Objective To explore an artificial intelligence (AI)-based method for automated hand hygiene monitoring and to compare the effectiveness of three algorithms (UniFormerV2, TDN, C3D) in recognizing hand hygiene steps in surgical settings, thereby aiding hospital infection control. Methods From April to October 2024, we non-invasively collected 641 video recordings of healthcare staff performing hand hygiene at four-bay scrub sinks in two tertiary hospitals using overhead HD cameras. The dataset was annotated by five trained experts for model training and validation. Results Following training on 385 samples, internal validation (n=119) showed the C3D model achieved 81% accuracy, 87% recall, and an 83% F1-score. The TDN model achieved 93%, 91%, and 92% for the same metrics. The UniFormerV2 model outperformed both, with an accuracy, recall, and F1-score of 93%—an improvement of over 10 percentage points compared to traditional CNNs (TDN, C3D). It also achieved an 84% accuracy in external validation, demonstrating strong generalization. Conclusion The UniFormerV2 model is more accurate than CNN-based models for hand hygiene step recognition and shows robust performance in external validation. It presents a viable tool for healthcare facilities to enhance hand hygiene management, ultimately improving medical quality and patient safety.

ObjectiveThis paper aims to investigate and evaluate the staged efficacy and safety of the representative empirical prescription of the “Zhu Ke Jiao” theory, Qijia Rougan prescription, combined with entecavir in the treatment of hepatic fibrosis in chronic hepatitis B. MethodsA multicenter randomized controlled clinical study was conducted, and 101 patients diagnosed with chronic hepatitis B-related hepatic fibrosis (CHB-HF) who met the diagnosis and inclusion criteria were randomly assigned to an observation group (Qijia Rougan prescription + entecavir) and a control group (entecavir). The treatment duration was 24 weeks. Liver stiffness measurement (LSM), fibrosis-4 index (FIB-4), portal vein diameter, hepatitis B serology, biochemical indicators, hepatic fibrosis markers in serum [hyaluronic acid (HA), laminin (LN), procollagen Ⅲ peptide (PⅢP), and type Ⅳ collagen (Ⅳ-C)], and traditional Chinese medicine syndrome scores were used as efficacy evaluation indicators. Efficacy assessments and explorations of different staged subgroups of Qijia Rougan prescription were conducted according to LSM values based on the Metavir pathological staging standard. ResultsA total of 98 cases were included for statistical analysis, with 49 cases in the observation group and 49 in the control group. The general data of the patients in both groups were comparable. Compared with the same group before treatment, the observation group showed a significant reduction in LSM and FIB-4 (P<0.01), as well as notable improvements in LN, Ⅳ-C, and various TCM syndrome scores (P<0.05, P<0.01). When compared to the control group after treatment, the observation group demonstrated significant improvements in LSM, FIB-4, and various TCM syndrome score indicators (P<0.05, P<0.01), indicating that the observation group performed better than the control group. Subgroup analysis of the regression of hepatic fibrosis stages showed that compared to the same group before treatment, the observation group had better improvement in regression of stages F2 and F3 (P<0.05). When compared to the control group after treatment, the observation group exhibited superior improvement in regression of stage F3 (P<0.05). No adverse events occurred in either group during the treatment period. ConclusionCompared with entecavir alone, the combination of Qijia Rougan prescription and entecavir significantly improves the degree of hepatic fibrosis and clinical TCM symptoms in patients. The optimal intervention period is primarily during stage F3, which is a potential “interception” point of the “Zhu Ke Jiao” theory.

ObjectiveTo evaluate the public health governance capacity in Jiangsu Province and provide an optimized pathway for the construction of a “strong, rich, beautiful, and high-quality” new Jiangsu. MethodsA total of 806 policy documents, 658 public information reports, and 148 research literatures related to public health governance capacity in Jiangsu Province from January 1995 to December 2023 were collected. The status of current public health goverance was assessed based on the evaluation criteria suitable for public health systems, and the strengths and the weaknesses of the system were identified. ResultsThe public health governance capability of Jiangsu Province was scored at 738.3 points, ranking 3rd nationally. Maternal health care and emergency response capacities achieved leading positions nationwide, both ranking 2nd. Jiangsu had exhibited a standardized guidance in the strategic level, a well-established management mechanism, an extensive coverage in information collection, and a scientifically established health targets setting. However, bottlenecks remained, including an unclear division of responsibilities across organizational departments, an insufficient public-health workforce, the absence of a stable growth mechanism for government funding investment, and difficulties in promptly identifying public needs. ConclusionJiangsu’s public-health system demonstrates leading nationally, yet several components remain underdeveloped. Future efforts should consolidate advantages while addressing weaknesses, further diversify content and forms, establish a stable funding increase mechanism, and clarify departmental functions, thereby providing solid health support for realizing the developmental goals of a “strong, rich, beautiful and high-quality” new Jiangsu.

Journal of China Pharmaceutical University 2026;57(2):233-239
Icodextrin peritoneal dialysis solution may produce cytotoxic α-dicarbonyl degradation products during heat sterilization, which must be monitored and controlled. The study established an o-phenylenediamine (OPD) derivatization HPLC-MS/MS method for the detection of these degradation products, enabling qualitative and quantitative analysis of α-dicarbonyl degradation products in icodextrin peritoneal dialysis solution. The results indicated that the main α-dicarbonyl degradation products in icodextrin peritoneal dialysis solution are 3-deoxyglucosone (3-DG), 3-deoxygalactosone (3-DGal), 4-deoxyglucosone (4-DG), and 3,4-dideoxyglucosone-3-ene (3,4-DGE), along with two monocarbonyl degradation products, furfural and 5-hydroxymethylfurfural. The quantitative method for 3-DG, 3-DGal, 3,4-DGE and their structural analog, glucosone, was validated. 3-DG, 3-DGal, and glucosone exhibited good linear relationships within the range of 5-150 ng/mL, while 3,4-DGE showed good linearity in the range of 1-150 ng/mL. The spiked recovery rates for all compounds were between 86.8% and 100.0%. The detection limits for glucosone, 3-DG, and 3-DGal were approximately 2.4 ng/mL, and approximately 0.5 ng/mL for 3,4-DGE. The method established in this study can accurately determine α-dicarbonyl degradation products in icodextrin peritoneal dialysis solution, providing an important basis for the quality control.

Journal of Clinical Neurology 2026;22(2):212-220
Background:
and Purpose Live cell-based assays (LCBA) are increasingly used for serological antibody detection due to their ability to preserve antigen conformation, offering moderately higher sensitivity than fixed cell-based assays. However, the clinical necessity of prioritizing LCBA for the detection of aquaporin-4 immunoglobulin G (AQP4-IgG) in neuromyelitis optica spectrum disorder (NMOSD) remains unclear, especially when compared to its established role in diagnosing myelin oligodendrocyte glycoprotein antibody-associated disease.
Methods:
We compared the performance of live cell-based assays using immunofluorescence (LCBA-IF) and fixed cell-based assays using immunofluorescence (FCBA-IF) in detecting AQP4-IgG in 90 cases of NMOSD meeting 2015 International Panel for Neuromyelitis Optica Diagnosis criteria, alongside 40 controls. Additionally, we further investigated the relationship between AQP4-IgG titers as measured by LCBA-IF and FCBA-IF and clinical parameters in NMOSD patients.
Results:
Results showed 96.9% agreement between LCBA-IF and FCBA-IF (Cohen’s κ=0.935), with a strong Spearman correlation (0.977, p<0.001). Both methods demonstrated 100% specificity, with LCBA-IF showing slightly higher sensitivity compared to FCBA-IF. Within LCBAIF-tested groups, statistically significant differences in annualized relapse rates were observed across all pairwise comparisons (low-titer vs. moderate-titer, low-titer vs. high-titer, and moderate-titer vs. high-titer; all p<0.050). However, this association reached statistical significance in some FCBA-IF-tested groups.
Conclusions
Overall, there is a strong concordance between LCBA-IF and FCBA-IF in detecting AQP4-IgG, where LCBA-IF shows slightly higher sensitivity. Furthermore, there is a potential link between elevated AQP4-IgG titers and an increased risk of relapse, and this correlation may appear more pronounced when using LCBA-IF.

Purpose:
This study aimed to evaluate the safety and efficacy of gemcitabine and cisplatin (GP) regimen in combination with immune checkpoint inhibitor sintilimab as neoadjuvant therapy for muscle-invasive bladder cancer (MIBC) patients and the feasibility of the following selective bladder sparing surgery.
Materials and Methods:
Patients with histopathologically confirmed urothelial carcinoma without distant metastases (T2-4a, N ≤ 1, M0, American Joint Committee of Cancer 8th) and with adequate organ function will be enrolled. The therapeutic regimen was sintilimab 200 mg once on day 8, gemcitabine 1,000 mg/m2 and cisplatin 35 mg/m2 once on days 1 and 8, every 21 days for four cycles. The primary endpoint was pathologic complete response (pCR, pT0N0) rate. The secondary end points were ypT < 2 rate, R0 resection rate, event-free survival, and safety.
Results:
From May 4, 2020, to May 20, 2023, 55 patients were enrolled. Forty-six patients were evaluated for efficacy. Among the 42 patients who underwent surgery, 16 patients (38.0%) achieved pCR. Thirty-three patients (78.6%) achieved pT < 2. With a median follow-up of 15.7 months, the 1-year event-free survival was 91.3%. Notwithstanding the poor pathological baseline characteristic of a high T3-T4a proportion (39.1%), a promising bladder preservation (including 22 patients transurethral resection of bladder tumor, 5 patients partial cystectomy, and 4 surveillances) rate was achieved (67.4%). The most common grade ≥ 3 treatment-related adverse events was neutropenia (n=15, 27.3%), which was related to chemotherapy. There were no grade 3 immune-related adverse events.
Conclusion
Neoadjuvant GP plus sintilimab is a promising regimen for MIBC patients, with relatively high pT < 2 rate and triggering the emerging roles for the multi-disciplinary team decision-making for bladder sparing surgery.

Chinese Journal of School Health 2026;47(6):869-872
Objective:
To investigate the heritability of axial length (AL) by comparing the consistency of AL in monozygotic (MZ) and dizygotic (DZ) twins, thereby providing data support for research on the genetic mechanisms and prevention and control practices of myopia in children and adolescents.
Methods:
A total of 137 twin pairs aged 3 to 16 years who attended the Optometry Center of the First Affiliated Hospital of Anhui Medical University from January 2021 to December 2024 were enrolled, comprising 79 DZ pairs and 58 MZ pairs. Biometry was used to measure children s axial length, and structural equation modeling within the OpenMx package was applied to calculate the heritability of axial length.
Results:
The overall mean axial length of the twin cohort was (23.7±1.5) mm. The intraclass correlation coefficient (ICC) for axial length in all twins was 0.82 (95% CI =0.76-0.87) ( P < 0.05 ). The ICC for MZ twins was 0.97 (95% CI =0.94-0.98), which was higher than that for DZ twins ( ICC =0.71, 95% CI =0.58-0.80) (both P <0.05). Maximum likelihood estimation using OpenMx revealed that the additive genetic plus unique environmental (AE) model provided the best fit. Specifically, additive genetic factors (A) accounted for 93.63% (95% CI =90.31%-95.69%) of the total variance, while unique environmental factors (E) accounted for 6.37% (95% CI =4.31%-9.69%).
Conclusion
Axial length in children is predominantly influenced by genetic factors, while unique environmental factors also play a contributory role.

Objective To understand mosquito density, species composition, and seasonal variations around the Beijing Capital International Airport(BCIA)and provide scientific evidence for mosquito-borne disease prevention and control. Methods Meteorological data were collected from 2016 to 2022, and mosquito density was monitored at seven surveillance sites around the BCIA from May to October each year using CO2-baited mosquito traps to analyze the relationship between mosquito density and meteorological factors. Results In total,68 518 female mosquitoes were captured, with a mosquito density of 10.20 per light · hour. The dominant species was Culex pipiens pallens(73.90%), followed by Aedes albopictus(9.71%)and Ae. vexans(9.29%);the mosquito density was highest in 2016(14.8 per light·hour)and lowest in 2022(4.46 per light·hour). Significant statistical differences were observed in mosquito density among different species(F=18.118, P<0.05). The density of Cx. pipiens pallens and other mosquito species showed a mutually exclusive trend; the peak of mosquito density varied based on years and habitats, with significant statistical differences in mosquito density among different habitats(F=8.504, P<0.05). The highest mosquito density was observed near the Wenyu River(21.50 per light·hour), whereas the lowest was recorded at the BCIA construction site(2.44 per light· hour). The monthly average temperature(r=0.595), monthly average highest temperature(r=0.575), and monthly average lowest temperature(r=0.624)showed moderate positive correlations, whereas the monthly average air pressure(r=-0.484)showed a moderate negative correlation. The monthly average minimum temperature was included in the regression model(F=25.575, P<0.000), and the equation Y=1.029X4-8.181 was used. The monthly average air pressure 2 months prior(b=-2.418, β=-1.619, P<0.05)and the monthly average relative humidity(b=-0.739, β=-1.201, P<0.05)significantly negatively predicted the mosquito density. Notably, the regression equation used was y=2526.170+(-2.418)X9+(-0.739)X13. The exposure-response analysis revealed that the density of mosquitoes was not linearly related to the average monthly air pressure, rainfall, and duration of sunshine. Conclusions The dominant mosquito species around the BCIA was Cx. pipiens pallens, Ae. albopictus, and Ae. vexans. During the control of Cx. pipiens pallens, the impact on other mosquito species must also be considered. Environmental management, breeding sites, and scientific use of pesticides should be prioritized based on the activity periods of mosquitoes. Further studies on mosquitoes and meteorological factors should be conducted to provide references and novel avenues for mosquito control.

Chinese Journal of Experimental Traditional Medical Formulae 2026;32(23):34-46
ObjectiveTo investigate the mechanism by which Sini San (SNS) treats liver fibrosis through regulating the polarization of intrahepatic Kupffer cells. MethodsSixty SPF-grade male Balb/c mice were randomized into blank control, model, silymarin (0.2 g·kg-1), and low-, medium-, and high-dose (2.93, 5.85, 11.70 g·kg-1, respectively) SNS groups. A mouse model of liver fibrosis was induced by intraperitoneal injection of CCl4. After 5 weeks of administration, the body mass, liver pathological changes [Hematoxylin and Eosin (HE), Masson, and Sirius red staining], serum biochemical indicators [alkaline phosphatase (ALP), aspartate aminotransferase (AST), and alanine aminotransferase (ALT)], liver fibrosis markers [procollagen type Ⅲ (PCⅢ), laminin (LN), and hyaluronic acid (HA)], inflammatory cytokines [interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), and interleukin-10 (IL-10)], and fibrosis-related proteins [α-smooth muscle actin (α-SMA), collagen type Ⅰ (Col Ⅰ), and collagen type Ⅲ (Col Ⅲ)] were assessed. A co-culture system of THP-1 cells (M1 macrophages) and human hepatic stellate cells (LX-2) was established. The cell experiment was designed with blank control (LX-2 alone), model (LX-2 + M1 macrophages), and low-, medium-, and high-dose SNS (co-culture with 5%, 10%, or 20% SNS-containing serum, respectively) groups. Cell viability, inflammatory cytokines, and macrophage phenotype were examined. Human primary hepatic Kupffer cells were allocated into blank control, M1 (LPS 100 μg·L-1+IFN-γ 20 μg·L-1), M2 (IL-4 20 μg·L-1+IL-13 20 μg·L-1), M1+M2, M1+SNS (supplemented with 10% SNS-containing serum for 2 h after culture under M1 conditions for 24 h), M2+SNS (supplemented with 10% SNS-containing serum for 2 h after culture under M2 conditions for 24 h), and M1+M2+SNS groups (supplemented with 10% SNS-containing serum for 2 h after culture under M1+M2 conditions for 24 h), and the expression of nuclear factor-κB (NF-κB) p65 and signal transducer and activator of transcription 3 (STAT3) at protein and mRNA levels was determined. ResultsCompared with the blank group, the model group exhibited increased serum levels of ALT, AST, ALP, PCⅢ, LN, HA, IL-6, and TNF-α, upregulated protein and mRNA levels of α-SMA, Col Ⅰ, and Col Ⅲ in the liver tissue (P<0.05, P<0.01), and decreased IL-10 level and STAT3 mRNA level (P<0.05, P<0.01), accompanied by marked collagen deposition and inflammatory infiltration in the liver. Compared with the model group, all doses of Sini San reversed these alterations (P<0.05, P<0.01), with the high-dose group showing the most pronounced effects. Cell experiments demonstrated that, compared with the blank control group, the model group had elevated levels of pro-inflammatory cytokines (IL-1β, IL-6, and TNF-α) and upregulated protein and mRNA levels of M1 markers [cluster of differentiation 86 (CD86, inducible nitric oxide synthase (iNOS), and monocyte chemoattractant protein-1 (MCP-1)] (P<0.01), along with declined level of IL-10 and downregulated expression of M2 markers [(CD163, CD206, and arginase-1 (Arg-1)] (P<0.05,P<0.01). Compared with the model group, treatment with Sini San-containing serum at various concentrations lowered the levels of pro-inflammatory cytokines and expression of M1 markers (P<0.05, P<0.01), while increasing the IL-10 level and M2 marker expression (P<0.05, P<0.01). Additionally, Sini San downregulated the mRNA level of NF-κB and upregulated the mRNA level of STAT (P<0.05, P<0.01). In the experiment with human primary Kupffer cells, compared with the blank group, the M1 group showed increased NF-κB p65 expression (P<0.01) and decreased STAT3 expression (P<0.05), whereas the M2 group exhibited increased STAT3 expression (P<0.01). Compared with the respective model groups, Sini San-containing serum reversed the M1-induced overexpression of NF-κB p65 (P<0.05, P<0.01) and further enhanced the M2-induced STAT3 expression (P<0.05, P<0.01). ConclusionSini San can inhibit the NF-κB p65/STAT3 signaling pathway and repolarize intrahepatic Kupffer cells from the pro-inflammatory M1 to the anti-inflammatory M2 phenotype to attenuate inflammation and suppress hepatic stellate cell activation and collagen deposition, thereby exerting anti-fibrotic effect.
