1.Mesenchymal stem cells and extracellular vesicles in repair of endometrial injury

BACKGROUND:Endometrial injury is a common benign disease in women of reproductive age,which seriously affects their fertility and reproductive health.In recent years,mesenchymal stem cells and their secreted extracellular vesicles,as a new therapeutic strategy,have received much attention in the repair of endometrial injury.OBJECTIVE:To review the application of mesenchymal stem cells and their secreted extracellular vesicles in the treatment of endometrial injury,to comprehensively understand the therapeutic mechanism and efficacy of endometrial injury,and to discuss the potential and challenges of the application of mesenchymal stem cells and their secreted extracellular vesicles in the treatment of endometrial injury,in order to provide theoretical basis for further basic and clinical research.METHODS:Literature searches were conducted in the China National Knowledge Infrastructure(CNKI),Chinese Medical Full-text Database,and PubMed electronic databases using search terms"mesenchymal stem cells,extracellular vesicles derived from mesenchymal stem cells,exosome,endometrial injury,intrauterine adhesion,Asherman's syndrome"in Chinese and English.Articles were selected through manual screening to exclude duplicates and irrelevant studies,resulting in the inclusion of 87 articles.RESULTS AND CONCLUSION:Mesenchymal stem cells and their secreted extracellular vesicles can participate in endometrial damage repair through a variety of molecular mechanisms,such as anti-fibrosis,promoting angiogenesis and cell proliferation,cell homing,immune regulation,and endometrial receptibility regulation.A large number of research results have been achieved in cell and animal experiments,and preliminary results of clinical studies have also achieved certain curative effects,including increased endometrial thickness,improved menstrual volume and fertility.However,difficulty and challenges persist in the application of mesenchymal stem cells and their secreted extracellular vesicles for endometrial injury treatment.During treatment,it is necessary to enhance the quality and stability of mesenchymal stem cells and their secreted extracellular vesicles,as well as clarifying their roles in processes such as cell proliferation,migration,and differentiation.Moreover,large-scale multicenter clinical trials are needed to validate the long-term safety and efficacy of mesenchymal stem cells and their secreted extracellular vesicles in endometrial repair,and to determine optimal dosages and administration routes.Continuous advancements in scientific technologies and the auxiliary application of bioengineering materials offer hope for developing more effective and safer therapeutic methods for endometrial injury treatment and the above problems will be solved in the future.
2.Effects of Zhikang Capsules Plus Compound Huangbai Lotion for Retention Enema on Inflammatory Indicators in Patients with Mild to Moderate Distal Ulcerative Colitis

Objective To investigate the clinical efficacy of Zhikang Capsules plus Compound Huangbai Lotion for retention enema in the treatment of patients with mild to moderate distal ulcerative colitis(UC)of damp-heat in large intestine type,and to observe their effects on the serum levels of pro-inflammatory cytokines of tumor necrosis factor α(TNF-α),interleukin 6(IL-6)and C-reactive protein(CRP).Methods A total of 110 patients with mild to moderate distal UC of damp-heat in large intestine type who were treated in the Outpatient Clinics and Wards of the Third Affiliated Hospital of Henan University of Traditional Chinese Medicine from October 2022 to December 2023 were randomly divided into an observation group and a control group according to random number table method,with 55 patients in each group.The control group was given retention enema with 1 g of Xilei Powder and 50 mL of Compound Huangbai Lotion before going to bed every night,while the observation group was given retention enema with 2.4 g of Zhikang Capsules and 50 mL of Compound Huangbai Lotion before going to bed every night The course of treatment for the two groups covered 14 days.The changes in the serum inflammation factors,coagulation function indicators,traditional Chinese medicine(TCM)syndrome scores,and the Inflammatory Bowel Disease Quality of Life Questionnaire(IBDQ)scores in the two groups were observed before and after treatment.After treatment,the clinical efficacy and drug safety in the two groups were evaluated.Results(1)After 14 days of treatment,the total effective rate of the observation group was 90.91%(50/55)and that of the control group was 80.00%(44/55),and the intergroup comparison(tested by rank sum test)showed that the therapeutic efficacy of the observation group was significantly superior to that of the control group(P<0.01).(2)After treatment,the serum levels of inflammation factors of TNF-α,IL-6,and CRP patients in the two groups were decreased compared with those before treatment(P<0.05),and the decrease in the observation group was significantly superior to that in the control group(P<0.05 or P<0.01).(3)After treatment,the prothrombin time(PT)and fibrinogen(FIB)of the two groups of patients were improved compared with those before treatment(P<0.05),while the improvement of thrombin time(TT)and activated partial thromboplastin time(APTT)were not obvious when compared with those before treatment(P>0.05);the comparison between the two groups showed that the improvement of PT and FIB in the observation group was significantly superior to that in the control group(P<0.05 or P<0.01).(4)After treatment,the scores of TCM symptoms of diarrhea,mucous-purulent and bloody stool,tenesmus,scorching sensation of the anus in the two groups were decreased compared with those before treatment(P<0.05),and the decrease in the observation group was significantly superior to that in the control group(P<0.05 or P<0.01).(5)After treatment,the scores of IBDQ items of general symptoms,intestinal symptoms,social functioning,emotional role functioning in the two groups were increased compared with those before treatment(P<0.05),and the increase in the observation group was significantly superior to that in the control group(P<0.01).(6)During the treatment,no obvious adverse reactions occurred in the two groups,with higher safety.Conclusion Zhikang Capsules plus Compound Huangbai Lotion for retention enema are effective and safe in treating mild to moderate distal UC of damp-heat in large intestine type.The therapy can effectively control the illness of UC,alleviate the clinical symptoms and negative emotions,improve the inflammatory response and coagulation function,and enhance the quality of life of the patients.
3.BK virus nephropathy after allogeneic hematopoietic stem cell transplantation: a case report and literature review

A 20-year-old male patient with T-lymphoblastic lymphoma/leukemia received 9/10 human leukocyte antigen-compatible unrelated peripheral blood stem cell transplantation. He was transplanted with 5.91×10 8 mononuclear cells/kg and 2.88×10 6 CD34 + cells/kg, and neutrophil engraftment was obtained at +11 days and platelet engraftment at +9 days. After transplantation, he presented with repeatedly increased serum creatinine levels, BK virus (BKV) -associated hemorrhagic cystitis, and BKV viremia. BK virus nephropathy was diagnosed based on renal biopsy and metagenomic next-generation sequencing. After adjusting the immunosuppressant, intravenous immunoglobulin, and donor lymphocyte infusion treatment, the patient’s renal function deteriorated progressively, and he eventually died of multiple organ failure at +289 days.
4.Clinical Efficacy of Qingchi San for Mild-to-Moderate Active Ulcerative Colitis:A Retrospective Cohort Study

OBJECTIVE To retrospectively analyze the clinical efficacy of Qingchi San in the treatment of mild-to-moderate ul-cerative colitis.METHODS A retrospective analysis was conducted on 221 ulcerative colitis patients treated at the Gastroenterology Department of First Teaching Hospital of Tianjin University of Traditional Chinese Medicine from December 2014 to February 2024.Participants were divided into an observation group(n=121)and a control group(n=100).The control group received oral me-salazine,while the observation group received Qingchi San enema in addition to the control group's treatment.Both groups received a 4-week course of treatment.The two groups were compared in terms of clinical efficacy,clinical remission rate,TCM syndrome score,time to intestinal bleeding remission,and changes in inflammatory markers(C-reactive protein,erythrocyte sedimentation rate,and white blood cells)and coagulation markers(platelets and D-dimer).RESULTS After treatment,the total effective rate and clinical remission rate in the observation group were significantly higher than those in the control group(P<0.05,P<0.01).The TCM syn-drome scores for each item(diarrhea,abdominal pain,bloody stools with mucus,and tenesmus)in the observation group were signifi-cantly improved compared with those in the control group(P<0.01).The time to intestinal bleeding remission in the observation group was significantly shorter than that in the control group(P<0.01).C-reactive protein,erythrocyte sedimentation rate,and white blood cell count in the observation group were significantly decreased compared with those in the control group(P<0.05,P<0.01).There was no statistically significant difference in coagulation markers(platelets and D-dimer)between the two groups(P>0.05).No signif-icant treatment-related adverse reactions,such as liver and kidney damage,were observed during treatment.CONCLUSION Qing-chi San enema can improve the clinical efficacy of patients with mild-to-moderate ulcerative colitis,improve clinical symptoms,short-en bleeding time,and reduce inflammatory markers.

8.Evaluation of retrograde single incision laparoscopic cholecystectomy with combined suspension of ligamentum teres hepatis

Objective:To explore the feasibility and safety of retrograde single incision laparoscopic cholecystectomy (SILC) with combined suspension of ligamentum teres hepatis.Methods:A retrospective cohort study was conducted to collect clinical data of 570 cases of laparoscopic cholecystectomy (LC) admitted to the Department of Pancreatic and Bariatric Surgery of Nanjing Drum Tower Hospital from December 2021 to December 2023. Based on the operation methods, 282 cases were classified into retrograde SILC group, 288 cases were classified into three incision LC group. Observation indicators including surgical related index, and incidence of postoperative complications. The continuity data were presented as medium, and compared using Mann-Whitney U test. The comparison of count data between groups was conducted using the chi-square test or Fisher exact probability test. Results:All cases were successfully performed cholecystectomy. The duration of surgery and the proportion of changes in surgical methods in the SILC group were 45(35, 55) minutes and 12.1%, respectively, while in the three port group they were 50(40, 65) minutes and 1.4%, respectively. The difference between the two groups was statistically significant ( P<0.05). The intraoperative bleeding volume, postoperative hospitalization time, and total hospitalization cost of the SILC group were 20(16, 28) mL, 1(1, 1) day, and 13.4(12.9, 14.1) thousand yuan, respectively. The three port group was 18(16, 23) mL, 1(1, 1) day, and 13.4(12.4, 14.6) thousand yuan, respectively. There was no statistically significant difference between the two groups( P>0.05). In terms of postoperative complications, the overall incidence of SILC and three port group were 4.6% and 3.5% respectively, which did not reach significant statistically difference( P>0.05). Meanwhile, there was no statistically significant difference( P>0.05) in the incidence of biliary tract injury, postoperative bile leakage, postoperative bleeding, incision liquefaction infection, and incision hernia between the two groups. Conclusion:The retrograde single incision laproscopic retrograde cholecystectomy combined with ligamentum teres hepatis suspension can achieve satisfactory therapeutic effects without increasing the surgical duration.

Depressive disorder ranks as a major bur-den of disease worldwide,yet the current antidepressant medications are limited by frequent non-responsiveness and significant side effects.The lateral septum(LS)is thought to control of depression,however,the cellular and circuit substrates are largely unknown.Here,we identified a subpopulation of LS GABAergic adenosine A2A receptors(A2AR)-positive neurons mediating depres-sive symptoms via direct projects to the lateral habenula(LHb)and the dorsomedial hypothalamus(DMH).Activa-tion of A2AR in the LS augmented the spiking frequency of A2AR-positive neurons leading to a decreased activation of surrounding neurons and the bi-directional manipula-tion of LS-A2AR activity demonstrated that LS-A2ARs are necessary and sufficient to trigger depressive pheno-types.Thus,the optogenetic modulation(stimulation or inhibition)of LS-A2AR-positive neuronal activity or LS-A2AR-positive neurons projection terminals to the LHb or DMH,phenocopied depressive behaviors.Moreover,A2AR are upregulated in the LS in two male mouse mod-els of repeated stress-induced depression.This identifica-tion that aberrantly increased A2AR signaling in the LS is a critical upstream regulator of repeated stress-induced depressive-like behaviors provides a neurophysiological and circuit-based justification of the antidepressant poten-tial of A2AR antagonists,prompting their clinical transla-tion.
10.Evaluation of the impact of extracurricular perception on major courses and extracurricular activities

Objective:To understand the development of extracurricular activities among students majoring in preventive medicine in a medical college and explore the impact of perception of extracurricular activities on major courses and extracurricular performances.Methods:A self-made questionnaire on the extracurricular perception was conducted, involving such aspects as organizational support, teacher guidance, student participation, etc. In August 2019, all senior students from Department of Preventive Medicine of a medical college were investigated by the questionnaire. At the same time, we had the extracurricular performances of the students from the Communist Youth League Committee and the major courses performances from the Academic Affairs Section of the Department. The Pearson test was conducted on the dimensions and total score of extracurricular perception, extracurricular performance and major courses performance to verify the impact of extracurricular perception on major courses and extracurricular performances. SPSS 19.0 was used for normality test, chi-square test and correlation analysis.Results:A total of 95 valid questionnaires were distributed and collected. It was noted that the total score of students' extracurricular perception was higher (66.23 points), among which, the highest score of teacher guidance was 73.10, while the lowest score of students' participation was 61.85. Extracurricular perception had a significant impact on major courses and extracurricular activities, especially in organizational support and teacher's guidance.Conclusion:Teachers play an important role in extracurricular activities, and students' participation is influenced by students and school factors. It is suggested that the school should pay attention to extracurricular activities in the process of cultivating college students, education environment should be optimized, funds should be guaranteed, the extracurricular courses should be improved, and the management system should be improved. For the teachers, teachers' professional level should be improved. Teachers should provide helpful guidance for students. At the same time, teachers should learn to use new technologies to build a better environment for students' learning. In addition, teachers should provide students with opportunities to be involved in extracurricular activities and create platforms. Finally, students should actively participate in all kinds of activities both inside and outside the school.
