Objective To investigate the clinical efficacy of arthroscopic debridement combined with platelet rich plasma(PRP)in treatment of supraspinatus calcified tendinitis. Methods 23 patients in the experimental group were treated with shoulder arthroscopic calcification clearing with PRP injection, while 23 patients in the control group were treated with simple arthroscopic clearing, and the curative effects were observed and compared. Results All the patients were followed up effectively.Of the 23 cases in the experimental group,5 cases were treated with rotator cuff repair(3 cases were diagnosed rotator cuff tear before operation,2 cases were diagnosed with large calcification, and rotator cuff repair was performed after cleaning),and 3 cases were treated with rotator cuff repair(2 cases were diagnosed with rotator cuff tear before operation,1 case was diagnosed with large calcification, and rotator cuff repair was performed after cleaning). The visual analogue scale(VAS)of the two groups one month after surgery was significantly lower than that before surgery, and the American Association of Shoulder and Elbow Surgeons(ASES)score of the two groups one month after surgery was significantly higher than that before surgery, with statistical significance(P<0.05).The VAS of the experimental group was significantly lower than that of the control group one month after surgery, and the ASES score was significantly higher than that of the control group one month after surgery, with statistically significant differences(t =-5.52,t = 8.73,P = 0.001).Conclusion Arthroscopic debridement combined with PRP is an effective and minimally invasive method for the treatment of supraspinatus calcified tendinitis. The calcium salt can be removed as much as possible under arthroscopy, and the scope of rotator cuff injury can be accurately evaluated. For the obvious rotator cuff injury with large damage, the rotator cuff repair with thread anchor nail can be performed. At the same time, combined with PRP,it can accelerate the repair of rotator cuff injury, relieve pain, and accelerate the functional recovery of shoulder joint.

A 44-year-old male patient presented with a subcutaneous nodule in the left little finger for 3 years. Skin examination showed a subcutaneous nodule with rubber-like hardness but no tenderness on palpation, measuring 0.4 cm × 0.4 cm in size at the dorsal distal aspect of the left little finger, and the movement of the distal interphalangeal joint was unrestricted. Postoperative histopathological examination revealed that the tumor contained abundant stroma consisting of variable fiberous, chondroid and myxoid materials; tumor cells were oval to short spindle-shaped with inconspicuous nucleoli but no mitosis; cells were arranged haphazardly or in small clusters. Immunohistochemical study showed positive staining for vimentin, CD34 and transcription factors ERG and SOX9, but negative staining for S100, P63, broad-spectrum cytokeratin AE1/AE3, epithelial membrane antigen, smooth muscle actin and desmin in tumor cells, and the Ki67 labeling index was below 1%. Finally, the patient was diagnosed with acral fibrochondromyxoid tumor.

To report a case of cutaneous metaplastic synovial cyst. A 14-year-old male patient presented with a cystic mass on the forehead for more than 6 months. Histopathological findings revealed an intradermal cystic structure with multiple intraluminal broad band-like villiform protuberances, which was lined with a membrane resembling hyperplastic synovium and composed of a variety of cellular structures; some areas of the cystic wall were covered with fibrin-like hyaline connective tissues, and some areas were highly cellularized and lined with multiple layers of epithelioid cells and spindle cells. Immunohistochemical study revealed diffuse expression of vimentin and positive staining for the histocyte marker CD68, but negative staining for cytokeratin and smooth muscle actin in epithelioid cells and spindle cells. According to the clinical manifestations and histopathological findings, the diagnosis of cutaneous metaplastic synovial cyst was confirmed.

Objective:
To evaluate the safety and efficacy of foam sclerotherapy for bleeding internal hemorrhoids.
Methods:
Consecutive adult patients (aged 18-60 years) with bleeding hemorrhoids were prospectively enrolled between March 2017 and March 2018. All patients were randomly assigned to the foam sclerotherapy (FS) group and the liquid sclerotherapy (LS) group. The foam and liquid sclerotherapy of hemorrhoids nucleus was performed by cap-assisted colonic endoscopy.
Results:
A total of 108 patients were enrolled, including 62 males (57.4%) with mean age of 44.0±7.6 years. There was no significant difference in gender, age, or body mass index between the FS (n = 55) group and the LS (n =53) group. The total amount of lauromacrogol injection in the FS group was 3.7±0.9 mL, which was significantly lower than that in the LS group (8.0±2.2 mL,
6.Application of norepinephrine in septic shock
Journal of Medical Postgraduates 2003;0(03):-
The inappropriate use of norepinephrine(NE) in hypovolemic shock may be deleterious with potentially adverse vasoconstriction and subsequent hypoperfusion.Dramatically, based on the sufficient fluid resuscitation, high doses of NE administered rather increases the blood flow and oxygen delivery during sepsis. The mechanism may contribute to the reverse of vascular hyporesponsiveness and realization of microcirculation resuscitation. However, the optimal time, dosage and combination of NE with other vasoactive agents still necessarites further investigation.
