1.Early results of modified double-orifice valve plasty via total thoracoscopy for partial atrioventricular septal defect

Objective:To evaluate the safety, efficacy, advantages and disadvantages of the modified double-orifice valve plasty via total thoracoscopy for partial atrioventricular septal defect(PAVSD) through a retrospective analysis of early postoperative results.Methods:Patients diagnosed with PAVSD who underwent the standardized modified double-orifice valve plasty via total thoracoscopy between September 2023 and August 2024 were retrospectively enrolled. Baseline characteristics, surgical parameters, and follow-up outcomes were systematically analyzed.Results:A total of 14 patients(6 males, 8 females) were included, with a mean age of(32.9±15.5) years old and body weight of(55.1±11.6) kg. All procedures were successfully completed. The mean operative time, cardiopulmonary bypass time, and aortic cross-clamp time were(266.6±67.2) min, (160.7±34.2) min, and(97.0±31.1) min, respectively. Postoperative ICU stay, hospital stay, and total hospitalization duration were(1.7±1.1) days, (6.4±4.2) days, and(12.6±4.5) days, respectively. The mean follow-up duration was(7.9±3.6) months. Preoperatively, left atrioventricular valve regurgitation(LAVVR) was graded as mild, moderate, or severe in 5, 4, and 5 patients. Postoperatively, 13 patients exhibited mild or less LAVVR, with 1 case of moderate regurgitation. By 3 months, all patients demonstrated LAVVR of mild or lower severity, which remained stable through follow-up. Peak LAVV gradients were(4.6±2.7) mmHg(1 mmHg=0.133 kPa)(range: 1.8-10.2 mmHg) postoperatively, improving to(3.6±0.6) mmHg(3.2-4.0) mmHg at 1 year. Right atrioventricular valve regurgitation improved from preoperative moderate-severe(50.0%) to LAVVR of mild or lower severity in all patients by 3 months. No mortality, residual shunts, or high-grade atrioventricular block occurred through follow-up. By 1 month, NYHA functional class improved to Ⅰ in all patients, which remained stable through follow-up.Conclusion:Standardized modified double-orifice valve plasty via total thoracoscopy for PAVSD demonstrates safety, minimal invasiveness, and rapid recovery, with favorable early outcomes.

Acute respiratory distress syndrome (ARDS) is the leading cause of respiratory failure with high morbidity and mortality. Pulmonary surfactant (PS)-based complementary therapies have exhibited potential for ARDS healing and applied as an adjunctive therapy strategy. Coacervate (Coac) has the characteristics of softness, deformability and excellent molecular enrichment properties, and has attracted extensive attention in the biomedical field. Here PS and coacervate were combined for the potential ARDS treatment. The Coac, fabricated from polyallylamine hydrochloride (PAH) and adenosine triphosphate (ATP) by simple mixing, exhibited soft droplet property and high enrichment for dexamethasone sodium phosphate (DSP). To avoid the fusion effect of membraneless coacervate and endow it with biological functions of PS, liposomes with PS-biomimetic lipid components (PS-lipo) were further introduced to construct PS-biomimetic membranized coacervate (DSP@PS-Coac). The DSP@PS-Coac demonstrated high lung targeting effect and significant penetration efficiency after intravenous injection. Furthermore, PS-lipo replenished the endogenous PS pool and facilitated the distribution of DSP in inflammatory cells in the lung. In the ARDS mouse model, PS-Coac and DSP exerted synergetic anti-inflammatory functions, via reducing the recruitment of inflammatory neutrophils and modulating macrophages into anti-inflammatory phenotype. The overall results confirmed that DSP@PS-Coac may provide a promising delivery option for the treatment of ARDS.

4.Establishment and exploration of a subcutaneous transplanted tumor model of hepatocellular carcinoma in mice with Qi stagnation and blood stasis syndrome

Objective To explore the establishment of a subcutaneously transplanted tumor model of hepatocellular carcinoma in mice with Qi stagnation and blood stasis syndrome.Methods Forty male C57BL/6 mice were randomly divided into 4 groups:NC group,QZXY group,Tumor group,and QZXY+Tumor group.They were categorized based on the modeling of Qi stagnation and blood stasis syndrome(7 days)combined with the modeling of subcutaneous transplantation of hepatocellular carcinoma tumor(20 days).Observations were conducted of the syndrome manifestations as well as the tumor size and weight of the mice after modeling.Results(1)Body weight:on the 7th day of modeling,the weights of the QZXY group and QZXY+Tumor group were significantly lower than that of the NC group(P<0.05).(2)Body temperature:on the 7th day of modeling,body temperature significantly decreased in the QZXY group(P<0.05),while it increased in the Tumor group(P<0.05)compared with the NC group.On the 27th day of modeling,the temperature of the QZXY+Tumor group was significantly lower than that of the NC group(P<0.05).(3)Syndrome manifestations:according to the syndrome scoring table,mice in both the QZXY group and QZXY+Tumor group exhibited Qi stagnation and blood stasis syndrome on the 7th day of modeling(P<0.05).As modeling time extended,the score of mice in the Tumor group increased with the formation of the tumor,and the score of mice in the QZXY+Tumor group was significantly higher than that of the other three groups(P<0.05).(4)Claw petechiae:the number of claw petechiae significantly increased in all three groups of modeled mice compared with the NC group(P<0.05),with the QZXY+Tumor group showing the highest number.(5)Claw r value:the r value of the claw was significantly lower in all three groups of modeled mice than that in the NC group(P<0.05).Additionally,the r value of the claw in the QZXY+Tumor group was consistently lower than that of the other three groups.(6)Open field activity:the vertical and horizontal activity of mice in the QZXY+Tumor group decreased significantly compared with that of the NC group(P<0.05).(7)Coagulation indexes:APTT,TT,and FIB were significantly increased in the QZXY+Tumor group(P<0.05 or P<0.01)compared with those in the NC group.(8)Tumor size and weight:compared with the Tumor group,the QZXY+Tumor group showed significantly increased tumor size and weight(P<0.05).Conclusions This study successfully established a subcutaneous transplanted tumor model of hepatocellular carcinoma in mice with Qi stagnation and blood stasis syndrome.The findings indicated that Qi stagnation and blood statsis syndrome may occur during the course of live cancer.Besides,the causes inducing the Qi stagnation and blood stasis syndrome will further accelerate the progression of liver cancer.

6.Study on the minimum inhibitory concentration of β-lactam and β-lactam resistance molecular mechanism of Haemophilus influenzae isolates from neonates

Objective:To study the changes of β-lactam resistance of Haemophilus influenzae (Hi) strain isolated from neonatal lower respiratory tract and the molecular mechanism of β-lactam resistance.Methods:Nineteen Hi strains isolated from neonatal lower respiratory tract infection in the previous multicenter prospective epidemiological study were re-identified, and the P6, fucK and Cap genes were detected by PCR.The minimum inhibitory concentration(MIC) of ampicillin, amoxicillin clavulanic acid and cefuroxime were detected by microdilution method, and tem-1 gene, rob-1 gene and ftsI gene were sequenced and analyzed.Results:(1) Nineteen strains of Hi were confirmed to be capsule-free type by P6 gene, fucK gene and cap gene, which was non-typeable Haemophilus influenzae(NTHi). (2)Compared with 2003-2004, the MIC values of ampicillin, amoxicillin clavulanic acid and cefuroxime of NTHi isolated from the lower respiratory tract of the newborn from 2013-2014 were significantly higher( P<0.05). (3)The rates of β-lactamase producing strains during 2003-2004 and 2013-2014 were 33.33% (3/9) and 30.00% (3/10), respectively.There was no significant difference between them during 10 years ( P>0.05). The detection of the β-lactamase gene showed that the β-lactamase of the all six strains were of the tem-1 type, and the rob-1 type was not detected.(4)Only one gBLNAR strain ( n=9) was found during 2003~2004, and gBLNAR 1, gBLNAI 3, gBLPAR 3, gBLPACR 1 ( n=10)appeared during 2013~2014.(5)There were 11 amino acid substitution patterns in ftsI gene during 2013~2014, but only five amino acid substitution patterns in 2003~2004.The mutation rate of the S357N, S385T, N526K and T532S of ftsI gene significantly increased during the past ten years ( P<0.05). One strain of gBLNAR/gBLNACR resistant to ampicillin, amoxicillin clavulanic acid and cefuroxime isolated in 2014 showed D350N, S357N, M377I, S385T, L389F, A502T and N526K variation at the same time. Conclusion:Neonatal patients with lower respiratory tract NTHi infection may rapidly face the severe challenge of multiple drug resistance of β-lactam antibiotics.
7.Study on construction and function of CNE1 cells stably over-expressing eIF1 gene

Objective To establish nasopharyngeal carcinoma cell line (CNE1) with eIF1 gene stable over-expression and to study its effects on the proliferation and migration of nasopharyngeal carcinoma cells.Methods EIF1 over-expression vector was constructed by adopting the pEGFPC1 eukaryotic expression system for transfecting nasopharyngeal carcinoma CNE1 cells.Thus the stably transfected EIF1-elF1 and its control cells were obtained.The over-expression situation of eIF1 in these cells was verified by real time fluorescence quantitative PCR(qPCR) and Western blot.The proliferation and migration activity of CNE1-eIF1 cells were tested by adopting the cell proliferation and migration tests.Results The enzyme digestion electrophoresis identification and sequencing showed that the pEGFPC1-eIF1 eukaryotic expression vector was successfully constructed.After mRNA and protein expression identification,compared with the reloading plasmid transfection group,the eIF1 gene mRNA and protein expression levels in nasopharyngeal carcinoma cell line CNE1 stably over-expressing eIF1 were up-regulated by 2.85 folds and 2.58 folds respectively (P< 0.05),while its proliferation and migration activities were down-regulated by 55 % and 36 % respective (P< 0.05).Conclusion The nasopharyngeal carcinoma cell line over-expressing elF1 is successfully constructed,the eIF 1 over-expression could significantly down-regulate the proliferation and migration activities of nasopharyngeal carcinoma cells,suggesting that eIF1 has potential anti-tumor effect.
8.Clinical effect of Hennipen method to treat elderly patients with anterior dislocation of the shoulder joint

Objective To analyze the clinical curative effect of Hennipen method to treat elderly patients with anterior dislocation of shoulder joint.Methods Between February 2013 and June 2015,57 elderly patients with acute anterior dislocation of shoulder joint were colleted in our hospital.The patients aged from 65 to 81,with average age of 72.3 years.Hennipen method was used to replace the dislocation of shoulder joint.Visual analogue scale (VAS)scores and joint range of motion before and after treatment were recorded to evaluate its clinical effect.Results All cases were checked with X -ray to comfirm had been reducted,and had no complications,such as proximal humeral fractures,axillary nerve injured.1 case failed maneuver reduction,so was tracted and reseted again after anesthesia.Before treatment,the VAS scores was (5.4 ±0.9)points,which after treatment was (1.9 ±0.7)points, and the difference was significant(t =12.43,P <0.01).The range of motion of the shoulder joint was significantly improved.Conclusion Hennipen method is simple,to threat anterior dislocation of shoulder joint of elderly patients, it can effectively relieve pain,reduce complications,which is worthy of clinical promotion.
9.Preliminary clinical oberservation on treatment of frozhen shoulder with local analgesic influsion equipment after arthroscopic arthrolysis

Objective To observe the clinical efficacies of local analgesic influsion equipment after arthroscopic arthrolysis in treating frozen shoulder. Methods 84 patients suffered from frozen shoulder were randomly divided into two groups from February 2012 to July 2014 , Group one was treated with local analgesic influsion equipment, another group was not treated with it. both groups received arthroscopic arthrolysis The mean duration of follow-up was 11.2 months after operation.The range of shoulder motion , VAS and ASES scores were recorded before the operation and one week, one month, three months after the operation. Results All patients were involved , the range of shoulder motion , VAS and ASES scores improved significantly after operation(P < 0.05). The differences of the range shoulder motion, ASES score after operation,the patients with local analgesic equipment improved significantly (P < 0.05),but there were no significant difference of VAS scores between two groups at 3months after operation (P > 0.05). Conclusion Local analgesic equipment could alleviate the pain of patients with frozen shoulder earlier and more effectively , promote recovery quickly , achieve expected effects.
10.Orthocord suture for treatment of avulsion fracture of posterior cruciate ligament from the tibia under arthroscope

Objective To evaluate the clinical results of a procedure for treatment of avulsion fracture of posterior cruciate ligament (PCL) using arthroscopic reduction and Orthocord suture.Methods The clinical data of 13 patients who underwent arthroscopic reduction and Orthocord suture fixation for PCL avulsion fracture of the tibial eminence from October 2009 to June 2010 was analyzed retrospectively.Follow-up assessment including Lysholm knee score,and radiographic evaluation.Results All 13 patients achieved follow-up with 10-14 ( 12 ± 2) months.No significant complications were noted,such as swelling,pain or disability of knee.Drawing test was negative.The preoperative Lysholm score in the 13 knees was (72.3 ±2.8) scores,the postoperative Lysholm score was (93.2 ± 1.8) scores,difference was statistically significant (t =15.8,P< 0.05 ).Conclusions Treating PCL avulsion fracture with arthroscopic fixation by use of Orthocord suture can restore ligament length,stabilize fragments,promote early motion.
