1.Inhibitory effects of different concentrations of auranofin on M1 macrophage function and its therapeutic potential in diabetic wound healing

BACKGROUND:During diabetic wound healing,the sustained activation of M1 macrophages exacerbates the inflammatory response and hinders wound repair.Auranofin,an anti-inflammatory drug,has not been clearly studied for its effects on M1 macrophages and its potential role in diabetic wound healing.OBJECTIVE:To investigate the effects of different concentrations of auranofin on the biological function of M1 macrophages and evaluate its potential application in diabetic wound healing.METHODS:RAW264.7 and THP-1 cells were used as research models.M1 polarization was induced using different concentrations of interferon-γ and lipopolysaccharide.M1 macrophages were treated with 1 and 2 μmol/L auranofin.Cell counting kit-8 assay was used to evaluate the effect of auranofin on cell viability.Quantitative real-time PCR was performed to detect mRNA expression of interleukin-1β,interleukin-6,and tumor necrosis factor-α.ELISA was employed to measure the levels of interleukin-1β,interleukin-6,and tumor necrosis factor-α in the supernatant.Western blot analysis was used to assess the expression of nuclear factor-κB(p65),phosphorylated mitogen-activated protein kinases(MAPK),and total MAPK proteins.Additionally,6-8-week-old male C57BL/6J and db/db diabetic mice were used for wound healing experiments,with the mice divided into C57 control,db/db control and auranofin treatment groups,each containing six animals.Dorsal skin defect modeling and treatment with intraperitoneal injection of auranofin were performed to observe wound healing in mice.RESULTS AND CONCLUSION:(1)Cell experiments showed that co-treatment with interferon-y(10 ng/mL)and lipopolysaccharide(100 ng/mL)significantly induced M1 polarization in RAW264.7 and THP-1 cells,resulting in increased mRNA expression of interleukin-1β,interleukin-6,and tumor necrosis factor-α.Treatment with auranofin(1 and 2 μmol/L)reduced the mRNA expression of these inflammatory factors in the cells and inhibited the secretion of inflammatory factors in the cell supernatant.(2)Auranofin treatment significantly suppressed the activation of nuclear factor-κB(p65)and phosphorylated MAPK signaling pathways.(3)Animal experiments showed that auranofin promoted wound healing in db/db diabetic mice,suggesting that auranofin has strong anti-inflammatory effects and may facilitate the healing of wounds in diabetic mice.

3.Analysis of prostate cancer screening results and exploration of screening model for elderly males in Songjiang Rural Areas of Shanghai City based on PSA preliminary screening under the community linkage mode

This study aims to analyze the screening results and epidemiological characteristics of prostate cancer (PCa) among elderly males in the rural areas of Songjiang, Shanghai City, through the implementation of a preliminary prostate-specific antigen (PSA) screening based on a community-linkage model, and to explore an effective screening approach. A retrospective observational study design was employed to collect data from residents who underwent PSA screening at Songjiang Hospital affiliated to Shanghai Jiao Tong University School of Medicine, in collaboration with multiple community health service centers in Songjiang District, Shanghai City, between June 2022 and June 2024, through free clinics and annual health examinations. Prostate biopsy was recommended for individuals with total PSA (tPSA) levels >10 ng/ml and those with 4 ng/ml≤tPSA≤10 ng/ml and abnormal free-to-total PSA (f/tPSA) ratios. Clinical characteristics of detected PCa patients were analyzed. Follow-up was conducted through phone calls and home visits by family doctors, coupled with enhanced health education. The results indicated that a total of 17 198 residents participated in the screening, among which 2 234 (12.99%) had tPSA levels between 4 ng/ml and 10 ng/ml, and 257 (1.49%) had tPSA levels >10 ng/ml. Ultimately, 417 residents underwent prostate biopsy, with 171 being diagnosed with PCa, yielding a positive biopsy rate of 41.00% and a PCa detection rate of 0.99%. The predominant pathological subtype among PCa patients was adenocarcinoma (168 cases, 98.24%). Of the 146 PCa patients who received treatment, the majority were classified as intermediate or high-risk (124 cases, 84.93%). Furthermore, with the optimization of the screening model, there was a significant increase in the proportion of subsequent outpatient visits. In conclusion, the community-linkage-based PSA screening model demonstrated high effectiveness in screening for PCa among elderly males in the rural areas of Songjiang, Shanghai City. Epidemiological findings revealed that PCa patients in this region are primarily composed of intermediate and high-risk groups, highlighting the need for intensified early screening and health education.

5.Research on the effect of fibroblast exosomes from diabetic rats on wound healing

Objective To expore the effect of fibroblast exosomes from diabetic rats on wound healing.Methods Male SD rats were randomly divided into diabetes mellitus group(DM group,n=12)and normal control group(NC group,n=15).Fibroblast exosomes were extracted and identified.After digestion of human epidermal keratinocyte line(HEK-a)and human epidermal microvascular endothelial cell line(HMEC-1),the cells were divided into diabetes exosomes group(DM-EOXa group),normal exosomes group(N-EXOa group)and normal control group(NCa group).CCK8 was used todetect the proliferation of fibroblast.fifteen male SD rats were randomly divided into diabetes exosomes group(DM-EXOb group),normal exosomes group(N-EXOb group)and normal control group(NCb group),with 5 rats in each group.Full-thickness wounds were created on rats back,and fibroblast exosomes were used to intervene wound healing.The wound healing was compared on 3th,7th,10th,and 14th day in each group.Results The proliferation of HEK-a and HMEC-1 was higher in N-EXOa and DM-EXOa groups than in NCa group(P<0.05).On the 7th,10th and 14th day,the average wound rate was lower in N-EXOb group than in NCb and DM-EXOb groups(P<0.05 or P<0.01).On the 10th day,the average wound rate was lower in NCb group than in DM-EXOb group(P<0.05).Conclusions Normal fibroblast exosomes can promote wound healing,while diabetic fibroblast exosomes from diabetic rats have no effect on wound healing,and even inhibit wound healing.
6.Research progress on fecal incontinence in patients undergoing transanal total mesorectal excision

Total mesorectal excision (TME) is the gold standard for the treatment of middle and low rectal tumors. Since the first patient who underwent transanal total mesorectal excision (taTME) was reported in 2010, taTME has attracted wide attention from scholars at home and abroad. Fecal incontinence is one of the common complications after taTME, which severely affects the postoperative quality of life of patients. This paper systematically summarizes the commonly used assessment methods, current situation, influencing factors, treatment and nursing progress of postoperative fecal incontinence patients with taTME, in order to provide evidence for improving the quality of life of postoperative defecation incontinence patients with taTME.
7.Application value of mesocolon approach in transanal total mesorectal excision

Objective:To investigate the application value of mesocolon approach in transanal total mesorectal excision (TaTME).Methods:The retrospective cohort study was conducted. The clinicopathological data of 61 patients with middle or low rectal cancer who were admitted to the Nanchong Central Hospital of North Sichuan Medical College from January to December in 2018 were collected. There were 41 males and 20 females, aged from 43 to 81 years, with an average age of 62 years. Of the 61 patients, 30 patients undergoing TaTME with the conventional approach were allocated into traditional approach group, and 31 patients undergoing TaTME with mesocolon approach were allocated into mesocolon approach group. Observation indicators: (1) surgical situations; (2) postoperative recovery; (3) follow-up. Follow-up was conducted by outpatient examination and telephone interview once every 3 months to detect local recurrence and metastasis of tumors in patients up to June 2019. Measurement data with normal distribution were expressed as Mean± SD, and comparison between groups was conducted using the t test. Measurement data with skewed distribution were expressed as M (range), and comparison between groups was conducted using the Mann-Whitney U test. Count data were expressed as absolute numbers or percentages, and comparison between groups were analyzed using the chi-square test or Fisher exact probability. Comparison of ordinal data between groups was analyzed using the Mann-Whitney U test. Results:(1) Surgical situations: patients in the two groups underwent TaTME successfully, without conversion to laparotomy. The transabdominal operation time, volume of intraoperative blood loss, length of distal margin from surgical specimen, length of proximal margin from surgical specimen, cases with complete mesentery or with nearly complete mesentery ( the integrity of mesentery ), positive rate of circumferential margin, positive rate of distal margin, and the number of lymph node dissected of the traditional approach group were (126±56)minutes, 41.0 mL (range, 17.5-71.4 mL), 1.3 cm (range, 0.8-2.0 cm), (10.0±5.0)cm, 10, 20, 3.3%(1/30), 0, 13.7 (range, 9.0-17.0), respectively, versus (101±30)minutes, 44.0 mL (range, 25.0-67.5 mL), 1.6 cm (range, 1.1-2.2 cm), (12.0±3.0)cm, 23, 8, 6.5%(2/31), 0, 13.0 (range, 10.9-17.3) of the mesocolon approach group. There were significant differences in the transabdominal operation time, length of proximal margin from surgical specimen, and the integrity of mesentery between the two groups ( t=2.133, -2.286, χ2=10.250, P<0.05). There was no significant difference in the volume of intraoperative blood loss, length of distal margin from surgical specimen, or the number of lymph node dissected between the two groups ( Z=-0.662, -1.107, 0.304, P>0.05). There was also no significant difference in the positive rate of circumferential margin or positive rate of distal margin between the two groups ( P>0.05). (2) Postoperative recovery: the time to first anal flatus of the traditional approach group was 51 hours (range, 48-64 hours). There were 3 patients with complications in the traditional approach group. One patient in the traditional approach group had postoperative anastomotic fistula of Clavien-Dindo classification Ⅱ, and was cured after conservative treatment including sufficient drainage, parenteral nutrition and anti-infective treatment. One patient had chylous fistula of Clavien-Dindo classification Ⅱ, and was cured after conservative treatment. One patient had pulmonary infection of Clavien-Dindo classification Ⅳa, and was cured after treatment in ICU. The duration of postoperative hospital stay of the traditional approach group was (11.3±4.5)days. The time to first anal flatus of the mesocolon approach group was 59 hours (range, 49-70 hours). One patient in the mesocolon approach group had paralytic ileus of Clavien-Dindo classification Ⅰ, and was cured after conservative treatment. The duration of postoperative hospital stay of the mesocolon approach group was (9.6±1.8)days. There was no significant difference in the time to first anal flatus or duration of postoperative hospital stay between the two groups ( Z=-0.554, t=1.884, P>0.05). There was no significant difference in the complications between the two groups ( P>0.05). (3) Follow-up: 61 patients were followed up for 6-18 months, with a median time of 12 months. There was no local recurrence or metastasis of tumors in patients during the follow-up. Conclusion:The mesocolon approach is safe and feasible in TaTME, which abides by the principle of radical resection, and can decrease the difficulty of mesocolon excision, shorten the time of transabdominal operation, increase the length of proximal margin from tumor specimen, improve the integrity of mesentery.
8.Value of narrow band imaging combined with endoscopic dentate line in evaluation of patients with gastroesophageal reflux disease

Objective To explore the value of narrow band imaging combined with endoscopic dentate line in evaluation of patients with gastroesophageal reflux disease.Methods A total of 200 patients with GERD were selected as GERD group and 160 normal people were selected as control group.Gred-Q score was used to record the symptoms.The morphology of the dentate line and microvessel were observed by narrow-band imaging.Gred-Q score,dentate line morphology,the relationship between esophageal microvascular manifestations and gastroesophageal reflux disease were analyzed.Results The abnormal positive rate of NBI microvessel density in GRED group was significantly higher than that in control group (P < 0.05).The abnormal rate of dentate line in GRED group was significantly higher than that in control group (P < 0.05).There were positive correlations between NBI microvessel performance and Gerd-Q score,dentate line morphology and Gerd-Q score,NBI microvessel performance and dentate line morphology (r =0.780,0.803,0.586,P < 0.01).Conclusion Narrow band imaging combined with endoscopic dentate line has an important clinical value in diagnosing patients with gastroesophageal reflux disease.

10.Influence of CCM on postoperative function training effect of anal sphincter in patients with anus preserving surgery for low rectal cancer

Objective To discuss influence of continuous collaboration care model (CCM) on postoperative function training effect of anal sphincter in patients with anus preserving surgery for low rectal cancer.Methods 86 cases with low rectal cancer treated by anus preserving surgery from July to December in 2013 were selected as control group,to whom routine nursing care was carried out.88 patients with low rectal cancer treated by anus preserving surgery from July to December in 2014 were selected as observation group,to whom continuous CCMwas implemented on the basis of routine nursing care in the control group.Defecation function and quality of life of patients in the two groups were compared 3 months and 6 months after the surgery.Results After 3 months of nursing intervention,81.8% of patients in the observation group had fine anal function,higher than the number in the control group,47.7%;after 6 months,the proportions of the two groups were 90.9% and 67.4% (χ2 =8.632,7.523;P <0.05).Physiological function,psychological function,social function,self-awareness of health and score of quality of life of patients in the observation group were all higher than those of the control group (t =5.264,4.635,4.265,4.145,3.958;P <0.01 ).Conclusions Implementation of continuous collaborative family care management can promote recovery of patients′defecation function and improve their quality of life.
