To summarize the clinical experience of Professor LIU Shangyi in treating pruritus vulvae. It is believed that women have the physiological characteristics of liver and kidney as the root, and their pubic area is easily attacked by wind-dampness pathogenic qi, so the core mechanism of pruritus vulvae is proposed as wind-dampness accumulation and deficiency of liver and kidney. The core treatment method is to dispel wind-dampness and nourish the liver and kidneys, and modify the Danggui Decoction (当归饮子) to form a self-prescribed Yinyang Formula (阴痒方) as the basic prescription to treat pruritus vulvaen.

This paper summarized the clinical experience of Professor HE Chengyao in treating recurrent miscarriage complicated with subclinical hypothyroidism. It is believed that kidney yang insufficiency is the root of the disease, while the functional decline of the five zang (脏) organs and the obstruction of the sanjiao (三焦) pivot are the key links of the pathogenesis. In clinical practice, the division of yang numbers in the Book of Changes (《周易》) is followed, and 9 months is advocated as the basic treatment cycle for recurrent miscarriage complicated with subclinical hypothyroidism. During the first half of the period before pregnancy (the first 3 months), it is recommended to warm the pivot and sanjiao (三焦), and Yougui Pills (右归丸) is commonly used as the basic prescription to warm and supplement the kidney yang, together with the medicinals of invigorating blood and dissolving stasis, regulating and unblocking qi movement. During the second half of the period before pregnancy (the second 3 months), it is better to nourish essence and nature the embryo commonly with Wuzi Yanzong Pills (五子衍宗丸) in modification. After pregnancy (the third 3 months), it is suggested to supplement kidney and consolidate chong mai (冲), replenish qi and nourish blood mainly, supplemented by warming and nourishing heart yang, and self-made Bushen Antai Formula (补肾安胎方) which is modified based on the combination of Shoutai Pills (寿胎丸) and Wenbao Beverage (温胞饮) is commonly used.Additionally, it is recommended to adjust lifestyle and diet so as to balance yin and yang and improve the physical condition.

Objective:To discuss the efficacy and safety of stereotactic electroencephalography (SEEG) in epileptogenic foci excision in pediatric patients with drug-resistant epilepsy. Methods:A total of 126 pediatric patients (<18 years old) with drug-refractory epilepsy who received SEEG-guided epileptogenic foci excision in Epilepsy Center, Aviation General Hospital from January 2015 to March 2022 were selected. The clinical data and efficacy were retrospectively analyzed, and prognoses of these pediatric patients were evaluated by Engel grading 1 year after resection.Results:(1) A total of 1289 electrodes were implanted, with a mean of (10.09±2.92) electrodes per pediatric patient; 55 pediatric patients had unilateral implant and 71 had bilateral implant. Mean EEG monitoring time was (8.69±5.71) d, ranged 3-28 d. Epileptogenic focus could be located in 114 pediatric patients (90.5%) after initial implantation under SEEG monitoring, and secondary implantation for accurate positioning was given in 12 pediatric patients (9.5%). (2) Lobectomy was performed in 27 pediatric patients (21.4%), multi-lobectomy or tailored cortical resection in 36 (28.6%), tailored cortical resection on single lobe in 60 (47.6%), and tailored cortical resections on single lobe or hippocampal amygdala resection combined with corpus callosotomy in 3 (2.4%). Minimally invasive exploring hemostasis under SEEG was performed in 13 pediatric patients (17 electrodes) and postoperative CT was normal. A little asymptomatic epidural, subdural or cerebral parenchymal hematoma spontaneously absorbed was noted in 4 pediatric patients after implantation under SEEG monitoring. No perioperative infection, CSF leakage, death or severe disability was noted. (3) Mean follow-up was performed for (26.1±7.26) months; 66 (52.3%) pediatric patients reached Engel grading I, 33 (26.2%) reached Engel grading II, 21 reached Engel grading III (16.7%), and 6 (4.8%) reached Engel grading IV. Thirteen pediatric patients with failed resection received SEEG-guided epileptogenic foci excision for the second time: 8 (76.9%) had Engel grading I and 2 had Engel grading II 1 year after follow-up, accounting for 76.9% totally.Conclusion:SEEG-guided epileptogenic foci excision is safe and effective in drug-refractory epilepsy; for pediatric patients with poor initial results, SEEG can be used to relocate the epileptogenic focus, and a second resection of epileptogenic focus can also obtain good results.

Objective:To explore the effect of vaginal microbiota disorder on pregnancy outcomes in the first-time frozen-thawed embryo transfer (FET) patients and perinatal outcomes in single pregnancy live delivery patients. Methods:The clinical data of 2 299 cycles of FET patients in the Reproductive Center of the Third Affiliated Hospital of Zhengzhou University from January 2021 to December 2022 were collected in a retrospective cohort study. According to the types of dominant bacteria in the vaginal microbiota before transplantation, they were divided into three groups: control group (dominant bacteria were Lactobacillus, which was Gram positive bacteria, 1 849 cycles), the Gram negative bacilli group (191 cycles), and the Gram positive cocci group (259 cycles). Baseline data and pregnancy outcomes were compared among the three groups. The perinatal outcomes of select single pregnancy live birth FET patients who met the inclusion criteria were further analyzed among the three groups. The main observation indicators were live birth rate, miscarriage rate, and preterm birth rate. A multivariate logistic regression model was used to control confounding factors in the main observation indicators, and to analyze the relationship between dominant bacterial types in the vaginal microbiota and live birth rate, miscarriage rate, and preterm birth rate. Results:The difference in endometrial thickness on the day of transplantation among control group, Gram negative bacilli group, and Gram positive cocci group was statistically significant [(9.38±1.58) mm, (9.56±1.70) mm, and (9.84±1.74) mm, respectively, P<0.001]. In the endometrium preparation methods, the proportion of down-regulation+artificial cycle patients in the Gram positive cocci group was higher than that in control group, and the difference was statistically significant [13.13% (34/259) and 7.46% (138/1 849), respectively, P<0.016 7]. In the pregnancy outcomes, there was a statistically significant difference in live birth rate among control group, Gram negative bacilli group, and Gram positive cocci group [49.86% (922/1 849), 49.21% (94/191) and 41.70% (108/259), respectively, P=0.048]. The live birth rate of the Gram positive cocci group was lower than that of control group, and the difference between the two groups was statistically significant ( P<0.016 7). There were no statistically significant differences in other pregnancy outcomes and perinatal outcomes of single pregnancy live birth FET patients (all P>0.05). The multivariate logistic regression model corrected for female age, infertility years, basal follicle stimulating hormone, anti-Müllerian hormone, proportion of single embryo transfer, proportion of single blastocyst transfer, endometrial thickness on transfer day, and endometrial preparation methods, Gram positive cocci were independent risk factors for live brith after FET transplantation (a OR=0.73, 95% CI: 0.55-0.95, P=0.021). Conclusion:The dominant bacteria in the vaginal microbiota before embryo transfer are Gram positive cocci, which may be related to a decrease in live birth rate in first-time FET patients, but not significantly related to the perinatal outcomes.

OBJECTIVE To study the tocolysis effects of Angelica sinensis polysaccharides on threatened abortion model rats and their impacts on Th1/Th2 balance by regulating the phosphoinositide 3-kinase (PI3K)/protein kinase B (AKT) signaling pathway. METHODS Pregnant rats were randomly grouped into the control group, model group, A. sinensis polysaccharide group (200 mg/kg), PI3K/AKT signaling pathway inhibitor LY294002 group (5 mg/kg), and A. sinensis polysaccharide+LY294002 group (200 mg/kg A. sinensis polysaccharide+5 mg/kg LY294002), with 10 rats in each group. Except for the control group, rats in all other groups were given mifepristone (8.3 mg/kg) and misoprostol (100 μg/kg) intragastrically to establish a threatened abortion model, and intragastric or intraperitoneal injection of corresponding drugs. The serum levels of estrogen, progesterone, interferon-γ (IFN-γ), tumor necrosis factor-α (TNF-α), and interleukin-4 (IL-4) in each group of rats were detected, and the uterine ovarian index and embryonic mortality rate of rats in each group were measured; the morphology of uterine tissue in rats was observed in each group; Th1/Th2 balance in peripheral blood of rats as well as the expression of PI3K/AKT signaling pathway-related proteins in the uterine tissues of rats in each group were detected. RESULTS Compared with the control group, the uterine tissue of rats in the model group showed pathological damage; the serum levels of estrogen, progesterone and IL-4, uterine ovarian index, peripheral blood Th2 cell ratio, and the ratios of phosphorylated PI3K (p-PI3K)/PI3K and phosphorylated AKT (p-AKT)/AKT in uterine tissue were all decreased (P<0.05); the embryo mortality rate, Th1 cell ratio, Th1/Th2 ratio, and serum levels of IFN-γ and TNF-α were increased (P<0.05). Compared with the model group, the pathological damage of uterine tissue in the A. sinensis polysaccharide group was reduced, and the above indexes were all improved significantly (P<0.05); LY294002 could weaken the effect of A. sinensis polysaccharide on model rats (P<0.05). CONCLUSIONS A. sinensis polysaccharides can improve Th1/Th2 imbalance in threatened abortion model rats by activating the PI3K/AKT signaling pathway, thereby inhibiting immune inflammation, and promoting embryo survival.
