1.Effect of ADAM12,S100A8,and Serum Tumor Markers on Chemotherapy Outcomes and Prognosis in Patients With Triple-Negative Breast Cancer

Objective To investigate the influence of a disintegrin and metalloproteinase 12(ADAM12),S100 calcium binding protein A8(S100A8),and serum tumor markers on chemotherapy outcomes and prognosis in patients with triple-negative breast cancer.Methods A totla of 300 patients with breast cancer admitted between January 2020 and January 2021 were included.Based on pathological immunohistochemistry findings,the patients were divided into a triple-negative group(n=98,triple-negative breast cancer)and a non-triple-negative group(n=202,non-triple-negative breast cancer).Serum tumor markers(carcinoembryonic antigen[CEA]and carbohydrate antigen 125[CA125]),the levels of ADAM12 and S100A8,and tissue protein expression levels of ADAM12 and S100A8 were compared between the two groups.The relationship between the protein levels of ADAM12 and S100A8 in the cancer tissues and the clinicopathological characteristics of the triple-negative group was analyzed.Differences in the protein levels of ADAM12 and S100A8 between patients with different chemotherapy outcomes(remission vs.non-remission)and different follow-up outcomes(survival vs.death)were analyzed.Kaplan-Meier survival analysis was used to examine the relationship between the protein levels of ADAM12 and S100A8 in cancer tissues and the prognosis.Results The protein expression levels of ADAM12 and S100A8 in cancer tissues from the triple-negative group were higher than those in the non-triple-negative group(P<0.05).The protein expression levels of ADAM12 and S100A8 in cancer tissues were correlated with tumor diameter,histological grading,axillary lymph node metastasis,TNM staging,and differentiation degree in the triple-negative group(P<0.05).The levels of CEA,CA125,ADAM12,and S100A8,as well as the protein expression levels of ADAM12 and S100A8 in cancer tissues in the non-remission group,were higher than those in the remission group(P<0.05).Similarly,these markers were also significantly elevated in the death group compared to those in the survival group(P<0.05).After 3 years of follow-up,the overall survival(OS)of patients with low ADAM12 expression was 36.0(8.0,36.0)months,while that of patients with high ADAM12 expression was 32.5(4.0,36.0)months,showing a statistically significant difference(log rank x2=12.913,P<0.001).The OS of patients with low S100A8 expression was 36.0(7.0,36.0)months,while that of the high-expression group was 31.0(4.9,36.0)months,also showing a statistically significant difference(log rank x2=24.151,P<0.001).Conclusion ADAM12 and S100A8 protein expression levels in triple-negative breast cancer tissues are higher than those in non-triple-negative breast cancer tissues.Serum tumor markers and ADAM12 and S100A8 protein expression levels(in both serum and tumor tissues)affect the chemotherapy outcomes and prognosis of triple-negative breast cancer patients,among which the expression levels of ADAM12 and S100A8 proteins in tumor tissues can serve as predictors of patient prognosis.

4.Construction and practice of a "three progressions, four integrations, and five cocultivation" talent cultivation system for nursing professional groups under the 1+X certificate system

Objective:To explore the talent cultivation model of nursing professional group under the 1+X certificate system, to build a talent cultivation system for nursing professional groups with "three progressions, four integrations, and five cocultivation"Methods:From 1 July 2021 to 31 December 2023, built a professional group course system of "mutual sharing of foundations, mutual generation of majors, mutual selection of positions, and progressive improvement of abilities". Aligned course content with X certificate content. Deepen the reform of the three education systems, create a "dual teacher" teaching team that combines full-time and part-time work, and build teaching resources that connect the cloud and the ground. The application effect of this model was evaluated in 2021 nursing majors in Cangzhou Medical College by cluster sampling.Results:There were 748 students majoring in nursing in class of 2021, 150 boys aged 19-22(20.00 ± 1.23), 598 girls, aged 19-21(19.00 ± 2.57). There were 118 students majoring in midwifery in class of 2021, all of them were girls, aged 18-20(18.00 ± 2.11). There were 128 students majoring in rehabilitation therapy technology in class of 2021, 66 boys aged 19-21(19.00 ± 2.11), 62 girls aged 19-21(19.00 ± 2.54). Nursing professional group completed the "Basic Nursing Technology Training Guidance", "Elderly Care" and other 5 self-compiled textbooks, 730 students had participated in the X certificate assessment, 729 students e passed the assessment, the passing rate was 99.9%.Conclusions:The talent cultivation model of "three progressions, four integrations, and five co cultivation" in the nursing professional group can cultivate students′ professional abilities, and cultivate a composite technical and skilled talent with multiple abilities and interconnections in one job.
5.Analysis of oligoclonal bands detection results of 3 217 patients with neurological disorders

Objective:To study positive rates and typing of oligoclonal bands (OCB) in patients with neurological disorders, and to reveal the clinical significance and applicational value of OCB test.Methods:A retrospective analysis was performed on the detection results of 3 217 patients with neurological disorders who undertook both serum and cerebrospinal fluid OCBs in the First Hospital of Peking University from January 2012 to August 2022. According to the final diagnosis, the patients were divided into 13 groups including multiple sclerosis (479 cases), neuromyelitis optica spectrum disorders (935 cases), autoimmune encephalitis (192 cases), viral encephalitis (94 cases), nervous system complication after HSCT (232 cases), Guillain-Barré syndrome (644 cases), chronic inflammatory demyelinating polyneuropathy (157 cases), etc. Cerebrospinal fluid and serum OCBs were detected using isoelectric focusing electrophoresis combining immunofixation, then classified into Ⅰ-Ⅴ types according to the morphology. Consequently, positive rates and types were analyzed for each group. χ2 test was used for comparison between groups. Results:The positive rates of cerebrospinal fluid OCB in multiple sclerosis, nervous system complication after hematopoietic stem cell transplantation (HSCT), autoimmune encephalitis, viral encephalitis, neuromyelitis optica spectrum disorders, Guillain-Barré syndrome and chronic inflammatory demyelinating polyneuropathy were respectively 66.8% (320/479), 48.7% (113/232), 46.4%(89/192), 19.1% (18/94), 17.6% (165/935), 9.9% (64/644), 5.1% (8/157). For patients with multiple sclerosis, neuromyelitis optica spectrum disorders, viral encephalitis, and autoimmune encephalitis, Type Ⅱ bands took the majority of cerebrospinal fluid OCB-positive cases with the rates of 94.1% (301/320), 78.7% (70/89), 77.8% (14/18), and 77.6% (128/165) respectively, indicating intrathecal IgG synthesis; for patients with nervous system complication after HSCT, Guillain-Barré syndrome and chronic inflammatory demyelinating polyneuropathy, type Ⅳ bands took the majority of cerebrospinal fluid OCB-positive cases with the rates of 94.7% (107/113), 82.8% (53/64) and 100% (8/8), indicating no obvious intrathecal IgG synthesis. The positive rates of cerebrospinal fluid oligoclonal bands were significantly different among all groups (χ 2=1 268.31, P<0.001). Conclusion:The positive rates of cerebrospinal fluid oligoclonal bands are different among different neurological disorders, in which the positive rate of cerebrospinal fluid OCB is higher with type Ⅱ bands as the majority type in multiple sclerosis, which indicates that the detection and typing of cerebrospinal fluid OCB are helpful for the diagnosis of various neurological diseases, especially for multiple sclerosis.
6.Effect of process optimization management mode in equipment management of Central Sterile Supply Department

Objective:To explore the effect of process optimization management mode in equipment management of Central Sterile Supply Department.Methods:The 1 000 pieces of equipment received by the Central Sterile Supply Department of the First Affiliated Hospital of Zhengzhou University were selected by convenience method, and the 500 pieces of equipment (425 pieces of hospital equipment and 75 pieces of loaner equipment) received by traditional process management from January to March 2021 were used as control group. The 500 devices (429 pieces of hospital equipment and 71 pieces of loaner equipment) received by the process optimization management mode from April to June 2021 were selected as the observation group. The equipment cleaning rate, cleaning pass rate and loaner equipment backwash rate were compared between the two groups.Results:The equipment cleaning rate in the observation group was 99.4% (497/500) , which was higher than 92.4% (462/500) in the control group, and the difference was statistically significant ( P<0.05) . The pass rate of medical equipment cleaning in the observation group was 98.0% (490/500) , which was higher than 90.2% (451/500) in the control group, with a statistical difference ( P<0.05) . The backwash rate of loaner equipment in the control group was 98.7% (74/75) , and the backwash rate in the observation group was 100.0% (71/71) , and there was no significant difference between the two groups ( P>0.05) . Conclusions:The application of the process optimization management mode to the Central Sterile Supply Department can improve the medical equipment cleaning rate and cleaning pass rate.

8.Effects of attentional bias training on attention bias, psychological craving and relapse rate in male patients with alcohol dependence

Objective:To explore the effect of attentional bias training on the attentional bias, psychological craving and relapse rate of alcohol dependent patients during rehabilitation.Methods:A randomized controlled trial was used to select 90 alcohol-dependent patients and randomly divide them into an intervention group and a control group. The alcohol cue pictures and neutral pictures were imported into E-Prime software at a ratio of 1∶1.The patients in the two groups were trained for eight times with attentional bias and without attentional bias by point delection paradigm. After each training, the software automatically records the response time and correct rate of the cue pictures and neutral pictures, and before intervention(T0), after 4 interventions(T4), and after 8 interventions(T8), the alcohol craving score was assessed by visual analogue scale and the relapse situation was followed up 1 month after the intervention. SPSS 24.0 software was used for statistical analysis.Repeat measure ANOVA, χ2 test and t-test were used to compare the response time, accuracy and psychological craving of the two groups. Results:Repeated measurement analysis of variance showed that there were interaction between group and time in cue-map response time, line chart accuracy and psychological craving score of the two groups ( F=5.218, 6.939, 147.14, all P<0.01). Simple effect analysis showed that there was no significant difference in cue pictures response time, cue pictures accuracy and psychological craving score between the intervention group and the control group at T0(all P>0.05), but there was significant difference at T4 and T8 (all P<0.05), and there was significant difference in neutral pictures reaction only at T8 ( P<0.05). The comparison between two groups showed that the intervention group showed a downward trend in cue pictures response, T4 and T8 were lower than T0, with statistical difference (both P<0.001), while there was no difference in the control group. In the intervention group, the correct rate of cue pictures increased, T4 and T8 were higher than T0, and there were statistical differences (both P<0.001), while there was no difference in the control group( P>0.05). The psychological craving scores of both the intervention group and the control group showed a downward trend, T4 and T8 were lower than T0, there were statistical differences (both P<0.001), but the difference between T4 and T8 in the control group remained unchanged. One month after the end of the intervention, the difference in the rate of relapse between the intervention group and the control group was marginal (11.10%, 26.70%, P=0.059). The total number of days of abstinence in the intervention group was longer than that in the control group ((28.33±4.99)d, (26.47±6.66)d, P=0.010). Conclusion:Attentional bias training can improve the mental craving and attentional bias of alcohol-dependent patients during the rehabilitation period, and the relapse rate decreased one month after the intervention.
9.A follow-up controlled study of repeated cue exposure therapy combined with biofeedback training for patients with alcohol dependence after the acute withdrawal

Objective:This study aims to investigate the effect of repeated cue exposure therapy combined with biofeedback training on patients with alcohol dependence after acute withdrawal.Methods:A total of 130 male alcohol dependent patients (PADs) who were hospitalized in Beijing Huilongguan Hospital from November 2016 to June 2018 were enrolled, and after the alcohol withdrawal syndrome eliminated, the PADs were divided into two groups by randomized controlled research methods as follows: repeated cue exposure combined biofeedback therapy group (study group, SG) and routine treatment group (control group, CG). After controlling the symptoms of acute alcohol withdrawal, the SG patients were given cue exposure therapy and biofeedback training 12 times, and the CG patients were given routine rehabilitation treatment and general psychological support over a four-week session. The psychological craving for alcohol of two groups were evaluated with Visual Analog Scale (VAS) and Alcohol Urge Questionnaire (AUQ) before and afer treatment. Both groups were discharged after four-week treatment and received follow-ups at the end of the 4th, 8th, 12th, 24th and 52th week, respectively. The alcohol consumption questionnaire was used to evaluated with the recurrence rate, cumulative abstinence duration (CAD), time to first re-drinking, and average daily alcohol consumption after relapse for each follow-up assessment. Independent sample t test or chi-square test was used for comparison between groups. Results:The decrement of VAS before and after cue exposure treatments in the SG was significantly higher than that in the CG (2.10(0.33,2.78) vs. 0.60(-0.30,1.70), Z=-3.50, P=0.001).The decrement of AUQ score was also significantly higher than the CG (3.00(1.00,6.00) vs.1.00(-2.00,4.00), Z=-3.02, P=0.003). There was a significant difference in the rate of recurrence at the end of week-4 between the SG and CG (30.0% vs. 54.2%, χ2=7.17, P=0.007). The time before the first re-drinking in the experimental group was longer than that in the control group (52.5(26.0,208.5) d vs. 21.0(5.0,183.0) d, Z=-2.5, P=0.013). The total average daily alcohol consumption of the experimental group was lower than that of the Control Group (11.0±8.5 vs.15.2±8.6, t=-2.69, P=0.008). Conclusions:The results suggest that repeated cue exposure combined with biofeedback therapy can reduce the psychological craving for alcohol and recurrence rate after a short term of treatment. Meanwhile, the long-term (follow-up 1 year after treatment) recurrence rate may not change significantly, however the first re-drinking can be postponed, and they tend to drink less even if they re-drink again.

Objective:To explore a new training mode for nursing professionals suitable for the 1+X certificate system, and realize the training goal of "one specialty and multi-ability" compound technical nursing talents.Methods:To take the "1+ X" certificate standard as the basis for the construction of nursing specialty, to reconstruct the talent training program of integration of graduation certificate and certificate. To take the content of "1+X" certificate as the basis for the construction of professional courses, construct the core curriculum system of integration of curriculum and certificate. To build a "new double-qualified" teaching staff and constructing of new double-qualified teachers and accelerate the development of quality resources.Results:The talent training mode of integration of graduation certificate and certificate under the 1+X certificate system was constructed. The curriculum structure has been optimized. The teachers′ability of teaching, training and examination was improved.Conclusions:The new mode of training nursing professionals under the 1+X certificate system meets 1 degree education and X vocational training of nursing students and achieve a "1" and "X" seamless convergence. It provides innovative ideas for the promotion and implementation of 1+X certificate system pilot work in the field of nursing education nationwide.
