Objectives:Coronary slow flow(CSF)has long been regarded as a marker of coronary microvascular dysfunction(CMD).This study aims to evaluate the diagnostic value of CSF for CMD in patients with angina and nonobstructive coronary arteries(ANOCA).Methods:The study data were derived from the ANOCA-CMD prospective cohort study. All enrolled patients underwent coronary angiography and concurrent coronary physiological assessments in the left anterior descending artery using pressure-wire and thermodilution techniques to obtain coronary flow reserve(CFR)and the index of microcirculatory resistance(IMR).Based on the results,CMD was classified into four subtypes:CMD with elevated IMR(IMR≥25),CMD with reduced CFR(CFR<2.5),CMD with either reduced CFR or elevated IMR(CFR<2.5 or IMR≥25),and CMD with both reduced CFR and elevated IMR(CFR<2.5 and IMR≥25).The corrected thrombolysis in myocardial infarction(TIMI)frame count(CTFC)in the left anterior descending artery was calculated from coronary angiography images, with CSF defined as CTFC>27.This study evaluated the correlation between CTFC,CFR,and IMR,and investigated the diagnostic value of CSF for CMD in ANOCA patients. Results:A total of 103 ANOCA patients were enrolled in this study, with a mean age of(64.2±10.6)years,and 53.4%were female.Among them,57 patients(55.3%)were diagnosed with coronary slow flow.Patients with slow flow had higher IMR(P<0.001)and CFR(P=0.041).Similarly,the proportion of CMD with elevated IMR was higher in the slow flow group(P<0.001),while the proportion of CMD with reduced CFR was lower(P=0.044).There was no significant difference between the groups in the proportions of CMD with either reduced CFR or elevated IMR or CMD with both reduced CFR and elevated IMR(all P>0.05).CTFC was positively correlated with hyperemic mean transit time(r=0.424,P<0.001),IMR(r=0.430,P<0.001),and CFR(r=0.211,P=0.032).The area under the curve(AUC)of CTFC for diagnosing CMD with elevated IMR was 0.721(95%CI:0.623-0.819)with an accuracy of 67%(57%,76%),for diagnosing CMD with reduced CFR was 0.610(95%CI:0.499-0.720)with an accuracy of 60%(50%,70%),for diagnosing CMD with either reduced CFR or elevated IMR was 0.549(95%CI:0.425-0.673)with an accuracy of 47%(37%,57%),and for diagnosing CMD with both reduced CFR and elevated IMR was 0.582(95%CI:0.471-0.693)with an accuracy of 47%(37%,57%).Thus,CSF demonstrated limited diagnostic values across all subtypes of CMD.Conclusions:In ANOCA patients,CSF cannot serve as an effective diagnostic marker for CMD.Therefore,in clinical practice, the slow flow phenomenon should not be directly equated with the presence of coronary microvascular dysfunction in ANOCA patients.

BACKGROUND:The combination of traditional Chinese medicine syndrome and semen quality-related parameters can jointly predict the occurrence of abnormal increase in sperm DNA fragmentation index (DFI) and draw a column chart, which can significantly improve clinical practicality and application efficiency, provide a basis for comprehensive evaluation of semen quality in clinical practice, take active intervention measures to improve clinical outcomes, and formulate personalized medical plans.OBJECTIVE:To explore the prediction model and verification of sperm DNA fragments based on traditional Chinese medicine syndrome and semen quality-related parameters.METHODS:Retrospective analysis was made on 420 infertile patients who received traditional Chinese medicine syndrome diagnosis and sperm DNA fragment rate examination in the Department of Traditional Chinese Medicine Andrology,Nanxishan Hospital of Guangxi Zhuang Autonomous Region from July 2019 to July 2021.According to the Manual of Human Semen Examination and Treatment Laboratories (6th Edition),137 patients with sperm DFI>30% were included in the group of abnormally high sperm DFI,and 283 patients with sperm DFI ≤ 30% were taken as the control group.First, univariate analysis was used to screen the influencing factors of the abnormal increase of sperm DFI.Then, the best matching factor was selected by using the collinearity problem of LASSO correction factors. Then,it was included in the multifactor forward stepwise logistic regression to find out its independent influencing factors and draw a nomogram. Finally, the receiver operating characteristic curve, calibration curve, decision curve analysis and clinical impact curve were used to verify the differentiation and accuracy of the prediction model and its clinical application effectiveness.RESULTS AND CONCLUSION:(1) The results of the univariate analysis showed that age, body mass index, forward motion rate, total sperm motility, sperm concentration, sperm morphology, kidney yang deficiency syndrome, damp heat downpour syndrome, and kidney sperm deficiency syndrome were the influencing factors for the abnormal increase of sperm DFI (P<0.05).(2) The best matching factors further screened by LASSO regression were age, body mass index, total sperm motility, sperm concentration, sperm morphology, kidney yang deficiency syndrome, damp heat downpour syndrome, and kidney essence deficiency syndrome (P<0.05).(3) Multifactor forward stepwise Logistic regression showed that age, body mass index, sperm concentration, total sperm motility, damp heat downpour syndrome, and kidney yang deficiency syndrome were six independent factors that caused the abnormal increase in sperm DFI.(4) Receiver operating characteristic curve showed that the area under the curve of the model group was 0.760(0.713,0.806),and the area under the curve of the validation group was 0.745(0.714,0.776).It showed that the prediction model had good discrimination.(5) The average absolute error of the calibration curve was 0.040,and the Hosmer Lemeshow test (P>0.05),suggesting that there was no significant statistical difference between the probability of the abnormal increase in DFI of spermatozoa predicted by the model and the probability of the abnormal increase in DFI of spermatozoa actually occurred, which confirmed that the model had good accuracy.(6) Decision curve analysis and clinical impact curve showed that the model group and validation group had the maximum clinical net benefit when the threshold probability values were (0.08-0.84) and (0.09-0.78) respectively, and had good clinical application efficiency within the threshold probability range.(7) These findings conclude that age, body mass index, sperm concentration, total sperm viability, damp heat downpour syndrome and kidney yang deficiency syndrome are independent factors that cause the abnormal increase in sperm DFI.The nomogram of the clinical prediction model constructed by them has good clinical prediction value and clinical application efficiency, and can provide the basis for comprehensive clinical evaluation of semen quality and individualized medical service.

BACKGROUND:The highest incidence of spinal fracture is in the thoracolumbar segment, and its symptoms are back pain, posterior convexity deformity, activity limitation,or with spinal cord nerve injury causing lower limb pain, numbness, and even paraplegia and other complications. The finite element method is a digital computer modeling technique, which can simulate the physical model and carry out force analysis realistically.OBJECTIVE:To review the application of finite element method in thoracolumbar spine fractures.METHODS:We searched the Chinese and English literature databases PubMed,Web of Science, and CNKI for relevant literature on the application of the finite element analysis method in spinal thoracolumbar fracture published before March 2024.The search terms in Chinese and English were:finite element analysis methods, biomechanical phenomena, stress analysis, thoracolumbar fractures, spinal fractures.Finally,55 papers were included.RESULTS AND CONCLUSION:(1)The exploration of thoracolumbar fractures caused by different etiologies(osteoporotic, traumatic, and pathological)through the finite element method is conducive to a deeper understanding of the biomechanics of various types of thoracolumbar fractures, and to improve the individualized and fine-tuned treatment of thoracolumbar fractures.(2)The finite element analysis of a single sample or a small number of samples has the chance, and a larger number of samples are required for the future finite element analysis to reduce the chance caused by the sample.(3)The rigid structure of bones alone cannot meet the biomechanical working conditions of the integrity of the physical object, and future finite element models need to incorporate all the structures of the physical object(e.g.,soft tissues, such as muscles and ligaments)as far as possible.(4)The finite element method has been used in more studies on osteoporotic and traumatic thoracolumbar spine fractures, which will need to be more in-depth in the future, and less in the field of pathologic thoracolumbar fractures, which has a wider scope for future research.
