Objective:A monoclonal cell line that stably expresses transmembrane protease serine 2 (TMPRSS2) was constructed using the PiggyBac (PB) transposon system to establish a technique for isolating human parainfluenza viruses (HPIVs) without rely on exogenous trypsin. Methods:The pB513B/TMPRSS2 recombinant expression plasmid was synthetized based on the principle of PB transposition, and co-transfected into LLC-MK2, HEp-2, and Vero cells, together with the helper plasmid. Positive cells were then identified using the puromycin resistance and limiting dilution method. Stable TMPRSS2 expression was verified by PCR and western blot (WB). Furthermore, the viral replication capacity of the cell lines was evaluated using digital PCR and immunostaining plaque assays. Results:The successful construction of the recombinant plasmid pB513B/TMPRSS2 was confirmed through restriction enzyme digestion and DNA sequencing. Following transfection, the LLC-MK2/TMPRSS2 cell survival rate was higher than those of the HEp-2/TMPRSS2 and Vero/TMPRSS2 cells. Subsequent PCR and WB analysis results revealed that the LLC-MK2/TMPRSS2 cells maintained stable TMPRSS2 expression after 20 successive passages. Cell counting kit-8 (CCK-8) assays confirmed that there was no significant difference in proliferative activity between LLC-MK2/TMPRSS2 cells and LLC-MK2 cells. Viral replication capability testing showed that the laboratory-adapted strain of HPIV3 (ATCC strain) exhibited exponential growth in both LLC-MK2 and LLC-MK2/TMPRSS2 cells in culture without exogenous trypsin. Notably, LLC-MK2/TMPRSS2 cells significantly supported the replication of clinical HPIV3 isolates (circulating strain), whereas LLC-MK2 cells failed to sustain their infectivity effectively. Conclusions:In this study, we successfully generated the LLC-MK2/TMPRSS2 cell line using the PB transposon system to stably express TMPRSS2. Our TMPRSS2-transfected cell line enables the efficient isolation and propagation of HPIVs in vitro, eliminating the need for exogenous trypsin and providing a vital technical platform for isolating HPIVs and studying their pathogenicity.

BACKGROUND:
Based on the China-VHD database, this study sought to develop and validate a Valvular Heart Disease- specific Age-adjusted Comorbidity Index (VHD-ACI) for predicting mortality risk in patients with VHD.
METHODS & RESULTS:
The China-VHD study was a nationwide, multi-centre multi-centre cohort study enrolling 13,917 patients with moderate or severe VHD across 46 medical centres in China between April-June 2018. After excluding cases with missing key variables, 11,459 patients were retained for final analysis. The primary endpoint was 2-year all-cause mortality, with 941 deaths (10.0%) observed during follow-up. The VHD-ACI was derived after identifying 13 independent mortality predictors: cardiomyopathy, myocardial infarction, chronic obstructive pulmonary disease, pulmonary artery hypertension, low body weight, anaemia, hypoalbuminaemia, renal insufficiency, moderate/severe hepatic dysfunction, heart failure, cancer, NYHA functional class and age. The index exhibited good discrimination (AUC, 0.79) and calibration (Brier score, 0.062) in the total cohort, outperforming both EuroSCORE II and ACCI (P < 0.001 for comparison). Internal validation through 100 bootstrap iterations yielded a C statistic of 0.694 (95% CI: 0.665-0.723) for 2-year mortality prediction. VHD-ACI scores, as a continuous variable (VHD-ACI score: adjusted HR (95% CI): 1.263 (1.245-1.282), P < 0.001) or categorized using thresholds determined by the Yoden index (VHD-ACI ≥ 9 vs. < 9, adjusted HR (95% CI): 6.216 (5.378-7.184), P < 0.001), were independently associated with mortality. The prognostic performance remained consistent across all VHD subtypes (aortic stenosis, aortic regurgitation, mitral stenosis, mitral regurgitation, tricuspid valve disease, mixed aortic/mitral valve disease and multiple VHD), and clinical subgroups stratified by therapeutic strategy, LVEF status (preserved vs. reduced), disease severity and etiology.
CONCLUSION
The VHD-ACI is a simple 13-comorbidity algorithm for the prediction of mortality in VHD patients and providing a simple and rapid tool for risk stratification.

Following the release of the Technical Requirements on Quality Control and Standard Establishment of Traditional Chinese Medicine Formula Granules by the National Medical Products Administration in 2021, Chinese Pharmacopoeia Commission has promulgated 296 national drug standards so far, and most provinces have started the work of establishing provincial standards as supplements. The promulgation of standards fostered high-quality development of the industry. Since the implementation of national and provincial standards for more than three years, enterprises have gained deep understanding and hands-on experiences on the characteristics, technical requirements, production process, and quality control of traditional Chinese medicine(TCM) formula granules. Meanwhile, challenges have emerged restricting the high-quality development of this industry, including how to formulate quality control strategies for medicinal materials and decoction pieces, how to reduce manufacturing costs, and how to improve the pass rate and product stability under high standards. Based on the work experiences from standard management and process research, this article analyzed the distribution of sources, processing methods, dry extract rate ranges, process requirements for volatile oil-containing decoction pieces, control measures of safety indices, characteristics and trends of setting characteristic chromatograms or fingerprints, characteristics and trends of setting content ranges, and main differences between national standards and provincial standards. On the one hand, this article aims to present main characteristics for deeply understanding different indicators in standards and provide basic ideas for establishing quality and process control systems. On the other hand, from the perspective of industrial practice, suggestions are put forward on the important aspects that need to be focused on in the quality and process control of TCM formula granules.

The aim of this investigation was to determine the optimal storage medium for testicular hypothermic transportation and identify the ideal concentration for the application of the protective agent 5-aminolevulinic acid (5-ALA). Furthermore, this study aimed to explore the underlying mechanism of the protective effects of 5-ALA. First, we collected and stored mouse testicular fragments in different media, including Hank's balanced salt solution (HBSS; n = 5), Dulbecco's Modified Eagle Medium/Nutrient Mixture F-12 (DMEM/F12; n = 5), and alpha-minimum essential medium (αMEM; n = 5). Storage of testicular tissue in HBSS preserved the integrity of testicular morphology better than that in the DMEM/F12 group ( P < 0.05) and the αMEM group ( P < 0.01). Testicular fragments were subsequently placed in HBSS with various concentrations of 5-ALA (0 [control], 1 mmol l -1 , 2 mmol l -1 , and 5 mmol l -1 ) to determine the most effective concentration of 5-ALA. The 2 mmol l -1 5-ALA group ( n = 3) presented the highest positive rate of spermatogonial stem cells compared with those in the control, 1 mmol l -1 , and 5 mmol l -1 5-ALA groups. Finally, the tissue fragments were preserved in HBSS with control ( n = 3) and 2 mmol l -1 5-ALA ( n = 3) under low-temperature conditions. A comparative analysis was performed against fresh testes ( n = 3) to elucidate the underlying mechanism of 5-ALA. Gene set enrichment analysis (GSEA) for WikiPathways revealed that the p38 mitogen-activated protein kinase (MAPK) signaling pathway was downregulated in the 2 mmol l -1 5-ALA group compared with that in the control group (normalized enrichment score [NES] = -1.57, false discovery rate [FDR] = 0.229, and P = 0.019). In conclusion, these data suggest that using 2 mmol l -1 5-ALA in HBSS effectively protected the viability of spermatogonial stem cells upon hypothermic transportation.

This study was aimed at establishing a method of ultra-fast quantitative PCR for Brucella detection. We used an exogenous recombinant plasmid as the internal reference and targeted the T4SS secretion system,an important Brucella viru-lence factor,to design specific primers and probes. The sensitivity, specificity, and repeatability of this method were evaluated, and a standard curve was constructed. The coincidence rate of detection findings with this method versus quantitative PCR was determined. This method markedly decreased the detection time to only 10 minutes. The standard curve demonstrated a good linear relationship(Y=-3.410 7x+38.357,R2=0.998 5)with a low minimum detection limit of 10 copies/μL.The method exhibited good specificity and did not specifically amplify several common clinical bacteria other than Brucella. The de-tection of three concentrations of positive plasmids yielded coefficients of variation(CVs)of 0.20%to 0.91%,thus demonstra-ting the method's excellent repeatability.Furthermore,140 clinical samples were analyzed concurrently with the fluorescence PCR method, which yielded a 100%compliance rate and consistent results. Our findings indicated that the Brucella ultra-fast quantitative PCR was ultrafast;had high sensitivity, high specificity, and good specificity;and can be used for the clinical de-tection of Brucella and emergency investigation of epidemics. Therefore, this method is valuable for the early diagnosis of Bru-cella.

Aim To investigate the antidepressant mechanism of hyperforin via the utilization of single-cell sequencing technology. Methods C57BL/6 mice were randomly divided into the control group,depres-sion model group, and hyperforin intervention group. The chronic unpredictable mild stress(CUMS)model was induced and drug interventions were administered for 28 d.Behavioral experiments were conducted to as-sess depressive symptoms, and hippocampal tissue was collected for single-cell RNA sequencing.Key cell populations and differentially expressed genes across groups were identified, followed by PPI network,GO,and KEGG enrichment analysis. Results Behavioral experiments indicated that CUMS successfully induced depressive symptoms in mice, while hyperforin im-proved depressive behavior. In the depression model group, the proportion of brain perivascular macrophages(PVM)increased, and this proportion decreased after hyperforin intervention, approaching the level seen in the control group. The top 20 common differentially ex-pressed genes in the PVM subpopulation were Saa3,Hbb-bs and Ccl24.PPI network analysis identified core targets, including Ccl2,Dhx9,C3,Msr1,Cxcl2 and Cx3cr1.KEGG enrichment analysis revealed pathways related to chemokines, phagosome formation, and inosi-tol phosphate metabolism. Conclusion The antide-pressant mechanism of hyperforin may be related to the regulation of Ccl24 and its related chemokine signaling pathway by PVM.

Objective:To investigate the prognostic value of peripheral blood absolute monocyte count(AMC)in non-severe aplastic anaemia(NSAA)patients.Methods:178 patients with NSAA who attended the Affiliated Hospital of Xuzhou Medical University from April 2008 to September 2020 were retrospectively analyzed, and the optimal cut-off value of peripheral blood AMC was determined by the receiver operating characteristic curve of the subjects, and they were divided into low AMC group(48 patients)and normal AMC group(130 patients),and the differences in clinical characteristics between the two groups were compared.Overall survival(OS)and progression-free survival(PFS)were analyzed by Kaplan-Meier.Univariate and multivariate Cox regression analysis were used to determine the independent prognostic value of AMC.Results:Among 178 NSAA patients,105(59.0%)were male and 73(41.0%)were female, with a median age of 31(18-87)years old,a median follow-up time of 58 months(range:6 months-175 months),and a median AMC of 0.15 × 109/L[range:(0.01-0.59)× 109/L)].The proportion of granulocytes(27.5%vs 36.0%,P<0.05),and the proportion of mature monocytes(1%vs 2%,P<0.05)in the low AMC group were lower than that in the normal AMC group;the proportion of mature lymphocytes in the low AMC group was higher than that in the normal AMC group(54%vs 50%,P<0.05).However,there was no significantly different in the proportion of erythropoietic cells and stages of the erythropoietic cells between the two groups(P>0.05).CR(27.7%vs 10.4%)and ORR(75.4%vs 56.3%)in the normal AMC group were higher than that in the low AMC group.Compared with patients in the low AMC group,AA patients in the normal AMC had better 5-year OS(98.5%vs 86.9%,P<0.01),and the 5-year PFS(86.0%vs 58.9%,P<0.01).Also,the 10-year survival rate of patients in the normal AMC group was higher than that in the low AMC group(98.5%vs 60.5%,P<0.01).Univariate analysis showed that age, reticulocyte count,AMC<0.1 × 109/L and the proportion of bone marrow mature monocytes were related with patients survival.Multivariate Cox regression analysis showed that monocyte count reduction was not an independent poor prognostic factor in NSAA patients(HR=4.474,95%CI:0.508-44.390;P=0.172).Conclusion:Low AMC level at initial diagnosis is not an independent prognostic factor for NSAA patients, but still suggest potential prognostic value of AMC.
