1.Compact Fundus Imaging System Using Shack-Hartmann Wavefront Sensing for High-speed Auto-focus

ObjectiveThe widespread adoption of portable fundus cameras for primary care and community screening is hindered by limitations in current autofocus(AF) technologies. Image-based methods relying on sharpness evaluation require iterative searches, resulting in slow convergence, while projection-based techniques are susceptible to optical artifacts and calibration errors. To address these challenges, this study introduces a novel AF system based on direct wavefront sensing, designed to deliver simultaneous high speed, high precision, and operational robustness within the compact form factor essential for portable ophthalmic devices. MethodsOur approach fundamentally reimagines the AF process by directly measuring the ocular wavefront aberration. We developed a custom portable fundus camera integrating a miniaturized Shack-Hartmann wavefront sensor (SHWS) into the optical path. An 850 nm laser diode projects a point source onto the retina via oblique illumination to minimize corneal reflections. Light scattered from this spot carries the eye’s refractive error through the imaging optics and is directed to the SHWS, positioned at a plane optically conjugate to the primary color CMOS imaging sensor. A microlens array within the SHWS samples the incident wavefront, generating a pattern of focal spots on a CCD. Real-time centroid analysis of these spots provides a map of local wavefront slopes. These measurements are processed through a singular value decomposition (SVD) algorithm to fit a Zernike polynomial basis set, enabling real-time reconstruction of the wavefront phase. The defocus component (S) is extracted from the second-order Zernike coefficients, providing a direct, quantitative measure of the refractive error in diopters. This value serves as a precise error signal in a closed-loop control system, which commands a voice-coil actuated focusing lens to its null position in a single, deterministic step, eliminating the need for iterative search algorithms. ResultsComprehensive evaluation demonstrated the system’s high performance. Testing on a calibrated model eye (OEMI-7) established a highly linear relationship between the computed defocus S and the focusing lens position across a ±20 Diopter (D) compensation range, achievable within a 5 mm mechanical travel. The system achieved a focusing precision of 0.08 D, corresponding to an 18-fold improvement over a conventional projection spot-size method tested under identical conditions. The total focus acquisition time, encompassing wavefront measurement, computation, and lens actuation, averaged under 0.5 s. Clinical validation with 25 human volunteers (50 eyes, refractive range -15 D to +10 D) confirmed practical efficacy. The wavefront-sensing AF succeeded in 92% of attempts with a mean time of 0.5 s, substantially outperforming a projection-based benchmark which achieved only a 32% success rate with an average time of 4.25 s. The system provided instantaneous directional guidance and maintained stability during minor ocular movements. Objective assessment of image quality, via amplitude contrast of retinal vasculature, showed consistent and significant enhancement following AF correction across the entire tested diopter range. ConclusionThis work successfully implements and validates a direct wavefront-sensing autofocus paradigm for portable fundus cameras. By directly quantifying and compensating for the optical defocus aberration, this method bypasses the fundamental limitations of image-processing and projection-based techniques, enabling rapid, precise, and deterministic diopter compensation. The developed system delivers an exceptional combination of a wide operational range (±20 D), high accuracy (0.08 D), fast convergence (0.5 s), and a compact physical footprint. This technology provides a practical and high-performance focusing solution capable of enhancing the reliability, throughput, and diagnostic utility of portable retinal imaging in large-scale screening applications. Future efforts will be directed towards system cost optimization and performance adaptation for diverse ocular conditions.
2.Prediction of testicular histology in azoospermia patients through deep learning-enabled two-dimensional grayscale ultrasound.

Testicular histology based on testicular biopsy is an important factor for determining appropriate testicular sperm extraction surgery and predicting sperm retrieval outcomes in patients with azoospermia. Therefore, we developed a deep learning (DL) model to establish the associations between testicular grayscale ultrasound images and testicular histology. We retrospectively included two-dimensional testicular grayscale ultrasound from patients with azoospermia (353 men with 4357 images between July 2017 and December 2021 in The First Affiliated Hospital of Sun Yat-sen University, Guangzhou, China) to develop a DL model. We obtained testicular histology during conventional testicular sperm extraction. Our DL model was trained based on ultrasound images or fusion data (ultrasound images fused with the corresponding testicular volume) to distinguish spermatozoa presence in pathology (SPP) and spermatozoa absence in pathology (SAP) and to classify maturation arrest (MA) and Sertoli cell-only syndrome (SCOS) in patients with SAP. Areas under the receiver operating characteristic curve (AUCs), accuracy, sensitivity, and specificity were used to analyze model performance. DL based on images achieved an AUC of 0.922 (95% confidence interval [CI]: 0.908-0.935), a sensitivity of 80.9%, a specificity of 84.6%, and an accuracy of 83.5% in predicting SPP (including normal spermatogenesis and hypospermatogenesis) and SAP (including MA and SCOS). In the identification of SCOS and MA, DL on fusion data yielded better diagnostic performance with an AUC of 0.979 (95% CI: 0.969-0.989), a sensitivity of 89.7%, a specificity of 97.1%, and an accuracy of 92.1%. Our study provides a noninvasive method to predict testicular histology for patients with azoospermia, which would avoid unnecessary testicular biopsy.

3.Association of higher serum follicle-stimulating hormone levels with successful microdissection testicular sperm extraction outcomes in nonobstructive azoospermic men with reduced testicular volumes.

To investigate the impact of preoperative serum follicle-stimulating hormone (FSH) levels on the probability of testicular sperm retrieval, we conducted a study of nonobstructive azoospermic (NOA) men with different testicular volumes (TVs) who underwent microdissection testicular sperm extraction (micro-TESE). A total of 177 NOA patients undergoing micro-TESE for the first time from April 2019 to November 2022 in Shenzhen Zhongshan Obstetrics and Gynecology Hospital (formerly Shenzhen Zhongshan Urology Hospital, Shenzhen, China) were retrospectively reviewed. The subjects were divided into four groups based on average TV quartiles. Serum hormone levels in each TV group were compared between positive and negative sperm retrieval subgroups. Overall sperm retrieval rate was 57.6%. FSH levels (median [interquartile range]) were higher in the positive sperm retrieval subgroup compared with the negative outcome subgroup when average TV was <5 ml (first quartile [Q1: TV <3 ml]: 43.32 [17.92] IU l -1 vs 32.95 [18.56] IU l -1 , P = 0.048; second quartile [Q2: 3 ml ≤ TV <5 ml]: 31.31 [15.37] IU l -1 vs 25.59 [18.40] IU l -1 , P = 0.042). Elevated serum FSH levels were associated with successful micro-TESE sperm retrieval in NOA men whose average TVs were <5 ml (adjusted odds ratio [OR]: 1.06 per unit increase; 95% confidence interval [CI]: 1.01-1.11; P = 0.011). In men with TVs ≥5 ml, larger TVs were associated with lower odds of sperm retrieval (adjusted OR: 0.84 per 1 ml increase; 95% CI: 0.71-0.98; P = 0.029). In conclusion, elevated serum FSH levels were associated with positive sperm retrieval in micro-TESE in NOA men with TVs <5 ml. In men with TV ≥5 ml, increases in average TVs were associated with lower odds of sperm retrieval.

4.Discovery of FAM3 A-targeting Small Molecule Agents Using Integrated Virtual Screening and SPR Technology

Family with sequence similarity 3 member A(FAM3A),a novel mitochondrial protein,plays a pivotal role in hepatic glucose and lipid metabolism by enhancing ATP synthesis and secretion and mod-ulating the ATP-P2 receptor-Akt signaling pathway.Dysregulation of FAM3A is closely associated with the pathogenesis of non-alcoholic fatty liver disease(NAFLD)and type 2 diabetes mellitus(T2DM).In this study,targeting FAM3A as a therapeutic candidate,we conducted virtual screening to identify 47 small-molecule compounds with potential binding activity.Surface plasmon resonance(SPR)analysis re-vealed three compounds exhibiting high binding affinity to FAM3 A.Further structural characterization of the FAM3A-compound complexes,combined with intermolecular interaction analysis,elucidated the binding mode of the lead compound Index 2(taxifolin)to FAM3A at atomic resolution.These findings provide critical insights into the molecular mechanisms underlying ligand-FAM3A interactions and deliver valuable chemical scaffolds for the development of therapeutics targeting NAFLD and T2DM.This work establishes a foundation for advancing drug discovery efforts focused on FAM3A-mediated metabolic disor-ders.
5.Results of Cancer Screening Program in Urban Areas in Shaanxi Province from 2021 to 2022

[Purpose]To analyze the screening results of urban cancer early diagnosis and treat-ment program in Shaanxi Province from 2021 to 2022.[Methods]The risk assessment and clini-cal screening data of five high incidence malignant tumors(female breast cancer,lung cancer,upper gastrointestinal cancer,liver cancer and colorectal cancer)in the urban cancer early diag-nosis and treatment program in Shaanxi Province were collected and analyzed.The high-risk rate,screening compliance rate,and positive detection rate of the above 5 types of malignant tumors were calculated.[Results]A total of 17 921 urban residents in Shaanxi Province completed the cancer risk survey from 2021 to 2022(44.12%were males and 55.88%were females),9 270 were identified as high-risk(51.7%):34.2%for breast cancer,24.2%for lung cancer,20.7%for upper gastrointestinal cancer,7.5%for liver cancer,and 29.8%for colorectal cancer.Among 18 145 person-times of high-risk,6 357 person-times underwent clinical screening,with an overall com-pliance rate of 35.0%.The participating number and compliance rate of individual cancer screening were 1 451 person-times(42.3%)for female breast cancer,2 064 person-times(47.7%)for lung cancer,1 125 person-times(30.4%)for upper gastrointestinal cancer,468 person-times(35.0%)for liver cancer,and 1 249 person-times(23.4%)for colorectal cancer.The detected positive cases and the detection rate were 142 cases(9.8%)for frmale breast cancer,373 cases(18.1%)for lung cancer,13 cases(1.2%)for upper gastrointestinal cancer,8 cases(1.7%)for liver cancer,and 68 cases(5.4%)for colorectal cancer.[Conclusion]The screening results of early diagnosis and treatment project for urban cancer in Shaanxi Province shows a relative higher risk rate and lower compliance rate with significant differences among various types of cancer.
6.The Mechanism of Calcium Handling Proteins and NF-κB in Calcium Dyshomeo-stasis of Cardiomyocytes Caused by Acute MDMA Exposure

Objective To explore the mechanism of myocardial toxicity caused by N-methyl-3,4-methyle-nedioxyamphetamine(MDMA),the changes of intracellular calcium oscillation mode and calcium han-dling proteins during acute exposure to different concentrations of MDMA were detected,and the in-volvement of nuclear factor κB(NF-κB)and its effect on calcium handling proteins were investigated.Methods Primary rat cardiomyocytes were cultured to establish MDMA acute exposure model,and a control group was set up.The MDMA poisoning model was divided into three concentration groups of 10,100 and 1 000 μmol/L.After 1 h of exposure,the morphological changes of cardiomyocytes were ob-served,the cytotoxicity and changes in calcium signals were measured,and the changes in calcium handling proteins RyR2,SERCA2a,PLN,NCX1 and Cav1.2 were detected.The changes of NF-κB activity and the expression of nucleoprotein p-p65(Ser311)and PKCζ after MDMA exposure,and the intervention of NF-κB inhibitors pyrrolidine dithiocarbamate ammonium(PDTC)and protein kinase C(PKC)inhibitor chelerythrine(CHE)were detected by electrophoretic mobility shift assay(EMSA)and Western blotting.The effects of PDTC intervention on calcium signals,and the expressions of RyR2,SERCA2a,PLN,NCX1 and Cav1.2 after acute MDMA exposure were also observed.Results No obvious changes were observed in the morphology of cardiomyocytes after acute exposure to MDMA,whereas the oscillation waveform of intracytoplasmic calcium ion showed irregular changes with increased oscillation amplitude,intense fluctuations,irregular frequency,and increased fluctuation range of relative optical density values.The expression of RyR2,SERCA2a and NCX1 increased,while the expression of Cav1.2 and PLN de-creased.Acute MDMA exposure could increase NF-κB activity,while PDTC and CHE intervention could inhibit NF-κB activity.In MDMA exposed group,the expression of PKCζ and nucleoprotein p-p65(Ser311)both increased and could be inhibited by CHE.After the intervention of PDTC to block NF-κB,the amplitude of calcium oscillation was lower than that of the MDMA exposed group,and the expres-sion of RyR2,SERCA2a and NCX1 decreased.There was no significant change in PLN,while the ex-pression of Cav1.2 increased.Conclusion MDMA can lead to an increase of calcium ion concentration in cardiomyocytes.Calcium ions are involved in myocardial toxicity of MDMA.The mechanism is re-lated to changes in calcium handling proteins,mainly associated with the increased expression of RyR2.MDMA can up-regulate the intracellular activity of NF-κB through the PKCζ-NF-κB pathway and affect calcium handling proteins,which aggravate intracellular calcium overload during acute MDMA exposure.
7.Effects of simultaneous transplantation of multiple foot flaps in repairing defects of varying degrees on the palmar side of multiple fingers

Objective:To explore the effects of simultaneous transplantation of multiple foot flaps in repairing defects of varying degrees on the palmar side of multiple fingers.Methods:This study was a retrospective observational study. From February 2020 to March 2023, 11 patients aged 23-52 years with defects on the palmar side of multiple fingers (with 38 affected fingers) meeting the inclusion criteria were hospitalized at Longgang Orthopedics Hospital of Shenzhen, including 8 males (with 31 affected fingers) and 3 females (with 7 affected fingers). All affected fingers sustained skin and soft tissue defects, along with exposed deep tissue such as finger bones and tendons, and 2 thumbs exhibited nail bed and distal phalanx defects. The defect area of each finger ranged from 1.5 cm×1.0 cm to 5.5 cm×3.0 cm. Foot flaps were designed based on the area and depth of the finger defects, and multiple foot flaps were transplanted simultaneously to repair the wounds of fingers. The area of single flap resected ranged from 2.0 cm×1.0 cm to 5.5 cm×3.0 cm. Donor site wounds were closed directly or covered using artificial dermis or split-thickness skin grafts. The number of resected flaps, the number of affected fingers repaired, and the surgical duration were recorded. The flap survival and foot donor site wound healing were observed postoperatively. The flap and donor and recipient sites recovery were observed during follow-up. At the final follow-up, the functional recovery of affected fingers was evaluated based on the trial criteria of the Chinese Medical Association's Hand Surgery Society for function evaluation of thumb and finger reconstruction, the sensory function of the flaps was assessed by the sensory function evaluation standard of the British Medical Research Council, the cold tolerance of the affected hands was evaluated with the Cold Intolerance Severity Scale, the scars at the recipient and donor sites were assessed using the Vancouver scar scale (VSS), and the function recovery of flap donor foot was evaluated with the Maryland Foot Score criteria.Results:Donor sites were selected from 16 feet with 2-4 flaps harvested from each foot. Specifically, skin and soft tissue defects in 27 affected fingers were repaired using free lateral toe flaps, skin and soft tissue defects in 9 affected fingers were repaired using free medial foot flaps, and skin and soft tissue defects with finger bone defects in 2 affected fingers were repaired using free toenail osteocutaneous flaps. The surgical duration ranged from 5.60 to 9.25 hours. Postoperatively, all transplanted flaps in affected fingers survived, and all foot donor site wounds healed. Follow-up for 12-25 months showed that the appearance, color, and texture of the flaps were similar to the surrounding normal skin. The affected finger pulp was full with the fingertip having restored pain and touch sensation. There was no pain in the donor site of foot, and the walking ability of patients was not affected. At the final follow-up, the functional recovery was evaluated as excellent for 36 affected fingers and good for 2 affected fingers. The sensory function rating of the flaps was graded as S3-S4, and all affected fingers were mild intolerant to cold. VSS scores of the scars at the recipient sites ranged from 1-3, and VSS scores of the scars at the donor sites ranged from 1-5. The functional recovery of all flap donor feet was excellent.Conclusions:Simultaneous transplantation of multiple foot flaps is an effective treatment method for repairing defects of varying degrees on the palmar side of multiple fingers, resulting in excellent aesthetic and functional recovery of the affected fingers while causing minimal damage to the donor site.

The surgical duration ranged from 5.60 to 9.25 hours. Postoperatively, all transplanted flaps in affected fingers survived, and all foot donor site wounds healed. Follow-up for 12-25 months showed that the appearance, color, and texture of the flaps were similar to the surrounding normal skin. The affected finger pulp was full with the fingertip having restored pain and touch sensation. There was no pain in the donor site of foot, and the walking ability of patients was not affected. At the final follow-up, the functional recovery was evaluated as excellent for 36 affected fingers and good for 2 affected fingers. The sensory function rating of the flaps was graded as S3-S4, and all affected fingers were mild intolerant to cold. VSS scores of the scars at the recipient sites ranged from 1-3, and VSS scores of the scars at the donor sites ranged from 1-5. The functional recovery of all flap donor feet was excellent.Conclusions:Simultaneous transplantation of multiple foot flaps is an effective treatment method for repairing defects of varying degrees on the palmar side of multiple fingers, resulting in excellent aesthetic and functional recovery of the affected fingers while causing minimal damage to the donor site.
