1.Research on the influencing factors of intertemporal health decision-making based on an interpretive structural model

Objective:To identify the key individual factors influencing intertemporal health decision-making and construct an interpretive structural model to analyze the logical relationships between these factors, providing intervention targets for making health-related intertemporal decisions.Methods:Through literature review and expert discussion, individual factors influencing health intertemporal decision-making were identified. The interpretive structural model and cross-impact matrix multiplication method were used to analyze the hierarchical relationships and category distribution among these factors.Results:A hierarchical structural model consisting of three levels and eight elements was established, with category distribution including independent, associated, and dependent groups. Education level and economic status were identified as fundamental factors at the bottom level, influencing each other and belonging to the independent group. Health cognition, health status, self-control, and emotional state were middle-level indirect factors. Self-control was influenced by health status and also affected emotional state and health cognition, while health cognition, in turn, influenced health status, placing these factors in the associated group. Future time perception and future situational expectations were direct factors at the top level, influencing each other and belonging to the dependent group.Conclusions:Education level, economic status, health cognition, health status, self-control, emotional state, future time perception, and future situational expectations are individual factors influencing intertemporal health decision-making, with an inherent relationship from bottom to top. Clarifying the pathways between these factors will help to provide targeted interventions based on individual conditions, promoting correct health intertemporal decision-making.
2.Association between dietary diversity and the risk of MACE after PCI in patients with coronary heart disease

Objective:To investigate the association between dietary diversity and the risk of major adverse cardiovascular events (MACE) after percutaneous coronary intervention (PCI) in patients with coronary heart disease (CHD) .Methods:A total of 553 patients diagnosed with CHD and undergoing PCI in the Department of Cardiology at the Second Affiliated Hospital of Harbin Medical University between May and November 2023 were enrolled using a convenience sampling method. A Semi-Quantitative Food Frequency Questionnaire was used to assess patients' dietary intake after PCI, and the Dietary Diversity Score (DDS) was calculated. Patients were followed up for one year to determine the incidence of MACE.Results:History of hypertension, history of hyperlipidemia, body mass index, use of antiplatelet agents, use of diuretics, triglycerides, smoking index and DDS were identified as factors influencing the occurrence of MACE after PCI ( P<0.05) . Among these, higher dietary diversity had a protective effect against MACE. Conclusions:After PCI, patients with lower DDS experienced MACE more frequently than those with higher scores. Increased dietary diversity can effectively help prevent MACE in patients after PCI.
3.Impact of sleep quality and sleep chronotype on severity of coronary artery lesions in patients with coronary heart disease

Objective:To investigate the independent and interactive effects of sleep quality and sleep chronotype on the severity of coronary artery lesions in patients with coronary heart disease.Methods:From September 2023 to January 2024, 322 inpatients with coronary heart disease diagnosed by coronary angiography in the Department of Cardiovascular Medicine of the Second Affiliated Hospital of Harbin Medical University were selected for the study using the convenience sampling method. Pittsburgh Sleep Quality Index and the Chinese version of the Munich Chronotype Questionnaire were used to investigate the sleep quality and sleep chronotype, and the Gensini score was used to evaluate the severity of coronary artery lesions. Multiple ordered Logistic regression was used to explore the factors influencing the severity of coronary artery lesions in patients with coronary heart disease.Results:Multiple ordered Logistic regression analysis showed that total cholesterol, history of diabetes mellitus, sleep quality, and sleep chronotype were influencing factors in the severity of coronary artery lesions in patients with coronary heart disease ( P<0.05) . There were statistically significant main and interaction effects of sleep quality and sleep chronotype on the severity of coronary artery lesions ( P<0.05) . Conclusions:Sleep quality and sleep chronotype in patients with coronary heart disease have not only independent but also interactive effects on the severity of coronary artery lesions. Nursing staff can provide guidance and interventions for sleep quality and sleep chronotype that may help control the progression of coronary artery lesions in patients with coronary heart disease.

Patients were assessed using a general information questionnaire, Semi-Quantitative Food Frequency Questionnaire, Food Choice Questionnaire, Nutritional Knowledge Questionnaire, 7-Item Generalized Anxiety Disorder Scale, and Patient Health Questionnaire-9.Results:A total of 340 questionnaires were distributed, and 333 valid questionnaires were collected, with an effective response rate of 97.94% (333/340). Two dietary patterns were identified among patients with CHD in severe cold regions: a "fruit-nut-dairy" dietary pattern and a "meat-seafood-grain" dietary pattern. Factors influencing the "fruit-nut-dairy" dietary pattern included gender, nutritional knowledge, and food choice motivations such as accessibility, sensory appeal, brand and packaging, and emotional dimensions ( P<0.05). Factors influencing the "meat-seafood-grain" dietary pattern included body mass index (BMI), living alone or not, monthly income, and food choice motivations such as convenience, health attributes, sensory appeal, and price ( P<0.05) . Conclusions:When providing dietary interventions for CHD patients, targeted control of factors such as gender, BMI, living alone or not, monthly income, nutritional knowledge, and key elements of food choice motivations may optimize patients' irrational dietary structures and reduce adverse disease outcomes.

9.Health behavior decision-making experience and needs of patients after percutaneous coronary intervention:a qualitative research

Objective To explore the experiences and needs of patients making health behavior decisions after percutaneous coronary intervention(PCI),and to provide evidence for formulating targeted decision support strategies.Methods Based on the Ottawa decision support framework and descriptive qualitative study,16 patients with unhealthy behaviors 1 year after PCI were selected as research subjects in a tertiary A general hospital in Harbin from July to December 2022 by purpose sampling method,and their decisional experience and needs were deeply interviewed by semi-structured interview method.The data were sorted out and analyzed by content analysis method.Results After the analysis of the interview data,the 2 themes and 6 sub-themes were extracted,including the experience of health behavior decision(uncertainty about the benefits of health behavior,low recognition of health behavior,and worry about the negative impact of behavior change);health behavior decision-making needs(health behavior knowledge needs,medical resources needs,emotional support needs).Conclusion PCI patients have a variety of conflicting experiences and multi-dimensional health behavior decision-making needs.Clinical nurses should start from the experience and needs of patients,pay attention to and identify the conflict experience of patients early,meet the individual needs of patients from the aspects of knowledge,medical resources,emotional support,etc.,and promote the transformation and long-term maintenance of PCI patients'health behaviors.
