1.Esophageal carcinoma with ductal differentiation of esophageal gland:clinicopath-ological characteristics and whole exome sequencing analyses

Purpose To summarize the clinical pathological and immunohistochemical characteristics of esophage-al carcinoma with ductal differentiation of esophageal gland,and analyze the somatic mutation characteristics,key driv-ing mutation genes,and significantly mutated genes based on whole exome sequencing.Methods The clinicopatho-logical features of 9 cases of esophageal carcinoma with esophageal duct differentiation were retrospectively analyzed,and the immunohistochemistry EnVision two-step method was used to stain them,and 3 of the samples were subjected to whole exome sequencing and data analysis.Results Among the 9 patients,6 were males and 3 were females.The average age was 68.3 years old(61-80 years old).All 9 cases were located in the middle-lower segment of the e-sophagus.The diameter of the lesion was from 1.5 cm to 3.5 cm.Most areas of the tumor had a double-layer epithelial structure,including the inner layer of luminal epithelium and the outer layer of basal epithelium.Focal areas could be seen with keratinization and mucinous cells.Immunohistochemistry showed that CK7 was positive in the inner epitheli-um,while p63 was positive in the outer basal epithelium.S-100,SOX10 and c-myb were all negative,and p53 was mutated(diffuse strongly positive).The results of whole exome sequencing analysis showed somatic mutation character-istics(796 SNV,37 InDel,482 CNV),key driving mutation genes(12),and significantly mutated genes(TP53).No intraepithelial neoplasia was observed on the surface squamous epithelium of all cases,and no Barrett's esophagus or ectopic gastric mucosa was observed.The average follow-up time was 21.9 months(8 days-51 months),with 8 ca-ses surviving and 1 case dying of severe pulmonary infection 8 days after surgery.Conclusion Esophageal carcinoma with ductal differentiation of esophageal gland is a rare epithelial derived malignant tumor of the esophagus,character-ized by unique morphological,immunohistochemical,and molecular changes.

3.Cerebral syphilitic gumma misdiagnosed as multicentric glioma: a case report

Neurosyphilis is a group of clinical syndromes caused by treponema pallidum invading the central nervous system and causing varying degrees of damage to the brain parenchyma, brain (spinal) membrane or spinal cord. Cerebral syphilitic gumma is the rarest form of neurosyphilis, and its clinical manifestations lack specificity, which is easily confused with other intracranial space occupying lesions on imaging. In this article, a patient with cognitive dysfunction, intracranial hypertension, and imaging findings similar to multicentric glioma was diagnosed as cerebral syphilitic gumma by serology, cerebrospinal fluid test and brain histopathology. The symptoms basically disappeared after treatment with standard penicillin. This report aims to raise awareness of cerebral syphilitic gumma in order to reduce missed diagnosis and misdiagnosis.
4.T2 mapping for quantitatively evaluating changes of junctional zone and outer myometrium caused by endometrial fibrosis

Objective To observe the value of T2 mapping for quantitatively evaluating the changes of junctional zone and outer myometrium caused by endometrial fibrosis.Methods A total of 73 infertility patients with endometrial fibrosis confirmed by hysteroscopy(disease group)and 33 healthy women of childbearing age(control group)were prospectively enrolled,and MR examinations were performed at the late proliferative phase of endometrium.The thickness and T2 value of junctional zone,T2 value of outer myometrium on anterior,posterior and fundus wall of midsagittal corpus uteri were measured,and the mean value of the above measurements on the three walls were calculated.Receiver operating characteristic curves were constructed,the areas under the curves(AUC)were calculated to explore the efficacy of those with significant difference among the mean thickness and the mean T2 value of junctional zone,the mean T2 value of outer myometrium and their combination for evaluating endometrial fibrosis.Results The thickness and T2 value of anterior wall,posterior wall,fundus wall and the mean junctional zone in disease group were all significantly higher than those in control group(all P<0.001).No significant difference of T2 value of anterior wall,posterior wall,fundus wall nor the mean outer myometrium was found between groups(all P>0.05).The mean thickness and the mean T2 value of junctional zone and their combination could be used to effectively evaluate endometrial fibrosis,with AUC of 0.839,0.822 and 0.922,respectively,and their combination had the best performance(both P<0.01).Conclusion T2 mapping could be used to quantitatively evaluate the injury of junctional zone caused by endometrial fibrosis.
5.Study of safety and clinical outcomes of ommaya reservoir combined with external ventricular drainage for intraventricular hemorrhage

Objective To evaluate safety and clinical effect of Ommaya reservoir combined with external ventricular drainage for intraventricular hematoma(IVH),and to analyze the effects on neurological recovery,blood-brain-barrier(BBB)protection and the incidence of complications.Methods A total of 100 patients admitted to Yichun People's Hospital of Jiangxi Province for IVH and underwent surgical treatment from January 2021 to January 2023 were selected as the study subjects.Using a random number table,the patients were divided into control group(n=50)and treatment group(n=50).The treatment group received Ommaya reservoiv combined with external ventricular drainage,while the control group received conventional external ventricular drainage.The postoperative improvement rate,neurological recovery,BBB index,S100 calcium binding protein beta(neural)(S100β),myelin basic protein(MBP)levels,and hematoma clearance rate were compared between two groups.The incidence of complications in both groups was monitored over a 24-month follow-up period.Results At 3d,7d,and 14d post-surgery,the hematoma clearance rate in treatment group was higher than that in control group.At 3,12,and 24 months post-surgery,the patients of treatment group showed a higher improvement rate then that of control group,with better performance in National Institutes of Health stroke scale and Canadian stroke scale scores for neurological recovery,all differences being statistically significant(P<0.05).During follow-ups at 1,3,12,and 24 months post-surgery,the BBB index,S100β,and MBP levels in treatment group were all lower than those in the control group,with all differences being statistically significant(P<0.05).At 24 months post-surgery,the patients of treatment group had a lower incidence of complications than that of control group(P<0.05).Conclusion Ommaya reservoir combined with exteral ventricular drainage can significantly improve neurological function,protect the BBB,and show good long-term efficacy in follow-up.
