Real-time, noninvasive programmed death-ligand 1 (PD-L1) testing using molecular imaging has enhanced our understanding of the immune environments of neoplasms and has served as a guide for immunotherapy. However, the utilization of radiotracers in the imaging of human brain tumors using positron emission tomography/computed tomography (PET/CT) remains limited. This investigation involved the synthesis of [18F]AlF-NOTA-PCP2, which is a novel peptide-based radiolabeled tracer that targets PD-L1, and evaluated its imaging capabilities in orthotopic glioblastoma (GBM) models. Using this tracer, we could noninvasively monitor radiation-induced PD-L1 changes in GBM. [18F]AlF-NOTA-PCP2 exhibited high radiochemical purity (>95%) and stability up to 4 h after synthesis. It demonstrated specific, high-affinity binding to PD-L1 in vitro and in vivo, with a dissociation constant of 0.24 nM. PET/CT imaging, integrated with contrast-enhanced magnetic resonance imaging, revealed significant accumulation of [18F]AlF-NOTA-PCP2 in orthotopic tumors, correlating with blood-brain barrier disruption. After radiotherapy (15 Gy), [18F]AlF-NOTA-PCP2 uptake in tumors increased from 9.51% ± 0.73% to 12.04% ± 1.43%, indicating enhanced PD-L1 expression consistent with immunohistochemistry findings. Fractionated radiation (5 Gy × 3) further amplified PD-L1 upregulation (13.9% ± 1.54% ID/cc) compared with a single dose (11.48% ± 1.05% ID/cc). Taken together, [18F]AlF-NOTA-PCP2 may be a valuable tool for noninvasively monitoring PD-L1 expression in brain tumors after radiotherapy.

Journal of Clinical Medicine in Practice 2025;29(19):73-78
Objective To investigate the biomechanical differences in plantar pressure, postural stability, and plantar Visual Analogue Scale(VAS)scores between normal feet and unilateral/bilat-eral pes cavus, reveal their unique neuromechanical compensation mechanisms, and construct a sta-bility early warning model based on the minimum center of pressure(COP)trajectory classification. Methods A total of 70 patients with pes cavus from December 2023 to October 2024 were selected as study subjects, including 33 patients in the unilateral pes cavus group and 37 patients in the bilat-eral pes cavus group.During the same period,32 normal feet were included as normal foot group.A flat-panel plantar pressure testing system was used to collect dynamic plantar pressure data and COP trajectories from three groups at a self-selected walking speed. There were no statistically significant differences in baseline data such as age, gender, and body mass index among the three groups(P>0.05).One-way analysis of variance and the Wilcoxon rank-sum test were used to compare the differences in maximum pressure, contact area,VAS scores, and the 95%confidence ellipse area of the COP among the three groups in 10 plantar regions. Results Patients with pes cavus exhibited lower peak pressure in the MF region compared to normal feet, while higher peak pressure in the M2,M3,and MH regions.Patients with bilateral pes cavus showed lower peak pressure in the T1 region compared to normal feet, and patients with unilateral pes cavus had lower peak pressure in the LH region compared to the normal group(P<0.05).The plantar contact area in patients with pes cavus was reduced in the T1,M2,M3,M4,MF,and MH regions compared to normal feet(P<0.05).The 95%confidence ellipse area of the COP was larger in both the bilateral and uni-lateral pes cavus groups compared to the normal foot group(P<0.001).Unilateral pes cavus pres-ented a specific lateral COP drift(amplitude of 3 to 4 cm),which is a biomechanical manifestation of compensatory eversion of the unaffected foot.Patients with bilateral pes cavus exhibited a"bimod-al oscillation"trajectory(amplitude of 6 to 8 cm),suggesting possible vestibular-spinal regulatory dysfunction and the poorest postural stability. In the pes cavus group, there was a significant in-crease in pressure in the M2,M3,and MH regions, with peak pressures exceeding 190 kPa in pa-tients with bilateral pes cavus, which was highly correlated with plantar pain and could serve as a pain early warning threshold. Conclusion Unilateral and bilateral pes cavus exhibit significantly different neuromechanical compensation patterns. The classification based on the"lateral drift"and"bimodal oscillation"characteristics of the COP trajectory can serve as a stability early warning indi-cator for assessing fall risk.Decompression interventions targeting the key pressure regions of M2,M3,and MH(such as customized orthotic insoles)are the core strategies for alleviating pain and optimizing dynamic gait stability.

Hyperkalemia is one of the common ion metabolism disorders in clinical practice. Hyperkalemia is defined as serum potassium higher than 5.0 mmol/L according to the guidelines at home and abroad. Acute severe hyperkalemia can cause serious consequences, such as flaccid paralysis, fatal arrhythmia, and even cardiac arrest. The use of renin-angiotensin- aldosterone system inhibitors, β-blockers and diuretics, low-sodium and high-potassium diets, and the presence of related comorbidities increase the occurrence of hyperkalemia. Hyperkalemia risk exist in all clinical departments, but there is a lack of a standardization in the management of multi- department cooperation in hospital. Therefore, a number of domestic nephrology and cardiology department experts have discussed a management model for multi-department cooperation in hyperkalemia, formulating the management standard on hospital evaluation, early warning, diagnosis and treatment, and process. This can promote each department to more effectively participate in nosocomial hyperkalemia diagnosis and treatment, as well as the long-term management of chronic hyperkalemia, improving the quality of hyperkalemia management in hospital.

Acta Universitatis Medicinalis Anhui 2023;58(5):806-811
Objective:
To investigate the effect of QDPR on renal interstitial fibrosis and Beclin1 expression of unilateral ureteral obstruction ( UUO) rat model.
Methods:
The UUO model was constructed by unilateral ureteral ligation. The fibrosis progression , QDPR and Beclin1 expression of UUO model were evaluated by Masson , COL Ⅰ staining and Western blot techniques. UUO model which was over expression QDPR and its control were constructed by retrograde ureteral infusion of corresponding lentivirus. Infection efficiency and QDPR expression level were further evaluated. The QDPR overexpression on fibrosis progression , and Beclin1 was further analyzed.
Results:
Compared with the sham operation group , the collagen fibers level in the UUO model group significantly increased (P <0. 05) , the QDPR expression level was significantly reduced (P < 0. 05) , and the Beclin1 significantly increased (P< 0. 05) . Retrograde injection of lentivirus through the ureter could successfully infect the kidney and increase the expression level of QDPR in the kidney. The overexpression of QDPR in kidney could significantly decrease collagen fibers (P < 0. 05) and Beclin1 (P < 0. 05) expression.
Conclusion
Overexpression of QDPR inhibits the expression level of Beclin1 and progression of fibrosis , suggesting that overexpression of QDPR may relieve renal interstitial fibrosis by inhibiting Beclin1expression.
