1.Correlation of childhood trauma and leisure activities with psychological distress among upper grade elemetary school students

Objective:
To understand the impact of childhood trauma on psychological distress among upper grade elemetary school students, and to explore the mediating role of leisure activities in the relationship, so as to provide a basis for developing mental health intervention strategies.
Methods:
From August to November 2024, a combination of convenience sampling and stratified cluster random sampling was employed to recruit 1 373 fourth to sixth grade students from four primary schools in Harbin. The Childhood Trauma Questionnaire(CTQ), a self designed leisure activity scale (including active and passive leisure activities), and the Kessler Psychological Distress Scale (K10) were used to assess childhood trauma experiences, leisure activities, and levels of psychological distress. Spearman correlation analysis and linear regression analysis were conducted to explore the relationships among childhood trauma, leisure types, leisure time, and psychological distress. Based on the mediation analysis framework proposed by Hayes (Model 4), the mediating role of leisure types in the relationship between childhood trauma and psychological distress was examined.
Results:
Totally 19.1% of the upper elemetary school students exhibited psychological distress, while 30.2% had experienced childhood trauma. During school days, 64.6% of the students were reported of having leisure time concentrated between 1 and 5 hours per day, whereas 67.4% reported leisure time exceeding 5 hours per day on weekends. After controlling for potential demographic confounders such as gender, grade, ethnicity, household registration, being an only child, parents educational level, co residence, and whether parents are first time married,linear regression analysis showed that childhood trauma experience had positive predictive effect on psychological distress in upper primary school students( β =0.20, P <0.01). Leisure time showed no statistically significant association with psychological distress, both on school days ( β =-0.58 to -0.56) and weekends ( β =0.26- 0.98 )(all P >0.05). Active leisure activities were negatively associated with psychological distress ( β =-0.20), while passive leisure activities were positively associated with psychological distress ( β =0.29)(both P <0.01). Leisure type partially mediated the relationship between childhood trauma and psychological distress, accounting for 11.7% of the indirect effect.
Conclusion
Childhood trauma experiences positively predict psychological distress in upper elementary school students, and affect psychological distress through active leisure and passive leisure.
2.Application of glucocorticoids in severe non-antineoplastic drug-induced liver injury

Objective To explore the application and efficacy of glucocorticoids in severe non-antineoplastic drug-induced liver injury (DILI). Methods A retrospective study was conducted on patients with severe non-antineoplastic DILI who were hospitalized at Zhongshan Hospital, Fudan University from November 2016 to October 2025. Clinical data, laboratory results, suspected causative drugs, glucocorticoid use, and prognosis were collected. The time for peak total bilirubin decreased by 50% was used as an efficacy indicator. Patients were divided into a glucocorticoid group and a non-glucocorticoid group based on glucocorticoid use. Results A total of 102 patients with severe DILI were included, and 58 (56.9%) received glucocorticoid therapy. The most common suspected causative drugs were traditional Chinese medicine (65.7%), and the primary type of liver injury was hepatocellular (72.5%). The proportion of female patients (P=0.030), baseline peak total bilirubin (P<0.001), and international normalized ratio (P=0.034) were significantly higher in the glucocorticoid group than in the non-glucocorticoid group. The median time for peak total bilirubin decreased by 50% was 9.5 days in the glucocorticoid group and 11 days in the non-glucocorticoid group, with no statistically significant difference between the two groups. No significant benefit of glucocorticoid therapy was observed in subgroups with positive autoantibodies, elevated immunoglobulin G (IgG), or elevated γ-globulin ratio. Low-dose and gradual tapering were the main methods of glucocorticoid administration. Adverse reactions occurred in 8 patients (13.8%) in the glucocorticoid group, which resolved after drug withdrawal or symptomatic treatment. Conclusions The usage rate of glucocorticoids is relatively high in patients with severe non-antineoplastic DILI. Glucocorticoid treatment may accelerate the improvement of cholestasis in these patients, with mild adverse reactions and good safety.
3.Impact of childhood maltreatment and sleep quality on depressive symptoms among middle school students

Objective:
To explore the impact of sleep quality, experience of childhood maltreatment, and their interaction on depressive symptoms among middle school students, so as to provide the reference for early intervention of depressive symptoms among middle school students.
Methods:
From September to December 2023, a questionnaire survey was conducted among 1 231 students from two secondary schools in Harbin, Heilongjiang Province by a convenient sampling method. The survey included general demographic information, Childhood Trauma Questionnaire Short Form, Pittsburgh Sleep Quality Index and Short Version of Center for Epidemiological Studies Depression Scale. The Chi square test was used to analyze the differences in depressive symptom, sleep quality and childhood maltreatment among students with different demographic characteristics. Correlation analysis was conducted using Logistic regression, and interaction analysis was performed by both additive and multiplicative interaction models.
Results:
The detection rate of depressive symptoms among middle school students was 22.7%, and the rate for high school students (35.2%) was significantly higher than that for middle school students (17.0%) ( χ 2=50.35, P <0.01). The detection rates of depressive symptoms among middle school students with a history of childhood maltreatment and poor sleep quality were 45.8% and 44.0%, respectively. Multivariate Logistic regression analysis showed that compared to students without a history of childhood maltreatment, students with a history of childhood maltreatment had a higher risk of depressive symptoms ( OR =4.49,95% CI =3.31~ 6.09 , P <0.01);students with poor sleep quality had a higher risk of depressive symptoms than students with good sleep quality ( OR = 5.99,95% CI =4.37~8.22, P <0.01).The interaction results showed that the presence of childhood maltreatment and poor sleep quality had an additive interaction on the occurrence of depression in middle school students. Compared with students without childhood maltreatment and having good sleep quality, students with childhood maltreatment and poor sleep quality had a 22.49 times higher risk of developing depression ( OR =22.49,95% CI =14.22~35.59, P <0.01).
Conclusion
Depressive symptoms among middle school students are associated with childhood maltreatment and poor sleep quality, and there is an additive interaction between childhood maltreatment and poor sleep quality on the impact of depressive symptoms.
4.Exploration of the practice of establishing a target management system for department heads in large public hospitals

With the deepening of the reform of the medical and health system,the high-quality development of public hospitals has put forward higher requirements for the modern management system.This article takes a top-tier hospital in Huai'an as an example to explore the practical path and effectiveness of constructing a target management system for department directors in large public hospitals.The research achieved a comprehensive improvement in management efficiency by building a multi-di-mensional discipline evaluation system,promoting the construction of key specialties,creating a'trinity'medical quality man-agement system,strengthening the construction of quality control centers,and combining personalized target management respon-sibility agreements with the selection mechanism for outstanding department directors.Over the three years of implementation,key indicators of the hospital,such as the number of key specialties,CMI value,proportion of fourth-level surgeries,and average length of stay,have significantly improved.The research shows that this system,through the synergistic effect of goal orientation and performance incentives,effectively promotes the coordinated development of discipline construction,medical quality,and op-erational efficiency,providing a reference management paradigm for public hospitals to transition from scale expansion to connota-tion enhancement.
5.Horizontal sound localization in young and middle-aged patients with symmetric sensorineural hearing loss in noisy environments

Objective:This study evaluates the horizontal sound localization ability of young and middle-aged individuals with symmetric sensorineural hearing loss (SNHL) in noisy environments. It also examines the impact of hearing loss severity and signal-to-noise ratio (SNR) on localization accuracy.Methods:In this cross-sectional study, conducted from April 2023 to April 2024, 135 young and middle-aged patients (73 males and 62 females, aged 18-60 years) with SNHL who sought care at Beijing Chaoyang Hospital, were categorized into mild, moderate, and moderate-to-severe hearing loss groups (45 per group), with 45 normal-hearing controls (23 males and 22 females, aged 20-60 years). Participants completed localization tasks in quiet and noisy environments with SNR levels of 5 dB, 0 dB, -5 dB, and-10 dB. Root mean square error (RMSE) was used to measure localization accuracy. Repeated measures ANOVA assessed the effects of hearing loss and SNR on RMSE, while, Pearson correlation evaluated the relationship between binaural 4-frequency pure-tone average (4fPTA) and RMSE. Multiple linear regression analyzed the predictive role of 4fPTA and age.Results:(1) Two-way repeated measures ANOVA showed that both hearing loss severity and SNR significantly affected RMSE ( F=92.67, P<0.01; F=430.29, P<0.01), with a significant interaction between the two factors( F=92.67, P<0.01). (2) RMSE increased with hearing loss severity. At SNRs of 5 dB, 0 dB, and-5 dB, the moderate-to-severe group had significantly higher RMSE than the mild and moderate groups ( P<0.01). No significant differences were found between mild and moderate groups ( P=0.53, 0.57, 0.22). At-10 dB SNR, significant differences were observed across all groups ( P<0.01). (3) RMSE increased non-linearly as SNR decreased. Mean RMSE values under quiet conditions and at SNRs of 5 dB, 0 dB, -5 dB, and-10 dB were (7.43±5.01)°, (9.80±5.74)°, (11.60±6.22)°, (14.56±7.07)°, and (18.74±8.02)°, respectively. (4) RMSE was significantly positively correlated with binaural 4fPTA ( r=0.54-0.58, P<0.01). Multiple linear regression analysis indicated that the binaural average 4fPTA significantly predicted RMSE ( P<0.01), explaining 30.5%-34.1% of RMSE variance. Age did not significantly contribute to RMSE variation. Conclusions:The degree of hearing loss and background noise SNR significantly affect horizontal sound localization in young and middle-aged SNHL patients. RMSE increases with hearing loss severity and decreases with higher SNR. The interaction between hearing loss and SNR is significant, and RMSE correlates with binaural 4fPTA. However, the regression model based on 4fPTA and age explains only part of the RMSE variance, suggesting other contributing factors.
6.Identification of associated factors and construction of a predictive model for membranous nephropathy patients with IgM deposition

Objective:To explore the associated factors for membranous nephropathy (MN) patients with IgM deposition, and to construct a prediction model.Methods:This study was a retrospective cohort study. Patients diagnosed with MN with IgM deposition by renal biopsy in the Fourth Hospital of Hebei Medical University from February 2017 to December 2023 were retrospectively included. Clinical and pathological data were collected. The study population was randomized into a training set and a validation set at a 7:3 ratio. The endpoint event was defined as the remission of MN, and the patients were divided into remission group and non-remission group to compare the clinical and pathological examination results. Least absolute shrinkage and selection operator regression analysis and Cox regression analysis were used to explore the associated factors of poor prognosis of MN patients with IgM deposition. Internal validation was conducted using the validation set data. The clinical efficacy of the predictive model was evaluated by calculating the area under the receiver operating characteristic (ROC) curve and generating calibration curves. The total nomogram score for each patient was calculated based on the training set data, and the predictive performance was assessed by plotting the ROC curve. Patients were then stratified into low-risk and high-risk groups according to the optimal cut-off value derived from the ROC analysis of the total nomogram score. Kaplan-Meier survival analysis was performed to compare the remission rate between the two groups. Model performance was evaluated using the validation set.Results:A total of 200 MN patients with IgM deposition were included, and 49.0% of them achieved clinical remission. In the training set, statistically significant differences were observed in 24-hour urine protein quantification ( Z=-2.638, P=0.008), renal arteriolar wall thickening ( χ2=6.891, P=0.009), the proportion of patients receiving immunosuppressive therapy ( χ2=21.381, P<0.001), and the proportion of patients treated with corticosteroids combined with cyclophosphamide ( χ2=10.107, P=0.001). Through least absolute shrinkage and selection operator regression and Cox regression, 2 factors associated with clinical remission in MN patients with IgM deposition were simultaneously identified from 16 potential associated factors, including the use of immunosuppressants ( HR=3.823, 95% CI 2.055-7.113, P<0.001), and renal arteriolar wall thickening ( HR=0.428, 95% CI 0.221-0.831, P=0.012). Incorporating the clinical measurement of phospholipase A2 receptor (PLA2R) antibodies, a predictive model was established. The performance of the model was evaluated using the training dataset, yielding an area under the ROC curve of 0.731 (95% CI 0.648-0.814), with a sensitivity of 88.7% and a specificity of 55.1%. The optimal cut-off value was a total nomogram score of 41.7 points. The Kaplan-Meier survival analysis showed that the remission rate was significantly higher in the low-risk group than that of the high-risk group (Log-rank test, χ2=33.525, P<0.001). Model validation was performed using the validation dataset, which showed an AUC of 0.715 (95% CI 0.591-0.839), sensitivity of 70.4%, and specificity of 63.6%. Similarly, the Kaplan-Meier survival analysis demonstrated a significantly higher remission rate in the low-risk group than in the high-risk group (Log-rank test, χ2=8.467, P=0.004). Conclusion:A nomogram predictive model for remission of MN patients with IgM deposition, based on serum PLA2R antibody levels, the use of immunosuppressive therapy, and renal arteriolar wall thickening is developed. The model demonstrates a moderate clinical applicability.
7.Adipose tissue derived extracellular vesicles promote nerve regeneration and functional recovery after sciatic nerve injury in rat

Objective:To study the effect of adipose tissue derived extracellular vesicles (AT-EVs) on nerve regeneration and functional reinnervation after sciatic nerve injury (SNI) in rat.Methods:From April 2023 to April 2024, the experiment was completed by the Department of Orthopaedics, the First Affiliated Hospital of Zhengzhou University in the key laboratory of the School of Medical Sciences, Zhengzhou University. AT-EVs were isolated and extracted from subcutaneous adipose tissue of 12 healthy male SD rats using ultracentrifugation. The morphology, size and distribution of AT-EVs were characterised via transmission electron microscopy (TEM) and nanoparticle tracking analysis (NTA). The effect of AT-EVs on the proliferation ability of pheochromocytoma cells of rat (PC12) and rat Schwann cells (RSC 96) were detected by cell proliferation assay. Twenty-four healthy male SD rats were randomly divided into 3 groups with 8 rats per group, including a sham group, a SNI group and an AT-EVs treatment group. Right sciatic nerves of rat models in sham group were exposed without injury. For the rat models in SNI group and AT-EVs treatment group, right sciatic nerves were injected with phosphate buffered solution (PBS) and AT-EVs under the epineurium of the nerve respectively following sciatic nerve crush. At 28 days after surgery, Sciatic function index (SFI) and pace analysis was performed. Then, rats in all groups were sacrificed, and bilateral gastrocnemius muscles were collected to measure the muscle wet weight ratio. The right sciatic nerves were removed for further exploration of nerve regeneration. The histopathological changes and axon arrangement of sciatic nerve were evaluated by Hematein Eosin (HE) staining and Masson staining. The regeneration efficiency of Myelin forming protein (MBP) and Neuronal marker [β-tubulin (Tuj-1)] were analysed by immunofluorescence staining of sciatic nerve. Statistical analysis was conducted using GraphPad Prism 8.0. P<0.05 were considered statistically significant. Results:The results of TEM and NTA revealed that AT-EVs exhibited a typical disc-shaped bilayer membrane structure with a predominant particle size of 184.5 nm. AT-EVs significantly enhanced the proliferation ability of PC12 and RSC96, and the difference was statistically significant ( P<0.05). At 28 days after surgery, the SFI and muscle wet weight ratio were higher in the AT-EVs treatment group compared to the SNI group, and the difference was statistically significant ( P<0.05). Axons in AT-EVs treatment group were arranged more closely and orderly than those in SNI group at 28 days after surgery, and there were less axon disintegration and vacuolation caused by injury. The immunofluorescence assay indicated that the MBP and Tuj-1 fluorescence intensity in AT-EVs treatment group was significantly higher than those in the SNI group, and the difference was statistically significant ( P<0.05). Conclusion:AT-EVs can enhance the proliferation ability of PC12 and RSC96 to promote axon regeneration and functional reinnervation following SNI.
8.Advances in tissue kallikrein for acute ischemic stroke

Acute ischemic stroke(AIS)is a common neurological disorder that still presents many challenges despite advancements in diagnostic and therapeutic techniques.Tissue kallikrein(TK),a cardiovascular and cerebrovascular protective factor,is involved in the entire process of AIS and holds significant clinical value.This review explores the mechanisms of TK in AIS,including targeting collateral circulation improvement in ischemic areas,protecting neurovascular coupling,reducing neuronal injury,and participating in remote ischemic conditioning.Additionally,plasma TK levels are negatively correlated with AIS incidence and recurrence risk,which can serve as a potential biomarker.TK therapy has been shown to improve neurological function in AIS patients,reduce recurrence rates and enhance outcomes when combined with reperfusion therapies.TK also demonstrates therapeutic benefits in AIS patients with multiple organ comorbidities.This review concludes the important roles of TK in the diagnosis,treatment,and prevention of AIS,providing new insights for future research and clinical applications.
9.Efficacy analysis of endoscopic endonasal and craniotomy surgery in the treatment of craniopharyngioma Based on QST Classification

Objective This study aims to explore the efficacy and complication rates of the transcranial approach(TCA)and extended endoscopic endonasal approach(EEEA)for the treatment of craniopharyngiomas based on the QST classification,providing a scientific reference for clinical decision-making on surgical approach.Methods A total of 151 patients who underwent craniopharyngioma surgery at our center from January 2018 to December 2023 were enrolled.The patients were categorized into Q-CP(suprasellar type),S-CP(infundibular type),and T-CP(tuberal type)according to the QST classification.Systematic collection and analysis were performed on the outcomes of TCA and EEEA treatments,respectively.The differences in effectiveness between the two approaches were evaluated based on the QST classification.Results The improvement rate of visual symptoms was overall higher in the EEEA group than in the TCA group(59.1%vs.36.5%,P=0.006),and the visual deterioration rate was lower(3.0%vs.14.1%,P=0.006).However,the incidence of cerebrospinal fluid leakage was higher in the EEEA group(15.2%vs.3.5%,χ2=4.986,P=0.026).The incidence of postoperative seizures(8.2%vs.0,P=0.019),brain contusions(10.6%vs.0,P=0.005),and subdural hematoma(9.4%vs.0%,P=0.01)was higher in the TCA group.For patients with Q-CP type,the EEEA group had a higher rate of total tumor resection(92.9%vs.65.2%,P=0.025)and a lower recurrence rate(3.6%vs.21.7%,P=0.047),with shorter hospital stays and lower postoperative costs.The TCA group had higher intraoperative blood loss in this type(300 mL vs.200 mL,Z=-2.261,P=0.024).For S-CP type patients,the EEEA group showed a higher rate of total tumor resection(91.3%vs.74.2%)and a lower recurrence rate(0 vs.12.9%,P=0.031),with lower postoperative hospital costs.In T-CP type,due to the deeper location,EEEA showed limitations in protecting hypothalamic function and the TCA group had a better postoperative hypothalamic function score(P=0.035).Conclusion Based on QST classification,EEEA has advantages in Q-CP and S-CP types and is recommended as the preferred surgical procedure;In the T-CP type,TCA surgery is more helpful in protecting hypothalamic function.
