1.Predicting Postoperative Motor Function in High-risk Glioma Based on The Morphology Change of Motor Fiber Tracts

ObjectiveGliomas in the motor functional area can damage the corticospinal tract (CST), leading to motor dysfunction. Currently, there is a lack of unified methods for evaluating the extent of CST damage, especially in patients with high surgical risk where the minimum distance from the lesion to the CST is less than 10 mm. This study aims to further clarify the classification method and clinical significance of CST morphological changes in these patients. MethodsThis retrospective study analyzed 109 high-risk functional area glioma patients who underwent neurosurgical treatment with preoperative diffusion tensor imaging (DTI) imaging and intraoperative neurostimulation guidance between 2014 and 2024. All patients had a lesion-to-tract distance (LTD) of less than 10 mm between the CST and the lesion. Preoperative DTI evaluation of CST involvement-induced morphological changes were reviewed. Patients were divided into 3 groups: 17 cases (15.6%) with symmetric CST morphology compared to the healthy side (CST symmetry), 48 cases (44.0%) with significant CST morphology changes compared to the healthy side (CST deformation), and 44 cases (40.4%) with CST overlap with the tumor (CST overlap). Then we classified patients according to preoperative assessment of tumor-induced morphological changes, and analyze postoperative motor function for each category. ResultsPostoperative pathology showed a significantly higher proportion of high-grade gliomas (HGG) in the CST overlap group compared to the other two groups (P=0.001). Logistic regression analysis showed that CST overlap was a predictor of HGG (P=0.000). The rate of total tumor resection in the CST deformation group and overlap group was lower than in the CST symmetric group (P=0.008). There was a total of 41 postoperative hemiplegic patients, with 4 cases (23.5%) in the CST symmetric group, 11 cases (22.9%) in the CST deformation group, and 26 cases (59.1%) in the CST overlap group. CST overlap with the tumor predicted postoperative hemiplegia (P=0.016). Two-way ANOVA analysis of the affected/healthy side and CST morphology groups showed significant main effects of CST grouping and healthy-affected side (P=0.017 and P=0.010), with no significant interaction (P=0.31). The fractional anisotropy (FA) value in the CST overlap group and the affected side was lower. A decrease in the FA value on the affected side predicted postoperative hemiplegia (sensitivity 69.2%, specificity 71.9%). ConclusionWe have established a method to predict postoperative hemiplegia in high-risk motor functional area glioma patients based on preoperative CST morphological changes. CST overlap leads to a decrease in CST FA values. This method can be used for precise patient management and aid in accurate preoperative surgical planning.
2.Acetylation of Rehmannia glutinosa polysaccharides and antioxidant activity of acetylated derivatives.

This study aims to acetylate Rehmannia glutinosa polysaccharides by acetic anhydride method, optimize process parameters and evaluate their antioxidant activity. With the degree of substitution(D_s) as a criterion, the effects of reaction time, acetic anhydride-to-polysaccharides ratio and temperature were investigated. Process parameters were optimized by single-factor experiment and response surface methodology. The infrared spectroscopy(IR) and scanning electron microscopy(SEM) proved the successful acetylation and were employed to preliminarily analyze the structural characteristics of acetylated derivatives. The results showed that the D_s was 0.327 under the optimal technological conditions, including m(acetic anhydride):m(R. glutinosa polysaccharides)=2.70, reaction time 3.0 h and temperature 48 ℃. Further, the antioxidant properties of acetylated derivatives were investigated in vitro and acetylation was found effective to improve the antioxidant activity of R. glutinosa polysaccharides. This study provides a reference for the further development and application of R. glutinosa polysaccharides.

Objective:
The relationship between serum uric acid (SUA) levels and glycemic indices, including plasma glucose (FPG), 2-hour postload glucose (2h-PG), and glycated hemoglobin (HbA1c), remains inconclusive. We aimed to explore the associations between glycemic indices and SUA levels in the general Chinese population.
Methods:
The current study was a cross-sectional analysis using the first follow-up survey data from The China Cardiometabolic Disease and Cancer Cohort Study. A total of 105,922 community-dwelling adults aged ≥ 40 years underwent the oral glucose tolerance test and uric acid assessment. The nonlinear relationships between glycemic indices and SUA levels were explored using generalized additive models.
Results:
A total of 30,941 men and 62,361 women were eligible for the current analysis. Generalized additive models verified the inverted U-shaped association between glycemic indices and SUA levels, but with different inflection points in men and women. The thresholds for FPG, 2h-PG, and HbA1c for men and women were 6.5/8.0 mmol/L, 11.0/14.0 mmol/L, and 6.1/6.5, respectively (SUA levels increased with increasing glycemic indices before the inflection points and then eventually decreased with further increases in the glycemic indices).
Conclusion
An inverted U-shaped association was observed between major glycemic indices and uric acid levels in both sexes, while the inflection points were reached earlier in men than in women.

4.The influence factors and the clinical significance of the tumor abnormal protein expression in papillary thyroid carcinoma

Objective:To investigate the potential risk factors of the tumor abnormal protein (TAP) in papillary thyroid carcinoma (PTC) and analyze the clinical significance of TAP.Methods:The clinical data of 792 patients who underwent thyroid surgery in the thyroid surgery department of the First Affiliated Hospital of Zhengzhou University from Jun. 2018 to Jun. 2019 was collected, of whom 564 were PTC patients and 228 were benign thyroid tumor patients. Patients were divided into two groups by postoperative pathology: PTC group and thyroid benign tumor group. The potential risk factors the TAP in PTC were analyzed by univariate and multivariate Logistic regression analysis, and then the diagnostic value of TAP for PTC was assessed by ROC curve analysis.Results:Multivariate analysis showed that tumor maximum diameter (≥1.0 cm) ( OR:1.555; 95% CI: 1.031~2.344, P=0.035) and multifocal tumor ( OR:1.789; 95% CI: 1.098~2.916, P=0.019) were risk factors for elevation of TAP. Gender, age, body weight, extracapsular extension of cancer, invasive growth of cancer, Hashimoto's thyroiditis, central lymph node metastasis, lateral lymph node metastasis, and BrafV600E mutation were not associated with TAP ( P>0.05) . TAP in patients with PTC was significantly higher than that in patients with benign thyroid tumors[ (122.36±49.37) μm 2 vs (105.04±40.61) μm 2, P<0.001]. The sensitivity and specificity of TAP in diagnosing PTC were 78.90% and 40.35%, respectively. Conclusions:Tumor maximum diameter (≥1.0 cm) and multifocal tumor in PTC were risk factors for elevation of TAP. TAP has certain clinical value in differentiating PTC and thyroid benign tumor, which could be used as an auxiliary indicator.

OBJECTIVE:
To investigate the clinical features and outcome of neonatal acute respiratory distress syndrome (ARDS) in southwest Hubei, China.
METHODS:
According to the Montreux definition of neonatal ARDS, a retrospective clinical epidemiological investigation was performed on the medical data of neonates with ARDS who were admitted to Department of Neonatology/Pediatrics in 17 level 2 or level 3 hospitals in southwest Hubei from January to December, 2017.
RESULTS:
A total of 7 150 neonates were admitted to the 17 hospitals in southwest Hubei during 2017 and 66 (0.92%) were diagnosed with ARDS. Among the 66 neonates with ARDS, 23 (35%) had mild ARDS, 28 (42%) had moderate ARDS, and 15 (23%) had severe ARDS. The main primary diseases for neonatal ARDS were perinatal asphyxia in 23 neonates (35%), pneumonia in 18 neonates (27%), sepsis in 12 neonates (18%), and meconium aspiration syndrome in 10 neonates (15%). Among the 66 neonates with ARDS, 10 neonates (15%) were born to the mothers with an age of ≥35 years, 30 neonates (45%) suffered from intrauterine distress, 32 neonates (49%) had a 1-minute Apgar score of 0 to 7 points, 24 neonates (36%) had abnormal fetal heart monitoring results, and 21 neonates (32%) experienced meconium staining of amniotic fluid. Intraventricular hemorrhage was the most common comorbidity (12 neonates), followed by neonatal shock (9 neonates) and patent ductus arteriosus (8 neonates). All 66 neonates with ARDS were treated with mechanical ventilation in addition to the treatment for primary diseases. Among the 66 neonates with ARDS, 10 died, with a mortality rate of 15% (10/66), and 56 neonates were improved or cured, with a survival rate of 85% (56/66).
CONCLUSIONS
Neonatal ARDS in southwest Hubei is mostly mild or moderate. Perinatal asphyxia and infection may be the main causes of neonatal ARDS in this area. Intraventricular hemorrhage is the most common comorbidity. Neonates with ARDS tend to have a high survival rate after multimodality treatment.

OBJECTIVE:
To investigate the incidence of neonatal asphyxia and possible contributing factors for the development of severe asphyxia in Hubei Enshi Tujia and Miao Autonomous Prefecture, China.
METHODS:
A total of 16 hospitals in Hubei Enshi Tujia and Miao Autonomous Prefecture were selected as research centers. A retrospective analysis was performed for the clinical data of 22 294 live births in these 16 hospitals from January to December, 2016 to investigate the incidence rate of neonatal asphyxia and possible contributing factors for the development of severe asphyxia.
RESULTS:
Of the 22 294 neonates born alive, 733 (3.29%) were diagnosed with neonatal asphyxia, among whom 627 had mild asphyxia and 106 had severe asphyxia. The neonates with low maternal education level, maternal anemia during pregnancy, chorioamnionitis, abnormal amniotic fluid, abnormal umbilical cord, placenta previa, placental abruption, Tujia Minority, preterm birth, and low birth weight had a higher incidence of severe asphyxia (P<0.05).
CONCLUSIONS
The incidence rate of neonatal asphyxia in Hubei Enshi Tujia and Miao Autonomous Prefecture is higher. Low maternal education level, maternal anemia during pregnancy, chorioamnionitis, abnormal amniotic fluid, abnormal umbilical cord, placenta previa, placental abruption, Tujia Minority, preterm birth, and low birth weight may be related to the development of severe neonatal asphyxia.

7.Update on Molecular Imaging in Parkinson's Disease.

Advances in radionuclide tracers have allowed for more accurate imaging that reflects the actions of numerous neurotransmitters, energy metabolism utilization, inflammation, and pathological protein accumulation. All of these achievements in molecular brain imaging have broadened our understanding of brain function in Parkinson's disease (PD). The implementation of molecular imaging has supported more accurate PD diagnosis as well as assessment of therapeutic outcome and disease progression. Moreover, molecular imaging is well suited for the detection of preclinical or prodromal PD cases. Despite these advances, future frontiers of research in this area will focus on using multi-modalities combining positron emission tomography and magnetic resonance imaging along with causal modeling with complex algorithms.

8.Correlation research of renal perfusion and diffusion function using MRI in renal allograft early after renal transplantation

Objective To explore the correlationships between microperfusion diffusion indexes derived from intravoxel incoherent motion(IVIM)and perfusion values measured by arterial spin labeling (ASL)in renal allograft. Methods A total of 76 renal allograft recipients and 26 age-matched volunteers (group 0)were included in this prospective study. All subjects were underwent conventional MRI, IVIM and ASL MRI which were performed in the oblique-sagittal plane. Seventy-six recipients were divided into two groups based on the estimated glomerular filtration rate(eGFR):recipients with good allograft function(group 1, eGFR≥ 60 ml · min-1 · 1.73m-2,n=44)and recipients with impaired allograft function(group 2, eGFR<60 ml · min-1 · 1.73m-2,n=32). Three IVIM indexes values, including true diffusion coefficient(ADCslow), pseudo-diffusion coef fi cient(ADCfast), perfusion fraction(PF), and one ASL index value of renal cortex(renal blood flow, RBF)were measured. One-way analysis of variance and the least significant difference were used to compare the different of each cortical index values among three groups. Correlations between the ADCslow, ADCfast, PF, RBF and eGFR as well as the correlation among the indexes were evaluated using Pearson correlation coefficients. Results For cortical ADCslow, ADCfast, PF and RBF values, allografts with good function and impaired function showed significantly differences compared healthy controls(all P<0.01). In allografts with good function, cortical ADCslow,ADCfast,PF showed no significantly differences compared with controls(all P>0.05), but RBF value was significantly lower(P<0.05). The ADCslow, ADCfast, PF and RBF values of renal cortex were significantly lower in allografts with impaired function compared to allografts with good function(all P<0.01). In renal allografts, there were significant positive correlations between cortical ADCslow, ADCfast, PF, RBF value and eGFR(r values were 0.604, 0.552, 0.579 and 0.673, all P<0.01). Cortical ADCfast and PF value exhibited a significant correlation with RBF for recipients(r values were 0.501 and 0.423, all P<0.01). Conclusion Cortical ADCfast and PF values derived from IVIM and RBF measured by ASL show a significant positive correlation in renal allografts.
9.Differential gene expressions in the stromal cells of the human prostate peripheral zone in males of different ages.

OBJECTIVETo study the gene expressions in the stromal cells of the human prostate peripheral zone (PZ) in men of different ages.
METHODSWe primarily cultured stromal cells from the normal prostate PZ of men aged 23 -32 (young group) and 56 -75 years (old group), profiled the gene signature of the PZ cells by cDNA microarray, and defined the differential gene expression patterns by hierarchical cluster analysis. Among the differential genes, we selected and confirmed up-regulated genes by quantitative real time PCR (Q-PCR), and identified their protein coding by Western blotting.
RESULTSThere were significant differences in the gene expressions of the PZ cells between the old and young groups. Based on the fold change ratio of > or = 2 or < or = 0.5, 509 up-regulated and 188 down-regulated genes were selected in the PZ cells. A subset of significantly differential genes influencing the growth of adjacent epithelial cells were identified, including HGF, IGF2, IGFBP5 and MMP1 in the old males.
CONCLUSIONStromal cells in the prostate PZ were more active in older males in promoting the malignant progression of adjacent prostate epithelial cells, which might be due to the increased expression of extracellular paracrining mediators.

10.Separation of rod- and cone-driven oscillatory potential from retinal cone degeneration rat and congenital stationary night blindness rat

BackgroundOscillatory potentials (OPs) has been used extensively in experimental research and clinical diagnosis,but it is well known that OPs are the summating potentials originated from retinal rod and cone.To separate the rod and cone OPs is helpful for us to diagnose some retinal diseases.ObjectiveThis study was to analyze the characteristics of cone-and rod-driven OPs. Methods The retinal cone degeneration ( RCD ) and congenital stationary night blindness(CSNB) rats were used in this study and SD rats served as control,and 6 rats for each kinds of animals.Scotopic and photopic OPs were recorded in each rat under the dark adaptation for 12 hours and light adaptation for 10 minutes at the stimulate light intensities of -35,-25,-15,-5,0,5 db respectively with RETIscan Visual Electrophysical System.The scotopic and photopic OPs were extracted from flash electroretinogram (FERG) with Maflab7.0 Butterworth filtering waves,and the frequency spectrum of the OPs was analyzed by fast Fourier transform.The characteristics of OPs from RCD rats and CSNB rats were assessed and compared.Results The a wave and b wave of ERG were showed under the dark adaptation condition in both SD and RCD rats,but the b wave was absent in CSNB rat.Under the light adaptation condition,b wave was seen in both SD and CSNB rats,but a wave and b wave of RCD rat were absent.Two peaks were exhibited in both SD and RCD rats under the darkadaptation condition and high intensity of stimulate light at the lower frequency( domain frequency) of 75-110 Hz,90-120 Hz and high frequency ( minor frequency) 90- 120 Hz,110- 135 Hz respectively.In various intensities of stimulate light,CSNB rats appeared a peak at 70-100 Hz.But in light-adaptation and various intensities of stimulate light,the frequency peaks were seen at 75-95 Hz and 70-85 Hz from both SD and CSNB rats respectively.However,under the light adaptation condition,only one peak was seen in SD and CSNB rats at the 75-95 Hz and 70-85 Hz respectively.Compared with SD rats,the mean implied time of b wave was delayed and the amplitude was lowed under the light adaptation (P<0.05 ),however,no significant differences were found in the implied time and amplitude of b wave of scotopic ERG between SD rats and RCD rats( P>0.05 ).The scotopic OPs showed the prolong implied time and depressed amplitudes in RCD rats and CSNB rats compared with SD rats( P<0.05 ),and the photopic OPs presented the prolong implied time and lowed amplitude in CSNB rat in comparison with SD rats (P < O.05 ).ConclusionsCone- and rod-driven OPs appear very different characteristics.The results of this study imply that rod pathway gives more contribution to OPs than cone pathway.Analysis of these results will be helpful for the exploration of the origin of OPs and the diagnosis of the related disease.
