1.Comparative analysis of the characteristics of imported malaria cases in Nanning City in 2024 and the same period of the previous year

Objective To investigate the epidemiological characteristics of malaria and provide a basis for developing improved prevention and control measures. Methods Data were obtained from the Chinese Disease Prevention and Control Information System. Malaria surveillance data for Nanning City from January 1,2023, to December 31,2024, were exported from the Infectious Disease Reporting Information Management Subsystem. The characteristics of the two groups of malaria cases were compared. Results A total of 103 imported malaria cases were reported in Nanning City in 2024, representing a 38.32% decrease compared with the same period of the previous year. No statistically significant difference were observed between cases reported in 2023 and 2024 in terms of average age, gender ratio, proportion of parasite species, and monthly reporting distribution;however, statistically significant differences were found in the proportion of reporting areas and current residence areas(χ2= 13.572 and 10.355, respectively; P = 0.001 and 0.035, respectively). The proportion of cases reported in Shanglin County and the proportion of cases residing in Shanglin County were both lower than those during the same period of the previous year. Conclusions The high aggregation of imported malaria cases in Nanning City has decreased. Medical institutions in areas other than Shanglin County should strengthen their vigilance against malaria.
2.Analysis of the correlation between medication regimen complexity and glycemic control among type 2 diabetes mellitus in the elderly

Objective To explore the correlation between glycemic control and complexity of drug treatment regimens in elderly patients with type 2 diabetes mellitus.Methods A retrospective,cross-sectional study was conducted to collect the elderly patients with type 2 diabetes mellitus who were hospitalized in the Department of Endocrinology at the Geriatric Hospital of Nanjing Medical University from June 2022 to December 2024.The clinical characteristics and medication compliance of the patients were recorded through pharmaceutical consultations.The medication regimen complexity index(MRCI)and diabetes-specific medication regimen complexity index(D-MRCI)were calculated for each patient.Patients were divided into the target-achieving group and the non-target-achieving group based on whether their blood glucose met the criteria.Univariate analysis and multivariate Logistic regression analysis were used to identify the factors associated with adequate blood glucose control.Results A total of 152 patients were included,123(80.9%)had poorer glycemic control,31.6%exhibited good medication adherence,and 76.3%demonstrated good adherence to diabetes medications.The mean total MRCI score was(25.90±8.63),while the median D-MRCI score was 9.00(5.25,11.75),with 44.7%of patients classified as having high D-MRCI scores(D-MRCI>9).The results of Spearman correlation analysis indicated that the number of medications used by patients was highly correlated with the MRCI score(r=0.899,P<0.001),while the number of diabetes medications used was highly correlated with the D-MRCI score(r=0.705,P<0.001).Univariate analysis results revealed that high complexity diabetes medication regimen,duration of diabetes,the number of types of antidiabetic drugs,and the number of injectable antidiabetic medications were associated with glycemic control(P≤0.05).Multivariate analysis results showed that,high diabetes medication complexity was an independent risk factor for inadequate blood glucose control[OR=4.119,95%CI(1.121,15.139),P=0.033].Conclusion Higher D-MRCI(>9)was associated with poorer glycemic control.Simplifying medication regimens and optimizing medication management strategies may improve patient outcomes.
3.Distribution of pathogens and changes in serum HBD3 and Flt3L levels in patients with concurrent pulmonary infections after esophageal cancer radical surgery

Objective To investigate the distribution of pathogens and changes in serum humanβ-defendin-3(HBD3)and FMS like tyrosine kinase 3 ligand(Flt3L)levels in patients with pulmonary infection after esophageal cancer radical surgery.Methods Prospectively,patients who underwent radical resection of esophageal cancer and developed pulmonary infection from January 2022 to January 2024 were selected as the infection group(75 cases),and patients who underwent radical resection of esophageal cancer and did not develop pulmonary infection were selected as the non-infection group(93 cases).Patients in the infection group were divided into the mild group(25 cases),the moderate group(39 cases),and the severe group(11 cases)based on the CURB-65 score.The distribution of pathogenic bacteria in patients of the infection group was analyzed by the fully automatic microbial identification instrument.The expression levels of HBD3 and Flt3L in serum were detected by enzyme-linked immunosorbent assay(ELISA).Multivariate Logistic regression was applied to analyze the risk factors for postoperative pulmonary infection in esophageal cancer patients.ROC curve was applied to analyze the diagnostic value of HBD3 and Flt3L levels for postoperative pulmonary infection in esophageal cancer patients.Results Among 75 infected patients,90 strains of pathogens were detected,including 49 strains(54.44%)of Gram negative bacteria,29 strains(32.22%)of Gram positive bacteria,and 12 strains(13.33%)of fungi.The serum HBD3 and Flt3L levels in the infection group were greatly higher than those in the non infection group(P<0.05).The serum HBD3 and Flt3L levels in the severe group were higher than those in the moderate group and mild group(P<0.05),while the serum HBD3 and Flt3L levels in the moderate group were higher than those in the mild group(P<0.05).Multivariate Logistic regression analysis showed that HBD3,Flt3 L,tumor location in the upper/middle segment,intraoperative bleeding ≥ 500 ml,diabetes,and smoking history were all factors influencing the pulmonary infection after radical resection of esophageal cancer(P<0.05).According to the ROC curve,the AUC value for diagnosing postoperative pulmonary infection in esophageal cancer patients with serum HBD3 level alone was 0.789.The AUC value for diagnosing postoperative pulmonary infection in esophageal cancer patients with serum Flt3L level alone was 0.863,the AUC value of the combined diagnosis of the two was 0.934,which was greatly higher than that of the individual diagnosis(Zcombination vs HBD3=3.723,Zcombination vs Flt3L=2.098,P<0.05).Conclusion The serum HBD3 and Flt3L levels in patients with pulmonary infection after esophageal cancer radical surgery are highly expressed,and the serum HBD3 and Flt3L levels are correlated with the severity of pulmonary infection.The two are risk factors for postoperative pulmonary infection in esophageal cancer patients after radical surgery,and their combination can effectively diagnose postoperative pulmonary infection in esophageal cancer patients.
4.Lidocaine Intravenous Anesthesia Assists in Enhanced Recovery after Surgery of Patients Undergoing Pituitary Tumor Resection Surgery

Objective:To explore the application effect of continuous intravenous infusion of lidocaine on patients undergoing transnasal pituitary tumor resection.Methods:Ninety patients undergoing transsphenoidal pituitary tumor resection under general anesthesia from May 2022 to January 2025 were selected.The participants were randomly divided into two groups:a control group and a lidocaine group,with 45 patients in each group.Before starting anesthesia,the lidocaine group received intravenous injection of 1.5 mg/kg lidocaine,followed by continuous intravenous injection at a rate of 2 mg/(kg·h)throughout the entire surgical process.The control group received an equal amount of physiological saline.The other anesthesia regimens and drugs for the two groups of patients were consistent,and all anesthesia related drugs were stopped from infusion at the end of the surgery.Compare the intraoperative mean arterial pressure(MAP)and heart rate changes between two groups,evaluate visual analog scale(VAS)scores at different time points,postoperative opioid dosage and recovery level,and incidence of adverse reactions.Results:There was a certain degree of fluctuation in MAP and heart rate in both groups during the operation,with the lidocaine group showing a smaller level of fluctuation.There was no significant difference in MAP and heart rate levels between the two groups at T1 and T6 times(P>0.05),while the MAP and heart rate levels in the lidocaine group were higher than those in the control group at T2,T3,T4,and T5 times(P<0.05);Compared with the control group,there was no statistically significant difference in VAS scores between the two groups at T0 time(P>0.05).The VAS scores of the two groups showed an upward trend at T1,T2,T3,and T4 time,and a downward trend at T5 time.However,the VAS scores of the lidocaine group were lower than those of the control group at T1,T2,T3,T4,and T5 time(P<0.05);The dose of remifentanil administered via intravenous pump during the lidocaine group,the duration of first postoperative ventilation,and the length of hospital stay were all lower than those in the control group(P<0.05);The incidence of nausea,vomiting,and coughing related adverse reactions in the lidocaine group was lower than that in the control group(P<0.05).Conclusion:Lidocaine assisted intravenous anesthesia can promote stable hemodynamic fluctuations during pituitary adenoma resection surgery,reduce postoperative pain,decrease opioid dosage,shorten hospital stay,and reduce adverse reactions such as coughing,nausea,and vomiting.
5.Sodium lactate modulates TLR4/NF-κB signaling pathway for treatment of right heart failure

Aim To investigate the effects of sodium lactate(NALA)on right heart failure induced by monocrotaline(MCT)-induced pulmonary arterial hy-pertension in rats and to reveal the underlying mecha-nisms.Methods Forty male Sprague-Dawley(SD)rats were randomly allocated into four groups,with ten rats in each group,namely,MCT group,NALA group,and NALA+MCT group;the MCT and NALA+MCT groups were administered a single intraperito-neal injection of MCT at 60 mg·kg-1 to induce pul-monary hypertension,and one week later,the NALA and NALA+MCT groups received intraperitoneal in-jections of NALA at 0.1 g·kg-1(once a day,for 5 weeks),while the CON and MCT groups received e-qual volumes of physiological saline(once a day,for 5 weeks);right heart function was assessed using echo-cardiography,right ventricular and pulmonary artery remodeling were evaluated via histopathological sec-tions,and the expression levels of ANP,BNP,and in-flammatory factors were measured by ELISA,along with assessments of oxidative stress levels,Western blot detection of the expression levels of proteins in the TLR4/NF-κB signaling pathway.Results Compared to the CON group,the MCT group exhibited increased RVSP and RVHI,decreased right heart function,in-creased collagen fiber deposition,and elevated oxida-tive stress and inflammatory factor expression,and the expression levels of proteins in the TLR4/NF-κB signa-ling pathway increased(P<0.05);compared to the MCT group,the NALA+MCT group showed reduced RVSP and RVHI,improved right heart function,atten-uated pulmonary vascular remodeling,decreased ex-pression of ANP,BNP,inflammatory factors,and H2O2,along with increased antioxidant enzyme expres-sion,and the expression levels of proteins in the TLR4/NF-κB signaling pathway decreased(P<0.05).Conclusion NALA can inhibit right ventric-ular remodeling in rats with pulmonary hypertension,and the underlying mechanism may involve the allevia-tion of inflammatory responses and oxidative stress through the inhibition of the TLR4/NF-κB signaling pathway.
6.The value of total volume response and total mass response in the therapeutic evaluation of lung metastasis of hepatocarcinoma

Objective To analyze the correlation between lesion volume,lesion mass,and maximum lesion diameter in the assessment of advanced hepatocarcinoma with lung metastasis,and to evaluate the application value of total volume response and total mass response of lung metastatic lesions in efficacy assessment.Methods A retrospective analysis was conducted on the CT imaging data of 20 patients clinically confirmed with hepatocarcinoma and lung metastases,followed by subsequent follow-up to monitor their survival outcomes.Volume measurement software was used to measure the volume of lesions before and after treatment.We recored lesion diameter,volume measurements and CT values,calculated the mass of the lesions.The correlation between lesion volume,mass and diameter was analyzed,as well as the correlation between the change rates of volume,mass and lesion diameter.Additionally,the total volume and total mass of all lesions were calculated.The correlation between the change rates of total volume/total mass and the change rate of pulmonary lesion diameter under the RECIST 1.1 criteria,as well as the correlation with changes in patients'tumor markers,were analyzed.Furthermore,the overall volume response and overall mass response of lesions were evaluated based on changes in total volume and total mass,and their consistencies with the RECIST 1.1 criteria for efficacy evaluation were analyzed.Finally,univariate Cox regression analysis was performed to explore the association between these variables and patient survival outcomes.Results There was strong correlation between lesion volume,mass and tumor diameter(r=0.771,0.775),between the rate of change in mass and the rate of change in lesion diameter(r=0.846),and between the rates of change in total volume/total mass and the rate of change in pulmonary lesion diameter under the RECIST 1.1 criteria(r=0.800,0.896).The correlation between the rates of change in total volume/total mass and patients'tumor markers was not statistically significant.There was moderate correlation between the rate of change in volume and the rate of change in lesion diameter(r=0.692).The evaluation results of total volume response and total mass response for pulmonary lesions in advanced hepatocarcinoma with lung metastasis were generally consistent with the RECIST 1.1 criteria(Kappa=0.486,0.426).Univariate Cox regression analysis revealed that total lesion volume(P=0.047)and total lesion mass(P=0.049)were independent prognostic factors for survival outcomes.Conclusion Lesion volume,mass,and diameter,as well as their respective change rates,were found to be interrelated.Furthermore,total lesion volume and total lesion mass were identified as independent prognostic factors for survival outcomes.The total volume response and total mass response are promising evaluation methods in evaluating the efficacy of lung metastasis of hepatocarcinoma,which are different from the RECIST 1.1 evaluation criteria.

9.A qualitative study on the dietary management experience of gastric cancer patients with diabetes during chemotherapy

Objective:To understand the real psychological experience of dietary management among gastric cancer patients with diabetes during chemotherapy, and to provide reference for the formulation of targeted intervention measures.Methods:A phenomenological approach in qualitative research was applied. Using purposive sampling, 15 gastric cancer patients with diabetes who were hospitalized for chemotherapy in Beijing Friendship Hospital, Capital Medical University, from January to December 2024 were selected as study participants and were interviewed face-to-face in depth. The interview process was audio-recorded, and within 24 h after each interview, two researchers transcribed the recordings verbatim into textual data. Colaizzi's seven-step method of phenomenological analysis was adopted to organize and analyze the interview data.Results:A total of 4 themes and 10 sub-themes were extracted: cognitive bias in dietary management of dual diseases (one-sided understanding of diseases, obstacles in information acquisition and transformation) ; difficulties in dietary management under the synergy of diseases (contradiction between special nutritional needs during chemotherapy and blood glucose control, conflict between chemotherapy adverse reactions and dietary requirements for diabetes, lack of coordination in interdisciplinary nutritional guidance) ; positive experiences in dietary management (improvement in physical condition from scientific diet, joint support of family and medical staff in dietary management, knowledge empowerment enhancing self-achievement) ; personalized needs from the patients' perspective (need for operability in dietary practices, need for emotional and cognitive support) .Conclusions:Gastric cancer patients with diabetes face multiple challenges in dietary management during chemotherapy. Medical staff should gain an in-depth understanding of patients' real experiences and develop more humanized dietary management strategies to improve patients' quality of life and treatment outcomes.
