BACKGROUND:There is still no consensus on the optimal internal fixation for the treatment of Pauwels Ⅲ femoral neck fracture, and most of the related finite element analyses have been performed using a single simplified loading condition, and the biomechanical properties of commonly used internal fixation devices need to be further investigated. OBJECTIVE:To analyze the biomechanical characteristics of Pauwels Ⅲ femoral neck fractures treated with cannulated compression screw, dynamic hip screw, and femoral neck system by finite element method under different loading conditions of single-leg standing loads and sideways fall loads. METHODS:The DICOM data of healthy adult femur were obtained by CT scanning, imported into Mimics 15.0 software to obtain the rough model of bone tissue. The data exported from Mimics were optimized by Geomagics software, and then three internal fixation models were built and assembled with the femur model according to the parameters of the clinical application of the cannulated compression screw, dynamic hip screw, and femoral neck system by using Pro/E software. Finally, the three internal fixation models were imported into Ansys software for loading and calculation to analyze the stress distribution and displacement of the femur and the internal fixation under different working conditions of single-leg standing loads and sideways fall loads,as well as the stress characteristics of the calcar femorale and Ward's triangle. RESULTS AND CONCLUSION:(1)Under the single-leg standing load and the sideways fall load, the proximal femoral stress of the three internal fixation models was mainly distributed above the fracture end of the femoral neck. The peak stress of the proximal femoral end, fracture end,Ward triangle, and calcar femorale of the three internal fixation models were the smallest in the femoral neck system model and the largest in the cannulated compression screw model.(2)Under the single-leg standing load and the sideways fall load, the peak displacement of the proximal femur of the three internal fixation models was all located at the top of the femoral head, and the peak displacement was the smallest in the femoral neck system model and the largest in the cannulated compression screw model.(3)The peak displacement of the three internal fixation models was all located at the top of the internal fixation device under the single-leg standing and sideways fall loading conditions, and the peak displacement values were the smallest in the femoral neck system internal fixation model and the largest in the cannulated compression screw internal fixation model.(4)The internal fixation stress of the three internal fixation models was mainly distributed in the area near the fracture end of the internal fixation device under the single-leg standing and sideways fall loads, and the peak value of internal fixation stress was the smallest in the femoral neck system model and the largest in the cannulated compression screw model.(5)These results suggest that the mechanical stability of the femoral neck system is the best, but there may be a risk of stress shielding of the fracture end and calcar femorale. The stress of the internal fixation device of the femoral neck system is more dispersed, and the risk of internal fixation break is lower.

Objective To investigate the rate of greater omentum metastasis in gastric cancer(GC). Methods General informations of patients with GC who underwent radical gastrectomy at Shanghai Ruijin Hospital in May 2020 were collected, and their clinicopathological characteristics were analyzed to find risk factors of greater omentum metastasis. Recurrence and survival were also assessed. Results A total of 59 patients with GC were included in the study, of which 2(3.4%) had greater omentum metastasis. One patient presented a pathological stage of pT4aN3bM0 and another ypT4bN1M0. The 3-year overall survival rate of patients in the study was 87.9%. Conclusions The rate of greater omentum metastasis was relatively low, and patients with greater omentum metastasis had an more advanced pathological stage. To further validate this clinical issue, a prospective randomized controlled clinical study should be conducted between radical gastrectomy with omentectomy and omentum-preserving radical gastrectomy.

Objective To explore the biomechanical difference between bilateral pedicle screws fixation and unilateral pedicle screw combined with contralateral translaminar facet screw fixation under two endo-scopic approaches(interlaminar approach and transforaminal approach)through finite element analysis.Meth-ods The fusion model of simple fusion device(CAGEF model),interlaminar approach and transforaminal ap-proach models for bilateral pedicle screw fixation(BPSP model and BPSL model),and interlaminar approach and transforaminal approach for unilateral pedicle screw fixation combined with contralateral translaminar fac-et screw fixation(TLFSP model and TLFSL model)were established. The peak value of spinal displacement, maximum stress of the intervertebral fusion device, maximum stress of the internal fixation device, and maxi-mum stress of the L3 and L4 intervertebral disc of each model under six motion states:forward flexion,back-ward extension,left/right flexion, and left/right rotation were compared among the models. Results Com-pared with the CAGEF model, the peak spinal displacement and the maximum stress of the interbody fusion cage in the other four models were significantly reduced, and the values of the four fixed models were close to each other. The maximum stress of the internal fixation device in the three motion states of left flexion, left rotation, and right rotation in the BPSP model was greater than that in the BPSL model, while the maximum stress of the internal fixation device in TLFSP model was less than that in the TLFSL model only under the forward flexion motion state.Compared with the TLFSP model and TLFSL model, the maximum stress of the internal fixation devices in the BPSP model and BPSL model was greater only in the two motion states of flex-ion and extension, while which in the other four cases was smaller.Except for the maximum stress on the L3 and L4 intervertebral disc during rotation in the CAGEF model was higher than the other 4 models, the maxi-mum stress on the L3 and L4 intervertebral disc was similar in all other models under other motion states. Conclusion Under both interlaminar and foraminal approaches, the bilateral pedicle screws fixation and uni-lateral pedicle screw combined with contralateral translaminar facet screw fixation are both effective minimally invasive spinal fusion operations, and their biomechanical conditions are satisfactory. If choosing the interlami-nar approach,it is recommended to perform bilateral pedicle screw fixation, meanwhile decreasing the forward flexion and backward extension movements.Alternatively,a unilateral pedicle screw combined with contralat-eral interlaminar screw fixation can also be considered, while reducing left/right flexion movements.Howev-er,the specific treatment choice still needs to consider the patient's personalized treatment and the surgical operation ability of the clinical physician.

This paper summarizes Li Fei's academic insights and clinical experience in treating intractable facial paralysis.Li Fei posits that prolonged illness inevitably leads to deficiency and stasis, and thus, the treatment of intractable facial paralysis should first focus on identifying the etiology and syndrome differentiation, resolving stasis and unblocking stagnation, and regulating qi and blood, with particular emphasis on the relaxation of the meridian sinew system. The meridian sinew system, affiliated with the meridian and collateral, serves as the framework through which qi and blood nourish muscles, tendons, and joints, playing a crucial role in facial paralysis treatment.Guided by the theory of the meridian sinew system,Li Fei employs syndrome-based treatment, integrating the anatomy of facial expression muscles. His approach includes needle-knife therapy to release adhesions and break stasis, acupuncture to harmonize qi and blood, and intradermal needle therapy for sustained stimulation.Through these methods, the meridian sinew system is relaxed,qi and blood are regulated, and facial muscles are nourished, leading to gradual recovery from facial paralysis.

Objective To explore the molecular mechanism of miR-16-5p promoting apoptosis and inflammatory response by targeting GPR30 expression in an in vitro ischemic stroke microglia model(BV-2).Methods An ischemic stroke cell model was established by subjecting BV-2 cells to oxygen-glucose deprivation(OGD).qRT-PCR was utilized to assess the levels of miR-16-5p and GPR30 mRNA in OGD cells.A miR-16-5p inhibitor was transfected into OGD cells to silence miR-16-5p expression, and alterations in inflammatory response and apoptosis were measured using ELISA kits and Annexin V-FITC/PI staining.Starbase was employed to predict interactions, and dual-luciferase reporter gene assays were conducted to confirm that miR-16-5p targets the 3'-untranslated region(UTR)sequence of GPR30.Changes in cellular inflammatory response and apoptosis were evaluated by knocking down miR-16-5p and/or GPR30 in OGD cells. Results miR-16-5p expression was significantly elevated(P<0.01),while GPR30 expression was notably decreased(P<0.01)in OGD-induced cells.Knockdown of miR-16-5p reduced the expression levels of inflammatory factors and the cell apoptosis ratio(P<0.01).Inhibition of miR-16-5p expression led to an upregulation of GPR30 mRNA and protein levels(P<0.01).Simultaneous silencing of both miR-16-5p and GPR30 partially enhanced inflammatory factor expression levels and the cell apoptosis ratio compared to cells transfected solely with the miR-16-5p inhibitor(P<0.05).Conclusion In the microglia OGD model,miR-16-5p triggers inflammatory responses and enhances apoptosis by inhibiting GPR30 expression.
