1.Predicting Postoperative Motor Function in High-risk Glioma Based on The Morphology Change of Motor Fiber Tracts

ObjectiveGliomas in the motor functional area can damage the corticospinal tract (CST), leading to motor dysfunction. Currently, there is a lack of unified methods for evaluating the extent of CST damage, especially in patients with high surgical risk where the minimum distance from the lesion to the CST is less than 10 mm. This study aims to further clarify the classification method and clinical significance of CST morphological changes in these patients. MethodsThis retrospective study analyzed 109 high-risk functional area glioma patients who underwent neurosurgical treatment with preoperative diffusion tensor imaging (DTI) imaging and intraoperative neurostimulation guidance between 2014 and 2024. All patients had a lesion-to-tract distance (LTD) of less than 10 mm between the CST and the lesion. Preoperative DTI evaluation of CST involvement-induced morphological changes were reviewed. Patients were divided into 3 groups: 17 cases (15.6%) with symmetric CST morphology compared to the healthy side (CST symmetry), 48 cases (44.0%) with significant CST morphology changes compared to the healthy side (CST deformation), and 44 cases (40.4%) with CST overlap with the tumor (CST overlap). Then we classified patients according to preoperative assessment of tumor-induced morphological changes, and analyze postoperative motor function for each category. ResultsPostoperative pathology showed a significantly higher proportion of high-grade gliomas (HGG) in the CST overlap group compared to the other two groups (P=0.001). Logistic regression analysis showed that CST overlap was a predictor of HGG (P=0.000). The rate of total tumor resection in the CST deformation group and overlap group was lower than in the CST symmetric group (P=0.008). There was a total of 41 postoperative hemiplegic patients, with 4 cases (23.5%) in the CST symmetric group, 11 cases (22.9%) in the CST deformation group, and 26 cases (59.1%) in the CST overlap group. CST overlap with the tumor predicted postoperative hemiplegia (P=0.016). Two-way ANOVA analysis of the affected/healthy side and CST morphology groups showed significant main effects of CST grouping and healthy-affected side (P=0.017 and P=0.010), with no significant interaction (P=0.31). The fractional anisotropy (FA) value in the CST overlap group and the affected side was lower. A decrease in the FA value on the affected side predicted postoperative hemiplegia (sensitivity 69.2%, specificity 71.9%). ConclusionWe have established a method to predict postoperative hemiplegia in high-risk motor functional area glioma patients based on preoperative CST morphological changes. CST overlap leads to a decrease in CST FA values. This method can be used for precise patient management and aid in accurate preoperative surgical planning.
2.Causal association between periodontitis and hepatobiliary diseases: genetic insights from Mendelian randomization

Objective:
To investigate the reciprocal causal relationships between periodontitis and hepatobiliary diseases through Mendelian randomization (MR) analyses, to provide evidence for joint prevention and clinical decision-making in patients with concurrent periodontitis and hepatobiliary diseases.
Methods:
Single nucleotide polymorphisms (SNPs) were extracted from the largest genome-wide association study on periodontitis (17 353 cases, 28 210 controls) and hepatobiliary diseases within the European ancestry and used as instrumental variables (IVs). The strength of the associations was examined by calculating the F-statistic. The SNPs significantly associated with the outcome were removed by scanning on Phenoscanner platform. Bidirectional causal associations between periodontitis and hepatobiliary diseases were estimated using inverse variance weighted (IVW), MR-Egger, and Weighted Median methods. The robustness of the findings was further verified through additional sensitive MR approaches, including Cochran’s Q statistic (IVW), Rucker’s Q statistic (MR-Egger), MR-PRESSO and Leave-one-out analysis. Further MR analyses, utilizing other available genome-wide association studies (GWAS) on hepatobiliary diseases, were conducted to validate the results.
Results:
The IVW method found that periodontitis had a causal impact on acalculous cholecystitis (odds ratio = 1.277, 95% CI 1.097-1.485, P=0.002), implying an increased risk of acalculous cholecystitis associated with periodontitis, while the MR-Egger regression and Weighted Median failed to observe significant causal effects of periodontitis on acalculous cholecystitis. However, no bidirectional causal associations between periodontitis and nonalcoholic fatty liver disease, cirrhosis or liver cancer were observed using IVW, MR-Egger regression and Weighted Median. The bidirectional causal relationships were deemed unlikely to be influenced by horizontal pleiotropy. Further, the validation analysis based on alternative GWAS data suggested parallel results.
Conclusions
The MR analyses suggest that periodontitis may elevate the risk of acalculous cholecystitis. Further investigations, including clinical studies and mechanistic explorations, are warranted to validate these findings. However, the MR analyses do not support bidirectional causal associations between periodontitis and nonalcoholic fatty liver disease, cirrhosis or liver cancer.
3.Determination of aluminum content in hemofiltration base solution by graphite furnace atomic absorption standard addition method

Abtract Objective:To establish a graphite furnace atomic absorption spectroscopy,and to determine the content of aluminum in the hemofiltration base solution.Methods:The standard addition method of atomic absorption of graphite furnace was used to add matrix modifier to determine the content of aluminum.Results:Aluminum has a good linear relationship in the range of 0-20 μg·L-1,r=0.998;The detection limit concentration was 0.91 μg·L-1.The average recovery rate was 96.5%.The results of the three batches were 3.299,1.232 and 2.431 μg·L-1,respectively.Conclution:This method can effectively measure the content of aluminum in hemofiltration base solution products,and control the raw materials,production and packaging of products that may introduce pollution pathways.It is recommended that enterprises pay attention to the detection of aluminum content in the on hemofil-tration base solution to minimize the risk of contamination and ensure the quality of products.
4.Simultaneous determination of 4 bacteriostatic agents in triamcinolon acetonide econazole cream by HPLC under gradient elution

Objective:To develop an HPLC method with gradient elution for the simultaneous determination of 4 bacteriostatic agents in triamcinolone acetonide econazole cream from the different manufacturers.Methods:The test was performed on a Waters Symmetry C18 column(250 mm ×4.6 mm,5 μm)under gradient elution of metha-nol(A)and methanol-0.02 mol·L-1 sodium dihydrogen phosphate solution(B).The flow rate of 1.0 mL·min-1 and the column temperature was 35 ℃.The detection wavelengths were 228 nm and 254 nm.Results:The good separation of the bacteriostatic agents peaks were achieved.The linear ranges of benzoic acid,methyl hydroxybenzoate,ethylparaben and propylparaben fell into 0.010 348-0.155 22 mg·mL-1,0.009 876-0.148145 mg·mL-1,0.010 106-0.151 588 mg·mL-1 and 0.010 259-0.153 882 mg·mL-1 respectively.The av-erage recoveries were 99.68%(RSD=2.35%),100.21%(RSD=1.78%),100.47%(RSD=1.59%)and 100.06%(RSD=1.65%)respectively.The LODs were 3 × 10-5 mg·mL-1,1.67 × 10-5 mg·mL-1,1.67 × 10-5 mg·mL-1 and 1.67 × 10-5 mg·mL-1 respectively.Conclusion:The established method is sensitive and accurate,and has the good separation.It provides the reliable basis for the quality control of triamcinolone ace-tonide econazole cream.
5.Clinical characteristics of patients with MitraClip operation and predictors for the occurrence of afterload mismatch

Objective To explore the risk factors related to afterload mismatch(AM)after transcatheter mitral valve repair(MitraClip).Methods This was a retrospective cohort study.48 patients hospitalized in the Department of Cardiovascular Medicine,the First Affiliated Hospital of Sun Yat-sen University from December 2021 to December 2023,who underwent MitraClip due to severe mitral regurgitation(MR)were included.Preoperative clinical data,laboratory tests,and preoperative and postoperative color Doppler echocardiographic examination results of surgical patients were collected.AM was defined as the left ventricular ejection fraction(LVEF)decreased by 15%or more after surgery compared with the one before(dLVEF≤-15%).Patients were divided into AM group and non-AM group according to whether afterload mismatch occurred.Univariate and multivariate logistic regression were used to analyze the risk factors of postoperative AM.Results Among 48 patients who underwent MitraClip,14 of them(29.2%)developed afterload-mismatched.For those without AM,their overall LVEF was improved after the operation;for patients in both AM group and non-AM group,their overall left ventricular end-diastolic diameter(LVEDd),left ventricular end-diastolic diameter volume index(LVEDVi)was reduced compared with the preoperative ones.Univariate regression analysis showed that C-reactive protein levels(OR 1.98,95%CI 1.02-3.83),platelets(OR 2.22,95%CI 1.08-4.53),systemic immune inflammation index(OR 1.96,95%CI 1.03-3.71)were associated with an increased risk of AM in patients undergoing MitraClip(all P<0.05),while those with larger right atrial diameter(OR 0.35,95%CI 0.13-0.93)or moderate to severe tricuspid regurgitation(OR 0.19,95%CI 0.05-0.81)were less likely to develop into AM(both P<0.05),which is still satisfied after adjustment.Conclusions For patients who underwent MitraClip,C-reactive protein levels,platelets and systemic immune inflammation index(SII)are associated with an increased risk of afterload mismatched,while those with larger right atrial diameter or moderate to severe tricuspid regurgitation were less likely to develop into AM.
6.Characteristics and influencing factors of post-intensive care syndrome in elderly patients during the transitional period

Objective:To investigate the current situation and characteristics of post-intensive care syndrome in elderly patients during the transitional period, and explore its influencing factors.Methods:From December 2022 to September 2023, convenience sampling was used to select 119 elderly patients in the Intensive Care Unit of China-Japan Friendship Hospital as the research subject. The General Information Questionnaire, Short Physical Performance Battery, Fatigue Scale-14, Barthel Index, Hospital Anxiety and Depression Scale, and Mini-mental State Examination were used to evaluate patients for post-intensive care syndrome from cognitive, psychological, and physiological aspects 7 days after their transfer from the Intensive Care Unit. Binomial Logistic regression was used to analyze the influencing factors of post-intensive care syndrome in elderly patients.Results:Among 119 elderly patients, 84 developed post-intensive care syndrome, with an incidence of 70.6%. The binomial Logistic regression showed that women and high nutritional risk were risk factors for the occurrence of post-intensive care syndrome in elderly patients ( P<0.05) . Conclusions:The incidence of post-intensive care syndrome is high in elderly patients during the transition period, with females and patients with high nutritional risk being prone to developing post-intensive care syndrome. Medical and nursing staff should pay attention to identifying gender differences, focus on high-risk populations, and dynamically evaluate different symptoms early on to provide precise interventions for elderly patients.
7.Investigation of tick species and potential pathogenic ricks in certain areas of Wuwei City,Gansu Province

To understand the distribution of ticks in the Wuwei Region,enrich tick species data,and provide a basis for the prevention of tick-borne diseases,tick were collected using flagging and tick-picking methods during the highest activity period from April to September in 2021 and 2022 in the mountainous areas of Wuwei City.The ticks were identified based on morpho-logical and molecular biological characteristics,and characteristic sequences were obtained.A systematic evolutionary tree was constructed using the neighbor-joining method in MEGA 11.0 software.In total,7 342 ticks collected in Wuwei,which be-longed to 5 species from 4 genera with in the Ixodidae family,which included Dermacentor nuttalli,Hyalomma asiaticum,Ixodes canisuga,Haemaphysalis longicornis and Haema-physalis danieli.Ticks of the same species clustered together into the same branch of an evolutionary tree.In the Wuwei Re-gion,five common tick species are found across various habi-tats,with each habitat featuring different distributions of tick species and populations.The Dermacentor nuttalli is the dom-inant tick species in this area.

9.Clinical Significance of the Levels of Peripheral Blood Tregs and Cytokines IL-35,TGF-β and IL-10 in Hemophilia A Patients with FⅧ Inhibitor

Objective:To explore the levels of regulatory T cells(Tregs)and cytokines IL-35,TGF-β and IL-10 in peripheral blood of hemophilia A(HA)patients with F Ⅷ inhibitor and their clinical significance.Methods:43 HA patients admitted to the Hematology Department of the Affiliated Hospital of North China University of Science and Technology from October 2019 to December 2020 were selected,including 6 cases with F Ⅷ inhibitor and 37 cases without FⅧ inhibitor.In addition,20 healthy males who underwent physical examinations were selected as healthy controls.Flow cytometry was used to detect the levels of CD4+CD25+CD127-Tregs in peripheral blood of the HA patients and healthy controls,and ELISA assay was used to detect the expression levels of IL-35,TGF-β and IL-10 in serum,and their differences between different groups were compared.Results:Compared with the healthy control group,the level of Tregs in HA patients was decreased,and the level of Tregs in the FⅧ inhibitor positive group was the lowest,the difference was statistically significant(P<0.05).There was no significant difference in the expression level of Tregs in HA patients of different severity levels.The serum IL-35,TGF-β,and IL-10 levels in both FⅧ inhibitor negative and positive groups were significantly lower than those in healthy control group,and those in FⅧ inhibitor positive group were significantly lower than those in FⅧ inhibitor negative group(all P<0.05).Conclusion:The decrease of Tregs,IL-35,TGF-β,and IL-10 levels in HA patients may be related to the formation of FⅧ inhibitors.
10.Impact of ginkgo biloba extract on the malignant biological behavior of colon cancer cells by regulating CXCL12/CX-CR4 signal pathway

Objective:To investigate the impact of ginkgo biloba extract(GK)on the malignant biological behavior of colon cancer(CC)cells by regulating the chemokine 12(CXCL12)/chemokine re-ceptor 4(CXCR4)signal pathway.Methods:Colon cancer HCT116 cells were treated with different concentrations of GK(0,2.5,5,10 μ mol/L)for 48 hours,MTT assay was used to detect the survival rate of HCT116 cells and screen the appropriate GK concentration.HCT116 cells in logarithmic growth phase were divided into control group,GK group(5 μ mol/L GK),CXCL12 overexpression re-combinant adenovirus(Ad CXCL12)group,negative controI(Ad NC)group,Ad CXCL12+CXCR4 small interfering RNA(si CXCR4)group,and Ad CXCL12+negative control(si NC)group.Transwell assay was used to detect cell migration and invasion;MTT and Tunel were used to detect cell proliferation and apoptosis;and the mRNA and protein expression levels of CXCL12 and CXCR4 were detected by qRT PCR and Western blot respectively.Results:The survival rate of cells treated with 5μ mol/L GK was the closest to 50%.Follow up studies were conducted at this concentration.Com-pared with the control group,the cell proliferation rate,migration,invasion numbers,the expression levels of CXCL12,CXCR4 mRNA and protein in GK group decreased obviously,and the apoptosis rate increased obviously(P<0.05);Ad CXCL12 reversed the inhibitory effect of GK on HCT116 cells.si CXCR4 reversed the promoting effect of Ad CXCL12 on HCT116 cells.Conclusion:GK inhibits the malignant biological behavior of HCT116 cells by inhibiting CXCL12/CXCR4 signaling pathway.
