This paper aims to contribute to guaranteeing the stable development and enhancing the understanding of ecological agriculture of Chinese materia medica so that the national strategy and industrial demand can be better served. It first introduces current traditional Chinese medicine(TCM)policy and industrial development status from five aspects, including policy guarantee, theoretical support, technological innovation, standardization system, and brand influence. Then, the paper analyzes the development dilemma of TCM agriculture in production and quality increase and ecological environment protection. It also proposes the development goals of ecological agriculture of Chinese materia medica that meet the current industrial development demand, which are reducing chemical fertilizers, pesticides, and carbon emissions, improving quality, increasing efficiency, and protecting ecological environment. In addition, the new development goals are interpreted through case studies. Finally, this paper proposes four development strategies for ecological agriculture of Chinese materia medica: conducting research on the pattern and spatial and temporal variations of nationwide TCM production areas; studying the internal and external ecological memories of medicinal plant growth from the perspectives of genetic variations and environmental adaptation variations and elucidating their contributions to the formation of quality; carrying out selection and breeding of stress-resistant varieties for ecological agriculture of Chinese materia medica, the optimization of key technologies for soil improvement and restoration and green prevention and control against diseases and pests, and the improvement of quality; carrying out research on the quality assurance and value realization of ecological products made from TCM. This research can provide guidance for policy formulation, theoretical development of the discipline, and the enhancement of industrial technology for ecological agriculture of Chinese materia medica.

Hepatocellular carcinoma (HCC) is a leading cause of cancer-associated death globally. Liver transplantation (LT) has emerged as a key treatment for patients with HCC, and the Milan criteria have been adopted as the cornerstone of the selection policy. To allow more patients to benefit from LT, a number of expanded criteria have been proposed, many of which use radiologic morphological characteristics with larger and more tumors as surrogates to predict outcomes. Other groups developed indices incorporating biological variables and dynamic markers of response to locoregional treatment. These expanded selection criteria achieved satisfactory results with limited liver supplies. In addition, a number of prognostic models have been developed using clinicopathological characteristics, imaging radiomics features, genetic data, and advanced techniques such as artificial intelligence. These models could improve prognostic estimation, establish surveillance strategies, and bolster long-term outcomes in patients with HCC. In this study, we reviewed the latest findings and achievements regarding the selection criteria and post-transplant prognostic models for LT in patients with HCC.

Objective:To construct a Logistic regression model based on the energy spectral computed tomography(CT)parameters and clinical parameters characteristics, and to evaluate its diagnostic efficacy in the benign and malignant pulmonary ground glass nodules(GGN).Methods:162 GGN patients who were admitted to our hospital from April 2022 to March 2024 were selected, they were divided into benign group and malignant group according to pathological results. The difference of CT parameters[water content in normal scan period, water content in arterial period, slope of energy spectral curve in normal scan period and k value in arterial period]and clinical data between the two groups was compared.Multivariate Logistic regression model was used to analyze the independent influencing factors of malignant GGN.Receiver operating characteristic(ROC)curve was used to evaluate the diagnostic efficacy of the model for malignant GGN.Results:Water content in normal scan period in malignant group was significantly higher than that in benign group(P<0.05).k value in normal scan period in the malignant group was significantly higher than that in the benign group(P<0.05).Water content in arterial period in malignant group was higher than that in benign group(P<0.05).k value in arterial period in the malignant group was higher than in benign group(P<0.05).There were significant differences in smoking history, family history of tumor and chronic obstructive pulmonary disease(COPD)between benign group and malignant group(P<0.05).Multivariate Logistic regression model analysis showed that, water content in normal scan period,k value in normal scan period,k value in arterial period, water content in arterial period, smoking history and family history of tumor were independent influencing factors of malignant nodule in GGN patients(P<0.05).Based on the results of multi-factor Logistic regression analysis, the Logistic regression prediction model was constructed:logit(P)=ln(P/l-P)=0.015× water content in arterial period+1.214× smoking history+1.506× family history of tumor+0.013× water content in normal scan period 0.553× k value in normal scan period+0.202 × k value in arterial period.ROC curve results showed that the area under the curve(AUC)of the combined prediction model was 0.852,which was significantly higher than 0.654,0.607,0.628,0.759,0.707,0.682 of water content in normal scan period,k value in normal scan period,k value in arterial period, water content in arterial period, smoking history and family history of tumor. Conclusion:The Logistic regression model constructed based on the characteristics of energy spectral CT parameters and clinical parameters characteristics has good diagnostic efficacy for benign and malignant GGN.

Objective To evaluate the direct economic burden caused by surgical site infection(SSI)in patients with orthopaedic trauma under the payment management of disease diagnosis-related groups(DRG).Methods Clinical data of patients with orthopaedic trauma in a tertiary first-class hospital from May 1,2022 to May 30,2023 were surveyed retrospectively.Patients were grouped based on whether SSI occurred.Differences in average length of hospital stay, average hospitalization expense, and other indicators between SSI patients and non-SSI patients in the same DRG subgroup were compared, and the direct economic burden caused by SSI was analyzed. Results A total of 435 patients who paid according to the DRG payment management were included in the study.Twenty-two pa-tients had SSI,with an SSI incidence of 5.06%.Both the average length of hospital stay and average hospitalization expense of patients in the SSI group were higher than those in the non-SSI group, with statistically significant differ-ences(P<0.05).The DRG subgroups of SSI patients were mainly four groups:IF45,IF15,IJ13,and ZC13.Among them, the average length of hospital stay of SSI patients in the IF45,IF15,and ZC13 groups increased sig-nificantly(P<0.05),and the average hospitalization expense of SSI patients in the IJ13 group increased significantly(P<0.05).Conclusion Under the DRG payment management, the direct economic burden of orthopaedic trauma patients with SSI increases significantly. It is necessary to periodically evaluate and identify high-risk DRG subgroup patients,so as to adopt precise infection control interventions and reduce SSI incidence.

This paper introduced the overview of the "eight trends" of Chinese medicinal materials(CMM) industry in 2021, analyzed the problems of CMM production, and put forward development suggestions. Specifically, "eight trends" could be summarized as follows.(1) The growing area of CMM tended to be stable, and some provinces began to release the local catalog of Dao-di herbs.(2) The protection process of new varieties accelerated, and a number of excellent varieties were bred.(3) The theory of ecological cultivation was further enriched, and the demonstration effect of ecological cultivation technology was prominent.(4) Some CMM realized complete mechanization and formed typical model cases.(5) The number of cultivation bases using the traceability platform increased, and provincial internet trading platforms were set up.(6) The construction of CMM industrial clusters accelerated, and the number of provincial-level regional brands increased rapidly.(7) Many new agricultural business entities were founded nationwide, and a variety of methods were used to drive the intensified development of CMM.(8) A number of local TCM laws were promulgated, and the management regulation of food and medicine homology substances catalogs was issued. On this basis, four suggestions for CMM production were proposed.(1) It is suggested to speed up the formulation of the national catalog of Dao-di herbs and carry out the certification of Dao-di herbs production bases.(2) Ecological planting of forest and grassland medicine should be further strengthened in terms of technical research and promotion based on the principle of ecological priority.(3) The basic work of disaster prevention should be paid more attention and technical measures for disaster mitigation should be developed.(4) The planted area of commonly used CMM should be incorporated into the national regular statistical system.
