1.Clinical evidence Map Analysis of Chinese Medicine Promoting Wound Healing After Anal Fistula Operation in Recent Ten Years

Objective To sort out the clinical research evidence on the promotion of wound healing after anal fistula surgery by traditional Chinese medicine in the past decade,and understand the evidence distribution in this field,the evidence map system was used.Methods Computer retrieval of CNKI,VIP,Wanfang,PubMed,Web of Science,EMbase,The Cochrane Library and other databases were searched.Interventional studies,observational studies,systematic review/meta-analysis on the treatment of postoperative anal fistula wounds by traditional Chinese medicine in the past decade were included.The evidence was analyzed and presented in the form of text and graphics.And the quality of clinical RCT was evaluated using Cochrane systematic review manual.AMSTAR was used to evaluate the quality of meta-analysis,and the JBI quality evaluation tool was used to evaluate the methodological quality of retrospective studies.Results A total of 544 articles were included.In recent 10 years,the number of published documents showed an increasing trend.The intervention measures of TCM to promote wound healing after anal fistula operation included single intervention mode and combined intervention mode.The outcome indexes mainly included clinical efficacy,basic wound condition,TCM syndrome score,adverse reactions,quality of life,long-term efficacy and so on.The overall quality of clinical RCT,retrospective analysis and systematic review/meta-analysis is low.Most studies showed that Chinese medicine had advantages in the treatment of vertigo,and more high-quality studies were still lacking.Conclusion More large-sample,multi-center RCTS should be carried out in the future,and the quality of relevant systematic review/meta-analysis should be improved.The advantages of TCM in promoting wound-heal after anal fistula surgery can be fully explored,which provide strong support for the effectiveness and safety of traditional Chinese medicine in improving wound management.
2.Application of 3D-HRAM model in diagnosis and localization of anal sphincter injury after surgery for complex anal fistula

Objective:To explore the application value of three-dimensional high-resolution anorectal manometry(3D-HRAM)model in the diagnosis and localization of anal sphincter injury after surgery for complex anal fistula.Methods:A prospective study was con-ducted on 92 patients who had undergone surgery for complex anal fistula in the Department of Coloproctology of The People's Hospi-tal Affiliated to Fujian University of Traditional Chinese Medicine from January 2023 to January 2024.The study was conducted in two stages.In the first stage,92 patients underwent 3D-HRAM and endoanal ultrasonography(EAUS)to examine the sphincter,and the data were compared with intraoperative records to evaluate the diagnostic efficiency and localization accuracy of 3D-HRAM and EAUS.In the second stage,92 patients who had been examined were grouped according to the presence or absence of symptoms of anal dysfunction to further verify the diagnostic and localization values of 3D-HRAM and EAUS for postoperative compensated/decompen-sated sphincter injury.Results:Among 92 patients,41 and 57 patients were diagnosed with sphincter injury through 3D-HRAM and EAUS,with a sensitivity of 61.19%and 85.07%,a specificity of 88%and 76%,an area under the curve of 0.739 and 0.813 calcu-lated by plotting the receiver operating characteristic curve,and a localization accuracy of 44.77%and 71.64%,respectively,showing significant differences between 3D-HRAM and EAUS(P=0.002),indicating that 3D-HRAM had lower diagnostic efficiency and local-ization accuracy than EAUS for anal sphincter injury after surgery for complex anal fistula.After grouping by symptoms,the diagnostic efficiency and localization accuracy of 3D-HRAM for symptomatic sphincter injury(decompensated)were significantly better than those for asymptomatic sphincter injury,and were similar to those of EAUS(P=0.763).Conclusion:The application advantage of 3D-HRAM lies in the diagnosis and localization of decompensated anal sphincter injury after surgery for complex anal fistula,which provides effective technical support for the study of anal defecation control mechanisms and injury compensation mechanisms.

4.Bibliometric analysis of radiomics research

Objective:To analyze the development status,frontiers and hotspots of radiomics research in the past five years from 2019 to 2023,and to provide theoretical reference and guidance for radiomics research in China.Methods:The relevant literature in the field of radiomics published in the core database of Web of Science(WOS)from January 1,2003 to August 10,2023 were searched.According to the inclusion and exclusion criteria,6,777 eligible literatures were screened and obtained,including 6,254 articles in the past five years from January 1,2019 to August 10,2023.Bibliometric methods were used to analyze the clustering of countries and regions,institutions,journals,authors,keywords and draw visual maps.Results:The 6,777 radiomics-related articles published between 2003 and 2023 were first published in 2011,and the number of papers tended to stabilize in 2018,and then the number showed a significant trend of increasing year by year.Among the 6,254 articles published from 2019 to 2023,China(3,564 articles),United States(1,164 articles),and Italy(530 articles)ranked the top 3 in terms of publication volume,with close cooperation between countries.General Electric of the United States published the most papers(448 articles),and the journal Frontiers in Oncology(704 papers)ranked first in terms of paper publication volume.From 2019 to 2023,the diseases of concern in the field of radiomics are rectal cancer,hepatocellular carcinoma,breast cancer,and lung cancer(especially non-small cell lung cancer).Conclusion:Although China ranks first in the number of national publications,the quality of research still needs to be improved.In the future,the research trend in the field of radiomics may be the diagnosis and differential diagnosis of various diseases,the prediction and evaluation of curative effect,the evaluation of tumor disease metastasis and the identification of gene phenotype based on radiomics combined with multiple imaging techniques.
