Objective To investigate the composition ratios and trends of different etiologies after liver biopsy for patients with unexplained liver diseases.Methods A retrospective analysis was performed for the etiology of 960 patients with unexplained liver diseases who were hospitalized and underwent liver biopsy in The Third Affiliated Hospital of Sun Yat-Sen University from January 2011 to December 2020,and the etiologies were categorized by year and age group.The chi-square test was used for comparison of categorical data between multiple groups,and the Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between multiple groups.Results There was a tendency of increase in the overall composition ratio of unexplained liver diseases over the past decade.The leading diagnosis after liver biopsy was autoimmune liver disease(AILD)in 306 patients(31.9%),followed by nonalcoholic fatty liver disease(NAFLD)in 95 patients(9.9%)and drug-induced liver disease(DILI)in 82 patients(8.5%),and there were still 320 patients with undetermined causes(33.3%).There were significant differences in sex ratio and median age distribution between the patients with different etiologies(sex ratio:χ2=155.36,P<0.001;median age distribution:H=182.48,P<0.001).AILD had been the leading etiology in 2011-2020,and there was a tendency of reduction in the composition ratio of AILD(χ2=24.40,P<0.001).NAFLD accounted for the highest proportion of 17.6%in the adolescent stage,while AILD accounted for the highest proportion of 17.8%,47.3%,and 56.3%,respectively,in the young,middle-aged,and elderly stages.Conclusion There is a tendency of increase in the composition ratio of unexplained liver diseases in patients undergoing liver biopsy,with AILD being the main disease diagnosed after liver biopsy,followed by NAFLD and DILI,but one-third of the patients still have an unclear etiological diagnosis.

Objective To identify children with high-risk systemic adverse reactions(SARs)associated with dust mite subcutaneous immunotherapy prior to immunotherapy.Methods An analysis was conducted on the high-risk factors in patients who experienced SARs prior to initial immunotherapy.A total of 40 pediatric patients with dust mite allergic rhinitis who underwent treatment with Dermatophagoides pteronyssinus(Dp)extracts at our hospital's pediatric department from April 2021 to January 2025 and developed SARs were selected as the observa-tion group,while 54 patients who did not experience SARs served as the control group.Differences in gender,age,disease duration,eosinophil count,polysensitization,multisystem involvement,total IgE(tIgE),and dust mite-specific IgE(sIgE)were evaluated.Results Significant differences were observed between the observation and control groups in polysensitization,multisystem involvement,and tIgE levels.The polysensitization rates were 70%vs.48.1%(P=0.034),multisystem involvement 55%vs.22.2%(P=0.001),and high tIgE levels(tIgE≥327.8 IU/mL)82.5%vs.62.1%(P=0.025).Multivariate logistic regression identified multisystem involvement[OR(95%CI)=4.278(1.749～10.461)]as an independent risk factor for SARs during allergen immunotherapy(AIT).Conclusion Patients with dust mite allergic rhinitis accompanied by multi-system damage exhibit poorer conventional prevention efficacy against SARs when undergoing AIT.They remain highly susceptible to SARs occur-rence,necessitating more robust preventive measures against SARs to enhance the safety of AIT.

Objective To investigate the role and molecular mechanisms of bladder cancer-derived exosomal long non-coding RNA(lncRNA)CIAT1 in mediating collective invasion and to evaluate its clinical significance and potential therapeutic value.Methods High-throughput sequencing was used to identify lncRNAs that are highly expressed in exosomes from bladder cancer and lymph node metastatic tissues.CIAT1 was selected for further validation in clinical bladder cancer samples.By constructing CIAT1-overexpressing and knockdown bladder cancer cells,we demonstrated in vitro that CIAT1-contained exosomes target cancer-associated fibroblasts(CAFs)to induce collective invasion.The underlying mechanism of CIAT1 in bladder cancer collective invasion was explored through RNA pull-down,RNA immunoprecipitation(RIP),dual-luciferase reporter assays,chromatin isolation by RNA purification(ChIRP)and chromatin immunoprecipitation(ChIP).Results CIAT1 was significantly upregulated in exosomes derived from bladder cancer tissues compared to adjacent normal tissues by High-throughput sequencing(fold change>1.5,P<0.05)and clinical sample validation(P<0.01).In vitro experiments,exosomal CIAT1 was selectively internalized by cancer-associated fibroblasts(CAFs),significantly enhancing collective invasion of bladder cancer via regulating CAFs.In co-culture models,CIAT1 overexpression group showed increased total number and total length of collective invasion chains compared to the control group(P<0.01 for both).Mechanistically,CIAT1 was packaged into exosomes via binding to hnRNPA2B1,and internalized by CAFs,where it activated N-cadherin transcription by modulating H3K4me3 histone modification at the N-cadherin promoter.Consistently,the CIAT1 overexpression group exhibited elevated collective invasion chain numbers and lengths compared to the control group(P<0.01 for both).However,blocking N-cadherin reversed the pro-invasive effects of CIAT1,with no significant differences in chain numbers or lengths between the CIAT1 overexpression+N-cadherin blockade group and controls(P>0.05 for both).Further clinical correlation analysis confirmed that CIAT1-regulated N-cadherin is closely associated with collective invasion in bladder cancer patients(P<0.01).Conclusions Exosomal CIAT1 derived from bladder cancer cells targets CAFs to activate N-cadherin transcription,thereby promoting bladder cancer collective invasion.

Objective:To investigate the prognostic and immunotherapeutic significance of androgen receptor V1 (ARV1) in colorectal cancer (CRC) and to explore its mechanism in CRC progression.Methods:The relationship between ARV1 expression and CRC prognosis was analyzed using data from The Cancer Genome Atlas Program (TCGA). Gene ontology (GO) analysis was performed to identify potential mechanisms through which ARV1 regulates CRC progression. Multiple public databases were used to analyze the correlation between ARV1 and immune cell infiltration and to predict the sensitivity of ARV1 to immunotherapy and chemotherapeutic drugs. Immunohistochemical validation was conducted using postoperative specimens from 199 CRC patients, and clinical correlations were analyzed.Results:ARV1 expression was significantly lower in CRC tissues compared to normal tissues ( P<0.001). Patients with high ARV1 expression exhibited better overall survival than those with low expression ( P=0.016). Clinical analysis indicated that ARV1 serves as an independent prognostic factor in CRC, and its expression was associated with age and clinical stage (all P<0.01). GO analysis revealed that ARV1 influenced multiple pathways in CRC. Immune-related analysis demonstrated that ARV1 participated in regulating immune cell infiltration in CRC. Drug sensitivity analysis showed differences in responses to various chemotherapeutic agents between high and low ARV1 expression groups ( P<0.01). In immunotherapy, significant differences in Immunophenotype Score (IPS) were observed between high and low ARV1 expression groups in PD1-negative/CTLA4-negative patients ( P<0.05). Immunohistochemical results from 199 CRC patients confirmed that low ARV1 expression was associated with poorer prognosis ( P<0.001). Conclusions:ARV1 expression affects CRC prognosis and may serve as a potential novel biomarker for immunotherapy in CRC.

Rehabilitation is an effective measure to improve the quality of life for cancer patients,and strong rehabilitation motivation is an important influencing factor for patients' compliance with rehabilitation.This paper mainly reviews the concepts,assessment tools,influencing factors,and intervention strategies of rehabilitation motivation in cancer patients,with the aim of providing references for medical staff to improve the level of rehabilitation compliance in cancer patients and develop targeted rehabilitation management strategies.

Objective To systematically evaluate the rehabilitation effect of immersive virtual reality technology for patients with peripheral vestibular dysfunction,and to provide theoretical basis and practical guidance for the design of efficient rehabilitation training programs.Methods PubMed,Embase,Cochrane Library,Web of Science,CINAHL,CNKI,Wanfang Database,VIP Database,and China Biomedical Literature Database were searched for randomized controlled trials on the intervention effect of immersive virtual reality technology in patients with peripheral vestibular dysfunction from inception to February 2025.RevMan5.4 software was used to perform meta-analysis of the literature that met the quality standards.Results A total of 10 papers with 491 patients with peripheral vestibular dysfunction were included.Meta-analysis showed that immersive virtual reality technology improved vertigo symptoms in patients with peripheral vestibular dysfunction compared with conventional vestibular rehabilitation care[SMD=-1.41,95％CI(-1.58,-0.70),P＜0.001],and subgroup analysis showed that the frequency of intervention was＜5 times/week[SMD=-1.31,95％CI=(-1.96,-0.66),P＜0.001],single intervention duration＜30 min[SMD=-1.45,95％CI=(-2.21,-0.70),P＜0.001],and intervention period≤7 weeks[SMD=-1.49,95％CI(-2.04,-0.94),P＜0.001]were more significant.Immersive virtual reality technology reduced the risk of falls[MD=4.66,95％CI(3.84,5.48),P＜0.001],and improved anxiety and depression[SMD=2.65,95％CI(1.98,3.33),P＜0.001].Conclusion Immer-sive virtual reality technology improves vertigo symptoms,reduces the risk of falls,and improves anxiety and depres-sion symptoms in patients with peripheral vestibular dysfunction,in which the intervention period of ≤7 weeks,＜5 training sessions per week,and each training session of＜30 min are better for improving vertigo symptoms in patients with peripheral vestibular dysfunction.
