1.Relationship between sarcopenia and cardiovascular disease among middle-aged and older adults with normal weight in China: functional limitation plays a mediating role.

BACKGROUND:
Cardiovascular disease (CVD) is the predominant cause of mortality in China. However, the mechanisms linking sarcopenia to CVD remain poorly understood, particularly in normal-weight populations. Individuals with the absence of overweight or obesity may tend to experience missed opportunities for timely intervention. This study aimed to investigate the longitudinal association between sarcopenia and incidence of new-onset CVD in a normal-weight population, and to examine the mediating effect of functional limitation in this relationship.
METHODS:
We conducted a closed-cohort analysis using a nationwide sample of 4,147 middle-aged and older adults with normal weight in China. We performed Cox proportional hazards regression analysis to explore the associations of baseline sarcopenia with incident CVD. The difference method was applied to estimate the mediation proportion of functional limitation in this association.
RESULTS:
Over a mean follow-up period of 7.62 years, CVD occurred in 835 participants. In the multivariable-adjusted Cox model, individuals with sarcopenia exhibited a significantly higher likelihood of developing incident CVD compared to those without sarcopenia (adjusted hazard ratio [aHR] = 1.45, 95% confidence interval [CI]: 1.21-1.73, P < 0.001). Similar associations were observed for the incidence of heart disease and stroke. Functional limitation accounted for approximately 15.0% of the total effect of sarcopenia on incident CVD (P < 0.001).
CONCLUSIONS
Sarcopenia exerts both direct and indirect effects on incident CVD among middle-aged and older adults who are normal weight, with functional limitation serving as a significant mediator. Interventions targeting both sarcopenia and functional limitation may offer a promising strategy for enhancing cardiovascular health in this population.

2.A cross-lagged analysis of relationship between online psychological needs satisfaction dominance and phubbing among college students

Objective:To explore the dynamic relationship between college students' online psychological needs satisfaction dominance(OPNSD)and phubbing.OPNSD refers to the tendency of individuals to fulfill their basic psychological needs primarily through online environments.Methods:A sample of 340 college students from a university in Beijing participated in two data collections(T1 and T2)at 3-month intervals.OPNSD was calculated as the difference between scores on the Need Satisfaction Perceived Online Scale(NSPOS)and the Basic Need Sat-isfaction in General Scale(BNSGS).Phubbing and its 4 dimensions(self-isolation,nomophobia,interpersonal con-flict,and problem acknowledgment)were measured with the Generic Scale of Phubbing(GSP).A cross-lagged panel model was used to explore the relationship between the online psychological needs satisfaction dominance and phubbing.Results:Significant simultaneous(r=0.60,P<0.001)and successive temporal(r=0.56,P<0.001)correlations were observed between the OPNSD score and GSP score.T1 OPNSD score was positively associated with T2 GSP score in the dimensions of self-isolation,nomophobia,and interpersonal conflict(β=0.14,0.10,0.11;P<0.001 or P<0.05),but showed no association with the problem acknowledgement dimension score(β=0.07,P>0.05).However,the four dimensions of T1 GSP score were not associated with T2 OPNSD score(Ps>0.05).Conclusion:This study suggests that college students' online psychological needs satisfaction dominance positively predicts phubbing,highlighting the critical influence of online environments on mobile phone behaviors.
3.Clinical features and analysis of 10 children with Kawasaki disease shock syndrome combined with macrophage activation syndrome

Objective:Kawasaki disease shock syndrome(KDSS)and macrophage activation syndrome(MAS)are both severe forms of Kawasaki disease. The coexistence of these two critical illnesses is extremely rare,which can be life-threatening in severe cases. The purpose of this study is to summarize the clinical features of children with KDSS complicated with MAS,and provide a basis for precise diagnosis and treatment.Methods:A retrospective analysis was conducted on the clinical manifestations,laboratory tests,imaging characteristics,treatment,and prognosis of 10 children with KDSS and MAS admitted to Beijing Children's Hospital,Capital Medical University from January 2021 to June 2024.Results:Among the 10 children,six were male and four were female,and the age of onset was three months to eleven years old. Acute kidney injury was observed in five patients. Laboratory tests revealed significant increases in serum ferritin,C-reactive protein,B-type natriuretic peptide,aspartate aminotransferase,alanine aminotransferase,creatinine,triglycerides,and interferon-γ,while platelet count and albumin were significantly decreased. Six patients had cardiac enlargement,three had reduced ejection fraction,seven had pericardial effusion,and seven had coronary artery damage. All children were treated with immunoglobulin and methylprednisolone pulse therapy,as well as vasoactive drug infusion to improve circulation and maintain blood pressure. All children were discharged after clinical improvement,and most had a good prognosis.Conclusion:Children with KDSS may develop MAS,which present a complex and rapidly progressing condition often accompanied by a significant increase in ferritin levels. Early diagnosis and treatment can lead to a favorable prognosis.
4.Diagnostic value of combined detection of ascites and serum extracellular vesicle contents for HBV-related primary hepatocellular carcinoma

OBJECTIVE To explore the diagnostic value of combined detection of microRNA(miRNA)and alpha-fetoprotein(AFP),protein induced by vitamin K absence or antagonist-Ⅱ(PIVKA-Ⅱ)in ascites and serum ex-tracellular vesicles(EVs)for hepatitis B virus(HBV)-related primary hepatocellular carcinoma(HCC).METHODS From Nov.2023 to Nov.2024,41 patients with liver cancer and 26 patients with liver cirrhosis who underwent ascites placement or ascites concentration and reinfusion procedures at the Fifth Medical Center of Chi-nese PLA General Hospital were selected as study subjects.Ascites and serum samples were collected.Real-time quantitative reverse transcription polymerase chain reaction(qRT-PCR)was used to detect the expression levels of miR-21,miR-125a,miR-150 and miR-200a in EVs.Chemiluminescence was used to measure the levels of AFP and PIVKA-Ⅱ in ascites,serum and EVs from ascites and serum.An artificial neural network was utilized to con-struct a combined diagnostic model of serum and ascites markers.RESULTS The area under the curve(AUC)for distinguishing HCC from liver cirrhosis using a combination of serum and other indicators was 0.933.The AUC for distinguishing HCC from liver cirrhosis using a combination of ascites and other indicators was 0.912.By screening all detected indicators using an artificial neural network and incorporating indicators with a relative im-portance>0.5 into the diagnostic model,the model included four indicators:ascites AFP,ascites EVs miR-21,ascites EVs miR-200a and serum EVs miR-200a.This model had a sensitivity of 80.77%,a specificity of 87.80%and an AUC of 0.960 for distinguishing HCC from liver cirrhosis patients.CONCLUSION The combined diagnos-tic markers of miRNA,AFP and PIVKA-Ⅱ in ascites and serum-derived EVs have good application value in the diagnosis of HCC.

Creutzfeldt-Jakob disease(CJD)is a rapidly progressive and fatal neurodegenerative disorder caused by prions,with certain infectivity and iatrogenic transmission risks.With the rapid progress and application of new dia-gnostic biomarkers and detection methods,as well as the construction and improvement of surveillance and reporting systems,the detection of CJD in patients domestically and internationally has shown an increasing trend year by year.Due to its long incubation period and heterogeneity of early symptoms,early identification and diagnosis of the disease is difficult,increasing the risk of transmission within medical institutions.Currently,there is a lack of con-sensus on the infection prevention and control of CJD.In order to timely identify and diagnose CJD as well as effec-tively block its transmission in medical institutions,this consensus summarizes 15 clinical concerns and formulates 24 specific recommendations based on the latest domestic and international research findings and clinical evidence,as well as combines with clinical practice,aiming to standardize healthcare-associated infection prevention and control measures for CJD and reduce its transmission risk in medical institutions.

10.Association between baseline hemoglobin level and early neurological deterioration after intravenous thrombolysis in patients with acute ischemic stroke

Objective:To investigate the association between baseline hemoglobin level and early neurologic deterioration(END)after intravenous thrombolysis in patients with acute ischemic stroke(AIS).Methods:Data of AIS patients who received intravenous thrombolytic therapy at multiple hospitals across the country between January 2017 and July 2020 were collected from the online database Acute Stroke Patients for Stroke Management Quality Evaluation(CASE-Ⅱ,NCT04487340).Binary logistic regression analysis was used to study the factors affecting the occurrence of END after intravenous thrombolytic therapy,and the correlation between baseline hemoglobin level and END was investigated by limiting cubic spline curve analysis.Results:A total of 8162 patients were included.Patients with END had lower baseline hemoglobin levels(136 and 140 g/L,P<0.01)and higher rates of anemia(24.2%and 16.9%,P<0.01)compared with non-END patients.Binary logistic regression analysis showed that baseline hemoglobin level(OR=0.995,95%CI:0.991-0.999,P<0.05)and anemia(OR=1.238,95%CI:1.055-1.454,P<0.01)were independently correlated with the occurrence of END after intravenous thrombolysis in AIS patients.Restricted cubic spline regression showed that there was a U-shaped relationship between hemoglobin level and the risk of END after intravenous thrombolysis in AIS patients(P<0.01),although this relationship was only significant in male patients(P<0.05)and not in female patients(P>0.05).Conclusion:There is a correlation between baseline hemoglobin level and the risk of END in AIS patients after intravenous thrombolysis,especially in male patients,in whom both lower and higher hemoglobin level may increase the risk of END.
