1.Achievements,Challenges and Pathways for Digital and Intelligent Transformation of Traditional Chinese Medicine

Digital and intelligent technologies serve as the core engine driving the inheritance of the essence and the innovation while upholding the fundamentals of traditional Chinese medicine(TCM). Currently, the digital and intelligent transformation of TCM has undergone four developmental stages, exhibiting inherent characteristics such as long-term inevitability, objective standardization, and ecological evolution. By introducing quantitative metrics, digital and intelligent technologies have achieved breakthroughs in TCM knowledge inheritance and innovation, clinical diagnosis and treatment, and herbal medicine supply. The practical applicability of methodological innovations has been empirically validated, though significant disparities exist in technological adaptability and application depth across different fields. Overall, the digital and intelligent transformation of TCM remains in its nascent stage, grappling with multiple structural challenges:weak data foundations, inadequate technological adaptability, incomplete institutional frameworks, shortages of multidisciplinary talent, lagging policies and regulations, and urban-rural digital divide. In order to foster sustainable development and modernization of TCM, this paper establishes a six-dimensional collaborative governance framework of encompassing data, technology, organization, institutions, environment and ethics, which is rooted in data governance and digital governance theories. Future efforts should center on standardization, integration, and ecosystem development to build a data and technology foundation. Focus should be placed on deepening innovation and application of key TCM-specific technologies, while simultaneously strengthening interdisciplinary talent cultivation, improving institutional mechanisms and policy frameworks, and increasing support for rural areas. By adopting a people-centered and technology-empowered approach, we can overcome developmental constraints and unleash the powerful driving force of digital and intelligent technologies for the inheritance of TCM.
2.Textual Analysis of Relevant Policies on Children's Medicines in China Based on the Perspective of Policy Tools

Objective To analyse China's policies related to children's medicines based on the perspective of policy tools,explore their multidimensional characteristics,and provide reference for the formulation and optimisation of China's policies related to children's medicines.Methods A two-dimensional analysis framework of policy tools-policy objectives was constructed,and the policies related to children's medicines issued at the national level from 2011 to 2023 were selected,and the policy texts were coded,classified and statistically analysed.Results Thirty-five policy texts were included and 117 units of analysis were obtained through coding.Of these,52.14%were environment-based policies,38.46%were supply-based policies and 9.40%were demand-based policies.The policy objectives were categorised as ensuring the safety of medicines for children,improving the level of paediatric diagnosis and treatment and increasing the accessibility of medicines for children,and the policy instruments applied to the above policy objectives accounted for 35.7%,13.5%and 50.8%respectively.Conclusion The distribution of policy instruments is not reasonable,the structure of policy objectives is unbalanced,environmental policy instruments dominate and demand-oriented policy instruments are missing.
3.The correlation between intracranial arterial calcification and white matter hyperintensities in patients with cerebral small vessel disease

Objective:To investigate the correlation of intracranial arterial calcification (IAC) and its different subtypes with the severity of white matter hyperintensities (WMHs) in patients with cerebral small vessel disease (CSVD).Methods:Consecutive CSVD patients admitted to the Department of Neurology, the First Affiliated Hospital of Zhengzhou University from March 2018 to March 2022 were enrolled. Baseline demographic, laboratory, and imaging data were collected. Based on a developed and validated IAC grading scale using head CT, IAC was classified into intimal and medial types, and further categorized as focal or diffuse based on the extent of involvement. The severity of WMHs on magnetic resonance imaging was assessed using the Fazekas Scale, with patients divided into those with moderate-to-severe (total score>2) and non-moderate-to-severe WMHs (total score≤2). Subgroups were stratified based on baseline characteristics (patients′ sex, age, hypertension history, stroke history, diabetes, coronary heart disease, hyperlipidemia, smoking, and alcohol consumption). Multivariate Logistic regression was used to analyze the correlation between IAC′s subtypes and the severity of WMHs, with forest plots illustrating the interaction between medial IAC and subgroup variables.Results:A total of 490 patients with CSVD who met the inclusion and exclusion criteria were ultimately included, with a age of (60.88±10.99) years, including 162 females (33.1%). Moderate-to-severe WMHs were present in 245 patients (50.0%). Among the 490 CSVD patients, 395 (80.6%) had IAC, including 335 (68.4%) with intimal IAC and 207 (42.2%) with medial IAC. Diffuse IAC was observed in 126 patients (25.7%), all of whom had medial IAC. Intracranial arterial stenosis was present in 271 patients (55.3%). Multivariate Logistic regression showed that IAC ( OR=2.073, 95% CI 1.142-3.761, P=0.016) was significantly associated with moderate-to-severe WMHs and medial IAC ( OR=3.230, 95% CI 1.800-5.797, P<0.001) and advanced age ( OR=1.046, 95% CI 1.019-1.074, P=0.001) were significantly associated with moderate-to-severe WMHs. Subgroup analysis revealed medial IAC had no significant interaction with patients′ gender, age, hypertension, diabetes, coronary heart disease, hyperlipidemia, alcohol or smoking consumption except for stroke history. Conclusion:In the CSVD patients, IAC, especially medial IAC, is significantly associated with the severity of WMHs.

6. Microglial Exosome miR-7239-3p Promotes Glioma Progression by Regulating Circadian Genes

Glioma-associated microglial cells, a key component of the tumor microenvironment, play an important role in glioma progression. In this study, the mouse glioma cell line GL261 and the mouse microglia cell line BV2 were chosen. First, circadian gene expression in glioma cells co-cultured with either M1 or M2 microglia was assessed and the exosomes of M2-polarized and unpolarized BV-2 microglia were extracted. Subsequently, we labeled the exosomes with PKH67 and treated GL261 cells with them to investigate the exosome distribution. GL261 cell phenotypes and related protein expression were used to explore the role of M2 microglial exosomes in gliomas. Then a specific miR-7239-3p inhibitor was added to verify miR-7239-3p functions. Finally, the mouse subcutaneous tumorigenic model was used to verify the tumorigenic effect of M2 microglial exosomes in vivo. Our results showed that in gliomas co-cultured with M2 microglia, the expression of the BMAL1 protein was decreased (P < 0.01), while the expression of the CLOCK protein was increased (P < 0.05); opposite results were obtained in gliomas co-cultured with M1 microglia. After treatment with M2 microglial exosomes, the apoptosis of GL261 cells decreased (P < 0.001), while the viability, proliferation, and migration of GL261 cells increased. Increased expression of N-cadherin and Vimentin, and decreased E-cadherin expression occurred upon treatment with M2 microglial exosomes. Addition of an miR-7239-3p inhibitor to M2 microglial exosomes reversed these results. In summary, we found that miR-7239-3p in the glioma microenvironment is recruited to glioma cells by exosomes and inhibits Bmal1 expression. M2 microglial exosomes promote the proliferation and migration of gliomas by regulating tumor-related protein expression and reducing apoptosis.
7.International Status of the Extended Clinical Trial System and Its Enlightenment to China

OBJECTIVE:To pr ovide reference for improving extended clinical trial system in China. METHODS :The implementation experience of the international extended clinical trial system was introduced from three aspects : system development,application scope and principles ,application and approval ;the evolution and challenges of the system in China were further analyzed ,and relative suggestions were put forward. RESULTS & CONCLUSIONS :The development of international extended clinical trial system in the United States ,the United Kingdom ,Australia and the European Union had been relatively mature. The specific system had not formally been established in China ,and the implementation of the system faced many challenges,such as the ethical problems caused by drug use risk ,the sponsor dilemma caused by the difficult balance between the resistance of initiating application and the benefits ,the difficulty of review caused by the unknown responsible party of risk assessment and the urgent time of approval. It can be dealt with by the following measures :improving the existing laws and regulations,strengthen information disclosure and risk prevention work ,make clear the division of responsibilities in the process of system operation. Meanwhile ,based on the existing international model ,the management system of the expanded clinical trials in China is established. Corresponding application paths are set up for different application types such as “single patient emergency ”, “single patient non emergency ”and“two or more patients group ”,and all parties should be mobilized to supervise so as to promote the improvement and implementation of the extended clinical trial system.
8.Application value of DSC-PWI in ischemic cerebrovascular disease

Objective:To explore the application value in cerebral blood perfusion status of dynamic susceptibility contrast enhanced perfusion weighted imaging (DSC-PWI) in ischemic cerebrovascular disease (ICVD).Methods:Retrospective analysis of 31 cases ICVD head magnetic resonance imaging (MRI), diffusion weighted imaging (DWI), magnetic resonance angiography (MRA) and DSC-PWI image in clinical diagnosis, and selectively analyze parameters including relative cerebral blood flow (rCBF), relative cerebral blood volume (rCBV), mean transit time (MTT) and time to peak (TTP) for generalize its characteristics.Results:31 cases of ICVD, 16 cases hypoperfusion, rCBF decreased significantly, rCBV decreased, MTT and TTP prolonged, include 7 cases of large vessel disease and 5 cases of small vessel disease. The collateral circulation formed 9 cases, including 3 cases with good compensation, rCBF normal, rCBV normal, MTT and TTP prolonged, 6 cases with bad compensation, rCBF decreased, rCBV normal or increased, MTT and TTP prolonged. The blood reperfusion in 3 cases, rCBF normal or slightly increased, rCBV increased, MTT shortened or normal, and TTP shortened. The excessive perfusion in 3 cases, rCBF significantly increased and rCBV significantly increased, with MTT and TTP shortened.Conclusions:DSC-PWI can reliably reflect the perfusion state and collateral circulation compensation of ICVD, so as to guide the selection of clinical treatment program and significantly improve the prognosis of patients.
9.Legal Protection of the Tacit Technology Secret of Traditional Chinese Medicine

OBJECTIVE: To provide reference for the thought and operation of the law to protect the tacit technology secrets of traditional Chinese medicine (TCM). METHODS: Through the analysis of the composition, categories and legal attributes of tacit technology secrets of TCM, legal protection methods were sought from the legal perspective. RESULTS & CONCLUSIONS: In the field of TCM, tacit knowledge, represented by the TCM processing, TCM diagnosis and treatment, is mostly inherited and studied in the form of technology secrets. The tacit technology secrets of TCM have the characteristics of personality dependence and inheritance, the protection of them by law can not be achieved directly through the field of intellectual property rights, but through the protection of the interests of the inheritors. Public law and private law are indispensable to the protection of tacit technology secrets of TCM. Public law should focus on improving the mechanism of technological inheritance and protecting the rights and interests of TCM inheritors. Private law should overcome the risk of infringement by signing contracts and transforming tacit knowledge of TCM.
10.Current Situation and Consideration of TCM Time-honored Formula and Processing Technology Protection

OBJECTIVE:To provide reference for improving legal protection system of proprietary technology in TCM time-honored enterprises. METHODS:By studying the relevant literatures and data,main type of proprietary technology,formula, legal characteristics of processing technology in TCM time-honored enterprises were analyzed;the protection forms and existing problems of proprietary technology were summarized to put forward countermeasures. RESULTS & CONCLUSIONS:The most important proprietary technology of TCM time-honored brand is formula and TCM processing technology,and legal characteristics of it include practicality,value,confidentiality and teachability. The proprietary technology protection of TCM time-honored brand included various methods such as business secret,patent protection and administrative protection. But,there still are problems such as the incompatibility of patent protection,the conflict between the secret protection and information disclosure,gradually weakened means of administrative protection,weak awareness of legal protection in time-honored brand of TCM. It is suggested to use comprehensive protection means of patent system and business secret,coordinate intellectual property protection and administrative protection,establish proprietary technology protection directories and databases and incentive and constraint mechanism of internal intellectual property rights in enterprises so as to seek effective way to protect proprietary technology of TCM time-honored brand.
