Vertebral refracture following percutaneous vertebral augmentation (PVA) is commonly seen in elderly patients with osteoporotic thoracolumbar compression fractures (OTLCF). It can lead to recurrent pain, loss of vertebral height, progression of kyphosis, and even neurological dysfunction, significantly impairing patients′ quality of life. Current diagnosis and treatment face multiple challenges, including high misdiagnosis rate, difficulty in choosing between surgical and non-surgical treatment options, lack of standardized surgical protocols, interference from intralesional bone cement during procedures, inadequate stability of internal fixation in osteoporotic bone, and suboptimal compliance of anti-osteoporotic therapy. Establishing a standardized diagnostic and therapeutic framework is urgently needed. To standardize the management process and improve outcomes for vertebral refractures after PVA in elderly OTLCF patients, Spinal Trauma Group of the Orthopedic Branch of Chinese Medical Doctor Association organized experts in the field to develop Guideline for the diagnosis and treatment of vertebral refracture after percutaneous vertebral augmentation in elderly patients with osteoporotic thoracolumbar compression fractures ( version 2025), based on current literature and clinical experience, and adhering to principles of scientific rigor and clinical applicability. A total of 11 recommendations were proposed, encompassing diagnosis, treatment, and rehabilitation of vertebral refracture after PVA in elderly patients with OTLCF, aiming to provide a foundation for a standardized management.
2.Clinical efficacy of curcumin-mediated photodynamic therapy for treatment of cervical intraepithelial neoplasia complicated with human papillomavirus infection

Objective:To explore the clinical efficacy of curcumin-mediated photodynamic therapy (PDT) for the treatment of cervical intraepithelial neoplasia (CIN) combined with human papillomavirus (HPV) infection.Methods:A prospective randomized controlled study was conducted. Eighty patients with CIN combined with HPV infection in Dingzhou People's Hospital from January 2022 to January 2023 were selected and divided into the observation group and the control group using the randomized numerical table method, with 40 cases in each group. The observation group was treated with curcumin-mediated PDT, and the control group was treated with recombinant human interferon α-2b suppository for topical application. The efficacy, HPV-DNA conversion rate, high-risk human papillomavirus (HR-HPV) load, recovery status, inflammatory cytokine level, expression of Ki-67, expression of Survivin, and occurrence of adverse reactions were compared between the two groups.Results:Eighty patients aged (35±4) years old. The age, disease duration, age of first sexual experience, CIN grading, HPV typing and other general data of the two groups were compared, and the differences were not statistically significant (all P > 0.05). The total effective rate of the observation group was higher than that of the control group [95.0% (38/40) vs. 75.0% (30/40)], and the difference was statistically significant ( χ2 = 6.28, P = 0.012). The HPV-DNA conversion rates in the observation group were higher than those in the control group at 1, 3 and 6 months after treatment [1 month: 65.0% (26/40) vs. 42.5% (17/40); 3 months: 87.5% (35/40) vs. 67.5% (27/40); 6 months: 95.0% (38/40) vs. 80.0% (32/40)], and the differences were statistically significant ( χ2 values were 4.07, 4.59 and 4.11, respectively, all P < 0.05). HR-HPV loads in the observation group were lower than those in the control group at 1, 3 and 6 months after treatment (1 month: 42.4±5.8 vs. 56.7±6.7; 3 months: 31.0±4.5 vs. 40.3±3.7; 6 months: 14.5±2.4 vs. 28.8±2.4), and the differences were statistically significant ( t values were 10.24, 10.17 and 26.77, respectively, all P < 0.001). The healing time of cervical wound in the observation group was shorter than that in the control group [(30±3) d vs. (39±5) d, t = 9.37, P < 0.001]. The levels of interleukin 4, interleukin 6, interleukin 10 and relative expressions of Ki-67 and Survivin in the observation group were lower than those in the control group at 6 months after treatment (all P < 0.001). The difference in the incidence of adverse reactions between the observation and control groups was not statistically significant [12.50% (5/40) vs. 7.50% (3/40), χ2 = 0.56, P = 0.456]. Conclusions:Curcumin-mediated PDT for the treatment of patients with CIN combined with HPV infection has a high total effective rate; it can reduce the HR-HPV load and inflammatory cytokine levels, and improve the HPV-DNA conversion rate.
3.Comparative study of high-risk human papillomavirus E6/E7 mRNA detection and pathological examination of cervical lesions

Objective:To analyze the clinical significance of high risk human papillomavirus (HR-HPV) positive female cervical lesion screening HR-HPVE6/E7 mRNA detection.Methods:A total of 80 HR-HPV-positive female patients with cervical lesions were retrospectively selected in Dingzhou People′s Hospital from August 2021 to August 2023. According to the pathological examination results, the patients were divided into inflammatory response group (16 cases), cervical intraepithelial neoplasia (CIN) Ⅰ group (15 cases), CINⅡ group (22 cases), CINⅢ group (17 cases) and cervical cancer group (10 cases). The viral load and positive rate of HR-HPVE6/E7 mRNA in the five groups were compared. Receiver operating characteristic (ROC) curve was drawn to analyze the diagnostic efficacy of HR-HPVE6/E7 mRNA in cervical lesions.Results:The viral load of HR-HPVE6/E7 mRNA in the cervical cancer group, CINⅢ group, CINⅡ group, CINⅠ group and inflammatory response group was gradually decreased: 3 152.62(894.26, 21 986.24), 812.62(465.05, 3 945.06), 481.64(215.62, 2 451.49), 38.62(0, 612.47), 0(0, 316.28) × 10 3 copy/L, there was statistical difference among the five groups ( P<0.05). The positive rate of HR-HPVE6/E7 mRNA in cervical cancer group (10/10) was higher than that in CINⅢ group (10/17), CINⅡ group [27.27%(6/22)], CINⅠ group (2/15) and inflammatory response group (0), and there were statistical differences ( P<0.05). ROC curve analysis results showed that the area under the curve of HR-HPVE6/E7 mRNA for diagnosis of cervical lesions was 0.901(95% CI 0.837 - 0.942), sensitivity was 92.53%, specificity was 89.12%. Conclusions:Quantitative detection of HR-HPVE6/E7 mRNA viral load has high sensitivity and specificity in the diagnosis of cervical lesions.

7.Network Pharmacology Study of Compound Ligustrazine in Gastric Cancer Therapy

Objective To explore the potential role and mechanism of compound tetramethylpyrazine in gastric cancer therapy by using network pharmacology analysis combined with gene function annotation and clinical data analysis.Methods SwissTargetPrediction database was used to screen the potential drug action sites of compound tetramethylpyrazine,and the OMIM and Genecard databases were used in combination to obtain gastric cancer-related targets.Intersection analysis was performed to identify potential therapeutic targets.Subsequently,the method of ClusterProfiler was used to perform functional annotation of the downstream targets of intersection.In addition,The Cancer Genome Atlas(TCGA)database was used to obtain the original data of gastric cancer patients,and the immune infiltration analysis,miRNA analysis,transcriptional regulation analysis of key genes,gene set enrichment analysis(GSEA),gene set variation analysis(GSVA),nomogram model construction,and genome-wide association studies(GWAS)were performed.Results Through network pharmacological screening,we found 14 potential therapeutic targets through which tetramethylpyrazine acted on gastric cancer.Functional annotation showed that these targets were mainly involved in the pathways for hormone metabolism,drug metabolism,and signal transduction.Based on log rank test,the expression of the key genes,ELANE and MPO,showed significant difference in the comparison of gastric cancer survival curves(P<0.05),and were closely associated with immune cell infiltration.In addition,GSEA and GSVA results suggested that ELANE and MPO might influence the development of gastric cancer through multiple signaling pathways.Conclusion In this study,by using multiple analysis methods in an integrated way,we found that ligustrazine may have therapeutic effects on gastric cancer by regulating the potential targets of ELANE and MPO,as well as the relevant signaling pathways.
8.The surgical management of benign tumors of the lateral skull base with intracranial invasion: experience from a single centre over ten years

Objective:To investigate the clinical features, pathological types, imaging features, and surgical strategies of lateral skull base benign tumors with intracranial invasion.Methods:From January 2011 to March 2021, 36 patients of lateral skull base benign tumors with intracranial invasion were included in this retrospective study. Among the 36 patients, 14 cases were male, 22 cases were female, the aged range from 20-67, with the median age of 48. The clinical manifestations, characteristic imaging findings, pathological types, surgical approach selection, and prognosis were analyzed.Results:36 cases of lateral skull base tumors with intracranial invasion were all accepted surgeries. 23 cases were neurogenic tumors, facial nerve tumors ( n=8), neurogenic tumors in jugular foramen with unknown origin( n=6), hypoglossal schwannoma ( n=3), transotic intralabyrinthine schwannoma ( n=3), vestibular schwannoma involving the middle ear( n=2), vagal nerve schwannoma( n=1). Other types of tumors included meningioma ( n=10) and paraganglioma (Di 1 or 2, n=3). Different pathological types of tumors had different clinical manifestations and imaging manifestations. Sixteen cases were subjected to primary resection, while, other 20 cases underwent staged operation. Among the patients with staged operation, 10 patients had completed the second stage operation, five patients were waiting for the second stage operation, the other five patient′s residual intracranial tumor were significantly reduced and the space between tumor and brain tissues widened after the first stage operation, so, the following up with "wait and scan”policy was suggested. The total resection rate of tumors was related to the pathological nature, in which neurogenic tumors were 15/17, and meningiomas were 5/8. The main postoperative complications were cerebrospinal fluid leakage and infection in the operation area. There were two cases of postoperative intracranial infection, and three cases of cerebrospinal fluid leakage occurred in non staged operation cases. Conclusions:Lateral skull base tumors with intracranial invasion are rare. The most common pathological type is schwannoma, followed by meningioma and paraganglioma. For this type of tumor, if there is infection in the operation area and neck invasion is large, it is suggested to choose staged surgery, which can reduce the risk of intracranial infection and the incidence of cerebrospinal fluid leakage. Staged surgery strategy can also reduce the difficulty of second stage surgery, so the operation is much safer than non staged surgery.
9.Analysis of the changes of Th17,Th22,Treg cells and related cytokines in peripheral blood of patients with cervical cancer

Objective To investigate the changes of helper T cell(Th)17,Th22,regalatory T cell regula (Treg)and related cytokines in peripheral blood of patients with cervical cancer.Methods 95 patients with cervical cancer were enrolled in the study from the hospital from January 2015 to December 2016 as the obser-vation group,meanwhile 61 patients with cervical intraepithelial neoplasia(CIN)treated in the hospital were enrolled in the study as the CIN group,and 56 healthy people who underwent healthy assessment were en-rolled as the healthy group.Peripheral blood was extracted from those people,from which peripheral blood mononuclear cells were isolated,then the percentages of Th17,Th22 and Treg cells were measured,and the se-rum levels of IL-17,IL-21 and IL-22 were measured by using enzyme-linked immunosorbent assay.Results The percentages of Th17,Th22 and Treg cells in the observation group were higher than those in the CIN group and the healthy group;the percentages of Th17,Th22 and Treg cells in the CIN group were higher than those in the healthy group,and the differences were statistically significant(P<0.05);the serum levels of IL-17,IL-21 and IL-22 in the observation group were higher than those in the CIN group and the healthy group, the serum levels of IL-17,IL-21 and IL-22 in the the CIN group were higher than those in the healthy group, and the differences were statistically significant(P<0.05);the precentages of Th17,Th22 and Treg cells in the Ⅲ - Ⅳ stage group were higher than those in Ⅰ - Ⅱ stage group,and the differences were statistically significant(P<0.05);serum levels of IL-17,IL-21 and IL-22 in the Ⅲ ~ Ⅳ stage group were higher than those in Ⅰ - Ⅱ stage group,and the differences were statistically significant(P< 0.05).Conclusion Peripheral blood Th17,Th22,Treg cells and related cytokines may play an important role in the pathogenesis of cervical cancer.
