Under the background of breaking the " five only" evaluation criteria, continuously optimizing the scientific research performance evaluation system of hospitals to mobilize the enthusiasm and creativity of scientific researchers and guide the direction of scientific research development plays an important role in enhancing the overall scientific research capability of hospitals. Through literature analysis and expert consultation, a certain hospital has constructed a personal scientific research performance evaluation index system for medical staff in tertiary public hospitals, oriented towards innovation quality and member contributions, and began to implement it throughout the hospital in 2021. This index system included four categories of scientific research performance: vertical scientific research projects, academic influence, science and technology awards, and transformation of achievements, with a total of 20 indicators. The annual scientific research performance score of an individual would serve as the basis for the distribution of year-end scientific research performance and an important reference for applying for key and major projects within the hospital. After the application of this evaluation index system, the enthusiasm of medical staff for scientific research has been effectively stimulated. The average individual scientific research performance score increased from 0.974 in 2020 to 1.220 in 2023. All scientific research indicators involved in the evaluation system have shown growth, with a significant increase in high-quality results. This evaluation system can provide a reference for the scientific research performance evaluation of public hospitals under the background of breaking the " five only" evaluation criteria.

Chongqing Medicine 2025;54(10):2357-2360
Objective To evaluate the application of deep learning reconstruction(DLR)in high-resolu-tion T2WI of the uterus and compare it with traditional reconstruction method. Methods A total of 45 pa-tients diagnosed with cervical cancer and undergoing pelvic MRI scans at the hospital from May to August 2024 were prospectively included in the study.DLR technology was used to reconstruct high-resolution T2WI images, which were then compared with high-resolution T2WI images obtained using traditional reconstruc-tion techniques.Likert-type scale was employed for subjective quality evaluation of artifacts and tissue con-trast in high-resolution T2WI images, while relative contrast(RC)between the lesion area and uterine myo-metrium was used for objective quality assessment of the images. Results The artifact score of high-resolu-tion T2WI images obtained using DLR technology showed no significant difference compared to traditional re-construction method(4.22±0.42 vs.4.16±0.37,P=0.18).However,the tissue contrast score was signifi-cantly higher than that of traditional reconstruction methods(4.38±0.49 vs.3.98±0.26,P<0.001).The RC of high-resolution T2WI images obtained using DLR technology was superior to that of traditional recon-struction methods(0.74±0.06 vs.0.71±0.05,P<0.001).Conclusion DLR demonstrates significant ad-vantages in high-resolution uterine T2WI.Although it shows no significant difference in artifact suppression compared to traditional methods,it improves tissue contrast and enhances lesion visualization.

Chinese Journal of Hepatobiliary Surgery 2025;31(10):754-757
Objective:To explore the clinical manifestations, pathology, and prognosis of hepatic mesenchymal hamartoma (MHL).Methods:Clinical data of 10 MHL patients treated at the First Medical Center of Chinese PLA General Hospital from December 2009 to July 2024 were retrospectively analyzed, including seven males and three females, aged (48±23) years. Patients’ age, gender, clinical manifestations, treatment methods, prognosis, and other clinical data were collected. Results:Among them, four cases presented with upper abdominal distension and pain, while the other six showed no abnormal symptoms. All patients had palpable round masses of varying sizes in the upper abdomen. Nine patients had single lesion, among which five were located in the right hepatic lobe, three in the left hepatic lobe, and one in the caudate lobe. One patient had multiple lesions located in both the left and right anterior lobe. The maximum diameter of lesions ranged from 1.5 to 20.0 cm. Seven cases presented with unilocular cystic-solid masses, and three with multilocular cystic-solid masses, with varying sizes of cystic cavities. Microscopically, the tumors exhibited myxoid stroma with spindle or stellate cells, irregular branching bile ducts, residual hepatocyte islands, and variably sized cystic spaces. Four cases with malignant transformation additionally showed undifferentiated mesenchymal cells with atypia, mitotic figures, tumor giant cells, and eosinophilic cytoplasmic inclusions. Immunohistochemistry indicated tumor tissue exhibited positive expression of hepatocyte marker Hepacyte and negative expression of alpha-fetoprotein. The proliferative bile duct epithelium showed positive expression of CK7, CK19, and CK8/18. The spindle cell area expressed vimentin and SMA, with focal expression of desmin, and a Ki67 proliferation index of 5%. In the region of MHL complicated by undifferentiated embryonal sarcoma, there was positive expression of desmin, myogenin, and myoglobin, with a Ki67 proliferation index as high as 30%, and focal expression of S-100, α1-antitrypsin, and CK. A total of seven patients were followed up. Among them, two patients with MHL accompanied by undifferentiated embryonal sarcoma formation experienced tumor recurrence and metastasis after surgery, one patient died after the operation, and the remaining four patients showed no signs of recurrence or metastasis as of the last follow-up.Conclusion:MHL is a rare benign mesenchymal tumor of the liver, which is clinically and radiologically difficult to distinguish from other hepatic tumors. Definitive diagnosis requires percutaneous biopsy or postoperative pathological examination of the tumor. Patients with non-malignant MHL who undergo surgical treatment generally have a favorable prognosis.

Objective:To investigate the feasibility and effectiveness of robot-assisted surgery in children with cholangiectasis under 3 months of age. Methods:This is a retrospective cohort study. The clinical data of 53 children with cholangiectasis under 3 months admitted to Department of General Surgery of Children′s Hospital Affiliated to Zhejiang University School of Medicine from April 2019 to December 2024 were included retrospectively. According to the surgical method, the patients were divided into robot-assisted surgery(RAS) group(5 males, 24 females, age ( M(IQR)) 49.0(36.0)days (range: 19.0 to 90.0 days), weight 5.00(1.65)kg (range: 3.40 to 7.50 kg)) and laparoscopic-assisted surgery(LAS) group (5 males, 19 females, age 55.5(27.3)days (range: 18.0 to 87.0 days), weight 4.90 (1.62)kg (range: 3.50 to 6.20 kg)). Nineteen cases in each group were prenatally diagnosed. Data were statistically analyzed using the Mann-Whitney U test and the Pearson χ 2 test, respectively. Results:No statistically significant differences were observed between the two groups in age, sex, weight, or preoperative biochemical indices(all P>0.05). All procedures were successfully completed without conversion to open surgery. Postoperative biochemical indices, fasting duration, cyst diameter, operative time, drainage tube retention time, and follow-up duration showed no significant differences between the groups(all P>0.05). Compared to the LAS group, the RAS group demonstrated significantly shorter postoperative hospital stays (9.0(5.0)days (range:7.0 to 18.0 days) vs. 11.5(5.5)days (range:7.0 to 38.0 days), U=236.5, P=0.044) and hepaticojejunostomy time (15.0(3.5)minutes (range:11.0 to 22.0 minutes) vs. 18.0(3.0)minutes (range:13.0 to 25.0 minutes), U=144.5, P=0.001). However, the RAS group incurred higher surgical costs (78 099.9(10 100.1)yuan (range: 72 148.7 to 112 898.6 yuan) vs. 30 158.6(15 283.1)yuan (range: 25 041.7 to 107 673.1 yuan), U=41.0, P<0.01). In the LAS group, 3 patients developed anastomotic leakage requiring reoperation, while the RAS group had 1 case of anastomotic stenosis 2 years postoperatively (received second hepaticojejunostomy) and 1 case of incision infection(received incision debridement and dressing of the abscess). Conclusion:Robot-assisted surgery can be used to treat children with cholangiectasis under 3 months of age and may achieve good results.

Objective To observe the temporary loss(re-falling)of knee range of motion(ROM)during rehabilitation after arthroly-sis for post-traumatic knee stiffness, and analyze the factors related to it. Methods From July,2016 to September,2019,64 patients(68 knees)in Beijing Bo'ai Hospital accepted minimally inva-sive arthrolysis for post-traumatic knee stiffness, and were followed up for twelve months.ROM of flexion and extension of knee was measured before operation, and one,six and twelve weeks, and six and twelve months af-ter operation;while they were asssessed with Hospital for Special Surgery Knee Score(HSS).Multivariate Logis-tic regression was performed on re-falling. Results The ROM and HSS score improved as a whole after operation;however,HSS score improved constantly, but ROM decreased six weeks after operation compared with that one weeks after operation, involving 46 knees of 44 cases.Arthrolysis longer than 12 months from primary injuries, multiple complicated fracture and history of infection were the risk factors for re-falling(OR>8.058,P<0.05). Conclusion Minimally invasive arthrolysis is effective on knee function after arthrolysis for post-traumatic knee stiff-ness.However,re-falling of ROM may happen during rehabilitation.Delay of operation, multiple complicated fracture and history of infection may increase the risk of re-falling.
