Journal of Clinical Otorhinolaryngology Head and Neck Surgery 2025;39(6):582-589
Head and neck squamous cell carcinoma (HNSCC) is one of the ten most common cancers worldwide and is one of the refractory cancers with a poor prognosis in otorhinolaryngology-head and neck surgery. Cetuximab is widely used in the clinical treatment of HNSCC and has been approved by the FDA as a first-line chemotherapeutic agent. However, its efficacy varies significantly among different individuals. Therefore, exploring the resistance mechanisms of cetuximab in the treatment of HNSCC and screening for sensitive populations are essential for the precision treatment of head and neck cancer. This article summarizes the research progress on cetuximab resistance mechanisms in HNSCC, and the main aspects include: alterations in epidermal growth factor receptor (EGFR) and its ligands, changes in downstream effectors of EGFR, bypass activation and crosstalk, epithelial-mesenchymal transition, epigenetic modifications, and immunosuppression in the tumor microenvironment.

Journal of Guangzhou University of Traditional Chinese Medicine 2025;42(8):1977-1983
This paper summarizes Li Fei's academic insights and clinical experience in treating intractable facial paralysis.Li Fei posits that prolonged illness inevitably leads to deficiency and stasis, and thus, the treatment of intractable facial paralysis should first focus on identifying the etiology and syndrome differentiation, resolving stasis and unblocking stagnation, and regulating qi and blood, with particular emphasis on the relaxation of the meridian sinew system. The meridian sinew system, affiliated with the meridian and collateral, serves as the framework through which qi and blood nourish muscles, tendons, and joints, playing a crucial role in facial paralysis treatment.Guided by the theory of the meridian sinew system,Li Fei employs syndrome-based treatment, integrating the anatomy of facial expression muscles. His approach includes needle-knife therapy to release adhesions and break stasis, acupuncture to harmonize qi and blood, and intradermal needle therapy for sustained stimulation.Through these methods, the meridian sinew system is relaxed,qi and blood are regulated, and facial muscles are nourished, leading to gradual recovery from facial paralysis.

Chinese Critical Care Medicine 2025;37(10):950-955
OBJECTIVE:
To develop a digital intelligent multimodal shift handover system for the intensive care unit (ICU) and evaluate its application effect in ICU shift handovers.
METHODS:
A research and development team was established, consisting of 1 department director, 1 head nurse, 3 information technology engineers, 3 nurses, and 2 doctors. Team members were assigned responsibilities including overall coordination and planning, platform design and maintenance, pre-application training, collection and organization of clinical feedback, and research investigation respectively. A digital intelligent multimodal shift handover system was developed for ICU based on the Shannon-Weaver linear transmission model. This innovative system integrated automated data collection, intelligent dynamic monitoring, multidimensional condition analysis and visual reporting functions. A cloud platform was used to gather data from multi-parameter vital signs monitors, infusion pumps, ventilators and other devices. Artificial intelligence algorithms were employed to standardize and analyze the data, providing personalized recommendations for healthcare professionals. A self-controlled before-after method was adopted. Before the application of the ICU digital intelligent multimodal shift handover system (from December 2023 to March 2024), the traditional verbal bedside handover was used; from June 2024 to March 2025, the ICU digital intelligent multimodal shift handover system was applied for shift handovers. Questionnaires before the application of the shift handover system were collected in April 2024, and those after the application were collected in April 2025. The shift handover time, handover quality (scored by the nursing handover evaluation scale), satisfaction with doctor-nurse communication (scored by the ICU doctor-nurse scale) before and after the application of the handover system were compared, and nurses' satisfaction with the shift handover system (scored by the clinical nursing information system effectiveness evaluation scale) was investigated.
RESULTS:
After the application of the ICU digital intelligent multimodal shift handover system, the shift handover time was significantly shorter than that before the application [minutes: 20 (15, 25) vs. 30 (22, 40)], the handover quality was significantly higher than that before the application [score: 84.0 (78.0, 88.5) vs. 71.0 (55.0, 79.0)], and the satisfaction with doctor-nurse communication was also significantly higher than that before the application (score: 84.58±6.79 vs. 74.50±11.30). All differences were statistically significant (all P < 0.05). In addition, the nurses' system effectiveness evaluation scale score was 102.30±10.56, which indicated that nurses had a very high level of satisfaction with the ICU digital intelligent multimodal shift handover system.
CONCLUSIONS
The application of the ICU digital intelligent multimodal shift handover system can shorten the shift handover time, improve the handover quality, and enhance the satisfaction with doctor-nurse communication. Nurses have a high level of satisfaction with this system.

Chinese Journal of Anesthesiology 2025;45(10):1317-1321
Objective:To evaluate the efficacy of ultrasound-guided subcostal quadratus lumborum block combined with general anesthesia in laparoscopic bariatric surgery. Methods:In this prospective randomized controlled study, 80 American Society of Anesthesiologists Physical Status classification Ⅰ or Ⅱpatients, aged 18-40 yr, with a body mass index of 30-45 kg/m 2, of cardiac function classification Ⅰ or Ⅱ, who underwent laparoscopic bariatric surgery at the First Affiliated Hospital of Nanjing Medical University from February 2023 to May 2023, were selected and divided into 2 groups ( n=40 each) using a table of random numbers: general anesthesia group (GA group) and subcostal quadratus lumborum block combined with general anesthesia group (SQB+ GA group). Both groups received combined intravenous-inhalational anesthesia. Patients in SQB+ GA group underwent bilateral ultrasound-guided subcostal quadratus lumborum block with 20 ml of 0.375% ropivacaine before anesthesia induction. Oxycodone was administered for analgesia during the post-anesthesia care unit (PACU) stay. Patient-controlled intravenous analgesia with sufentanil and granisetron was used after returning to the ward. When the visual analog scale score >4 within 24 h after surgery, flurbiprofen axetil 50 mg was intravenously given as rescue analgesic. The intraoperative consumption of remifentanil and use of vasoactive drugs were recorded. The effective pressing times of patient-controlled analgesia and requirement for rescue analgesia were recorded. The tracheal extubation time, PACU stay time, use of oxycodone during PACU stay, and occurrence of hypoxemia after tracheal extubation and occurrence of adverse reactions within 24 h after surgery was also recorded. Arterial blood gas analysis was performed at discharge from PACU. Results:Compared with GA group, the intraoperative consumption of remifentanil was significantly reduced, the usage rate of oxycodone, effective pressing times of patient-controlled analgesia and rate of rescue analgesia were significantly decreased, the PaO 2 and PaCO 2 at discharge from PACU were decreased, the tracheal extubation time and PACU stay time were shortened, and the incidence of hypoxemia after tracheal extubation and postoperative nausea and vomiting was decreased in SQB+ GA group ( P<0.05). Conclusions:Compared with general anesthesia alone, ultrasound-guided subcostal quadratus lumborum block combined with general anesthesia can reduce the intraoperative consumption of opioids, alleviate postoperative pain, and decrease the occurrence of postoperative adverse reactions when used in laparoscopic bariatric surgery.

Chinese Journal of Microsurgery 2025;48(5):547-551
Objective:To explore the feasibility of obtaining three-dimensional (3D) models of intraosseous and periosteal arteries of hallux phalanx using micro-arteriography with micro-CT scan. Methods:From January 2022 to April 2025, the Department of Orthopaedic, the 988th Hospital of the Joint Logistics Support Force of the Chinese PLA conducted a study on 7 fresh-frozen specimens of distal lower limb (right lower limb) from an 85-year-old male, and both lower limbs from an 82-year-old male, a 78-year-old female and a 66-year-old male in the Department of Human Anatomy & Histology and Embryology, Peking University School of Basic Medical Sciences. Red lead oxide powder (Pb 3O 4) was ground and filtered through a 300 mesh, and then mixed with turpentine at ratios of 1 g ∶ 1.5 ml, 1 g ∶ 1.0 ml, and 1 g ∶ 0.5 ml to prepare lead-based contrast agent suspensions. After thawing the specimens at room temperature, the suspensions were injected via the popliteal artery in ascending order of concentration. After injections, the specimens were fixed in 10% methanal for 2 weeks. The proximal and distal phalanges of the hallux, with the surrounding periosteum preserved intact, were then harvested. The harvested specimens were scanned using micro-CT at an ultimate resolution of 12 μm. Subsequently, Mimics Medical software was used to reconstruct 3D models of the intraosseous and periosteal arteries within the phalanges. Results:Periosteal arteries in the proximal phalanx were primarily distributed near the joint region. A consistently large trunk artery entered from plantar side, supplying most of the diaphysis and head. There was a rich periosteal arterial network on both sides of the distal phalanx, which communicates with each other through the arterial arch in the bone groove. However, trunk intraosseous artery could be absent. Intraosseous arteries in the proximal ends of both the proximal and distal phalanges originated from periosteal arteries. These formed an interconnected arteriosomes and coursed parallel to the articular surfaces. Conclusion:The micro-arteriography acquired by micro-CT scan effectively visualizes intraosseous and periosteal arteries and enables the reconstruction and analysis of 3D models of the arteriosomes. The characteristics of arteriolar distribution provide a theoretical basis for osteotomy or internal fixation procedures involving a hallux phalanx.
