1.Research advances in the role of NLRP3 inflammasome in the pathological mechanisms of multiple sclerosis:a review

Multiple sclerosis(MS)is a complex autoimmune disorder characterized by inflammatory demyelination in the central nervous system.Prominent symptoms include damage to myelin sheaths in the brain,optic nerve,and spinal cord,as well as axonal dysfunction;however,the exact causes and mechanisms of MS remain unclear.Genetic and environmental factors are thought to interact via autoimmune mechanisms,potentially triggering the disease.Recent studies suggest that abnormal activation of the NOD-like receptor thermal protein domain associated protein 3(NLRP3)inflammasome may play a critical role in the pathogenesis of MS.In this context,this review summarizes the molecular mechanisms underlying NLRP3 activation and its connection to MS,considering relevant literature from the past decade.The findings aim to provide insights into the progression of MS and to identify potential therapeutic strategies by elucidating the underlying mechanisms.
2.Visualization analysis of traditional Chinese medicine in the treatment of multiple sclerosis

Objective The development status,hot spots and trends of traditional Chinese medicine in the treatment of multiple sclerosis(MS)were analyzed by bibliometric and visual analysis.Methods Literatures related to the treatment of MS by traditional Chinese medicine were searched from CNKI,Wanfang Data and VIP from January 2000 to July 2024.CiteSpace software was used to visually analyze the number of publications,authors,institutions and key words in relevant literatures.Results A total of 438 literatures were included,and the number of published literatures showed an overall increasing trend.Fan Yongping pubilshed the most papers.The research institutions were mainly Dongzhimen Hospital of Beijing University of Chinese Medicine and Beijing Tiantan Hospital Affiliated to Capital Medical University.The current high-frequency key words in this field included acupuncture,treatment based on syndrome differentiation,traditional Chinese medicine,etc.A total of 9 clusters were formed."Acupuncture"was the most prominent.Conclusion Scholars have carried out active exploration and research in the field of traditional Chinese medicine in the treatment of MS,and the overall development trend is good.The research hot spots include acupuncture,treatment based on syndrome differentiation,traditional Chinese medicine,etc.,and the etiology and pathogenesis may be the future research trend.

5.Expression of serpin family E member 1 in gastric cancer and its mechanisms in promoting gastric cancer

Objective·To investigate the expression of serpin family E member 1(SERPINE1)in gastric cancer and its potential mechanisms in promoting gastric cancer.Methods·Pan-cancer analysis of SERPINE1 was performed by using the TIMER2.0 online website,and the differences in the expression of SERPINE1 in samples with different tumor stages of gastric cancer were analysed by the clinical data of gastric cancer from The Cancer Genome Atlas(TCGA)database.Survival curves were plotted.Quantitative real-time PCR(qRT-PCR)was used to detect mRNA expression in paired samples of clinical gastric cancer tissues.The small interfering RNA(siRNA)transfection technique was used to knock down the expression of SERPINE1 in MGC-803 and SGC-7901 gastric cancer cell lines,and the migration and invasive abilities of gastric cancer cells were investigated by Transwell and invasion chamber experiments.The effects of SERPINE1 knockdown on the angiogenesis of gastric cancer cells were further explored by the migration assay of human umbilical vein endothelial cells(HUVEC)and tubule formation assay of HUVECs.The Gene Expression Omnibus(GEO)dataset was used for differential gene analysis in high and low expression groups based on the median value of SERPINE1 expression.The differentially expressed genes were further analyzed by Kyoto Encyclopedia of Genes and Genomes(KEGG)analysis and Gene Set Enrichment Analysis(GSEA).qRT-PCR was performed to verify the expression levels of key genes related to epithelial-mesenchymal transition(EMT)and angiogenesis.Results·Bioinformatics analysis showed that SERPINE1 was highly overexpressed in a variety of primary tumour tissues,including gastric cancer.SERPINE1 gene expression increased with increasing tumour stage(P<0.001),and increased expression of SERPINE1 was associated with poor prognosis of gastric cancer(P<0.001).qRT-PCR results showed that SERPINE1 was significantly highly expressed in gastric cancer tissues(P=0.038).After knocking down SERPINE1,gastric cancer cell lines MGC-803 and SGC-7901 cells had decreased migration and invasion(P<0.05).Gastric cancer culture supernatants from the SERPINE1-knockdown gastric cancer cell line reduced angiogenesis in HUVECs(P<0.05).Differential genes in gastric cancer patients in the SERPINE1 high-expression group in the GEO database were enriched in cancer-related pathways such as focal adhesion,extracellular matrix receptor interaction,and angiogenesis-related pathways like angiogenesis and the vascular endothelial growth factor(VEGF)signalling pathway.The expression of SERPINE1 was negatively correlated with cadherin 1(CDH1),and positively correlated with other key genes related to EMT and angiogenesis.Moreover,the expression levels of key genes related to EMT and angiogenesis detected by qRT-PCR in SERPINE1 knockdown gastric cancer cell lines(P<0.05),were consistent with the correlation analysis results mentioned above.Conclusion·SERPINE1 expression is elevated in gastric cancer tissues,which could regulate the expression levels of key genes related to EMT and angiogenesis to promote the migration,invasion and angiogenesis of gastric cancer cells.

7.Research progress on therapeutic effect of exosomes from different sources on ischemic cerebrovas-cular disease

Ischemic cerebrovascular disease(ICVD)refers to the degeneration,necrosis,or transient loss of function of local brain tissue due to blood supply disorders,and is one of the common neurological diseases.Exosomes are a subgroup of extracellular vesicles,characterized by good biocompatibility,low toxicity and immunogenicity,and strong designability.The components of exosomes include proteins,lipids,genetic materials,etc.They come from a wide range of sources and can be secreted by various cells under physiological or pathological conditions.The roles and related mechanisms of exosomes from different sources in the treatment of ICVD are different.This article reviews the therapeutic effects and related mechanisms of exesomes derived from mesenchymal stem cells,brain cells,blood,endothelial cells,and macrophages on ICVD,in order to provide reference for the clinical treatment of ICVD.

The awake prone position plays an important role in the treatment of hypoxemia and the improvement of respiratory distress symptoms in non-intubated patients. It is widely used in clinical practice because of its simple operation, safety, and economy. To enable clinical medical staff to scientifically and normatively implement prone position for awake patients without intubation, the committees of consensus formulation, guided by evidence-based methodology and Delphi method, conducted literature search, literature quality evaluation and evidence synthesis around seven topics, including indications and contraindications, evaluation, implementation, monitoring and safety management, termination time, complication prevention and health education of awake prone position. After two rounds of expert letter consultation, Expert consensus on implementation strategy of awake prone positioning for non-intubated patients in China (2023) was formulated, and provide guidance for clinical medical staff.

9.The role of intravoxel incoherent motion diffusion-weighted imaging in distinguishing diabetic nephropathy from non-diabetic renal disease in diabetic patients

Objective:To investigate the intravoxel incoherent motion (IVIM) diffusion-weighted imaging (DWI) in the differential diagnosis of diabetic nephropathy (DN) and non-diabetic renal disease (NDRD) among patients with type 2 diabetes mellitus (T2DM).Methods:A diagnostic test. In this prospective study, patients with T2DM who underwent both IVIM-DWI and renal biopsy at the First Medical Center of Chinese PLA General Hospital between October 2017 and September 2021 were consecutively enrolled. IVIM-DWI parameters including perfusion fraction (f), pure diffusion coefficient (D), and pseudo-diffusion coefficient (D*) were measured in the renal cortex, medulla, and parenchyma. Patients were divided into the DN group and NDRD group based on the renal biopsy results. IVIM-DWI parameters, clinical information, and diabetes-related biochemical indicators between the two groups were compared using Student′s t-test or Mann-Whitney U test. The correlation of IVIM-DWI parameters with diabetic nephropathy histological scores were analyzed using Spearman′s correlation analyzes. The diagnostic efficiency of IVIM-DWI parameters for distinguishing between DN and NDRD were assessed using the receiver operating characteristic (ROC) curves. Results:A total of 27 DN patients and 23 NDRD patients were included in this study. The DN group comprised 19 male and 8 female patients, with an average age of 52±9 years. The NDRD group comprised 16 male and 7 female patients, with an average age of 49±10 years. The DN group had a higher D* value in the renal cortex and a lower f value in the renal medulla than the NDRD group (9.84×10 -3 mm 2/s vs. 7.35×10 -3 mm 2/s, Z=-3.65; 41.01% vs. 46.74%, Z=-2.29; all P<0.05). The renal medulla D* value was negatively correlated with DN grades, interstitial lesion score, and interstitial fibrosis and tubular atrophy (IFTA) score ( r=-0.571, -0.409, -0.409; all P<0.05) while the renal cortex f value was positively correlated with vascular sclerosis score ( r=0.413, P=0.032). The renal cortex D* value had the highest area under the curve (AUC) for discriminating between the DN and NDRD groups (AUC=0.802, sensitivity 91.3%, specificity 55.6%). Conclusion:IVIM-derived renal cortex D* value can be used non-invasively to differentiate DN from NDRD in patients with T2DM that can potentially facilitate individualized treatment planning for diabetic patients.
10.Spatial memory impairment is related to the neuronal apoptosis in the CA1 region of the hippocampus and the acetylcholine release in the rat model of vascular dementia

Objective To investigate the correlations between the hippocampal acetylcholine (ACh) content and neuronal apoptosis in the hippocampal CA1 region as well as the spatial memory impairment in a rat model of vascular dementia (VaD).Methods Forty male healthy Sprague-Dawley rats were randomly divided into either a VaD group or a sham operation group (n =20 in each group).A VaD model was induced by intermittently clipping common carotid artery.Microdialysis was used to collect dialysis solutions in rat hippocampus.High-performance liquid chromatographic analysis was used to detect the ACh content in the dialysis fluid.Morris water maze test was used to test their learning and memory abilities.TUNEL staining was used to detect neuronal apoptosis in the hippocampal CA1 region.Results Microdialysis analysis showed that the ACh content in the hippocampus in the VaD group was significantly lower than that in the sham operation group (0.442 ± 0.028 μmmol/L vs.1.560 ± 0.092 μ mmol/L; t =51.697,P =0.000).TUNEL staining showed that the apoptosis rate in the hippocampal CA1 region in the VaD group was significantly higher than that in the sham operation group (55.652% ±2.051% vs.6.530% ± 1.872% ; t =79.114,P=0.000).The escape latencies at different detection time points were prolonged significantly (At day 3:49.713 ± 18.161 s vs.13.322 ± 2.454 s; t =-8.881,P =0.000; at day 4:34.368 ± 7.424 s vs.10.503±1.415 s; t=-14.121,P=0.000; at day 5:30.676± 6.669s vs.7.311± 1.534 s; t=-15.270,P =0.000),and the numbers of cross platform were reduced significantly (3.768 ± 1.072 vs.10.218 ± 1.165; t =18.224,P=0.000).Pearson correlation analysis showed that the ACh contents in the VaD group were negatively correlated with the escape latencies (at day 3:r =-0.476,P =0.034; at day 4:r=-0.700,P=0.001; at day 5:r=-0.693,P=0.001).They were positively correlated with the numbers of cross platform (r =0.689,P =0.001),and negatively correlated with the neuronal apoptosis rates in the hippocampal CA1 region (r =-0.271,P =0.031).Conclusions The decreased ACh content,the increased neuronal apoptosis rate in the rat hippocampal CA1 region in the VaD model may be one of the mechanisms of cognitive impairment in VaD rats.
