1.Biomechanical Analysis and Optimization of Minimally Invasive Pelvic Internal Fixation

Objective By comparing biomechanical properties of two-screw rod and three-screw rod for fixing pelvic fracture, the difference in mechanical effects of different screw rod fixation positions in pelvic minimally invasive surgery was studied.Methods The mechanical models of pelvis fixed by two-screw rod and three-screw rod were established, and biomechanical characteristics of the pelvis during standing on both legs, during single-legged standing on healthy side or affected side, as well as in sitting posture were compared and analyzed by finite element simulation, and the fixation effect of three-screw rod was verified by clinical experiments.Results Both fixation methods could restore mechanical transmission of the pelvis. But for three-screw rod fixation, the stress on both sides of the pelvis was more balanced, and the displacement of the whole body and fracture surface was also lower during single-legged standing.Conclusions The three-screw rod fixation has an excellent effect in stability, which is more beneficial for fracture recovery.
2.Research status and hot spot analysis of user information behavior in online health communities in China

This paper analyzed the current situation and research hotspots of user information behavior in online health communities in China, a study of practical and theoretical significance for relevant scholars to learn the forefront of user behavior research in online health communities and better engage in related research. Thanks to literature review, this paper analyzed the research status of user information behavior in online health communities in China, and summarized the main research hotspots. Generally speaking, the research results of user behavior in online health communities in China were rich in volume, yet there remained a gap between China and foreign researches in the depth of research and the breadth of research objects.Future research should focus on making up for these deficiencies.
3.Exploration and application of a novel attempt to recruit participants in clinical trials of vaccines under the emergency

Objective:To evaluate and share the novel method for recruiting participants in clinical trials of vaccines in emergency situations.Methods:To publish recruitment notice in local areas of Wuhan through websites and medium, and guide interested persons to log in to the“Clinical Trials of SARS-CoV-2 Vaccine Reservation and Health Declaration System”to appoint and register their health information. The "Health Declaration System" provides each volunteer evaluation and risk levels to preliminarily exclude those who do not meet the inclusion criteria. Researchers review the qualified volunteers by telephone, organize them to go to the vaccination site, and finally conduct a strict medical screening to determine the final subjects.Results:A total of 4 819 people and 5 132 people registered in the Phase Ⅰ and Phase Ⅱ recruitment system respectively, with men 2 912 (60.43%) and 2 887 (56.25%) more than women 1 907 (39.57%) and 2 245 (43.75%), mostly in the 20-39 age group, with 3 211 (66.63%) and 3 966 (77.28%). All 13 districts in Wuhan have interested residents to participate clinical research.The initial qualified rate of the Phase Ⅱ recruitment system was higher than that of Phase Ⅰ, with men 2 047 (70.28%) and 2 135(73.95%), higher than women 1 083 (56.80%) and 1 472 (65.57%); 440 and 689 people were reviewed by telephone in Phase Ⅰ and Phase Ⅱ respectively, and the number of verified volunteers was about 440 (35.00%) and 689 (67.20%); Of the 201 603 people who arrived at the vaccination site, 12 and 26 of them were positive for the SARS-CoV-2 antibody with an antibody positive rate of 6.00% and 4.31% respectively.Conclusion:The novel method for recruiting subjects in this clinical study is efficient and reliable, and the recruitment situation of Phase Ⅰ had set a good example for Phase Ⅱ but the medium-and long-term compliance of subjects and the separation of willingness and behaviors still need to be further studied.

5. Diagnostic value of multimodal magnetic resonance imaging in primary central nervous system lymphoma

Objective:
To investigate the diagnostic value of multimodal magnetic resonance imaging (MRI) in primary central nervous system lymphoma (PCNSL).
Methods:
The multimodal MRI findings of 16 patients with PCNSL confirmed by histopathology from January 2016 to December 2018 in Affiliated Hospital of North Sichuan Medical College were retrospectively analyzed. Routine MRI plain scan, enhancement, diffusion weighted imaging (DWI) and magnetic resonance spectroscopy (MRS) examination were performed in all cases.
Results:
All 16 patients were diagnosed with diffuse large B cell non-Hodgkin lymphoma. There were 7 cases with solitary lesions and 9 cases with multiple lesions. A total of 36 lesions were detected, with 30 above and 6 below the curtain. Sixteen lesions were located in the deep white matter near the ventricle (among which 2 lesions involved the corpus callosum), 5 lesions in the superficial cerebral hemisphere, 7 lesions in basal ganglia, 2 lesions in thalamus, 3 lesions in cerebellum, and 3 lesions were in brainstem. T1WI showed equal or slightly low signal, and T2WI showed equal or slightly high signal. There was different degree of edema around the lesions. 32 lesions were mild to moderate, and 4 lesions were severe. On enhancement scanning, 31 lesions showed patchy, nodular or lumpy uniform enhancement; 5 lesions showed irregular annular enhancement. "Incision" was found in 12 lesions, and "angular" was found in 7 lesions. Obvious enhancement of adjacent meninges or ependymal membranes occurred in 1 case each. DWI showed 29 high signals in the focal areas and 7 equal or slightly high signals, and all apparent dispersion coefficient (ADC) values were reduced. MRS showed that choline peak increased, and N-acetyl aspartate (NAA) peak and creatine peak decreased in all cases. highly elevated lipid peak was observed in 8 cases, inverted lactic acid peak in 3 cases, and both lipid peak and lactic acid peak were observed in 2 cases.
Conclusions
Multimodal MRI can reflect the characteristics of PCNSL from different aspects, which is of great value in improving the diagnostic accuracy of PCNSL.
6.Research on transfer of patient trust in online health websites

Objective To explore the determinants and mechanism of trust transfer among patients for online health websites using the equation model. Methods A trust transfer model was built based on the theory of group convergence and doctor-patient interaction. 225 doctor records of online consultations were collected from Haodf, an online health website by data crawling from May 1 to Nov.30, 2017. Multiple regression was used for an empirical analysis. Results The patients' online trust has a positive impact on the physicians'trust offline, with a regression coefficient of 0.94, being significant at the level 0.01. Patient incentives, group size, and frequency of group interaction are important for online patient trust. Meanwhile, patient motivation, group size, and patient online efforts all have significant effect on patient offline trust. Conclusions Online patients' trust affects their offline trust significantly. Developers of online health websites can increase the online trust of patients by motivation and interaction, which help create a desirable medical environment and encourage their trust to offline doctors, contributing to higher medical satisfaction.
7.Application of Multi-stage Countercurrent Extraction Technology in the Extraction Process of Aesculi

This study was aimed to optimize the extraction process of Aesculi. The extraction process of Aesculi wasoptimized through orthogonal experiment while the transfer rate of escin Ia and escin Ib. And the yield of dry extract. wasadopted as marks. And ratio of solid to liquid, the ethanol concentration, extraction time and extraction temperature werestudies. The results showed that optimized extraction conditions of Aesculi were determined as follows. The ratio of solidto liquid was 1: 10, the ethanol concentration was 70%, extracted for 20 minutes ever stage and the extractiontemperature was 60℃. It was concluded that the extraction process is efficacious for general flavone and icariinextraction. The method is suitable for the standardized production technology of Aesculi.
8.Clinical observation of growth hormone intrauterine perfusion combined with replacement cycle in the treatment of thin endometrium

Objective To investigate the effects of the frozen embryo transplantation for patients with poor outcome of endometrial growth by using growth hormone (GH) intrauterine perfusion combined with replacement cycle in the treatment of thin endometrium.Methods This was a prospective study and study participants were consecutively recruited between Jun 2014 and September 2015.A total of 88 frozen thawed embryo transfer cycles was divided into two groups from the Reproductive Center of Hunan Provincial Maternal and Child Health Hospital.Group A were 63 hormone replacement therapy (HRT) cycles and Group B were 25 GH intrauterine perfusion combined HRT cycles.Results The endometrial thickness of 22 thin endometrium patients from Group B were increased above 7 mm on progesterone day.The endometrial thickness on transplant day of Group A was (9.28 ± 1.64) mm,which was significantly higher than Group B (7.9 ± 0.86) mm (P < 0.05).The clinical pregnancy rate (50.79% vs 52.0%),implantation rate 31.1% vs 47.17%),miscarriage rate (9.38% vs 15.38%) had no significant difference between Groups A and B.The endometrial thickness from 7 mm to 7.9 mm on transplant day,the clinical pregnancy rate (30.76% vs 54.54%) had no significant difference in two groups (P >0.05),but the implantation rate of group A was significantly lower than that of group B (20% vs 52.17%) (P <0.05).When the endometrial thickness was above 8 mm on transplant day,the clinical pregnancy rate (58.33% vs 63.63%),implantation rate (36.36% vs 50%) had no significant difference between groups A and B (P > 0.05).Conclusions GH uterine cavity perfusion was a useful method for treatment of thin endometrium,and was helpful for improvement of endometrial thickness and receptivity,improved embryo implantation environment by assistance for HRT under the high estrogen levels.
9.Clinical trial data management and quality metrics system.

Data quality management system is essential to ensure accurate, complete, consistent, and reliable data collection in clinical research. This paper is devoted to various choices of data quality metrics. They are categorized by study status, e.g. study start up, conduct, and close-out. In each category, metrics for different purposes are listed according to ALCOA+ principles such us completeness, accuracy, timeliness, traceability, etc. Some general quality metrics frequently used are also introduced. This paper contains detail information as much as possible to each metric by providing definition, purpose, evaluation, referenced benchmark, and recommended targets in favor of real practice. It is important that sponsors and data management service providers establish a robust integrated clinical trial data quality management system to ensure sustainable high quality of clinical trial deliverables. It will also support enterprise level of data evaluation and bench marking the quality of data across projects, sponsors, data management service providers by using objective metrics from the real clinical trials. We hope this will be a significant input to accelerate the improvement of clinical trial data quality in the industry.
10.Nutritional risk screening and evaluation of clinical outcome and quality of life in inpatients of tuberculosis ;in Shenzhen

Objective To conduct nutritional risk screening and evaluate the relationship of nutritional risk to complication rate and quality of life in tuberculosis inpatients of Shenzhen. Methods A total of 1 374 patients from tuberculosis department and surgery department in the third people′s hospital of Shenzhen were consecutively enrolled. Data were collected on the nutritional risk screening, complication and quality of life. Results The occurence of nutritional risk at admission of inpatients with tuberculosis among total , younger , elderly patients was 77.7%, 70.9%, 88.9%, respectively. There was a significant increase in the occurrence from admission to 2 weeks in all patients (P < 0.05). On admission and 2 weeks after admission or discharge, the occurence in those patients ≥ 65 years was significantly higher than that in the younger ones (P < 0.05) On the scales of quality of life (SF-36), the scores of physical functioning, role-physical, bodily pain, fatigue and general health were significantly lower than in the patients at risk. Conclusion A large proportion of inpatients of tuberculosis is at nutritional risk and tended to be worsen during the course of admission , which has associated with increased complication rate and lower scores of quality of life.
