1.Correlation between insomnia,gastrointestinal symptoms,and glycated hemoglobin in patients with type 2 diabetes:a cross-sectional study based on the co-management platform of three disciplines of diabetes

Objective To investigate the relationship between insomnia,gastrointestinal symptoms,and glycosylated hemoglobin(HbA1c)levels in individuals diagnosed with type 2 diabetes mellitus(T2DM),as well as the related influencing factors.Methods A total of 910 T2DM patients treated in our multicenter from January 2022 to December 2022 were enrolled in this study.General information(gender,age,smoking and drinking history,exercise,course of disease,treatment and complications),HbA1c,Athens Insomnia Scale(AIS)scores and Gastrointestinal Symptoms Rating Scale(GSRS)scores of patients were collected.The differences of sleep and gastrointestinal symptoms between groups were analyzed,and the correlation between the differences and HbA1c was analyzed.Furthermore,the risk factors for non-standard HbA1c were analyzed.Results The AIS score and GSRS score in the HbA1c control group were less than those in the non-standard group(P<0.01).Insomnia was reported by 37.0%of T2DM patients,and the HbA1c level in the insomnia group was significantly higher than that in the non-insomnia group(10.00%±2.38%vs.8.26%±1.73%,P<0.01).Gastrointestinal symptoms were present in 57.5%of T2DM patients,and the HbA1c levels in the group with gastrointestinal symptoms were significantly higher than those in the group without gastrointestinal symptoms(9.26%±2.23%vs.8.43%±1.98%,P<0.01).Furthermore,26.3%of T2DM patients experienced both insomnia and gastrointestinal symptoms.Remarkably,the HbA1c levels in the group with both insomnia and gastrointestinal symptoms were significantly higher than those in the group without either condition(10.18%±2.44%vs.8.45%±1.86%,P<0.01).Correlation analysis demonstrated a significant association between sleep quality,gastrointestinal function,and HbA1c levels(P<0.01).The logistic regression analysis result revealed that age,GSRS score,AIS score,and the presence of insomnia combined with gastrointestinal symptoms were independent risk factors for predicting HbA1c≥6.5%(P<0.01).Having both insomnia and gastrointestinal symptoms concurrently was the strongest risk factor for substandard HbA1c control,and the risk of blood sugar control may increase about 5 times when both appear together.Conclusion Insomnia and gastrointestinal symptoms are common comorbidities in T2DM patients,showing a cross-interfering relationship,and they appear together with poor blood sugar control,interact causally,and amplify each other.
2.A multi-constraint optimal puncture path planning algorithm for percutaneous interventional radiofrequency thermal fusion of the L5/S1 segments

Objective To minimize variations in treatment outcomes of L5/S1 percutaneous intervertebral radiofrequency thermocoagulation(PIRFT)arising from physician proficiency and achieve precise quantitative risk assessment of the puncture paths.Methods We used a self-developed deep neural network DWT-UNet for automatic segmentation of the magnetic resonance(MR)images of the L5/S1 segments into 7 key structures:L5,S1,Ilium,Disc,N5,Dura mater,and Skin,based on which a needle insertion path planning environment was modeled.Six hard constraints and 6 soft constraints were proposed based on clinical criteria for needle insertion,and the physician's experience was quantified into weights using the analytic hierarchy process and incorporated into the risk function for needle insertion paths to enhance individual case adaptability.By leveraging the proposed skin entry point sampling sub-algorithm and Kambin's triangle projection area sub-algorithm in conjunction with the analytic hierarchy process,and employing various technologies such as ray tracing,CPU multi-threading,and GPU parallel computing,a puncture path was calculated that not only met clinical hard constraints but also optimized the overall soft constraints.Results A surgical team conducted a subjective evaluation of the 21 needle puncture paths planned by the algorithm,and all the paths met the clinical requirements,with 95.24%of them rated excellent or good.Compared with the physician's planning results,the plans generated by the algorithm showed inferior DIlium,DS1,and Depth(P<0.05)but much better DDura,DL5,DN5,and AKambin(P<0.05).In the 21 cases,the planning time of the algorithm averaged 7.97±3.73 s,much shorter than that by the physicians(typically beyond 10 min).Conclusion The multi-constraint optimal puncture path planning algorithm offers an efficient automated solution for PIRFT of the L5/S1 segments with great potentials for clinical application.

4.Prevention and Treatment of Obesity Type 2 Diabetes Mellitus by Traditional Chinese Medicine Through AMPK Signaling Pathway: A Review

Obesity type 2 diabetes mellitus (T2DM) strengthens insulin resistance (IR) and metabolic abnormalities and significantly increases the risk of heart disease, cancer, and other diseases, and it is characterized by IR and malnutrition. As a metabolic regulation center, adenosine phosphate activated protein kinase (AMPK) mainly responds to the changes in intracellular serine/threonine kinase adenosine monophosphate (AMP) levels. After its activation, AMPK converts the cell metabolism mode from synthesis to decomposition to improve energy metabolism and acts on pathological conditions such as inflammation, ischemia, obesity, and aging. In recent years, a large number of studies have found that AMPK is an important target for the treatment of obesity T2DM. Traditional Chinese medicine(TCM) monomers/extracts and TCM formulas mainly affect the mammalian target of rapamycin (mTOR), recombinant sirtuin 1 (SIRT1), nuclear factor erythroid 2-related factor 2 (Nrf2), nuclear factor kappa-B (NF-κB), and other key signaling factors by regulating the AMPK signaling pathway, so as to achieve a variety of effects such as regulating metabolism and autophagy, reducing oxidative stress and inflammatory response, and treating obesity T2DM. It also has advantages such as multiple targets, comprehensiveness, and low toxicity. The regulation of the AMPK pathway by TCM in the prevention and treatment of obesity T2DM has become an important research direction at the present and in the future, but there is no systematic summary and induction in this field. Therefore, this article attempts to summarize the composition and regulatory mechanisms of the AMPK signaling pathway in affecting obesity. It provides a review of the current research status of TCM in regulating the AMPK signaling pathway for the prevention and treatment of obesity T2DM, so as to provide a reference for the diagnosis and treatment of obesity T2DM in TCM and the development of new drugs.

Osteoporosis is an important cause of bone weakness and susceptibility to fractures. Anti-osteoporosis drugs of Western medicine cannot reverse its progression, and can only reduce the loss of bone density; long-term use of them is accompanied by certain adverse reactions. Traditional Chinese medicine focuses on syndrome differentiation and holistic approach, which can make up for the shortcomings of Western medicine’s treatment. The mammalian target of rapamycin (mTOR) signaling pathway is involved in the growth, proliferation, and differentiation of bone cells, and is closely related to the occurrence and development of osteoporosis. In recent years, various traditional Chinese medicine monomers (such as flavonoids, polysaccharides, alkaloids, etc.) and traditional Chinese medicine formulas (such as Bushen huoxue decoction, Liuwei dihuang pills, Erzhi pills, etc.) have been proven to promote bone formation, inhibit bone resorption, enhance bone cell autophagy, and delay the progression of osteoporosis by regulating the mTOR signaling pathway. Therefore, the article summarizes the traditional Chinese medicine monomer and formula that intervene in the mTOR signaling pathway for the treatment of osteoporosis, in order to provide medication ideas for the traditional Chinese medicine treatment of osteoporosis.
6.Traditional Chinese Medicine Monomers in Treatment of Diabetic Retinopathy by Regulating PI3K/Akt Signaling Pathway: A Review

Diabetes retinopathy (DR) is an important cause that threatens the visual health of adults. There are some treatment methods of western medicine with definite efficacy, such as anti-vascular endothelial growth factor and laser photocoagulation, but they have many adverse reactions such as intraocular infection and visual field damage. Traditional Chinese medicine (TCM) therapies are safe and effective, which can complement western medicine. Phosphatidylinositol3-kinase (PI3K)/protein kinase B (Akt) signaling pathway regulates a range of processes including glucose metabolism, cell proliferation, and cell transcription and apoptosis, which is closely related to the occurrence and development of DR. Numerous studies have shown that TCM monomers can participate in maintaining the integrity of blood-retinal barrier and inhibiting retinal neovascularization and neurodegeneration in many aspects such as inhibiting oxidative stress and alleviating inflammatory reaction by regulating the PI3K/Akt pathway, so as to delay the progress of DR. Therefore, this study reviewed PI3K/Akt pathway and its relationship with DR, as well as the TCM monomers in interfering with DR based on PI3K/Akt pathway to provide some ideas for the prevention and treatment of DR in integrated TCM and western medicine.
7.Research progress of active ingredients of traditional Chinese medicine in the intervention of diabetic nephropathy by regulating mTOR signaling pathway

Diabetic nephropathy (DN) has become one of the main causes of end-stage renal disease,and its pathogenesis is still unclear. Currently,it is believed to be closely related to kidney injury mediated by various factors such as autophagy disorder under the condition of high glucose,oxidative stress and inflammation. Mammalian target of rapamycin (mTOR) signaling pathway is crucial for protein synthesis and autophagy regulation,which plays an important role in the occurrence and development of DN. In recent years,the research on the prevention and treatment of DN with traditional Chinese medicine (TCM) has made important progress. Plenty of evidence has shown that the active ingredients of TCM can enhance autophagy,improve oxidative stress and inflammation,inhibit cell apoptosis and abnormal proliferation by regulating mTOR signaling pathway,so as to relieve pathological changes in the kidney such as podocyte injury,glomerular basement membrane thickening,mesangial tissue abnormalities and renal tubule dysfunction,thereby reducing proteinuria and improving renal function. All of the above are of great significance for delaying the progression of DN. This article systematically summarizes the research progress of saponins,flavonoids,polyphenols,alkaloids, terpenoids and other active ingredients of TCM intervening in DN through mTOR signaling pathway,in order to provide some reference for further basic research and the development of new drugs.
8.Selection strategies of hypoglycemic drug for special population with type 2 diabetes mellitus

The special population with type 2 diabetes mellitus(T2DM)are different from the ordinary T 2DM patients in terms of drug treatment due to age ,gender,physiological and pathological status . Based on the latest evidence of evidence -based medical evidence ,this article systematically reviews the selection strategies of hypoglycemic drug in special population such as T2DM patients with complications , elderly patients , children patients , pregnant/lactating women , etc. To formulate an individualized hypoglycemic treatment plan for T 2DM special population ,it is necessary to reasonably select hypoglycemic drugs according to their different characteristics and the latest evidence -based medical evidence ,so as to precisely reduce blood sugar and prevent and delay the occurrence of diabetic complications . For example ,for T 2DM patients with atherosclerotic cardiovascular disease,heart failure ,kidney disease and liver disease ,sodium glucose co -transporter 2 inhibitors or glucagon-like peptide 1 receptor agonists should be used additionally on the basis of metformin ;for T 2DM patients with gastroparesis ,insulin is recommended ;for T2DM patients with hyperuricemia or gout ,hypoglycemic drugs with uric acid lowering effects should be preferred ,and insulin should be used with caution ;for T 2DM patients with osteoporosis ,hypoglycemic drugs that do not affect bone metabolism or have bone protection effects should be selected ;for the elderly patients with T 2DM,it is recommended to give priority to the drugs with low risk of hypoglycemia alone or in combination .
9.Clinical analysis of partial cystectomy combined with chemotherapy in treating localized muscle invasive bladder cancer (a report of 53 cases)

Objective To evaluate the safety and effectiveness of partial cystectomy combined with chemotherapy in treating localized muscle invasive bladder cancer.Methods The patients admitted to localized muscle invasive bladder cancer from October 2011 to December 2015 were screened according to the strict inclusion criteria,and the final 53 patients were enrolled in the study,37 males and 16 females,aged 36 to 85 (63.72 ± 13.31)years.Tumor diameter 1.5-4.0 (2.52 ±0.56)cm,the number of≤ 3 and the edge of the tumor from the ureteral opening > 2 cm.All pathological types are urothelial carcinoma.All patients were examined by MRI clinical stage T2a-T2b period.A total of 53 patients with muscle invasive bladder cancer who refuse or older,merge the heavier medical disease,difficult to tolerate radical cystectomy.Using partial cystectomy,intraoperative electrosurgical excision to deep muscle,parallel to the tumor bed frozen to confirm negative margins,postoperatively systemic chemotherapy of gemcitabine combined cisplatin and bladder perfusion chemotherapy of hydroxycamptothecin.Results Patients were successfully opcrated,and no margin positive or incision planting cases.The operating time is 55-110 min and average (79.11 ± 17.25)min.There was no serious complication in operation and postoperative,pathological diagnosis was invasive epithelial cell carcinoma of the urinary tract,pathology classification:low level 41 cases,and high level 12 cases.Follow up of 12 to 62 months,an average of (27.11 ± 12.22)months,21 cases of recurrence,12 patients had superficial recurrence in the bladder who was treated successfully by transurethral resection of bladder tumor,5 cases of multiple myometrial invasion who was treated by radical cystectomy (3 cases died of bladder cancer after the surgery in 8,12,and 18 months later),4 cases of myometrial invasion with recurrence of pelvic lymph node metastasis or even distant metastasis to give up treatment (all died within six months of bladder cancer).The first tumor recurrence time 3-18 months,11 months and the median time.Conclusions Our data indicate that partial cystectomy combined with chemotherapy is a safe and effective in treating localized muscle invasive bladder cancer,with less trauma,shorter operative time and higher safety,so as to provide an alternative treatment for patients with advanced age,high risk or who can not be implemented transurethral resection of bladder tumor or who refused to undergo radical cystectomy.
10.Clinical nursing of four initial cases of imported yellow fever in China

Objective To observe the clinical symptoms of yellow fever patients,and to explore the related nursing intervention.Methods The clinical data of four initial cases of imported yellow fever was collected. Patient's condition changes were also carefully observed,so as to give them symptomatic support treatment and nursing intervention.Results Symptoms like hyperpyrexia,yellow staining,muscular soreness, dizziness,and bleeding tendency emerged in all of four cases. Two severe cases had severe hepatic and renal failure,and central nervous system damage.Conclusions Yellow fever is an acute disease with clinical manifestations such as fever,liver function damage,bleeding,central nervous system damage. Since there are no effective antiviral drugs for such disease,nurses should pay attention to patients' condition changes,and provide strengthened nursing care and symptomatic treatment. Besides,it is important to enhance public education, vaccination,and individual protection.
