1.Study on the correlation between the growth and eruption of the mandibular third molar and the crowding of the mandibular dentition

Objective To explore the association between the growth condition of mandibular third molars(M3)and the parameters of mandibular dental arch through a retrospective cross-sectional study on M3 in adults,and to provide a basis for the selection of clini-cal treatment of M3.Methods A total of 221 adult patients were randomly selected for our study.Dolphin software was used to analyze the CBCT of all the patients.Parameters of the mandibular dentition including the entire dental crowding(EDC)were recorded.Then the association between M3 growth condition and these parameters was analyzed.Results The mesio-impacted angle of M3 was posi-tively correlated with EDC(P<0.05),and negatively correlated with the retromolar space(RMS,P<0.0 1).It was worth noting that me-dian mesio-impacted M3 significantly increased EDC(P<0.01),and the erupting M3 in the vertical orthotopic position significantly in-creased RMS(P<0.01).Conclusion For patients with median mesio-impacted M3 or insufficient RMS,preventive removal of M3 may be considered clinically,which may help to reduce crowding and prevent relapse after orthodontic treatment.

ObjectiveTo evaluate the efficacy and safety of Xiaoji Hufei Formula in protecting children with close contact exposure to influenza, and to provide reference and evidence-based support for better clinical prevention and treatment of influenza in children. MethodsA multicenter, prospective, non-randomized, parallel, controlled trial was conducted from October 2021 to May 2022 in five hospitals, including Dongfang Hospital of Beijing University of Chinese Medicine. Confirmed influenza cases and influenza-like illness (ILI) cases were collected, and eligible children with close contact exposure to these cases were recruited in the outpatient clinics. According to whether the enrolled close contacts were willing to take Xiaoji Hufei formula for influenza prevention, they were assigned to the observation group (108 cases) or the control group (108 cases). Follow-up visits were conducted on days 7 and 14 after enrollment. The primary outcomes were the incidence of ILI and the rate of laboratory-confirmed influenza. Secondary outcomes included traditional Chinese medicine (TCM) symptom score scale for influenza, influenza-related emergency (outpatient) visit rate, influenza hospitalization rate, and time to onset after exposure to influenza cases. ResultsA total of 216 participants were enrolled, with 108 in the observation group and 108 in the control group. Primary outcomes: (1) Incidence of ILI: The incidence was 12.0% (13/108) in the observation group and 23.1% (25/108) in the control group, with the observation group showing a significantly lower incidence (χ2=4.6, P<0.05). (2) Influenza confirmation rate: 3.7% (4/108) in the observation group and 4.6% (5/108) in the control group, with no statistically significant difference. Secondary outcomes: (1) TCM symptom score scale: after onset, nasal congestion and runny nose scores differed significantly between the two groups (P<0.05), while other symptoms such as fever, sore throat, and cough showed no significant differences. (2) Influenza-related emergency (outpatient) visit rate: 84.6% (11 cases) in the observation group and 96.0% (24 cases) in the control group, with no significant difference. (3) Time to onset after exposure: The median onset time after exposure to index patients was 7 days in the observation group and 4 days in the control group, with a statistically significant difference (P<0.05). ConclusionIn previously healthy children exposed to infectious influenza cases under unprotected conditions, Xiaoji Hufei formula prophylaxis significantly reduced the incidence of ILI. Xiaoji Hufei Formula can be recommended as a specific preventive prescription for influenza in children.

5.Prognostic role of C-reactive protein to albumin ratio in lung cancer: An updated systematic review and meta-analysis

Background::C-reactive protein to albumin ratio (CRP/Alb ratio, CAR) has been suggested as a potential prognostic biomarker in lung cancer. This updated systematic review and meta-analysis aimed to assess the association between CAR and lung cancer prognosis in current literature.Methods::A systematic search of databases was conducted to identify relevant studies published up to April 2023. Pooled hazard ratios (HRs) and 95% confidence intervals (CIs) were calculated to assess the association between CAR and overall survival (OS) and progression-free survival (PFS) and recurrence-free survival (RF) in lung cancer patients.Results::This meta-analysis includes 16 studies with a total of 5337 patients, indicating a significant association between higher CAR and poorer OS, PFS, and RFS in lung cancer patients, with a pooled HR of 1.78 (95% CI = 1.60–1.99), 1.57 (95% CI = 1.36–1.80), and 1.97 (95% CI = 1.40–2.77), respectively.Conclusions::This updated meta-analysis provides evidence for the potential prognostic role of CAR in lung cancer, suggesting its utility as an effective and noninvasive biomarker for identifying high-risk patients and informing treatment decisions in a cost-effective manner. However, further large-scale studies will be necessary to establish the optimal cut-off value for CAR in lung cancer and confirm the present findings.

7.Design and application of "1+3" management module for medical high-value consumables in Operation Room

Objective:To establish the "1+3" management module of high-value consumables in Operation Room and verify its application, so as to provide new ideas for cost management of consumables in Operation Room.Methods:The Operating Room team of Shenzhen Hospital of Southern Medical University designed a "1+3" management module in 2022, where "1" referred to the management process of high-value consumables in Operation Room, and "3" referred to the precise management of consumables in Operation Room warehouse, the management of closed-loop use of Operation Room consumables and adverse event management of consumables. Surgeries using high-value consumables in the Thoracic Surgery Department, Gastrointestinal Surgery Department, and Urology Department of the hospital were selected as the research objects. The surgeries using conventional consumables from January to June 2022 were set as the control group, and the surgeries implementing the "1+3" management module from July to December 2022 were set as the observation group. The number of consumables received by the itinerant nurses before the operation and the number of high-value consumables returned after the operation were compared between the two groups. And the number of missed and error charges for high-value consumables in the two groups were counted and compared.Results:The number of consumables received before operation in the control group was higher than that in the observation group, and the difference was statistically significant ( P<0.05). The number of high-value consumables returned in the observation group was less than that in the control group, and the difference was statistically significant ( P<0.01). The proportion of missed charges for consumables in the observation group was lower than that in the control group, and the difference was statistically significant ( P<0.01), but there was no statistically significant difference in the proportion of incorrect charges between the two groups ( P>0.05) . Conclusions:The "1+3" management module for high-value consumables in Operation Room makes the process of receiving, returning, and charging high-value consumables clear, with traceable data, achieving refined management of high-value consumables in Operation Room, reducing the number of high-value consumables returned to the warehouse and reducing the proportion of missed consumables, which is conducive to effective cost control in Operation Room.
8.Tissue localization of insulin-like peptide in Haemonchus contortus and identifica-tion and expression of its encoding genes

Insulin-like peptides(ILPs)play a crucial role in the biological processes of nematode metabolism,signal transduction,and developmental processes,representing potential therapeutic targets for nematode infections in animals.In order to explore the specific role of ILPs in nematode development and infection,genome-wide identification,sequence and evolution analysis of the ILPs coding genes were conducted on Haemonchus contortus(H.contortus);transcriptional levels of these ILPs coding genes among developmental stages were determined by a real-time quantitative PCR method;recombinant ILPs were produced in prokaryotic system for polyclonal antibody prep-aration and Western blot analysis;indirect immunofluorescence localization experiment was used to reveal the tissue distribution of ILPs at different developmental stages.It was showed that the number of ILPs coding genes in H.contortus were significantly reduced compared with that of Caenorhabditis elegans,namely three members of ILPs coding genes exhibiting antagonistic fea-tures;the highest transcriptional levels of ILPs coding genes was detected in the infective third stage larvae of H.contortus;ILPs were found dominant in the intestine and hypodermis of the in-fective larvae of this parasitic nematode.In this study,three genes encoding antagonistic ILPs were identified in H.contortus,they might play a role in regulating the development and infection processes.The finding lay a foundation for the study of nematode hypobiosis(larval diapause)in animals and the screening of potential intervention targets.
9.A preliminary study on the construction and standardization of procedures for ambulatory joint arthroplasty surgery

Objective:To investigate the safety and effectiveness of ambulatory joint arthroplasty and to establish a standardized procedure for outpatient joint replacement surgery.Methods:The clinical data from patients who underwent joint replacement surgery at the Orthopaedics Department of the General Hospital of PLA Fourth Medical Center between April 2023 and January 2024 were retrospectively analyzed. After screening and evaluation, fifty-nine patients who met specific criteria were enrolled for elective ambulatory surgery, including unicompartmental knee arthroplasty (UKA), total knee arthroplasty (TKA), and total hip arthroplasty (THA). For comparison, a 1∶1 matched control group was created from patients who underwent primary joint arthroplasty with routine hospitalization during the same period, matched by gender and surgical procedure. Functional outcomes were assessed pre- and post-operatively using the visual analogue scale (VAS), Harris hip score (HHS), and American Knee Society score (KSS). Postoperative complications, post-discharge complications, unplanned 90-day readmissions, and reoperations were analyzed.Results:The ambulatory surgery group had an average age of 62.0±6.5 years (range 53-76 years), which was significantly younger than the inpatient group at 66.2±8.3 years (range 46-81 years; t=3.707, P=0.002). No significant differences were observed in demographics such as body mass index (BMI) and American Society of Anesthesiologists (ASA) classification ( P>0.05). The incidence of complications, including nausea, vomiting, and severe pain, was similar between groups, with no statistically significant difference (χ 2=0.083, P=0.752). One case of unplanned emergency treatment occurred in the day-surgery group, but post-discharge complication rates did not significantly differ between the groups (3 cases vs. 1 case, P=0.473). The rate of delayed discharge in the ambulatory surgery group was 9%, primarily due to acute complications such as nausea, vomiting, severe pain, and poor patient compliance. VAS scores were lower in the ambulatory group compared to the conventional group within two weeks post-surgery, showing a statistically significant difference ( P<0.05). At one week post-surgery, the Harris hip score of ambulatory patients was significantly improved compared to the inpatient group ( t=7.362, P=0.027). The KSS knee score and KSS function score also showed significant improvement at two weeks post-surgery in the ambulatory group ( P<0.05). Cost-benefit analysis indicated that the hospitalization cost for ambulatory UKA (19.5±0.42 k CNY, excluding prosthesis costs) was approximately 3,000 CNY less than that for conventional surgery (22.5±0.41 k CNY), a statistically significant difference ( t=3.699, P=0.001). Conclusion:Ambulatory joint arthroplasty is a safe and effective option for selected patients, with manageable short-term postoperative complications. This outpatient surgery model promotes early recovery of joint function and provides effective pain management. Ambulatory UKA, in particular, offers enhanced cost-effectiveness, reduced length of stay, and faster bed turnover, making it a valuable approach for wider adoption.
10.Finite element analysis of absorbable screws in Bernese periacetabular osteotomy

Objective:To investigate the stability and feasibility of using absorbable screws during Bernese periacetabular osteotomy.Methods:A retrospective analysis was conducted on a 36 year-old woman diagnosed with developmental dysplasia of the hip, who had undergone Bernese periacetabular osteotomy. Finite element analysis was used to simulate the stability of the acetabulum under loads of 10%, 20%, 50%, and 100% of the patient's weight. The structural stiffness of the pelvis and the maximum equivalent stress on the absorbable screws were observed under different conditions, including whether the acetabular bone block and the ilium were in contact, whether 3 or 4 screws were used, and whether a graft (including fibular cortical bone and PEEK grafts) was used.Results:The structural stiffness of the pelvis fixed with four screws increased by 67%-94% compared to that with three screws. After using a graft, the structural stiffness of the pelvis increased by 50%-83%. As the load increased, the maximum equivalent stress on the screws also increased. When the acetabular bone block and the ilium had no contact, no graft was used, and only three screws were used for fixation, the maximum equivalent stress could reach 518.9 MPa, while this value dropped to 61% when four screws were used (318.7 MPa). When the acetabular bone block and the ilium were in contact, the maximum equivalent stress was about 12% of that when there was no contact, regardless of the number of screws used. When a cortical bone graft or a PEEK graft was used, the maximum equivalent stress could drop to 21%-26% of that without a graft. When the screw strength was 130 MPa, a load of 20% of body weight was applied, and only three screws were used without a graft, the equivalent stress could exceed the strength of the screw; if four screws were used, the equivalent stress was slightly higher than the strength of the screw when a load of 50% of body weight was applied. However, when a graft was used (either cortical bone or PEEK), even when a load of 100% of body weight was applied, the equivalent stress was slightly lower than the strength of the screw.Conclusion:Absorbable screws can provide sufficient stability for Bernese periacetabular osteotomy. The contact between the acetabular bone block and the ilium, an increase in the number of screws, and the use of grafts (cortical bone and PEEK grafts) can further improve stability. Therefore, absorbable screws have broad application prospects in Bernese periacetabular osteotomy.
