples (5 mL each) were obtained from the proband and her family members for genomic DNA extraction, followed by whole-exome sequencing (WES) and Sanger sequencing validation. The pathogenicity of the detected variants was assessed according to the Classification Standards and Guidelines for Genetic Variants formulated by the American Society of Medical Genetics and Genomics (ACMG) (hereinafter referred to as the "ACMG Guidelines"). The study was approved by the Ethics Committee of the Affiliated Hospital of Jining Medical University (Ethics No. 2024-08-C012), and written informed consent for clinical research were obtained from all participants.
RESULTS:
The proband, a 35-year-old female, presented with translucent yellow primary teeth and progressive browning, darkening, and loss of permanent teeth, without skeletal abnormalities. Affected family members exhibited similar phenotypes. Genetic testing revealed a heterozygous COL1A2 variant (c.1503+1G>A) in the patient and other affected members, while unaffected family members all lacked this variant. Based on the ACMG Guidelines, this variant was classified as likely pathogenic (PM4 + PP1_Strong + PM2_Supporting).
CONCLUSION
The COL1A2 c.1503+1G>A heterozygous variant is the disease-causing mutation in this family. Above finding has expanded the mutational spectrum of the COL1A2 gene and provided a basis for genetic counseling and diagnosis in similar cases.

6.Textual analysis of provincial policy on nursing assistant training and management in China

Objective To analyze the deployment of policy instruments and the distribution of stakeholder engagement in provincial policies on nursing assistant training and management.Methods The relevant policy texts on nursing assistant training and management were systematically searched and collected from the official websites of provincial governments and their direct departments,CNKI,and the PKU Law Database.A two-dimensional framework of policy instruments-stakeholder was constructed,and the content analysis was used to classify,encode and quantify policy clauses.Results The study encompassed 20 provincial-level nursing assistant training and management policies,yielding a total of 359 codes.Within the policy instruments dimension,environmental,supply,and demand instruments constituted 66.30%,28.97%,and 4.74%,respectively.In terms of stakeholders,the management-side accounted for 56.55%,providers and trainers for 18.11%each,partners for 5.01%,and demand-side for 2.23%.Both management-side and trainers engaged with 3 policy instruments,providers with 2,and partners and demand-side with one each.Conclusion In the provincial nursing assistant training and management policies,there are differences in the deployment of policy instruments,and the distribution of stakeholders is uneven.Managers should pay attention to publicity guidance and platform construction,improve incentive mechanisms and training programs,and innovate cooperation models with stakeholders,as well as strengthen communication and exchange.
7.Research progress of trifluridine-tipiracil and probiotics in colorectal cancer

Trifluridine-tipiracil(TAS-102),as a novel fluorouracil-based chemotherapeutic agent,effectively overcomes fluorouracil resistance in colorectal cancer(CRC)through its unique pharmacological mechanisms,providing a critical treatment option for advanced CRC patients.Probiotics have shown unique value in CRC prevention and adjuvant therapy by regulating intestinal flora homeostasis and modulating host immune response.It is worth noting that the combination of chemotherapeutic drugs and probiotics not only enhances the anti-tumor activity through synergistic effects,but also reduces the adverse effects caused by chemotherapeutic drugs.Based on this,this paper systematically reviews the pharmacological properties of TAS-102 and describes its synergistic effects in combination with other antitumor drugs.It also summarizes the synergistic effect of fluorouracil drugs combined with probiotics to enhance the effectiveness and reduce the toxicity,and discusses the potential value of the combination of TAS-102 and probiotics,which provides a new research direction for optimizing the precision treatment of CRC and a scientific basis for improving the quality of life of patients.
8.Study on the association between epidemiological characteristics of mental disorders and rural revital-ization:an empirical analysis of a town in Huazhou city,Guangdong province

Objective This study aims to investigate the epidemiological characteristics of mental disorders in a rural town of Huazhou City,Guangdong Province,and explore their association with the rural revitalization strategy,so as to provide evidence for policy formulation.Methods A combination of questionnaire surveys and in-depth interviews was used to collect in-formation on the demographic characteristics,disease status,economic conditions,and policy awareness of registered mental dis-order patients in the town.Semi-structured interviews were also conducted with local doctors and village committee members.Da-ta were analyzed using descriptive statistics and logistic regression models to assess the impact of awareness of rural revitalization policies on patients'income.Results The average age of the study subjects was 47.71 years,with an average disease duration of 18.38 years.A total of 86.36%of patients had a family history of mental disorders.After onset,42.11%of patients had an annual income≥¥ 10 000,which was an increase compared to pre-illness levels,but 60.75%had completely lost their ability to work.Awareness of rural revitalization policies was low(only 18.29%were fully aware),but policy awareness was significant-ly associated with increased patient income(OR=18.03,P<0.001).Additionally,medical expenditures were significantly higher among female patients than male patients(P<0.001).Conclusion In a town in Huazhou City,Guangdong Province,60.75%of mental disorder patients have completely lost their ability to work,while 42.11%have an annual income exceeding ¥ 10,000.Income is positively correlated with policy awareness(OR=18.03).It is recommended to strengthen policy promo-tion,optimize the allocation of medical resources,and develop specialized assistance programs to promote the coordinated devel-opment of mental health and rural revitalization.
9.Research Progress on Targeted Therapy and Resistance Mechanisms of Malignant Melanoma

Malignant melanoma is an extremely aggressive tumor that can be surgically treated in its early stage.In advanced stages,the invasive and proliferative capabilities of melanoma cells continue to increase,and traditional treatment methods such as radiotherapy,chemotherapy,and immunotherapy have limited efficacy,resulting in poor patient prognosis.Due to the presence of mutations in multiple molecular pathways in malignant melanoma,targeted therapy is now regarded as a more viable treatment option for patients with advanced malignant melanoma,with most patients benefiting from it.However,the development of drug resistance to targeted therapy has always been a serious challenge,as the emergence of resistance limits the efficacy.Therefore,it is necessary to explore the mechanism and drug resistance of targeted therapy of malignant melanoma.
