1.Efficacy and safety of ablative fractional carbon dioxide laser combined with dye pulse light and transdermal delivery of triamcinolone acetonide in treatment of keloids

Objective:To investigate the efficacy and safety of transdermal triamcinolone acetonide delivery assisted by carbon dioxide dot matrix laser combined with narrow spectrum intense pulse laser in the treatment of keloid.Methods:A total of 82 patients with keloid in the Fifth Hospital of Huangshi City from March 2019 to March 2023 were retrospectively included and divided into two groups according to treatment methods. Combination group: 43 cases, including 20 males and 23 females, aged 20-55 (38.3±7.3) years, with a course of 3-8 (6.2±0.8) months, were treated with carbon dioxide dot matrix laser combined with narrow-spectrum intense pulse laser assisted transdermal administration of triamcinolone. Control group: 39 cases, including 23 males and 16 females, were treated with transdermal administration of triamcinolone acetonide alone, aged 21-54 (37.9±6.5) years, and the course of disease was 3-8 (6.2±0.7) months. Both groups were treated once a month for 9 months. The total effective rate, Vancouver scar scale score, bilateral scar assessment scale score, itching degree of scar, and incidence of adverse reactions were compared between the two groups after 9 months of treatment, and the cumulative recurrence rate of 12 and 24 months after treatment was assessed.Results:The total effective rate of the combined group was 93.0% (40/43), which was higher than that of the control group (76.9%) (30/39, P=0.014). After treatment, the color, vascular distribution, thickness, softness, total score and bidirectional scar assessment scale score of the combined group were lower than those of the control group, and the degree of scar itch was less than that of the control group (all P<0.05). The incidence of adverse reactions was 14.0% (6/43) in the combined group and 17.9% (7/39) in the control group, the difference was not statistically significant ( P=0.621). At 12 months after treatment, the recurrence rate was 4.7% (2/43) in the combination group and 5.1% (2/39) in the control group, with no statistical significance ( P=0.920). At 24 months after treatment, the recurrence rate in the combination group was 7.0% (3/43), lower than that in the control group (23.1%) (9/39, P=0.039). Conclusion:Carbon dioxide dot matrix laser combined with narrow spectrum intense pulse laser assisted transdermal triamcinolone acetonide in the treatment of keloid has a good effect, which can reduce the degree of scar itching, reduce recurrence, and do not increase adverse reactions.
2.A review of the current status of evaluation models for senior clinical physicians based on professional title assessment

Using clinical physicians as an example,this article describes the current status of evaluation models for sen-ior health technical personnel in China.It explores the professional title assessment mechanism-an important system reflecting the evaluation of clinical technical capabilities-as well as practical experiences and advanced practices adopted in various re-gions.The existing problems are summarized,including insufficient refinement in the classification of assessment systems,diffi-culties in quantifying clinical practice,lack of standardization in evaluation informatization,and disconnection between evaluation results and their application.Recommendations are proposed:optimizing the hierarchical and categorized evaluation mechanism,improving the evaluation system and standards,exploring quantitative assessment of clinical capabilities,innovating evaluation tools,and establishing a virtuous cycle between talent evaluation and talent utilization to promote the sustainable development of the healthcare sector.

5.Data-driven DRG-DIP-clinical pathway multidimensional fusion analysis and evaluation

Objective:To analyze the correlation between the grouping and weighting of two sets of disease combination systems, namely diagnosis-related groups(DRG) and diagnosis-intervention packet(DIP), and to establish a multidimensional analysis and evaluation mode by applying DRG, DIP, and clinical pathway to guide the standardized diagnosis and treatment and management of disease types.Methods:DRG grouping and DIP simulation full enrollment were applied to patients discharged from a tertiary Grade A general hospital in 2019. The correlation analysis between DRG, DIP, and clinical pathway inclusion(entry), correlation analysis between relative weight of DRG group and DIP standard score, and correlation analysis between clinical pathway entry and cost structure of the two disease groups were conducted by using chi-square test, Pearson correlation analysis, t-test, structural change value, degree of structural change, and incremental contribution rate. Results:Among the 130 395 patients, 41 460 cases entered the clinical pathway, 127 535 cases were enrolled in DRG, and 104 227 cases were enrolled in DIP. There was a correlation between the enrollment of DRG, DIP, and clinical pathway( P<0.05), and there was also a correlation between the relative weight of DRG groups and the enrollment of clinical pathway. The relative weight of the DRG disease group was positively correlated with the DIP standard score( r2=0.761 7, P<0.001). There was a significant difference in hospitalization costs between patients with and without clinical pathway access for some diseases( P<0.05), and different cost categories had different impacts on the total costs. Conclusions:The weight assignment and value orientation of DRG and DIP disease types are consistent, and the multi-dimensional fusion evaluation mode for DRG-DIP-clinical pathway is feasible. The correlation analysis of DRG, DIP, and clinical pathways can serve as the basis for disease classification and cost structure evaluation, which could help to carry out hospital′s refined management and optimize disease structure.
6.Construction and practice of an integrated clinical diagnosis curriculum system based on military medical service

Air Force Medical University has constructed an integrated clinical diagnosis curriculum system for undergraduate students based on military medical service and established the teaching methods of diagnostic evidence orientation, diagnostic method guidance, and diagnosed case practice. In the context of the new epidemic situation, we have also formed the blended teaching model of "triad training" and "four methods", optimized the teaching process of theory and practice, incorporated the contents of military medicine, determined the teaching objectives of this curriculum system at the three levels of knowledge, ability, and quality, completed the construction of the core curriculum, and applied it to the teaching practice. This curriculum system has achieved significant improvements in the degree of satisfaction among teachers and students and the excellent rate of military medical skill examination, which better meets the practical needs of medical service personnel training in air force.
