ObjectiveThis study systematically analyzed the arginine metabolic dysregulation in the rat model of heart failure （HF）， providing a modern scientific basis for elucidating the pathogenesis of HF and offering new insights for the prevention and treatment of HF with traditional Chinese medicine （TCM）. MethodsA thoracotomy was performed to ligate the left anterior descending coronary artery of rats， which induced acute myocardial ischemia and thus led to the development of post-myocardial infarction heart failure. The rats were divided into a sham surgery group and a model group， with eight rats in each group. Serum targeted metabolomics analysis was performed using ultra-performance liquid chromatography-triple quadrupole mass spectrometry （UPLC-TQ-S）， and the spatial distribution of metabolites in cardiac tissue was observed using airflow-assisted desorption electrospray ionizationmass spectrometry imaging （AFADESI-MSI）. Targets associated with HF and arginine metabolism were screened from databases including GeneCards and the Gene Expression Omnibus （GEO）， a protein-protein interaction （PPI） network was constructed， and enrichment analysis of the Kyoto Encyclopedia of Genes and Genomes （KEGG） pathway and Gene Ontology （GO） was performed. Finally， molecular docking was conducted to verify the binding between core metabolic components and key targets， and potential TCMs were predicted based on the core pathways and targets. ResultsCompared with the sham surgery group， the levels of arginine and citrulline in the serum of model rats were significantly decreased （P<0.01）， while those of proline， ornithine， creatine， creatinine and glutamate were significantly increased （P<0.05， P<0.01）. Cardiac mass spectrometry imaging showed a decreased abundance of arginine in the local myocardial tissue. Bioinformatics analysis identified 24 core functional targets， such as the angiotensin-converting enzyme （ACE）， neuronal nitric oxide synthase （NOS1）， 5-hydroxytryptamine receptor 2A （HTR2A）， and epidermal growth factor receptor （EGFR）， and enrichment analysis indicated that these targets were significantly involved in the calcium signaling pathway， neuroactive ligand-receptor interactions， and phosphatidylinositol signaling pathway. Molecular docking confirmed strong binding activities between arginine， citrulline and HTR2A， as well as between creatine， creatinine and EGFR. Based on pathway-target prediction， potential TCM interventions， such as ginseng and magnolia， were identified. ConclusionThis study revealed characteristic arginine metabolic disorder in HF， and the core targets of HF were closely associated with the phosphatidylinositol signaling pathway. It provides a modern biological interpretation of the pathogenesis of HF in TCM from the perspectives of metabolites and signaling pathways， and offers valuable insights for targeted therapy of HF and the development of TCM.

Objective To explore the value of ECG-less technology and deep learning image reconstruction(DLIR)algorithm for coronary CT angiography(CCTA).Methods Seventy-one patients with suspected coronary artery disease(CAD)were prospectively enrolled and randomly divided into group A(n=35,underwent CCTA using ECG-less technology)and B(n=36,underwent CCTA using physical electrocardiographic gating).CCTA images of group A were reconstructed using filtered back projection(FBP),adaptive statistical iterative reconstruction V(ASiR-V)(50％blending)and high-strength DLIR(DLIR-H),respectively,while those of group B were reconstructed using conventional ASiR-V(50％blending).The radiatio n doses and examination durations of CCTA were compared between groups.Then subjective and objective evaluation of imaging quality were compared between groups and within group A among 3 reconstruction algorithms.Results No significant difference of radiation dose of CCTA was found between groups(P＞0.05).The examination duration in group A was reduced by approximately 29.37％compared to group B(P＜0.05).No significant difference of subjective image quality scores nor objective evaluation of ASiR-V reconstructed CCTA was detected between groups(all P＞0.05).In group A,compared to FBP algorithm,subjective image quality scores of DLIR-H and ASiR-V upgraded,signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)increased and standard deviation(SD)values decreased(all P＜0.05),but no significant difference of CT values was found(all P＞0.05).Conclusion ECG-less technology could significantly shorten CCTA examination duration,while DLIR could reduce image noise and improve image quality of CCTA.

Objective:To examine the validity and reliability of the Basic Psychological Needs Satisfaction and Frustration Scale(BPNSFS)in the adult population.Methods:A total of 488 adults were recruited and random-ly divided into 2 groups.One half(n=244)was utilized for item analysis and exploratory factor analysis,while the other half(n=244)was reserved for confirmatory factor analysis.One month later,a convenient sample of 100 in-dividuals was selected for retesting.The Depression-Anxiety-Stress Scale-21(DASS-21)and the Satisfaction with Life Scale(SWLS)were employed as criterion measures.Results:Both subscales of the BPNSFS demonstrated a 3-factor structure,explaining a total variance of 60.99％and 60.31％,respectively.The 3-factor model exhibited good fit indices(x2/df=1.85,2.47;RMSEA=0.06,0.08;CFI=0.95,0.94;TLI=0.94,0.92;SRMR=0.04,0.05).Scores on both subscales and their dimensions were significantly correlated with DASS-21 and SWLS scores(|r|=0.16-0.57,Ps＜0.001).The Cronbach α coefficients for the 2 subscales were 0.87 and 0.89,with Cronbach α values for each dimension ranging from 0.73 to 0.81.The test-retest reliabilities(ICC)for the 2 sub-scales were 0.84 and 0.80,with ICC values for each dimension ranging from 0.65 to 0.83.Conclusion:The validi-ty and reliability of the Chinese version of the Basic Psychological Needs Satisfaction and Frustration Scale demon-strate good measurement properties in assessing the adult population.

BACKGROUND:In the follow-up after unicompartmental knee arthroplasty,some patients have knee pain,among which postmenopausal obese women are the most common. As an important index to measure the degree of body obesity,whether body mass index is related to the curative effect after unicompartmental knee arthroplasty and whether obesity will affect the function of knee joint after operation are worthy of further study.OBJECTIVE:To evaluate the clinical efficacy of postmenopausal obese women undergoing medial unicompartmental knee arthroplasty,and to determine the influence of body mass index on the quality of life after unicompartmental knee arthroplasty.METHODS:Female postmenopausal patients who underwent medial unicompartmental knee arthroplasty for the first time due to medial knee pain from January 2017 to January 2019 in the Fourth Clinical Medical College of Xinjiang Medical University were enrolled. A total of 270 cases were included according to inclusion and exclusion criteria,and were divided into 4 groups according to preoperative body mass index:There were 42 cases in normal group (body mass index 18.5-22.9 kg/m2),58 cases in overweight group (body mass index 23.0-24.9 kg/m2),122 cases in obese group (body mass index 25.0-29.9 kg/m2),and 48 cases in severely obese group (body mass index ≥30 kg/m2). Hospital for Special Surgery score,Western Ontario and McMaster Universities Osteoarthritis Index score,knee range of motion,visual analog scale score,and hip-knee-ankle angle were compared before,after and at the last time in each group. Patients were followed up to record the time of use of prostheses after surgery and reasons for failure or revision. The effective utilization rate of prostheses was calculated and compared in each group. Survival curve was used for statistical analysis of the effective utilization rate of prostheses.RESULTS AND CONCLUSION:(1) There was no significant difference in postoperative follow-up time,knee joint range of motion,visual analog scale score,and hip-knee-ankle angle between the groups (P>0.05). (2) The Hospital for Special Surgery score and Western Ontario and McMaster Universities Osteoarthritis Index score of each group in final follow-up were significantly improved compared with those before surgery (P<0.05),and the postoperative effect was obvious in each group (P<0.05). Regarding Hospital for Special Surgery score,the improvement effect was worse in the severely obese group. (3) The comparison of hip-knee-ankle angle between all groups immediately after surgery and the last follow-up showed that there were significant differences between the other groups at two time points (P<0.05) except the normal group (P>0.05). (4) The effective utilization rate of prosthesis in normal,overweight,obesity,and severely obese groups was 100％,95％,94％,and 94％,respectively,and there was no significant difference between the groups (x2=2.532,P=0.469). (5) It is indicated that body mass index of postmenopausal obese women had no significant effect on the effective utilization rate of medial unicompartmental prosthesis. Obesity is an important factor affecting the postoperative knee function score and the effective utilization rate of prosthesis.Weight should be properly controlled before and after unicompartmental knee arthroplasty. At the same time,female body mass index ≥ 30 kg/m2 is not the best indication for unicompartmental knee arthroplasty. It is suggested that female patients undergoing unicompartmental knee arthroplasty should controlbody mass index below 30 kg/m2.

Objective:To evaluate the imaging stability of a portable boom-type ophthalmic ultra-high-resolution optical coherence tomography (OCT) device in multiple application scenarios.Methods:The boom-type mode and handheld mode of the portable boom-type OCT and the desktop OCT were used to perform three-dimensional imaging tests on three healthy adults undergoing physical examinations at the Zhongshan Ophthalmic Center, Sun Yat-sen University as well as on OEMI-7 model eyes in a sitting position.The same two modes of the portable boom-type OCT were used to perform three-dimensional imaging on four awake non-sedated infants, two sedated infants and four healthy adults in the supine position.The obtained 3D imaging data were processed using a correlation analysis method between adjacent B-scans, and the offset of B-scan in the axial (z-axis) and the fast axis transverse (x-axis) were calculated.The procedures for in vivo human eye experiments followed the Declaration of Helsinki and were approved by the Ethics Committee of Zhongshan Ophthalmic Center, Sun Yat-sen University (No.2020 KYPJ154).All subjects and infant guardians signed the informed consent form. Results:Compared with the handheld imaging mode, the axial and fast axis lateral motion offsets of the model eye were significantly reduced in the boom-type imaging mode from (124.00±12.49)μm to (48.00±15.87)μm and from (24.00±1.00)μm to (2.67±0.57)μm, respectively ( t=2.932, 4.337; both P<0.001).In both human and model eyes, the axial and fast axis lateral motion offsets of the boom-type mode were significantly lower than in the traditional handheld operation mode (both P<0.001).The axial and lateral motion offsets between the boom-type mode and desk-top OCT imaging were comparable, without significant differences (both P>0.05).In both sedated and awake, non-sedated infants in the supine position, the axial offset of the portable boom-type OCT system was similar to that of the healthy adults, without significant difference in the overall comparison ( P=0.385), and the lateral offsets were higher than those of healthy adults, with statistically significant differences (both P=0.013).There was no significant difference in axial deviation between sedated and non-sedated infants ( P>0.05).The lateral deviation of non-sedated infants was higher than that of sedated infants, though the difference was not statistically significant ( P=0.247). Conclusions:The portable boom-type OCT system can maintain high-speed, high-resolution imaging performance while achieving imaging stability comparable to traditional desktop OCT systems.It is more suitable for bedside imaging of supine subjects, especially uncooperative infants, and has good clinical application prospects.

Objective To explore the association between the ratio of red blood cell distribution width to albumin level(RAR)and the risk of non-valvular atrial fibrillation(NVAF)in patients with hyperuricemia.Methods A total of 8 277 patients with hyperuricemia who underwent physi-cal examinations at the Physical Examination Center of Northern Jiangsu People's Hospital from January 2021 to June 2024 were enrolled.From them,185 patients with AF were assigned into a case group,and 740 patients without AF were into a control group in a ratio of 1∶4 with propen-sity score matching(PSM).Logistic regression analysis,restricted cubic spline analysis,and sub-group analysis were employed to investigate the association between RAR and the risk of NVAF in hyperuricemic patients.Results The case group had significantly larger ratio of smoking,high-er levels of red blood cell distribution width,total bilirubin,hematocrit,and RAR index,but lower albumin,low-density lipoprotein cholesterol(LDL-C)and triglyceride(TG)levels,and smaller ra-tio of fatty liver than the control group(P＜0.01).Multivariate logistic regression analysis showed that the patients at Q2,Q3,and Q4 quartiles of RAR index have higher risk for AF than those at Q1 quartile(OR=2.843,OR=4.263,OR=7.801,P＜0.01).The effect of RAR index was significant when below the breakpoint of 2.85(OR=1.492,95％CI:1.151-1.993,P=0.004).Subgroup analysis indicated significant interactions between RAR index and age as well as smok-ing(P interaction＜0.05).The area under curve value of RAR index in predicting NVAF occurrence in male hyperuricemic patients was 0.679(95％CI:0.632-0.725).Conclusion In patients with hyperuricemia,a higher RAR index is associated with an increased risk of NVAF.RAR index has guiding significance for diagnosing male hyperuricemic patients complicated with AF.

Ischemic bowel disease(IBD)is a prevalent intestinal vascular disease that poses a serious threat to patients' lives and health.Tthe key pathological process of ischemic bowel disease is intestinal ischemia-reperfusion(II/R)injury.If not diagnosed and treated in a timely manner,it can lead to life-threatening conditions such as peritonitis,intestinal gangrene perforation,and multiple organ dysfunction.Current treatments primarily focus on symptomatic and surgical interventions,lacking targeted pharmacological therapies.A substantial body of research has found that traditional Chinese medicine(TCM)exhibits a protective effect against II/R injury.This review systematically elaborated on the mechanisms by which various TCM bioactive components,including Astragaloside IV,Ginsenosides,Salvianolic acids,and Tetramethylpyrazine,as well as compound formulations such as Shenqi injection,Taoren Chengqi decoction,and Sini decoction ameliorate II/R injury.These findings provide novel research perspectives for developing TCM-based therapeutics for IBD.

Objective To improve the quality of diagnosis and coding on obstetric medical record home pages in our cit-y,identify major issues,and provide a basis for targeted training interventions.Methods Utilizing a medical record quality con-trol system and logic validation rules,we conducted verification and problem identification analysis on diagnosis and coding infor-mation from all obstetric medical record home pages throughout 2024 across five tertiary hospitals in the city.Results 40.37％of the front page of obstetric medical record had problems.The major problems included missing diagnosis(65.75％),wrong coding(25.02％)and wrong choice of main diagnosis(6.99％).Conclusion The establishment of logical validation rules based on the information on front pages of obstetric medical record can identify the problems in the diagnosis together with the coding of front page of obstetric medical record,which is conducive to targeted training of obstetrician and medical record coders,as well as improving the completeness and accuracy of the diagnosis and coding of the front page of obstetric medical record.
