1.Therapeutic strategies targeting CD47-SIRPα signaling pathway in gastrointestinal cancers treatment.

Gastrointestinal (GI) cancers are prevalent globally, with leading incidence and mortality rates among malignant tumors. Despite notable advancements in surgical resection, radiotherapy, and chemotherapy, the overall survival rates remain low. Hence, it is imperative to explore alternative approaches that enhance patient outcomes. Cluster of differentiation 47 (CD47), serving as an early diagnostic marker, is predominantly overexpressed in GI cancers and associated with poor prognosis. Targeting the CD47-signal regulatory protein alpha (SIRPα) signaling pathway may provide a novel strategy for GI cancers treatment. This study summarizes current knowledge of the structure and function of CD47 and SIRPα, their roles in signaling pathways, the prognostic significance of CD47, therapeutic strategies targeting the CD47-SIRPα signaling pathway in GI cancer, and highlights key issues for future investigations.
2.Comparison of the short-term outcomes in very/extremely low birth weight infants supplemented with fortified donor human milk or preterm formula

Objective:To compare the short-term outcomes of very low birth weight (VLBW) and extremely low birth weight (ELBW) infants supplementarily fed with fortified donor human milk (DHM) or preterm formula (PF) when the mother's own milk (MOM) is insufficient.Methods:This retrospective cohort study included 91 VLBW or ELBW preterm infants with birth weight<1 500 g who were hospitalized in Peking Union Medical College Hospital from October 1, 2017, to September 30, 2020. Based on the supplemental feeding method when MOM was insufficient, these infants were divided into the DHM group ( n=51) and PF group ( n=40). Mann-Whitney U, t-test, Chi-square test, or Fisher's exact test were used to compare the short-term clinical outcomes during hospitalization between the two groups. Results:(1) There were no statistically significant differences between the 91 preterm infants in the DHM group and PF group in their gestational age, birth weight, sex ratio, birth mode, mothers' age at delivery, or the proportion of infants of small gestational age (all P>0.05). (2) The feeding volume in the DHM group was significantly greater than that in the PF group on the 14th day after birth [(108.2±53.1) vs. (81.0±47.8) ml/(kg·d), t=0.78, P=0.020]. Moreover, the time to achieve the feeding amounts up to 120 ml/(kg·d) and 150 ml/(kg·d) for infants in the DHM group were significantly shorter than those in the PF group [(17.5±10.2) vs. (30.0±12.0) d, t=4.38; (22.1±13.3) vs. (32.3±11.9) d, t=0.02; both P<0.05]; (3) Lower proportion of peripherally inserted central catheter (PICC) [58.8% (30/51) vs. 100% (40/40), χ 2=21.88, P<0.001] and shorter PICC duration were observed in the DHM group [10.0 (0.0-19.0) vs. 29.0 (17.0-40.5) d, Z=5.56, P<0.001] compared to the PF group. The times of red blood cell transfusions and the incidence of late sepsis in the DHM group were less than those in the PF group [0.0 (0.0-2.0) vs. 2.0 (1.0-3.0) times, Z=4.44, P<0.001; 23.5% (12/51) vs. 50.0% (20/40), χ 2=6.39, P=0.011]. There were no statistically significant differences observed in the incidence of bronchopulmonary dysplasia, neonatal necrotizing enterocolitis, retinopathy of prematurity, and the length of hospitalization (all P>0.05). Conclusion:When MOM is insufficient, supplementing VLBW and ELBW infants with fortified donor human milk can shorten the time to achieve enteral nutrition and reduce the use rate and time of PICC, the incidence of late-onset sepsis, and the times of red blood cell transfusion.

Helicobacter pylori(Hp)is a common Gram-negative bacillus causing gastrointestinal infec-tions.It mainly exists on the surface of gastric epithelial cells and in mucus and is associated with gastric ulcers,gastric cancer,and gastric mucosa-associated lymphomas.Studies have shown that Hp can induce or exacerbate certain extragastric diseases and is associated with the occurrence of coronavirus disease 2019.It is hypothesized that Hp may be indirectly or directly involved in the occurrence and development of diseases by stimulating the production of inflammatory cytokines or inducing cross-immune reactions.In addition,Hp can enter Candida to release toxins continuously and play a role in escaping the recognition of the host immune system and the bacteri-cidal effect of drugs.This article reviews the research progress in Hp-associated extragastric diseases in recent years,aiming to draw the attention of clinical workers to Hp-associated extragastric diseases and enrich the knowledge about Hp infection for formulating countermeasures to avoid the aggravation or triggering of other disea-ses by Hp.
4.Graft-versus-host disease accompanied with new T-cell receptor genes clonal rearrangement after allogeneic hematopoietic stem cell transplantation: report of 2 cases and review of literature

Objective:To investigate the clinical significance of graft-versus host disease (GVHD)accompanied with new T-cell receptor (TCR) genes clonal rearrangement after allogeneic hematopoietic stem cell transplantation (allo-HSCT).Methods:The clinical data of 2 patients admitted to People's Hospital of Henan University from December 2018 to March 2020 who developed GVHD after allo-HSCT accompanied with TCR genes clonal rearrangement were retrospectively analyzed, and the related literatures were reviewed.Results:Patient 1 was diagnosed with peripheral T-cell lymphoma non-specific type (PTCL-NOS), and then developed severe acute GVHD (aGVHD) after identical sibling allo-HSCT, and gradually developed liver chronic GVHD (cGVHD), skin cGVHD and new TCR genes clonal rearrangement. Patient 2 was diagnosed with acute myeloid leukemia (AML)-M 4, and severe aGVHD, hepatic cGVHD, and clonal rearrangement of TCR genes were gradually detected after identical sibling allo-HSCT. Conclusions:The TCR genes clonal rearrangement after allo-HSCT is not necessarily suggestive of tumors, and it may be related to lymphocyte development disorder caused by GVHD, so the comprehensive judgement should be carefully made.

TSCI have dyskinesia and sensory disturbance that can cause various life-threaten complications. The patients with traumatic spinal cord injury (TSCI), seriously affecting the quality of life of patients. Based on the epidemiology of TSCI and domestic and foreign literatures as well as expert investigations, this expert consensus reviews the definition, injury classification, rehabilitation assessment, rehabilitation strategies and rehabilitation measures of TSCI so as to provide early standardized rehabilitation treatment methods for TSCI.
7.Clinical characteristics and etiology analysis of 176 children with liver dysfunction

Objective:To investigate the clinical features and etiology of 176 children with liver dysfunction, and to provide reference for the diagnosis and treatment of children with liver dysfunction.Methods:From April 2017 to February 2019, 176 children with liver function abnormalities from 2 months to 13 years old who were admitted to Hangzhou Children's Hospital were selected inthe study.The pathogens of the children's secretions were detected by fluorescence quantitative method.The blood was tested for hepatitis B virus, hepatitis C antibody and hepatitis A antibody, and blood culture method was used to detect and identify bacteria, and blood genetic metabolism and coagulation function were also screened.According to the age of the children, they were divided into infant group, toddlergroup, preschool group and school age group.Results:Among the 176 children, the infants had the most abnormal liver function, 93 cases(52.84%), followed by 43 children (24.43%) in the toddler group.Causes of the disease: 138 cases (78.41%) of the original diseases, 8 cases (4.55%) of non-infectious diseases, 30 cases (17.04%) of the undetected pathogens.Among the 138 cases with liver function abnormality due to infectious diseases, 123 cases (89.13%) were infected with virus, 8 cases (5.80%) were infected with bacteria, 6 cases (4.35%) were infected with mycoplasma, and 1 case (0.72%) was infected with Toxoplasma gondii.Among the 123 cases of liver dysfunction caused by viral infection, cytomegalovirus was detected in 62 cases (50.41%), followed by EB virus in 38 cases (30.89%), rotavirus in 10 cases (8.13%), and EV71 virus in 5 cases (4.07%), 4 cases of common enterovirus (3.25%), 2 cases of respiratory syncytial virus (1.63%), 1 case of herpes simplex virus and Coxsackie A16 virus (0.81%). Pre-school and above were mainly infected with EB virus; the age>1 year-old liver function abnormality was 37.10% (23/62) due to cytomegalovirus, and the proportion of EB virus infection was 92.11% (35/38). Compared with 62.90% and 7.89% of age 2 months to 1 year, the difference was statistically significant (χ 2=29.27, P=0.00). Eightcases of liver infection caused by bacterial infection: 3 cases of Staphylococcus aureus, 2 cases of Streptococcus pneumoniae, 2 cases of Escherichia coli, and 1 case of Pseudomonas aeruginosa.There were 3 cases of genetic metabolic diseases, including 2 cases of hepatolenticular degeneration and 1 case of glycogen accumulation.Characteristics of liver function abnormalities in children: the main clinical manifestations of children were jaundice, splenomegaly and hepatomegaly, cough, diarrhea, etc.Outcome: 98 cases (55.68%) were healed, 60 cases (34.09%) were improved, and 17 cases (9.66%) were aggravated, 1 case of death (0.57%). Conclusion:The abnormal liver function of infants was mainly caused by virus infection, especially cytomegalovirus.Infants and young children have no obvious symptoms in the early stage of the disease, severe cases can be manifested as jaundice and hepatosplenomegaly, mainly mild, with a good prognosis.
8.Questionnaire investigation on occurrence post-traumatic stress disorder and sleep quality of nurses in emergency department

Objective To investigate the relationship between post-traumatic stress disorder (PTSD) and sleep quality among emergency nurses, as well as the regulatory effects of gender on this relationship. Methods Two hundred and forty eight nurses in departments emergency of 8 tertiary general hospitals in Zhejiang province from October 2017 to February 2018 were enrolled as the respondents including 62 males and 186 females. The differences in sleep quality and PTSD among nurses of different genders were analyzed by questionnaires; the PTSD checklist and Pittsburgh sleep quality index (PSQI) were used to evaluate PTSD and sleep quality situations of different gender nurses; the correlations of PTSD, PSQI total score and each dimension score were analyzed by Pearson correlation analysis; and the regulatory role of gender on PTSD and PSQI score were analyzed by multiple linear regression method. Results The survey showed that the positive rate of sleep disorder was 46.77% (116/248) and the positive rate of PTSD was 43.95% (109/249) in emergency nurses. The total scores of PTSD and each score of following dimensions:re-experience symptoms, avoidance symptoms, high alert symptoms, and the total scores of PSQI and each score of dimensions of sleep quality, sleep entering time, sleep time, sleep efficiency, sleep disorders and using hypnotics of male nurses were significantly lower than those of the female nurses (total scores of PTSD: 33.94±9.36 vs. 38.05±5.69, re-experience symptoms: 9.43±2.62 vs. 10.40±2.39, avoidance symptoms: 14.85±4.43 vs. 16.54±3.69, high alertness symptoms: 9.66±2.35 vs. 11.04±2.49; total scores of PSQI: 7.39±1.42 vs. 8.32±3.52, sleep quality:0.91±0.42 vs. 1.08±0.49, sleep time: 1.82±0.77 vs. 1.69±0.69, sleep efficiency: 0.85±0.44 vs. 1.07±0.45, sleep disorders: 1.08±0.29 vs. 1.33±0.56, hypnotics: 0.23±0.08 vs. 0.46±0.12, all P < 0.05). Pearson correlation analysis showed that re-experience symptoms of female nurses had no significant correlations with sleep time and sleep efficiency (all P >0.05), but had significant positive correlations with other dimensions of PSQI (all P < 0.05), and the correlation coefficient was 0.22 - 0.64; in the aspect of male nurses, re-experience symptoms and hypnotic drugs, as well as high alert symptoms and sleep disorders, time function were of no correlations (all P > 0.05), but with other PSQI dimensions showed significant positive correlations (all P < 0.05), and the correlation coefficient was 0.11 - 0.43. The multiple linear regression analysis showed that the re-experiencing symptoms, avoidance symptoms and high alertness symptoms were positive predictive PSQI scores (β= 0.840, 0.970, 0.500, P = 0.026, 0.012, 0.000); the relationship between re-experiencing symptoms, high alertness symptoms and scores of PSQI could be adjusted by gender (β= 0.950, 0.290, P = 0.003, 0.032), the re-experiencing symptoms and high alertness symptoms could positively predict the PSQI scores of female nurses (β= 0.440, 0.570, P = 0.017, 0.001), and were slightly worse in predicting the PSQI scores of male nurses (β= 0.390, 0.110, P = 0.004, 0.048). Conclusion The predictive effect of PTSD different dimensions on PSQI score varies with difference in gender, and compared with male emergency nurses, the stress state of female emergency nurses is more easily to reduce sleep quality.
9.Effects of high-fat diet on liver function and intestinal bacterial community in rats

Objective To investigate the influence of high-fat diet on liver function and intestinal bacte-rial community through building rat models. Methods 20 rats of 21 days old were divided into two groups ran-domly as normal diet group fed with standard chow diet and high-fat group fed with high-fat diet. After 6 weeks, feces of rats in both groups were obtained for 16S rRNA high-through sequencing of the intestinal bacterial com-munity. Results After 6 weeks high-fat diet, total protein (TP) (55. 79±3. 75, P=0. 002), globin (GLB) ( 34. 9±2. 53, P<0. 001), albumin (ALB) /GLB (. 60±0. 02, P<0. 001), alkaline phosphatase (ALP) (373. 80±63. 05, P<0. 001), total cholesterol (TC) (1. 94±0. 23, P<0. 001), low density lipoprotein (LDL) (0. 76±0. 93, P<0. 001), LDL/high density lipoprotein (HDL) (1. 43±0. 22, P<0. 001), and tri-glyceride (TG) (1. 48±0. 50, P=0. 015) increased compared with the normal diet group. Additionally, intes-tinal bacterial diversity and evenness decreased significantly. The dominant bacteria were Bacteroidetes, Firmi-cutes, and Proteobacteria, with averaged relative abundances as 56. 36%, 35. 31%, and 6. 61%, respectively. The relative abundances of Bacteroidetes deceased (P=0. 007), those of Firmicutes increased (P=0. 020), and those of Proteobacteria were kept stable (P=0. 928) after a 6-week high-fat diet. Furthermore, the intesti-nal bacterial community structure changed distinctly between the two groups by 16s rRNA high-through sequen-cing. Conclusion High-fat diet can lead to change of intestinal bacterial community structure and further result in liver function damnification as well as obesity.
10.Incidence and impact of frailty on falls of patients with maintenance hemodialysis

Objective To investigate the incidence of frailty and its impact on falls of patients with maintenance hemodialysis (MHD). Methods 186 MHD patients meeting the criteria were recruited.The patient's general clinical data were collected.Frailty was assessed using a frailty questionnaire in all participants with prospective study. Results 70 patients contracted frailty,with the incidence of 37.6%.56 patients happened to fall,with the incidence of 30.1%.Logistic multivariate regression analysis confirmed that diabetes,peripheral vascular disease,IDH and frailty were independent risk factors of the falls.Conclusions MHD patients are at a high risk of falls and frailty.Frailty is an independent factor for predicting falls,which is of great significance for identifying patients with high risk of falls.
