1.Epidemiological Characteristics and infection sources of cholera in China from 2005 to 2024

Objective:To analyze the epidemiological characteristics and infection sources of cholera in China from 2005 to 2024.Methods:A total of 2 066 cholera cases were included in the study, which were obtained from the China Disease Control and Prevention Information System (CDPCIS) of China CDC. The information on cholera clusters was downloaded from the National Public Health Emergency Event Surveillance System (PHEESS) of China CDC. A total of 128 cholera clusters were included and analyzed in this study. The epidemiological characteristics and infection sources of cholera were analyzed. The Jointpoint model was applied to analyze the incidence trend, and annual percentage change (APC) was also quantified.Results:From 2005 to 2024, a total of 2 066 cholera cases were reported, with an average of 103 cases reported annually. Specifically, the incidence showed a marked downward trend from 2004 to 2015 ( APC=-26.78%, P=0.006). During 2015-2024, the disease remained at low endemic levels, with an average of 18 reported cases annually ( APC=-2.68%, P=0.807). Cholera peak season was from May to October. A total of 24 provinces reported cholera cases, which were mainly distributed in Zhejiang, Fujian, Beijing, Jiangsu, Anhui, Guangdong, and Hainan provinces, accounting for 78.03% of the total cases. Pathogen surveillance indicated an alternating prevalence of Vibrio cholerae serogroups O1 and O139 among laboratory-confirmed cases between 2005 and 2024. There was a disparity in the dominant serogroup of Vibrio cholerae by region. The results from 128 cholera clusters indicated that cholera outbreaks frequently occurred in rural banquets (64.84%), followed by regular restaurants (13.28%). Among these, 63 clusters (49.22%) with identified infection sources indicated that foodborne transmission (95.24%) was the primary mode of cholera transmission, which mainly through seafood and aquatic products, such as soft-shelled turtles, shrimp and shellfish. The characteristics of cholera clusters caused by Vibrio cholerae serogroups O1 and O139 showed statistically significant differences in scale, attack rate, place of residence, setting, and infection source ( P<0.05). Conclusion:Cholera incidence has remained consistently low since 2015 in China, mainly in sporadic cases. Rural gatherings (e.g., wedding banquets) are the main settings for cholera clusters. The main infection sources are predominantly caused by cross-contamination due to improper processing practices of aquatic products, such as soft-shelled turtles.

Objective:The objective of this study was to understand the incidence, spatial and temporal distribution characteristics and trends of bacillary dysentery in China from 2005 to 2024 in order to identify the high-risk groups and reveal the potential risk factors and to provide a scientific basis for optimizing the allocation of preventive and control resources, formulating targeted intervention strategies and assessing the effectiveness of the measures.Methods:The nationally reported incidence data of bacillary dysentery was collected from 2005 to 2024 in the Chinese Center for Disease Control and Prevention National Notifiable Diseases Reporting Information System. Descriptive epidemiological methods were used to analyze the population characteristics of bacillary dysentery cases. A Joinpoint regression model was constructed to examine long-term trends in reported incidence rates and spatial dynamic window scanning statistics were applied to detect spatial clusters of bacillary dysentery cases.Results:Between 2005 and 2024, 3 520 247 cases of bacillary dysentery were reported across China, with an average incidence rate of 12.88 per 100 000 people, after which the rate of decline decreased. The incidence rate showed a general downward trend, featuring a significant inflection point in 2016. It exhibited marked seasonality, peaking from May to October (summer-autumn), which weakened over time. From 2005 to 2024, the most likely clusters were in Beijing and Tianjin. Males, infants, the elderly, farmers, and children not in daycare showed many cases.Conclusions:The results revealed that the peak incidence of bacillary dysentery in China from 2005 to 2024 was featured in the summer-autumn months. High-incidence areas were mainly Beijing and Tianjin. The key groups, including males, infants, the elderly, farmers and children not in daycare, were identified. Enhancing surveillance, targeted health education, and preventive measures, especially in these key populations and in regions where the disease shows a high incidence should be strengthened.
3.Clinical application value of the combined detection of fecal SDC2 and TFPI2 gene methylation in the diagnosis of colorectal cancer and advanced adenoma

Objective To investigate the clinical application value of the combined detection of fecal syndecan 2(SDC2)and tissue factor pathway inhibitor 2(TFPI2)gene methylation in the diagnosis of colorectal cancer and advanced adenoma.Methods 124 pa-tients with colorectal lesions,including 43 colorectal cancer,52 advanced adenoma,and 29 non-advanced adenoma,and 46 patients with other gastrointestinal diseases such as colorectal inflammatory diseases,ulcers,etc.,who visited 5 hospitals in Zhengzhou from 2020 to 2023 were enrolled as the disease group.In addition,78 healthy individuals who underwent physical examinations in the same period were selected as the healthy control group.Their fecal samples were collected before undergoing colonoscopy,and the methyla-tion levels of SDC2 and TFPI2 genes were detected using the quantitative methylation-specific PCR(qMSP).Results The positive rates of the combined detection of SDC2 and TFPI2 gene methylation in the colorectal cancer group and advanced adenoma group(95.34%and 88.46%)were significantly higher than those in the non-advanced adenoma group(48.27%,x2=18.75/13.63,P<0.05)and healthy controls(3.84%,x2=96.38/91.54,P<0.05).In the colorectal cancer group,the postoperative positive rate of the combined detection in 34 patients with positive methylation of SDC2 and TFPI2(5.88%)was significantly lower than the preoperative positive rate(95.34%,x2=54.74,P<0.05).Moreover,the mean Ct values of methylation of SDC2 and TFPI2 genes(32.38±3.72 and 31.85±3.21)in the colorectal cancer group were significantly lower than that in the advanced adenoma group(35.07±3.09 and 34.96±2.78,t=3.84 and 5.05,P<0.05).The analysis results of the ROC curve showed that the sensitivities of SDC2,TFPI2,and their combination detection in the diagnosis of colorectal cancer were 86.40%,88.37%,and 89.35%,respectively,while in differenti-ating advanced adenoma they were 73.52%,66.73%,and 80.15%,respectively.The combined detection of SDC2 and TFPI2 in differ-entiating colorectal cancer and advanced adenoma achieved the highest diagnostic efficacy(AUCROC=0.889 and 0.738,respectively).Conclusion The combined detection of methylation of SDC2 and TFPI2 genes in fecal samples has high diagnostic value for colorectal cancer and advanced adenomas,and also has certain clinical application value for the progression and postoperative monitoring of the disease.

7.Study on image quality optimization of abdominal digital radiography with different additional filters

Objective:To investigate the effect of different additional filters on the image quality and radiation dose in abdominaldigital radiography (DR).Methods:Retrospective analysis was performed on 10 cases of abdominal DR from December 2020 to January 2021 in Fuxing Hospital, and the average mAs was calculated. Using automatic exposure control (AEC) technology, a polymethyl methacrylate (PMMA) slab of the corresponding thickness (18 cm) at the same output of above mAs was confirmed and used as the attenuator to simulate the abdomen. The phantom of CDRAD 2.0 and 17 slices of 10 mm thick PMMA plates (total thickness 18 cm) were placed on the bed. The additional filters were selected as no additional filter, 2 mmAl, 0.1 mmCu+ 1 mmAl, 0.1 mmCu+ 2 mmAl respectively. The AEC levels were selected at -2, -1, 0, 1, and 2 respectively. The images were collected and analyzed by using CDRAD 2.0 software to obtain the image quality factor (IQF inv). The incident air kinetic energy on the surface of the PPMA was measured for each exposure, and PCXMC software was used to estimate the organ dose and effective dose, and a comparative analysis was made. Results:The skin incident doses at no additional filter, 2 mmAl, 0.1 mmCu+ 1 mmAl, 0.1 mmCu+ 2 mm Al were (0.546 1±0.200 8), (0.376 2±0.133 8 ), (0.285 3±0.100 1) and (0.289 9±0.099 2) mGy, respectively. The estimated effective doses were (79.63±29.24)×10 -3, (71.05±25.56)×10 -3, (63.58±22.18)×10 -3 and (67.64±23.11)×10 -3 mSv, respectively. The gonadal doses were (0.058 1±0.020 8), (0.050 0±0.018 0), (0.044 8±0.015 6) and (0.047 7±0.016 3) mGy.The IQF inv values were 4.70±0.61, 4.80±0.84, 4.60±0.55, 4.60±0.60, respectively. There were linear correlations between the effective dose and the skin incident dose under different additional filtration, and the effective doses increased with the increase of the incident doses. The dose was lowest at the addition filtration of 0.1 mmCu+ 1 mmAl. There was no significant difference in the IQF inv between groups( P>0.05). Conclusions:The optimal additional filtration for abdominal DR was 0.1 mmCu+ 1 mmAl with the image quality meeting the requirements of clinical diagnosis, the radiation dose reduced reasonably, and the objective of the optimization of radiographic parameters achieved.
8.Literature case analysis of liver injury caused by bosentan

Objective:To understand the clinical characteristics of bosentan-related liver injury.Methods:PubMed, Ovid, Web of Science, Embase, Springer-link, Wiley Oline Library, CNKI, Wanfang and VIP databases were searched as of April 2020 and case reports on bosentan-related liver injury were collected. General information, primary diseases, comorbidities, bosentan use, combined medication, and occurrence, treatment and outcome of liver injury in patients were extracted and analyzed by descriptive statistical method.Results:A total of 10 case reports on bosentan-related liver injury were included, involving 12 patients (3 from China, 3 from Japan, 2 from Italy, and 4 from Australia, Brazil, Canada, and Sweden respectively). The 12 patients were all female, aged from 29 to 79 years with an average age of 55 years. The primary diseases were systemic sclerosis-associated pulmonary hypertension (PAH) in 5 patients, connective tissue disease-associated PAH in 2 patients, idiopathic, secondary, chronic thromboembolic, biliary cirrhosis-associated, and non-hepatic portal hypertension-associated PAH in 5 patients, respectively. Four patients had comorbidities. All the 12 patients had combined medications,which included 1-9 kinds of drug. Liver injury occurred in patients 20 days to 57 months after bosentan administration (<1 month in 3 patients, 1-3 months in 4 patients, and >12 months in 5 patients) and was manifested as elevated transaminases, accompanied by elevated bilirubin in 5 patients; the clinical manifestations in 6 patients were described, including yellow staining of skin and sclera in 4 cases, anorexia in 4 patients, nausea in 3 patients, fatigue in 3 patients, and vomiting, weight loss, and drowsiness in 3 patients, respectively; liver biopsy was performed in 1 patient, showing autoimmune hepatitis. After liver injury, bosentan was discontinued or continued with a halved dose; symptomatic treatments were given depending on the specific situation of liver injury; among the 12 patients, 9 had normal liver function, 2 had improved liver function, and 1 died of multiple organ failure.Conclusions:Bosentan-related liver injury may occur within 20 days to 57 months of medication and often within 3 months, with similar clinical manifestations to those induced by other drugs. Some patients may have no clinical symptoms but only abnormal liver function while severe liver injury may occur in a small number of patients. Liver function can usually return to normal or be improved after discontinuation of bosentan.

10.Two-year prospective study of natural course and risk factors of depressive symptoms in college students

Objective:To investigate the natural course and psychosocial risk factors of depressive symptoms in college freshmen.Methods:Changes in depressive symptoms among college freshmen were observed for two year periods,self-reporting questionnaires were used to collect data every half yearly.The depressive symptoms were assessed with the Centre for Epidemiological Study-Depression Scale,and psychosocial factors were tested as the potential risk factors,rating by Zung Self-Rating Anxiety Scale,Eysenck Personality Questionnaire-Neuroticism Subscale,Rosenberg Self-Esteem Scale,Frost Multidimensional Perfectionism Scale,Parental Rearing Scale,Adolescents Self-Rating Life Events Checklist,Epworth Sleepiness Scale,daytime sleep time,Godin Leisure-Time Exercise Questionnaire.Generalized estimating equations model was used to analyze the date.Results:Totally 1339 college students were recruited in the survey.It was found the depressive scores significantly increased in the second and third surveys,then decreased in the fourth and fifth surveys.Male (Coef=-1.01,SE =0.42,P =0.017),not interested in the major (Coef=3.89,SE =1.42,P =0.006),neuroticism(Coef =0.79,SE =0.23,P =0.001),self-esteem(Coef =-1.57,SE =0.25,P < 0.001),anxiety(Coef =4.79,SE =0.16,P < 0.001),life events (Coef =0.08,SE =0.01,P < 0.00l),daytime sleepiness (Coef =0.80,SE =4.56,P < 0.001) were significantly correlated with depressive symptoms.Conclusion:The first year of college is a critical time in the prevention depression.Not interested in the major,high neuroticism,low self-esteem,anxiety,life events,daytime sleepiness may be risk factors of depressive symptom in college students.
