The National Essential Medicines System could protect public health and ensure access to essential medications.Although the current selection methods for China's National Essential Medicines Lists(NEMLs)are becoming more scientific and standardized, there are still problems such as much emphasis on expert experience and the lack of transparency of decision-making basis. To address these issues,it proposes an evidence-based decision-making pathway for NEMLs selection guided by clinical value. This approach ensures a strong integration of evidence and decision-making,offering valuable insights for improving the adjustment procedures and selection criteria of the NEMLs in China.

Lipophosphatidic acid(LTA)was used to stimulate Mac-T cells, and the expression lev-els and phosphorylation levels of key proteins of nuclear factor-κB(NF-κB)and mitogen-activated protein kinase(MAPK)signaling pathway and the expression levels of upstream key action factors TLR4 and MyD88 proteins were detected by Western blot, and EDU assay was used to detect cell proliferation levels and flow cytometry was used to detect apoptosis. The results showed that acti-vation of GPR120 significantly decreased the phosphorylation levels of LTA-induced NF-κB(P65 and IκBα)(P<0.01)and MAPK(JNK,ERK,p38)(P<0.01)in Mac-T cells;inhibition of GPR120 was able to upregulate LTA-induced NF-κB(p65 and IκBα)in Mac-T cells(P<0.01)and MAPK(JNK,ERK,p38)phosphorylation levels(P<0.01);and activation of GPR120 significantly allevia-ted LTA-induced upregulation of TLR4 and MyD88(P<0.01);inhibition of GPR120 significantly exacerbated LTA-induced upregulation of TLR4 and MyD88(P<0.05);LTA stimulation led to a trend of diminished Mac-T cell proliferation and significantly increased apoptosis, whereas activa-tion of the GPR120 gene significantly increased cell activity(P<0.01),promoted cell proliferation and significantly reduced apoptosis(P<0.05)thereby alleviating the damage to Mac-T cells by LTA;LTA stimulation led to a highly significant increase in apoptosis(P<0.01).In contrast,acti-vation of the GPR120 gene significantly reversed the increase in the apoptosis rate of Mac-T cells induced by LTA(P<0.01),while inhibition of the GPR120 gene enhanced the apoptosis-promo-ting effect of LTA(P<0.05),indicating that activation of the GPR120 gene attenuated the in-crease of apoptosis rate caused by LTA-induced inflammatory Mac-T cells. The results suggest that GPR120 can regulate inflammation by mediating TLR4 and MyD88 expression to inhibit NF-κB/MAPK inflammatory pathway activation and can promote cell proliferation.

Objective Search and collect the previous relevant literature on the influence of diabetes on the progress of tendinosis, summarize the relevant conclusions and discuss the pathogenesis of diabetes accelerating the progress of tendinosis, mainly including summarizing the biomechanical and histological changes of diseased tendons in diabetes patients,as well as its pathogenesis at the cellular level. Methods This paper reviewed and analyzed the domestic and foreign literature related to dia-betes tendinosis by searching some Chinese and English databases like PubMed,Springer Link,CNKI,Google Academic.Re-sults Most previous studies more support the theory of degeneration as the pathogenesis of tendinosis, but now more and more researchers find that diabetes,as a systemic metabolic disease, can induce the occurrence of tendinosis through a variety of mech-anisms,or aggravate the progress of tendinosis, such as the accumulation of advanced glycation end products(AGEs)at the le-sion site, chronic inflammatory reaction at the lesion site, increased oxidative stress level of tendon cells And changes in the ac-tivities of matrix metalloproteinases and glutamine transaminase.By summarizing and analyzing the various understandings of dia-betes on the pathogenesis of tendinosis proposed by previous researchers, the relationship between diabetes and tendinosis is grad-ually clear. Conclusion As a systemic metabolic disease, diabetes can induce the occurrence of tendinosis or aggravate the pro-gress of tendinosis through a variety of ways and mechanisms.Research and clarification of these ways can provide us with a pow-erful weapon to study and treat tendinosis. However,at present, there are many possible pathogenesis of diabetes tendinosis, which still needs to be studied step by step to further clarify its main pathogenesis.

International Journal of Pediatrics 2024;51(9):605-609
Vitamin D works in the active form 1,25(OH) 2D 3 after binding to vitamin D receptor.Vitamin D deficiency is common in children with renal disease, and the abnormalities of vitamin D and its metabolic pathways are closely related to the occurrence and development of proteinuria and the deterioration of renal function.Vitamin D not only plays its classic role in maintaining calcium and phosphorus homeostasis, but also protects the kidney by improving the immune function of children, blocking the renin-angiotensin-aldosterone system, preventing the occurrence and development of proteinuria, alleviating the damage of podocytes, and delaying renal interstitial fibrosis. Therefore, this article reviews the research progress of vitamin D and its receptor gene polymorphism and kidney disease,in order to serve the clinical practice.

Chinese Journal of Inflammatory Bowel Diseases 2024;08(4):304-308
The pulmonary complications associated with inflammatory bowel disease (IBD) predominantly affect the airways, with bronchiectasis being the most frequently observed condition. Concurrently, there is a notable increase in the prevalence of IBD among individuals with pulmonary disorders. This trend suggests a possible reciprocal relationship between IBD and pulmonary diseases, indicative of the "gut-lung axis" at play. The etiology of bronchiectasis in the context of IBD remains elusive, and there is a dearth of effective clinical tools for monitoring and prevention. This paper delves into the pathogenesis of IBD-related bronchiectasis, with a particular emphasis on the role of the gut-lung axis in this scenario. It scrutinizes the impact of immune responses and dysbiosis of the microbiome on the chronic inflammation observed in both the intestinal tract and the respiratory system.
