ObjectiveTo explore the target of Erzhiwan in reducing myocardial injury in ovariectomized mice through non-targeted myocardial metabolomics combined with experimental verification. MethodsOvariectomized mouse model was selected, 40 female C57BL/6 mice were randomly divided into sham operation group, model group, estrogen group(estradiol valerate, 1.3×10-4 g·kg-1), Erzhiwan low and high dose groups(3.12, 9.36 g·kg-1), with 8 mice in each group. Each administration group was given the corresponding dose of Erzhiwan by gavage, and the sham operation group and model group were given equal volume of distilled water by gavage for 12 weeks. Echocardiography was used to detect cardiac function, hematoxylin-eosin(HE) staining was used to observe myocardial morphological changes, and enzyme-linked immunosorbent assay(ELISA) was used to detect the levels of estrogen, N-terminal pro-brain natriuretic peptide(NT-proBNP), hypersensitive troponin T(hs-TnT), total cholesterol(TC), triglyceride(TG), low density lipoprotein cholesterol(LDL-C), high density lipoprotein cholesterol(HDL-C), interleukin(IL)-1β, IL-18 and tumor necrosis factor-α(TNF-α). The non-targeted metabolomics of mouse myocardium were analyzed by ultra performance liquid chromatography-quadrupole-electrostatic field orbital trap high-resolution mass spectrometry(UPLC-Q-Exactive Orbitrap MS), and the differential metabolites and corresponding metabolic pathways were obtained. The mRNA expression levels of phosphatidylinositol 3-kinase(PI3K) and protein kinase B(Akt) in mouse myocardial tissues were detected by real-time fluorescence quantitative polymerase chain reaction(Real-time PCR), and the protein expression levels of PI3K, Akt, phosphorylated(p)-Akt were detected by Western blot. ResultsCompared with the sham operation group, the model group showed abnormal cardiac function, increased myocardial fiber space, cardiomyocyte atrophy, sarcoplasmic aggregation, and occasional dissolution or rupture of muscle fiber, the level of estrogen in the serum was decreased, the levels of NT-proBNP, hs-TnT, IL-1β, IL-18, TNF-α, TG, TC and LDL-C were increased, and the level of HDL-C was decreased(P<0.01). Compared with the model group, Erzhiwan could increase the level of estrogen, improve the abnormal cardiac function, reduce the pathological injury of myocardial tissue, decrease the levels of myocardial injury markers(NT-proBNP, hs-TnT) and inflammatory factors(IL-1β, IL-18, TNF-α), decrease the levels of TG, TC, LDL-C, and increased the level of HDL-C(P<0.01). The results of non-targeted myocardial metabolomics showed that 31 of the 162 differential metabolites between the model group and sham operation group were significantly adjusted after administration of Erzhiwan, which were mainly glycerol phospholipid metabolites. Pathway enrichment results showed that Erzhiwan mainly affected glycerophospholipid metabolic pathway, PI3K-Akt pathway, cyclic guanosine monophosphate(cGMP)-protein kinase G(PKG) pathway and other metabolic pathways. Compared with the sham operation group, the levels of phosphatidylcholine(PC, 11 types) and phosphatidylethanolamine(PE, 5 types) in mouse myocardial tissue of the model group were increased(P<0.05, P<0.01), and the mRNA and protein expressions of PI3K and p-Akt were decreased(P<0.05, P<0.01). Compared with the model group, the levels of PC(11 types) and PE(5 types) were decreased(P<0.05, P<0.01) in myocardial tissue of Erzhiwan group, the mRNA and protein expressions of PI3K and p-Akt were elevated(P<0.01). ConclusionErzhiwan can alleviate the pathological injury of myocardium in ovariectomized mice, improve the abnormal cardiac function, improve lipid metabolism disorder, and reduce the levels of myocardial injury markers and inflammatory factors, which involves a number of signaling and metabolic pathways in the heart, among which glycerophospholipid metabolism pathway and PI3K/Akt pathway may have key roles.

Objective To explore the serum placental growth factor (PlGF) and triggering receptors expressed on myeloid cell-1 (TREM1)levels in patients with diabetes foot, and the correlation between them and clinical grading and prognosis of patients. Methods From January 2021 to January 2023,97 patients with diabetes foot who were admitted to Affiliated Hospital of Guilin Medical University were collected as the study object (diabetes foot group),and 102 healthy people who were examined in the hospital were collected as the healthy control group. The patients with diabetic foot were categorized into 33 cases of grade 0~1,39 cases of grade 2~3,and 25 cases of grade 4~5 according to Wanger grading criteria. Patients with diabetes foot were followed up for 6 months and divided into good prognosis group (n=72) and poor prognosis group (n=25).Serum PlGF and TREM1 levels were compared between groups. Spearman correlation analysis was applied to explore the correlation between serum PlGF and TREM1 levels and clinical grading in patients, factors influencing the prognosis of patients with diabetic foot was analyzed by Logistic regression . Receiver operating characteristic (ROC) curve was applied to investigate the predictive effect of serum PlGF and TREM1 on the prognosis of patients with diabetes foot. Results The expression level of serum PlGF in the diabetes foot group was lower than that in the healthy control group (53.86±15.13 pg/ml vs 87.57±18.28 pg/ml),and the level of TREM1 was significantly higher than that in the healthy control group (279.56±32.25 ng/L vs 195.37±26.28 ng/L),and the difference was statistically significance (t=14.133,20.232,all P<0.05).Serum PlGF expression (39.41±13.28 pg/ml vs 66.13±16.30 pg/ml,52.75±15.32 pg/ml) was lower in patients with clinical grades 4~5 than in patients with grades 0~1 and 2~3. TREM1 expression (326.66±35.75 ng/L vs 248.21±29.83 ng/L,275.89±32.06 ng/L) was higher than in patients with grades 0~1 and 2~3. Serum PlGF expression was lower in patients with clinical grades 2~3 than in patients with grades 0~1. TREM1 expression was higher than in patients with grades 0~1,and the differences were statistically significant (q=5.120~12.945,all P<0.05).Spearman correlation analysis, the serum PlGF expression level in diabetes foot patients was negatively correlated with their clinical grading (r=-0.696,P<0.001) and the TREM1 expression level was positively correlated with their clinical grading (r=0.657,P<0.001).Multivariate Logistic regression anaalysis,PlGF was an independent protective factor[OR(95%CI):0.725(0.531~0.990)]and TREM1 was an independent risk factor[OR(95%CI):2.308(1.284~4.147)]affecting the prognosis of diabetic foot patients. The AUC(95%CI) of PlGF,TREM1 levels and their combination to predict the prognosis of diabetes foot patients was 0.898(0.820~0.950),0.817(0.725~0.888)and 0.949 (0.885~0.984),respectively. The clinical efficacy of the combination of the two in predicting the poor prognosis of patients with diabetes foot was better than that of serum PlGF and TREM1 alone (Z=2.150,2.713,P=0.032,0.007). Conclusion The expression of PlGF in serum of patients with diabetes foot is significantly reduced, and the expression of TREM1 is significantly increased. Both of them are closely related to the clinical grading of patients, and they have good clinical efficacy in predicting prognosis.

Objective:To investigate the value of pelvic floor reconstruction with gluteus maximus myocutaneous flap in second-stage surgery for patients with failed perineal wound healing after pelvic exenteration (PE).Methods:This was a descriptive case series study. The clinical data of 24 patients with locally advanced (LARC) or recurrent (LRRC) rectal cancer who underwent PE and had long-term nonunion of postoperative perineal wounds were collected from the department of colorectal surgery of the Second Affiliated Hospital of Navy Medical University (Shanghai Changzheng Hospital) from January 2022 to January 2023. The specific operation methods of pelvic reconstruction by gluteus maximus myocutaneous flap are as follows: the necrotic tissue of the perineal wound was debrided and rinsed repeatedly, the gluteus maximus muscle was cut and separated from the gluteus superior and inferior arteries, the middle muscle pedicle was retained, part of the skin and muscle were separated from the medial margin, part of the epidermis was removed, the muscle and subcutaneous tissue at the medial margin of the flap were fixed to the medial edge of the wound, negative pressure suction tubes were placed above and below the wound cavity and in the muscle space on the right side, and the subcutaneous muscle and fat layer were sutured. The skin was sutured intersegmentally, and a negative pressure suction device was placed on the wound surface. After surgery, the patient should remain prone, and the drainage tube should be placed for at least 7 days. The drainage tube can be removed after 24-hour drainage is less than 30 ml. Perineal wound healing and complications related to gluteal major myocutaneous flap were observed.Result:The median reconstruction time of 24 patients was 180 (150 ~ 230) minutes, and the median intraoperative blood loss was 100 (30 ~ 200) ml. 91.7% (22/24) patients had successful healing of perineal wound within 30 d after operation. After a follow-up of 6 months, no complete or partial flap necrosis occurred. The incidence of complications related to gluteus maximus myocutaneous flap was 8.3% (2/24). One patient had flap infection and sinus tract, and one patient had flap sinus tract. All patients healed after debridement under local anesthesia. Conclusion:For LARC/LRRC patients with poor perineal wound healing after PE, pelvic floor reconstruction with gluteus maximus myocutaneous flap in second-stage operation is safe and feasible, and could successfully close the perineal wound, and has a low incidence of postoperative flap-related complications.

Chinese Journal of Urology 2024;45(3):238-240
The pursuit of cosmetic effects in post-surgical wounds has led to the development of ultra-minimally invasive techniques in surgery. Minimal invasive surgery has replaced open surgery and has become the new gold-standard for treating diseases. One such technique is the single incision triangulated umbilicus surgery (SITUS), which offers several advantages over traditional laparoscopic and other scarless surgeries, including reduced trauma, faster recovery, and better cosmetic outcomes. SITUS also has a short learning curve, aligns with conventional instrumentation operating habits, and can be used for whole abdominal surgeries. Chinese scholars have made further improvements to the SITUS technology, including expanding its applicability in intra-abdominal surgery and refining its incision closure methods to achieve superior cosmetic results. Currently, SITUS technology is experiencing rapid development in urology applications and has demonstrated satisfactory results in both domestic and international reports. This review aims to discuss the effectiveness and development of the SITUS technique in urology.
