1.Pattern Identification and Treatment of Constipation-Predominant Irritable Bowel Syndrome Based on the Turbidity Toxin Theory

Guided by the turbidity toxin theory, it is believed that the key pathogenesis of constipation-predominant irritable bowel syndrome is the obstruction of turbidity toxin and the disruption of intestinal function. Treatment is based on the principles of dispelling turbidity toxin and promoting intestinal function. The clinical patterns can be divided into three types, turbidity toxin heat accumulation pattern, turbidity toxin combined with liver depression and qi stagnation pattern, and turbidity toxin combined with qi and yin deficiency pattern. The treatment can respectively use self-prescribed Tongfu Jiangzhuo Formula (通腑降浊方) to clear heat and unblock the bowels, direct the turbid downward and resolve toxins; use self-prescribed Shugan Jiangzhuo Formula (疏肝降浊方) to soothe the liver and move qi, direct the turbid downward and resolve toxins; use self-prescribed Mazhi Jiangzhuo Formula (麻枳降浊方) to boost qi and nourish yin, moisten the intestines to remove turbidity and resolve toxins.
2.Evaluation of health education and health promotion effect on coal-burning pollution-induced endemic fluorosis in Ankang City, Shaanxi Province from 2020 to 2023

Objective:To evaluate the effectiveness of health education and health promotion for coal-burning pollution-induced endemic fluorosis (coal-burning fluorosis) in Ankang City, Shaanxi Province, and provide reference for further developing health education and health promotion measures.Methods:From 2020 to 2023, one county (district) was selected as the project county in coal-burning fluorosis area of Ankang City each year, and 3 to 5 towns of each project county were selected as the project town. Health education and health promotion activities on coal-burning fluorosis were carried out for department heads, endemic disease propaganda workers of project county, and students in grades 4 to 6 from center primary schools, local residents and migrant workers of all affected villages of project county. Before and after the implementation of health education and health promotion projects, 30 to 45 primary school students and 30 local residents from each project town were selected to conduct a questionnaire survey on the prevention and control knowledge of coal-burning fluorosis every year, and a household survey was conducted to investigate the prevalence and changes of specific behaviors of local residents who open their stoves.Results:From 2020 to 2023, the coverage rate of intervention activities for department heads, endemic disease propaganda workers, and primary school students in the project county was 100%; the coverage rate of intervention activities for local residents in the project county was 94.44% (18 271/19 346), and for migrant workers was 97.86% (8 683/8 873). The awareness rate of prevention and control knowledge of coal-burning fluorosis among primary school students and local residents after health education and health promotion in each year was significantly higher than those before health education and health promotion ( P < 0.001), and showed an increasing trend year by year (χ 2trend = 36.73, 38.47, P < 0.001). From 2020 to 2023, a total of 420 local residents were surveyed, and 110 people used open stoves. The prevalence rate of specific behaviors was 26.19%, and it showed a decreasing trend year by year (χ 2trend = 0.29, P < 0.001). After health education and health promotion, the number of people who changed their behavior during a certain period was 106, and the specific behavior change rate was 96.36% (106/110). Conclusions:The health education and health promotion on coal-burning fluorosis in Ankang City has achieved significant results. In the future, we should continue to increase investment in health education and health promotion, continuously improve the long-term mechanism of endemic disease health education and health promotion, innovate health education models, further improve the effectiveness of health promotion, and consolidate the achievements of prevention and control.

AIM To establish a GC-MS/MS method for the analysis of volatile components in Yinhu Ganmao Powder,and to determine the contents of α-pinene,camphene,sabinene,β-pinene,α-terpinene,(+)-limonene,p-cymene,1,8-cineole,linalool,L-menthol,terpinen-4-ol,DL-menthol,α-terpineol,tridecane,pulegone,caryophyllene,humulene,n-hexadecane and patchouli alcohol.METHODS The analysis was performed on a DB-624 UI capillary column(30 m×0.25 mm×1.40 μm ),and electron ionization source was adopted with multiple reaction monitoring mode.RESULTS Fifty volatile components and twenty-five liposoluble components were identified in volatile oils and medicinal material powder,respectively.Nineteen constituents showed good linear relationships within their own ranges(r ≥ 0.999 0),whose average recoveries were 84.43%-113.31%with the RSDs of less than 9.15%.CONCLUSION This stable,accurate and reproducible method can provide a reference for the quality evaluation of Yinhu Ganmao Powder.
5.Clinical effects of Supplemented Buyang Huanwu Decoction on postoperative patients with lumbar vertebral fracture complicated with spinal cord injury due to Qi Deficiency and Blood Stasis Pattern

AIM To explore the clinical effects of Supplemented Buyang Huanwu Decoction on postoperative patients with lumbar vertebral fracture complicated with spinal cord injury due to Qi Deficiency and Blood Stasis Pattern.METHODS One hundred and twenty patients were randomly assigned into control group(60 cases)for 6-week intervention of conventional treatment,and observation group(60 cases)for 6-week intervention of both Supplemented Buyang Huanwu Decoction and conventional treatment.The changes in clinical effects,TCM syndrome scores,spinal cord conduction signals(SEP amplitude,MEP amplitude),serum neurotrophic factors(NGF,IGF-1,BDNF),coagulation and inflammatory indices(PT,APTT,TNF-α,IL-1 β)and incidence of adverse reactions were detected.RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05).After the treatment,the two groups displayed decreased TCM syndrome scores,TNF-α,IL-1β(P<0.05),increased spinal cord conduction signals,coagulation and inflammatory indices(P<0.05),and shortened PT,APTT(P<0.05),especially for the observation group(P<0.05).No significant difference in incidence of adverse reactions was found between the two groups(P>0.05).CONCLUSION For the patients with lumbar vertebral fracture complicated with spinal cord injury due to Qi Deficiency and Blood Stasis Pattern,Supplemented Buyang Huanwu Decoction can safely and effectively promote neurological function recovery.
6.Effect of fasting blood glucose on coronary fractional flow reserve in patients with borderline coronary artery disease

Objective To explore the correlation between fasting blood glucose(FBG)level and fractional flow reserve(FFR)in patients with borderline coronary artery disease,and to clarify its potential influence on FFR measurement.Methods From August 2020 to August 2023,the data of 135 patients with coronary atherosclerotic heart disease who received coronary angiography and FFR evaluation in the Fourth Affiliated Hospital of Harbin Medical University were retrospectively collected.According to the exclusion and inclusion criteria,85 cases of borderline diseased vessels of single coronary artery with stenosis degree of 50%-80%were screened out,and they were divided into FBG≥6.1 mmol/L group(47 cases)and FBG<6.1 mmol/L group(38 cases).The baseline data,angiographic and functional indexes of the two groups were compared,and the correlation between FBG and FFR was analyzed.Results Compared with the FBG<6.1 mmol/L group,the FBG≥6.1 mmol/L group had a higher proportion of FFR negative results(72.3%vs.23.7%,P<0.001),and the FFR measurement values were generally increased[0.84(0.80,0.90)vs.0.75(0.68,0.80),P<0.001],with statistically significant differences.Pearson correlation analysis was performed on all lesions,and FFR>0.80(negative result)was positively correlated with FBG≥6.1 mmol/L(r=0.484,P<0.001).Conclusions Among the patients with borderline coronary artery disease(50%-80%stenosis)included in this study,FBG≥6.1 mmol/L is significantly correlated with FFR>0.80.For patients with borderline coronary lesions with elevated FBG,the influence of blood glucose factors should be carefully considered in clinical interpretation of FFR results.
7.Regulatory effect of neutrophils in microglial polarization after permanent ischemic stroke

Objective To investigate the effects of peripheral blood neutrophil infiltration on the polarization regulation of cerebral resident microglia under a permanent ischemic stroke model.Methods Fifty-eight C57BL/6 mice were divided into two groups.One group was sham group,and the other group of mice was subjected to permanent middle cerebral artery occlusion surgery.Mice were euthanized 48 hours,7 days,14 days,and 30 days after surgery for tissue collection.Western blotting was used to detect expression levels of M1 microglia markers CD 16,M2 microglia marker arginase 1(Arg1),inflammatory cytokine interleukin-1 β(IL-1β),and neutrophil marker myeloperoxidase(MPO)in brain tissue.Immunofluorescence histochemical staining was used to assess neutrophil infiltration and M2 microglial distribution around the infarct area in brain sections.In vitro,purified neutrophils were co-cultured with BV2 microglial cells.After lipopolysaccharide stimulation,the phagocytosis of neutrophils by BV2 cells was observed,and the expression levels of CD16 and Arg1 proteins in BV2 cells were detected.Results Western blotting showed that the levels of CD16(P<0.05),IL-1β(P<0.001),and MPO(P<0.05)in brain tissue increased significantly 48 hours and 7 days after surgery,then decreased,with MPO expression returning to normal levels 30 days after surgery.Immunofluorescence showed a significant increase of MPO-positive cells around the infarct area of the mouse cerebral cortex 48 hours after surgery(P<0.001),followed by a decrease(P<0.05).The number of ionized calcium binding adapter molecule 1(Iba1)and MPO double-positive cells gradually increased after surgery,and reached their peak at 14 days(P<0.05).Iba1 and Arg1 double-positive cells also increased significantly 7 days(P<0.05)and 14 days(P<0.01)after surgery.In vitro,co-culture experiments showed that after BV2 phagocytosing neutrophils,CD 16(P<0.05)significantly decreased and Arg1 significantly upregulated(P<0.05).Conclusion In a permanent ischemic stroke model,microglia transition from M1 to M2 type after phagocytosing neutrophils,and the injured brain area changes from pro-inflammatory state to anti-inflammatory state.
8.A meta-analysis of platelet-rich plasma for treatment of diabetic foot ulcers

Objective To evaluate the clinical efficacy of platelet-rich plasma(PRP)in treat-ment of diabetic foot ulcers(DFU)using a meta-analysis approach.Methods Relevant literature on PRP for treatment of DFU was collected by searching databases including PubMed,Cochrane Library,Embase,Web of Science,NEJM,CNKI,Wanfang Data,VIP Database,and China Biology Medicine disc.Relevant literature on PRP for treatment of DFU was collected.The literature was read,and effect sizes were extracted.The extracted data were then subjected to a meta-analysis using RevMan 5.3 software.Results A total of 6 studies were included in this meta-analysis,involving 440 pa-tients.Among them,there were 208 patients in experimental group(PRP combined with conventional treatment)and 232 patients in control group(conventional treatment,blank control,or placebo treat-ment).The ulcer healing efficacy rate in the experimental group was higher than that in the control group(OR=1.29,95%CI,1.19 to 1.40,P<0.05).The incidence of adverse reactions in the experimental group was lower than that in the control group(OR=0.33,95%CI,0.12 to 0.93,P=0.94).The healing time(MD=-14.37,95%CI,-23.12 to-5.62,P=0.001),thickness of granulation tissue on the ulcer surface(MD=1.60,95%CI,1.31-1.88,P<0.000 01),coverage rate of granulation tissue on ulcer surface(MD=6.03,95%CI,3.79 to 8.26,P<0.05),and level of vascular endothelial growth factor on the ulcer surface(MD=7.62,95%CI,1.57 to 13.67,P=0.01)in the experimental group were all superior to those in the control group.Con-clusion PRP treatment for DFU has the advantages of improving the ulcer healing efficacy rate,shortening the ulcer healing time,promoting the growth of granulation tissue on the ulcer surface,and reducing the incidence of adverse reactions.
9.Evolution of gut microbiota and cognitive dysfunction from 2015 to 2025:a bibliometric and knowledge mapping analysis

Objective To analyze the development trends,research status,and hotspots in the field of gut microbiota and cognitive dysfunction from 2015 to 2025,providing a more comprehensive reference for researchers in this field and exploring cutting-edge research directions.Methods Liter-ature in relevant research fields was retrieved from the WoSCC database.Bibliometric and visualiza-tion analyses were conducted using VOSviewer,CiteSpace,ScimangoGraphica,and the R package"bibliometrix".Results A total of 665 eligible English articles were retrieved from the WoSCC data-base.The analysis revealed that the number of publications has shown an increasing trend annually since 2018.China and the United States had the leading roles globally in terms of the number of pub-lications and collaborative relationships.Huazhong University of Science and Technology are the most influential research institution in this field,with NUTRIENTS(IF=4.8)being the journal with the highest number of publications,and KIM D H ranking first in outstanding contributions and influ-ence.Keyword co-occurrence analysis indicated that"neuronal apoptosis","SCFAs","synaptic dysfunction",and"dementia prevention"were the research hotspots in this field.Furthermore,trend analysis revealed that short-chain fatty acids(SCFAs),important metabolites of gut microbiota,insulin resistance,and neuroinflammation were the research frontiers in this field,highlighting insulin resistance as a mediating hub in microbiota-cognition research.Additionally,traditional Chinese medicine interventions(such as electroacupuncture)were identified as an emerging characteristic direction in this field.Conclusion Through bibliometric research,the current status and develop-ment trends of gut microbiota and cognitive dysfunction research are analyzed from multiple perspec-tives in the paper.Comprehensive insights into the cutting-edge dynamics of this research field are provided,offering valuable references for further in-depth research by more scholars in this area.
10.Robot-assisted laparoscopic enucleation of endogenic renal sinus tumors

Removal of endogenic renal sinus tumors which are located at the renal hilum,is recognized as one of the most challenging surgeries in urology due to the complex anatomical structure.With the help of the high-definition field of vision and flexible robotic arms of the Da Vinci robot system,our team have carried out precision intra-sinus tumor enucleation based on preoperative interactive qualitative and Interactive Quantitative Quality Assurance for 3D Imaging(IQQA-3D)accurate reconstruction technology,as well as the clinical anatomical characteristics of patients with renal sinus tumors.During operation,the renal pedicle is fully dissected,the perihilar fat is cleared,and the tumor capsule is exposed.Subsequently,the blood supply to the kidney is blocked,and the tumor is precisely excised along the tumor capsule.The vascular stumps are sutured point-to-point,and the wound is closed and locked.After the blood supply restores,regional arterial occlusion is performed if necessary to ensure the safety of the surgery and maximize the preservation of renal function.This article discusses the definition of tumor boundaries during robot-assisted endogenic sinus tumor enucleation,the use of intraoperative cooling techniques,and the improvement of surgical safety.
