1.Real experience of HIV/AIDS adolescents transitioning to adult healthcare: a Meta-synthesis of qualitative studies

Objective:To systematically evaluate the qualitative study on real experiences of HIV/AIDS adolescents transitioning to adult healthcare.Methods:The qualitative research on the real experiences of HIV/AIDS adolescents transitioning to adult healthcare was electronically searched in PubMed, Web of Science, CINAHL, Embase, Cochrane Library, China National Knowledge Infrastructure, Wanfang Data, VIP, and China Biology Medicine disc. The search period was from January 1, 2010, to November 5, 2024. The methodological quality of the included literature was evaluated according to the Joanna Briggs Institute Center for Evidence-Based Health Care Quality Assessment Criteria for Qualitative Research. The results were synthesized through the aggregate integration method.Results:A total of 11 articles were included, 52 outcomes were distilled, and similar outcomes were grouped into nine new categories, which were synthesized to form three integrative outcomes (attitudes toward transitioning to adult healthcare, multiple dilemmas faced in transitioning to adult healthcare, and need for comprehensive support during transition to adult healthcare) .Conclusions:HIV/AIDS adolescents have complex attitudes about transitioning to adult healthcare and face multifaceted challenges in the transition process. Healthcare professionals should pay attention to the real experiences of adolescents during the transition period and work with parents and peers to provide holistic support, and meet the diverse needs of adolescents.

The search timeframe was from the inception of the database until September 11, 2024. The quality of the included studies was assessed using the criteria for evaluating the qualitative research quality of the Australia Joanna Briggs Institute Evidence-Based Health Care Centre. Meta-synthesis was employed to synthesize the results.Results:A total of ten studies were included, yielding 42 research results that were categorized into 11 themes. The final integrated findings were divided into three categories: professional caregivers' attitudes towards medical transition, barriers to medical transition for adolescents with chronic diseases, and factors facilitating medical transition for adolescents with chronic diseases.Conclusions:Professional caregivers' attitudes towards the medical transition of adolescents with chronic diseases are complex, with a perception of various barriers to the transition. It is essential for healthcare administrators to pay attention to these issues and implement targeted interventions. Furthermore, improving the transition service process will support adolescents in making a smooth and successful transition to the adult healthcare system.

5.Cloning and Expression Analysis of Squalene Epoxidase Gene in Poria cocos

ObjectiveTo clone squalene epoxidase (SE), a potential key rate-limiting enzyme involved in the synthesis pathway of Poria cocos triterpenes, from P. cocos and analyze for bioinformatics and expression. MethodThe total RNA was extracted by the kit and reverse-transcribed to cDNA. Specific primers were designed, and the cDNA was used as a template for cloning the SE gene, which was analyzed for bioinformatics. The expression of P. cocos qualene epoxidase(PcSE) was examined by Real-time polymerase chain reaction(Real-time PCR) in P. coco Shenzhou No. 10, Xiangjing 28, and 5.78 strains. ResultThe full length of PcSE is 1 571 bp, containing four exons and three introns. The obtained CDS sequence is 1 413 bp, encoding 470 amino acids. This protein is a hydrophobic protein with no signal peptide structure and has two transmembrane structural domains with a FAD/NAD (P) binding domain and SE structural domain localized to the mitochondrial membrane and the plasma membrane. The homologous sequence alignment with fungi of the Poriferae family is 80.92%, and the phylogenetic tree shows that PcSE protein is most closely related to P. cocos from the US. The results of Real-time PCR showed that the PcSE was expressed in all three strains, with the highest expression in 5.78 strain, and there was no significant difference in PcSE expression among the three strains. ConclusionFor the first time, the PcSE gene was cloned and analyzed from P. cocos, providing a basis for further research on the function of PcSE and the analysis of P. cocos triterpene biosynthesis pathway.

Medical insurance system plays a key role in China's social security system.In the social insurance system,medical insurance features the widest coverage and the most complex in mechanism,while China' s medical insurance system itself is found with many setbacks.This paper probed into such risks found in the county-level medical insurance system as excessive cost growth causing overpayment of the medical insurance fund and poor supervision of the fund.On such basis,the authors recommended such policy changes as payment reform,and enhanced supervision over the fund,the demand side,and the government,in an effort to optimize China' s medical insurance system for theoretical and decision reference of other county-level hospitals in their reforms.
7.Effects of oxidized high density lipoprotein on tissue factor expression in ECV304 cell line

AIM: To investigate the expression of tissue factor(TF) induced by oxidized high density lipoprotein(oxHDL) in human umbilical vein cell line,ECV304,and the related mechanisms.METHODS: Four main groups were designed: the negative,the positive(ECV304 with histamine),the HDL group and the oxHDL group.Quantitative real-time polymerase chain reaction(RQ-PCR) and Western blotting were used to detect the expression level of TF.The specific inhibitors of MAPKs,SP600125(c-jun terminal NH2 kinase,JNK),SB203580(p38 MAP kinase,p38 MAPK),PD98059(extracellular signal-regulated kinase,ERK1/2) were used to investigate the underlying mechanisms.RESULTS: The TF expression in normal ECV304 cell line was not detected.Histamine administration resulted in a significant expression of TF in ECV304 cell line,with strongest effect after 1 h co-incubation at concentration of 1?10-5 mol/L histamine(about 4.8-fold higher expression of TF compared with that of 1?10-9 mol/L histamine).Expression level of TF was detected after stimulated with oxHDL in dose-and time-dependent manners.The highest expression of TF mRNA was found at 20 mg/L oxHDL and 6 h co-incubation,with 1.8-fold and 5.3-fold increase in TF expression,respectively,compared with that at 10 mg/L oxHDL and 2 h co-incubation.20 mg/L oxHDL also caused an apparent augmentation of TF protein expression,about 1.5-fold higher compared with that stimulated by 40 mg/L oxHDL.HDL co-incubation did not cause a detectable expression of TF protein.The mRNA levels of TF in ECV304 cell line induced by oxHDL were decreased by 95.0%,81.0%,87.0%,respectively(all P
