1.Interpretation of the key points of the 2025 AHA/ACC guideline for the prevention, detection, evaluation and management of high blood pressure in adults

The American Heart Association (AHA) and the American College of Cardiology (ACC), in collaboration with multiple professional organizations, jointly released the "Guideline for the Prevention, Detection, Evaluation and Management of High Blood Pressure in Adults" in August 2025. Based on the latest evidence-based medical findings from February 2015 to January 2025, the guideline proposes an individualized treatment strategy grounded in total cardiovascular disease risk stratification, incorporates the novel PREVENT risk assessment model, lowers the medication initiation threshold and control targets for high-risk populations, and provides specific management recommendations for special populations. This article provides an interpretation of these updates and conducts a comparative analysis with the current status of hypertension prevention and treatment in China as well as Chinese guidelines, aiming to offer reference for hypertension control practices in China.

The American Heart Association (AHA) officially released the "2025 Heart Disease and Stroke Statistics: A Report of US and Global Data From the American Heart Association" on January 27, 2025. This report systematically compiles the latest statistics on major cardiovascular diseases worldwide, while simultaneously integrating relevant outcome indicators, including quality of care, procedures, and economic costs, and updating the global prevalence patterns and evolving trends of diverse risk factors impacting cardiovascular health, providing essential guidance for the prevention, diagnosis, and treatment of cardiovascular diseases. Synthesizing insights from this pivotal report and other relevant studies, this article highlights key findings concerning the global prevalence and mortality of heart diseases, associated risk factors, and emerging diagnostic and therapeutic technologies.
3.Intelligent handheld ultrasound improving the ability of non-expert general practitioners in carotid examinations for community populations: a prospective and parallel controlled trial

Purpose:
The aim of this study was to investigate the feasibility of an intelligent handheld ultrasound (US) device for assisting non-expert general practitioners (GPs) in detecting carotid plaques (CPs) in community populations.
Methods:
This prospective parallel controlled trial recruited 111 consecutive community residents. All of them underwent examinations by non-expert GPs and specialist doctors using handheld US devices (setting A, setting B, and setting C). The results of setting C with specialist doctors were considered the gold standard. Carotid intima-media thickness (CIMT) and the features of CPs were measured and recorded. The diagnostic performance of GPs in distinguishing CPs was evaluated using a receiver operating characteristic curve. Inter-observer agreement was compared using the intragroup correlation coefficient (ICC). Questionnaires were completed to evaluate clinical benefits.
Results:
Among the 111 community residents, 80, 96, and 112 CPs were detected in settings A, B, and C, respectively. Setting B exhibited better diagnostic performance than setting A for detecting CPs (area under the curve, 0.856 vs. 0.749; P<0.01). Setting B had better consistency with setting C than setting A in CIMT measurement and the assessment of CPs (ICC, 0.731 to 0.923). Moreover, measurements in setting B required less time than the other two settings (44.59 seconds vs. 108.87 seconds vs. 126.13 seconds, both P<0.01).
Conclusion
Using an intelligent handheld US device, GPs can perform CP screening and achieve a diagnostic capability comparable to that of specialist doctors.
4.Effects of Heixiaoyao Powder on neuroinflammation in APP/PS1 mice via ATP/P2X7R/NF-κB signaling pathway

AIM To investigate the effects of Heixiaoyao Powder on neuroinflammation in APP/PS1 transgenic mice.METHODS The 16-week-old male APP/PS1 transgenic mice were randomly divided into the model group,the BBG group(P2X7R specific antagonist,30 mg/kg)and high,medium and low dose Heixiaoyao Powder groups of(22.10,11.05,5.53 g/kg),in contrast to the male C57BL/6J mice of the same age and the same strain of the blank groups,with 12 mice in each group.When normal saline by gavage was dosed upon the blank group and the model group,and the other groups were treated with corresponding drug by gavage.After 90 days of administration,the mice had their learning and memory ability detected by Morris water maze test;their hippocampal pathological changes observed with HE staining;their MyD88 expression detected by immunofluorescence staining;their hippocampal levels of pro-inflammatory factors(TNF-α,IL-6),anti-inflammatory factors(IL-10),ATP and amyloid protein β(Aβ)detected by ELISA;their hippocampal mRNA expressions of P2X7R,TLR4,MyD88 and NF-κB-P65 detected by RT-qPCR method;and their hippocampal protein expressions of P2X7R,TLR4,MyD88,NF-κB-P65 and p-NF-κB-P65 detected by Western blot.RESULTS Compared with the blank group,the model group demonstrated prolonged escape latency and reduced frequency in crossing platform(P<0.01);decreased number of hippocampal neurons,deranged neurons,and darker cytoplasm staining;increased immunofluorescence expression of hippocampal MyD88(P<0.01);decreased IL-10 level(P<0.01);increased levels of TNF-α,IL-6,ATP and Aβ(P<0.01);increased mRNA and protein expressions ofP2X7R,TLR4 and MyD88(P<0.01),and increased protein expression of p-NF-κB-P65(P<0.01).Compared with the model group,the groups intervened with Heixiaoyao Powder or BBG demonstrated shortened escape latency and increased frequency in crossing platform(P<0.01);more number of neatly arranged hippocampal neurons;increased hippocampal IL-10 level(P<0.01),decreased levels of TNF-α,IL-6,ATP and Aβ(P<0.05,P<0.01);decreased mRNA and protein expressions of P2X7R,TLR4 and MyD88(P<0.05,P<0.01);and decreased protein expression of p-NF-κB-P65(P<0.05,P<0.01).Except the low dose Heixiaoyao Powder group,the other treatment groups shared decreased immunofluorescence expression of MyD88(P<0.05,P<0.01).CONCLUSION Heixiaoyao Powder can significantly improve the learning and memory ability of APP/PS1 model mice,and its mechanism may lie in its function in alleviating cerebral neuroinflammation by reducing the abnormal Aβ aggregation via inhibiting the activation of ATP/P2X7R/NF-κB signaling pathway.
5.Inhibitory control subcomponent characteristics of children with autism spectrum disorder aged 4-5 years

Objective To comprehensively investigate the subcomponent characteristics of inhibitory control for children with autism spectrum disorder(ASD)aged 4-5 years under experimental and natural environments.Methods Thirty children with ASD aged 4-5 years and 30 chronological-age and intellectual level-matched typically developing children were recruited.The Simon task,Go/nogo task,and Stroop task were used to examine the conflict response inhibition,prepotent response inhibition,and interference control subcomponents of inhibitory control,respectively.The inhibit subscale of the behavior rating inventory of executive function-preschool version was employed to assess children's inhibition in daily natural situations.Results Under the incongruent condition of the Simon task,there were no significant differences in mean reaction time and accuracy between ASD group and the control group(P>0.05).In the Go/nogo task,the ASD group demonstrated elevated false positive errors compared to the controls[3.10%(0,6.20%)vs.0(0,0.78%),P=0.005].However,there were no significant differences in mean reaction time and false alarm error between ASD group and the control group(P>0.05).In the Stroop task,there were no significant differences in the accuracy and mean reaction time between ASD group and the control group(P>0.05).Additionally,the ASD group scored significantly worse than the controls in the inhibit subscale of BRIEF-P[(60.47±9.63)vs.(54.23±7.45),P=0.007].Conclusions The inhibitory control of children with ASD aged 4-5 years are partially impaired in a structural experimental setting while severely deficient in a natural environment.
6.Design of new type of elastic gloves for burn scar rehabilitation

Objective To design a new type of elastic gloves for burn scar rehabilitation to solve the problems of conventional elastic gloves in pressure distribution,elasticity maintenance and absorption of sweat stains.Methods The new elastic gloves was made of non-woven fabric by spandex material,which was composed of external and internal parts.The main body of the external part was used as the primary structure of the gloves,which was provided with a sealing strip,a storage bag,a drawstring,etc.The internal part consisted of a bonding sheet,an elastic band,a fiber sheet,an absorbent sponge,some breathable holes,etc.Results The new elastic gloves designed could be used for the pressure therapy for the scars on the opisthenar,palm side,finger web and purlicue with scar proliferation inhibitted effectively,and the breathable hole and absorbent sponge contributed to the absorption of the sweat of the patient.Conclusion The new type of elastic gloves gains advantages in elasticity,wearing comfort and effectiveness of the pressure therapy for purlicue and finger web,and can be used for the pressure therapy to inhibit proliferative scarring after burns.[Chinese Medical Equipment Journal,2025,46(8):118-120]
7.Heart-sparing strategy for breast cancer radiotherapy based on nnU-Net: regional optimization and automatic segmentation

Objective:To investigate the feasibility and optimal expansion width of replacing the left anterior descending coronary artery (LADCA) with the region of heart sparing (RHS) to reduce cardiac radiation dose during breast cancer radiotherapy.Methods:Retrospective analysis was conducted on data from 88 patients with left-sided breast cancer who underwent radiotherapy at 2 centers: Nanfang Hospital of Southern Medical University (50 cases for the training set, 15 cases for the internal test set) and Ganzhou Hospital of Nanfang Hospital (23 cases for the external test set) from March 2022 to January 2024. All patients had left-sided invasive ductal carcinoma with axillary lymph node metastasis, and had undergone modified radical mastectomy and chemotherapy. Based on simulation CT images, 2 radiation oncologists delineated the LADCA and 8 RHSs. The RHSs were delineated by expanding the LADCA contour by 0.5 cm increments, totaling 8 expansions. The RHS widths were defined as 0.5, 1.0, 1.5, 2.0, 2.5, 3.0, 3.5, and 4.0 cm. The nnU-Net model was trained for 3D automatic segmentation of the LADCA and RHSs. Model performance was evaluated using the Dice similarity coefficient (DSC), relative volume error (RVE), sensitivity, specificity, and 95% Hausdorff distance (HD95). Additionally, the minimum, maximum, and average relative dose variations (RDV) as well as V5% and V20% indicators were calculated for the LADCA and each RHS. Correlation analysis was performed using the least squares regression, with the slope and coefficient of determination ( R2) employed to evaluate the accuracy of the model fitting, the relationship between the LADCA and RHS, and the degree of their correlation, thereby assessing the substitutive effect of the RHS for the LADCA. Results:The DSC for the LADCA was 0.415, while the DSCs for RHS widths of 0.5 cm and 4.0 cm were 0.718 and 0.835, respectively. Overall, the automatic segmentation performance improved with increasing RHS width. The DSC, RVE, sensitivity, specificity, and HD95 for the external test set were largely consistent with those of the internal test set, demonstrating the model's good robustness across different datasets. All RDVmin values were negative, while RDVmax and RDVmean showed a positive correlation with RHS width. RDVmean increased from 39.01% to 75.89% as the RHS width increased. In the correlation analysis, the slopes for RHS widths of 1.5 cm and 2.0 cm were 0.95 and 1.05, respectively, with R2 values and coefficients of variation of 0.79 and 0.73, and 21.11% and 24.03%, respectively. Conclusions:The automatic segmentation model trained on nnU-Net can accurately segment RHSs. Based on geometric and dosimetric indicators, a 1.5 cm-wide RHS is the most suitable substitute for the LADCA, effectively limiting the radiation dose to the LADCA without compromising target dose coverage.
8.Clinical study of hysteroscopic combined with laparoscopic repair and reconstruction of diverticulum in the treatment of post-cesarean section uterine diverticulum

Objective To observe the clinical effect of hysteroscopic combined with laparoscopic repair and reconstruction of diverticulum in the treatment of post-cesarean section uterine diverticulum(PCSD).Methods A total of 150 patients with PCSD admitted to the department of gynaecology of our hospital from January 2020 to February 2022 were selected and divided into the transvaginal group(transvaginal repair of diverticulum)and the combined group(hysteroscopic combined with laparoscopic repair and reconstruction of diverticulum)according to different surgical plans,with 75 cases in each group.The general condition of surgery,days of menstrual period,thickness of muscular layer,inflammation-related factor levels,complications and pregnancy in patients with birth plan were recorded in both groups.Results The operation time of patients in the combined group was longer than that in the transvaginal group,and the time of anal exhaustion and postoperative hospital stay of patients in the combined group were shorter than those in the transvaginal group,the differences were statistically significant(P<0.05);There was no significant difference in terms of the intraoperative bleeding volume of patients between the two groups(P>0.05).The high-sensitivity C-reactive protein(hs-CRP),interleukin-4(IL-4),interleukin-8(IL-8),and tumor necrosis factor-α(TNF-α)7 days after surgery of patients were lower in both groups compared with those before surgery,and the combined group was lower than the transvaginal group,the differences were statistically significant(P<0.05).Compared with that before the surgery,the days of menstrual period of patietns in both groups 6 and 12 months after the surgery decreased,and the thickness of muscular layer increased,with statistically significant differences(P<0.05);while no statistically significant difference was found in terms of the days of menstrual period or thickness of muscular layer of patients 6 and 12 months after surgery between the two groups(P>0.05).There was no significant difference in the incidence of cumulative complications such as incision infection,hyponatraemia,bladder injury and vaginal infection between the two groups(P>0.05).The intrauterine pregnancy rate of patients with birth plan and the full-term pregnancy rate in the combined group were higher than those in the transvaginal group,the differences were statistically significant(P<0.05).There was no statistically significant difference in the abortion rate or preterm birth rate between the two groups(P>0.05).Conclusion The hysteroscopic combined with laparoscopic repair and reconstruction of diverticulum for patients with PCSD has a fast postoperative recovery,a short hospital stay,and can increase the pregnancy rate of patients with birth plan.
9.Comparative study on the predictive abilities of multiple inflammatory markers for the poor outcome of intracerebral hemorrhage

Objective This study aimed to investigate the relationship between multiple inflammatory markers and poor outcome in patients with intracerebral hemorrhage,and to compare their predictive abilities.Methods We retrospectively analyzed the patients with intracerebral hemorrhage admitted to the Department of Neurology of the First Affiliated Hospital of Nanchang University from January 1,2015 to March 31,2023.According to the Modified Rankin Scale at 90 days after onset,the patients were divided into good outcome(mRS≤2 points)and poor outcome(mRS score≥3 points).Clinical information,laboratory examinations and follow-up data were compared between the two groups.Inflammatory markers include neutrophil to lymphocyte ratio(NLR),platelet to lymphocyte ratio(PLR),monocyte to high-density lipoprotein ratio(MHR),systemic inflammatory response index(SIRI),systemic immune-inflammation index(SII),white blood cell count to mean platelet volume ratio(WMR),lymphocyte to monocyte ratio(LMR),and systemic coagulation-inflammation index(SCI).Univariate and multivariate logistic regression analysis were used to analyze the predictors of poor prognosis after intracerebral hemorrhage,and receiver operating characteristic curve(ROC)was constructed to compare their predictive ability.Results A total of 510 patients with intracerebral hemorrhage were included.Of those,297(58.2%)had good outcome,and 213(41.8%)had poor outcome.Comparison of baseline characteristics demonstrated that patients with poor prognosis had higher levels of white blood cells,neutrophils,high-density lipoprotein,fibrinogen,NLR,PLR,SIRI,SII,WMR at admission,larger baseline hematoma volume and baseline perihematomal edema volume,a higher proportion of lobar hemorrhage,older age,and lower levels of platelets,lymphocytes,LMR,and SCI(P<0.05).Multivariate regression analysis showed that NLR(OR:1.081,95%CI:1.032~1.1131,P=0.001),SIRI(OR:1.089,95%CI:1.014~1.169,P=0.019),SII(OR:1.000,95%CI:1.000~1.001,P=0.011),WMR(OR:2.627,95%CI:1.267~5.445,P=0.009)were independently associated with poor prognosis in patients with ICH.In ROC analysis,the area under the curve of NLR(0.729,95%CI:0.685~0.774)was higher than SIRI(0.692,95%CI:0.645~0.738),SII(0.688,95%CI:0.641~0.735)and WMR(0.65,95%CI:0.602~0.698)for predicting poor outcomes.Conclusion NLR,SIRI,SII and WMR at admission are independently associated with poor outcomes in patients with intracerebral hemorrhage,and NLR has the strongest predictive ability.
10.Clinical efficacy observation of vital pulp therapy for mature permanent teeth with carious irreversible pulpitis

Objective To compare the pain relief and long-term clinical success rate of vital pulp therapy and root canal treatment in mature permanent teeth with carious irreversible pulpitis.Methods A total of 90 patients diagnosed with carious irreversible pulpitis in mature permanent teeth were collected at Shanghai Stomatological Hospital from Jan 2021 to Jun 2022.They were randomly divided into two groups:test group(n=45)undergoing vital pulp therapy(VPT)and control group(n=45)undergoing root canal treatment(RCT).Pain scores were recorded before treatment,24 hours after operation and 7 days after operation.We conducted clinical evaluation and imaging analysis at 1,6,and 12 months after the surgery,then compared the pain scores and treatment success rates between the two groups.Results Eighty-one patients,including 39 patients in group VPT aged(31.00±1.43)years old and 42 patients in group RCT aged(30.60±1.54)years old,received follow-up for more than 1 year,and the success rate of the test group and control was 97.44%and 95.24%.The pain degree of the two groups was reduced at 24 hours and 7 days after operation(P<0.05),and the pain score of the test group was reduced compared with that in the control group 7 days after operation(P<0.01).Conclusion Compared with root canal treatment,vital pulp therapy for mature permanent teeth with carious irreversible pulpitis can achieve good results in short-term pain evolution and long-term clinical success.
